<b><i>Background:</i></b> Hidradenitis suppurativa (HS) is a complex disease that is optimally managed with specialized care. Data on HS specialty clinics (HSSCs) are lacking. <b><i>Methods:</i></b> HSSCs in the USA were identified on the HS Foundation website and analyzed for geographic location and clinic director demographics. <b><i>Results:</i></b> We identified 29 HSSCs in 16 states, an increase from 22 in 2019. Thirty-four states currently lack a HSSC; the Mountain West and East South Central regions of the USA are particularly affected. Among HSSC directors, the majority (93.3%) are dermatologists, with slightly more women (53.3%) than men (46.7%). Most (86.7%) have an academic affiliation, and the majority (60%) graduated from residency &#x3c;10 years ago. All directors are involved in research, and over half of HSSCs serve as HS clinical trial sites. <b><i>Conclusions:</i></b> The number of HSSCs in the USA has been growing, yet there remains a dearth in certain regions. Given that HS is a disease with high unmet need and a rapidly evolving therapeutic landscape, we encourage the establishment of more HSSCs to improve access to specialized care.

Cat-associated Bartonella species, which include B. henselae, B. koehlerae, and B. clarridgeiae, can cause mild to severe illness in humans. In the present study, we evaluated 1362 serum samples obtained from domestic cats across the U.S. for seroreactivity against three species and two strain types of Bartonella associated with cats (B. henselae type 1, B. henselae type 2, B. koehlerae, and B. clarridgeiae) using an indirect immunofluorescent assay (IFA). Overall, the seroprevalence at the cutoff titer level of ≥1:64 was 23.1%. Seroreactivity was 11.1% and 3.7% at the titer level cutoff of ≥1:128 and at the cutoff of ≥1:256, respectively. The highest observation of seroreactivity occurred in the East South-Central, South Atlantic, West North-Central, and West South-Central regions. The lowest seroreactivity was detected in the East North-Central, Middle Atlantic, Mountain, New England, and Pacific regions. We observed reactivity against all four Bartonella spp. antigens in samples from eight out of the nine U.S. geographic regions.

BackgroundOlder patients compose approximately 30% of trauma patients treated in the USA but make up nearly 50% of deaths from trauma. To help standardize and elevate care of these patients, the American College of Surgeons (ACS) Trauma Quality Improvement Program’s best practice guidelines for geriatric trauma management was published in 2013 and that for palliative care was published in 2017. Here, we discuss how palliative care and geriatrics quality metrics can be tracked and used for performance improvement and leveraged as a strength for trauma verification.MethodsWe discuss the viewpoint of the ACS Verification, Review, and Consultation and three case studies, with practical tips and takeaways, of how these measures have been implemented at different institutions.ResultsWe describe the use of (1) targeted educational initiatives, (2) development of a consultation tool based on institutional resources, and (3) application of a nurse-led frailty screen.DiscussionSpecialized care and attention to these vulnerable populations is recommended, but the implementation of these programs can take many shapes.Level of evidence V

A challenge in the development of renewable energy is the ability to spatially assess the risk of feedstock supply to conversion facilities. Policy makers and investors need improved methods to identify the interactions associated with landscape features, socioeconomic conditions, and ownership patterns, and the influence these variables have on the geographic location of potential conversion facilities. This study estimated opportunity zones for woody cellulosic feedstocks based on landscape suitability and market competition for the resource. The study covered 13 Southern States which was a segment of a broader study that covered 33 Eastern United States which also included agricultural biomass. All spatial data were organized at the 5-digit zip code tabulation area (ZCTA). A landscape index was developed using factors such as forest land cover area, net forest growth, ownership type, population density, median family income, and farm income. A competition index was developed based on the annual growth-to-removal ratio and capacities of existing woody cellulosic conversion facilities. Combining the indices resulted in the identification of 592 ZCTAs that were considered highly desirable zones for woody cellulosic conversion facilities. These highly desirable zones were located in Central Mississippi, Northern Arkansas, South central Alabama, Southwest Georgia, Southeast Oklahoma, Southwest Kentucky, and Northwest Tennessee.

Crayfish have been in demand as desirable food items around the globe for centuries, and entrepreneurs have capitalized on this demand by developing and applying aquaculture principals for the intentional culture of this freshwater crustacean. The current state of the art has advanced within the last half century and is centered on a handful of species, represented by three different families, with some level of commercial production occurring on all continents except Antarctica. Procambarus clarkii (family Cambaridae), a native of south central USA, is cultured in the USA and China and easily forms the bulk of farm-raised and wild-captured crayfish globally. One North American species (Pacifastacus leniusculus) and two European species (Astacus astacus and A. leptodactylus) constitute the main cultured species from the family Astacidae and are grown in small operations throughout Europe and parts of Asia. Four species (Parastacidae), all natives of Oceania, are cultured in their native ranges and were also introduced for aquaculture in several locations around the globe. Cherax destructor and C. albidus, both commonly referred to as yabby, are medium-size crayfish and share similar life histories, whereas C. quadricarinatus (redclaw crayfish) and C. cainii (smooth marron) are larger and more valuable but have very different geographical origins. While commercial crayfish aquaculture is typically based on an extensive or semi-extensive production approach in earthen ponds, more intensive approaches may involve selective breeding, improved strains, brood or nursery phases, and use of raceways or recirculation systems. Pond size can range from 0.05 to 80 ha, depending on the species cultured. Harvesting is accomplished mainly by baited trap, although other gear and techniques are sometimes employed. Global crayfish aquaculture production has expanded significantly in the last decade, due largely to the integration of Procambarus clarkii with that of rice production in the USA and China. This integrated system of production works well because rice farming has similar requirements as crayfish aquaculture, such as clay soils, irrigation systems, and suitable climates; furthermore, the rice crop residue provides the base of the food web for furnishing sustenance to growing crayfish.

Abstract Background There is a significant variation in the reported prevalence of hidradenitis suppurativa (HS), ranging from 0.03–4%. We hypothesized that this significant variation may be due to different prevalence rates of HS according to geographical location as well as sex. Objective We aimed to perform a meta-analysis to determine pooled overall prevalence of HS, prevalence stratified according to geographical region and sex. Materials and methods A systematic review was performed by searching Ovid Medline, PubMed, Cochrane Library, DARE, and Embase, from inception to August 2018. A systematic review and meta-analysis was performed according to PRISMA guidelines. A meta-analysis of proportions was performed to determined pooled prevalence rates, with meta-regression based on geographic region. Prevalence in males versus females was also performed according to region. Results The overall pooled prevalence rate was 0.3% (0.2–0.6%) based on 118,760,093 HS cases available. Subgroup analysis demonstrated prevalence differences, with the highest being in Europe 0.8% (0.5–1.3%), compared to the USA 0.2% (0.1–0.4%), Asia-Pacific 0.2% (0.01–2.2%), and South America 0.2% (0.01–0.9%). Prevalence in males was lower compared to females in the USA (OR 0.403, 95% CI 0.37–0.439, P < 0.001) as well as in Europe (OR 0.635, 95% CI 0.397–1.015, P = 0.08) but not in the Asia-Pacific region (OR 0.936, 95% CI 0.319–2.751, P = 0.78). Conclusion Prevalence of HS varies significantly according to the geographical population. This variation is likely attributed to different ethnicity distributions amongst different continents. PurposeThe support of host country nationals (HCNs) is critical for expatriate adjustment and performance. Drawing from social identity theory and self-categorization theory, this study investigates the antecedents of HCNs' support toward expatriates in Central/South America, focusing on cultural similarities and expatriate race.Design/methodology/approachWe conducted a quasi-experimental study to understand the antecedents that promote the willingness of HCNs to offer required support to expatriates. Data were gathered from 117 Latin American participants, who were asked to respond to questions about their perceptions of expatriates from the USA and their willingness to offer support to those expatriates.FindingsOverall, our findings suggest that HCNs are likely to provide support to expatriates when they perceive the expatriates as similar in terms of culture and race. Specifically, African Americans received more positive attitudes and support than White Americans in South/Central America. The effect of cultural similarity on HCN willingness to support expatriates was mediated by perceived trustworthiness.Originality/valueThe present study extends the research on HCN support to expatriates, to Central/South America, an important region that has been under-studied in the expatriate–HCN context. Another novel feature of our study is that we investigate the role of expatriate race and cultural similarity and illuminate the underlying mechanism of the relationship between expatriate race and HCN support.

Abstract BACKGROUND Established by the Centers for Medicare and Medicaid Services (CMS), the Open Payments Database (OPD) has reported industry payments to physicians since August 2013. OBJECTIVE To evaluate the frequency, type, and value of payments received by academic neurosurgeons in the United States over a 5-yr period (2014-2018). METHODS The OPD was queried for attending neurosurgeons from all neurosurgical training programs in the United States (n = 116). Information from the OPD was analyzed for the entire cohort as well as for comparative subgroup analyses, such as career stage, subspecialty, and geographic location. RESULTS Of all identified neurosurgeons, 1509 (95.0%) received some payment from industry between 2014 and 2018 for a total of 106 171 payments totaling $266 407 458.33. A bimodal distribution was observed for payment number and total value: 0 to 9 (n = 438) vs &gt; 50 (n = 563) and 0-$1000 (n = 418) vs &gt;$10 000 (n = 653), respectively. Royalty/License was the most common type of payment overall (59.6%; $158 723 550.57). The median number (40) and value ($8958.95) of payments were highest for mid-career surgeons. The South-Central region received the most money ($117 970 036.39) while New England received the greatest number of payments (29 423). Spine surgeons had the greatest median number (60) and dollar value ($20 551.27) of payments, while pediatric neurosurgeons received the least (8; $1108.29). Male neurosurgeons received a greater number (31) and value ($6395.80) of payments than their female counterparts (11, $1643.72). CONCLUSION From 2014 to 2018, payments to academic neurosurgeons have increased in number and value. Dollars received were dependent on geography, career stage, subspecialty and gender.

ObjectivesHidradenitis suppurativa (HS) causes substantial morbidity and quality-of-life impairment. We examined demographic/clinical characteristics of patients with HS and treatment patterns, prevalence and healthcare resource utilisation/expenditures related to HS in the real-world.DesignRetrospective claims data of MarketScan Commercial, Medicare Supplemental and Medicaid databases (2009–2014).SettingUSA.ParticipantsPatients aged ≥12 years with ≥3 non-diagnostic outpatient or inpatient claims with an HS diagnosis code and ≥12 months continuous enrolment with medical and pharmacy benefits before (preindex) and after (postindex) the earliest diagnosis of HS (index) were included.ResultsThere were 11 325 Commercial/Medicare patients (mean age 37.4 years) and 5164 Medicaid patients (mean age 28.3 years). HS was more common in Medicaid than Commercial/Medicare patients (0.301% and 0.098%, respectively, in 2014). Cellulitis and psychiatric disorders were the most common comorbidities and oral antibiotics and narcotics were the most frequently prescribed drugs preindex, with ≥10% increase postindex in both populations. HS-related inpatient costs decreased while outpatient costs increased from preindex to postindex. Medicaid patients had several risk factors that may be associated with poor outcomes (eg, high rates of prescription pain medication use, comorbidities, drug discontinuation/interruption/holiday, emergency department (ED) visits and hospitalisation).ConclusionsCommercial/Medicare and Medicaid HS beneficiaries experience high comorbidity burden but use different treatment modalities to manage HS. Results suggest a substantial unmet need exists among this patient population, with Medicaid patients experiencing a particularly high burden of disease and expensive healthcare resource utilisation.
