Abstract Objectives The prevalence of ESBL-producing Escherichia coli (ESBL-E. coli) in community-acquired urinary tract infections (UTI) has been increasing worldwide since 2000, but with large geographical variations. The aim of this study was to determine whether the ESBL-E. coli rate in urine samples from individuals with community-acquired UTI was associated with the local socio-economic, environmental, agricultural and healthcare characteristics. Methods This was a cross-sectional study in western France using data on antibiotic susceptibility of E. coli isolated from urine samples of individuals with community-acquired UTI analysed in non-hospital laboratories from 2015 to 2017. The ESBL-E. coli rate was calculated for each laboratory. Data on socio-economic characteristics, human antibiotic consumption, hospital bed density, animal farming density and percentage of agricultural land and surface water were retrieved at the municipality level and aggregated by study area. Their association with ESBL-E. coli prevalence was quantified using multivariate linear regression models with a backward selection. Results From 358 291 E. coli isolates from urine samples tested in 92 laboratories, the mean ESBL-E. coli prevalence for the study period was 3.30%. In an adjusted model, the ESBL-E. coli rate was significantly (P < 0.05) and positively associated with the local percentage of people >65 years old, third-generation cephalosporin use (DDD/1000 inhabitants), number of hospital beds/km2, poultry density, pig density and percentage of agricultural land. Lower deprivation was associated with a higher ESBL-E. coli rate. Conclusions Several anthropogenic factors (primary care, hospitals and animal farming) are associated with the local ESBL-E. coli rate in community-acquired UTI. These results could contribute to improve risk management, including identification of at-risk patient groups.

Background and Objectives: Urinary Tract Infection (UTI) is one of the most common infections in the community and hospitalized patients. The aim of the present study was to investigate bacteria isolated from urinary tract infections and their antibiotic resistance in hospitalized patients. Subjects and Methods In the this descriptive-cross-sectional study, the results of about 5,000 urine samples sent for culture from the hospitalized patients of Golestan Hospital in Ahvaz in 2019 were examined. Relevant information was extracted from patients’ medical records. Results Out of 5000 samples studied (39.7% female and 60.3% male), about 468 samples (9.36%) showed positive urine culture. Of the patients with UTI, 205 patients were women (43.8%). The risk of UTI increased with age. The most common bacteria caused urinary tract infections were E. coli with 51.5%, followed by Klebsiella with 29.3%. It was also observed that bacteria isolated from urine samples of people with UTI had antibiotic resistance, and for each bacteria special groups of antibiotics were more effective. Conclusion The prevalence of UTI in the studied patients was 9.36%. In females and with increasing age, the prevalence of UTI was higher. The most common bacteria that caused UTI were E. coli and then Klebsiella. Due to the fact that the resistance and sensitivity of bacteria to antibiotic used, it is recommended that the most common bacterial agents of UTI must be diagnosed and then the most appropriate antibiotic must be prescribed

Abstract Background: Resistance of Gram-negative bacteria to antibiotics had increased at an alarming pace over the last two decades. Empirical antibiotic treatment of UTI practice created opportunity for frequent use of antibiotics that resulted in emergence of resistant strains. Extended spectrum beta-lactamase producing Enterobacteriaceae are emerging worldwide and are usually multi-drug resistant; thus, limiting treatment options for these organisms. Hence, it is mandatory to scrutinize the extent of the problem in the study area. Objective: We investigated the prevalence and correlates of extended spectrum beta-lactamase producing Enterobacteriaceae and their antibacterial resistance profile among children with urinary tract infection (UTI) at Hawassa University Comprehensive Specialized Hospital. Method: A hospital-based cross sectional study was conducted from February 1-July 30/ 2018. A semi-structured questionnaire was used to collect data from 284 children suspected of UTI. Bacterial species were isolated using standard bacteriological techniques. Antibiotic susceptibility testing was done using Kirby-Bauer disk diffusion method and ESBL production was detected by double disc synergy test. Data entry, cleaning and statistical analysis was done using SPSS version 20. Results: Overall, Enterobacteriaceae accounted for 75% (72/96) of UTI infections. The most frequent Enterobacteriaceae were E. coli , 44.4% (32/72) and K. pneumoniae , 27.8% (20/72). Overall, ESBL-producers within Enterobacteriaceae accounted for 41.7% (30/72) and magnitude of multi-drug resistance level was 86.1% (62/72). ESBL producing K. pneumoniae and E. coli accounted for 70% (14/20) and 37.5% (12/32), respectively of all the respective isolates. Previous UTI within the past 12 month (AOR= 0.08, 95% CI (0.01-0.57) and medium family wealth index (AOR = 0.03, 95% CI (0.00-0.27) were associated with infection of ESBL-producing Enterobacteriaceae . Conclusion: ESBL producing Enterobacteriaceae were prevalent among UTI cases; where E. coli and K. pneumoniae were the major isolated resistant strains that contributed mainly to the problem. Majority of Enterobacteriaceae isolated were resistant to commonly prescribed drugs. Key words : ESBL, Antimicrobial Resistance, UTI, Children.

Background: Urinary tract infections “UTIs” are one of the frequently encountered problems during pregnancy. Untreated UTI can be associated with serious obstetric complications.  Hence the management and prevention of UTI in pregnancy is the crucial factor for the sustained pregnant period. Hence the present study was designed to identify the pathogenic agents of UTI and its associated risk factors in antenatal mothers in two private hospitals at Kathmandu city. Aim: The main objective of the study was to identify the etiological agent and associated risk factor in Urinary tract infection among pregnant women attending the antenatal care service visit in a selected hospital. Study Design: Cross-sectional descriptive study. Place and Time of Study: The study was carried out under the supervision of University and field study was carried out in Kathmandu, Nepal from March 2019 to October 2019. Methods: The cross-sectional study was performed at the Norvic Hospital and Baidya and Banskota hospital from March 2019 to October 2019. In this cross-sectional study, a total of 510, clean catch midstream urine (MSU) samples were collected aseptically and analyzed using standard microbiology methods. Data for the factors associated with UTI were obtained by use of questionnaires and standard laboratory tests for selected underlying conditions concerning associated risk factors were collected using structured questionnaires and the sample was also processed for antimicrobial drug susceptibility testing. Results: The study revealed 193/510(37.84%) UTI prevalence among patients attending two hospitals in Kathmandu valley. Escherichia coli was the most prevalent bacterial uropathogen with 119/193(61.6%) followed by Klebsiella pneumoniae 35/193(18.13%), Staphylococcus aureus 22/193 (11.39%), Proteus mirabilis 7/193 (3.6%), Enterococcus faecalis 2/193 (1.0%), Enterobacter aerogenes2/193(1.0%) Serratia odorifera 3/193(1.5%), Streptococcus species3/193(1.5%). Conclusions: The high prevalence of urinary tract infection in pregnant women warrant the need to screen all pregnant women and treat those infected with appropriate antimicrobial regimens in order to reduce its complications. Urinary Tract Infection screening is essential in pregnant women.
