Abstract Background Promoting well-being and preventing poor mental health in young people is a major global priority. Building emotional competence skills via a mobile app may be an effective, scalable and acceptable way to do this. A particular risk factor for anxiety and depression is elevated worry and rumination (repetitive negative thinking, RNT). An app designed to reduce RNT may prevent future incidence of depression and anxiety. Method/design The Emotional Competence for Well-Being in Young Adults study developed an emotional competence app to be tested via randomised controlled trials in a longitudinal prospective cohort. This off-shoot study adapts the app to focus on targeting RNT (worry, rumination), known risk factors for poor mental health. In this study, 16–24 year olds in the UK, who report elevated worry and rumination on standardised questionnaires are randomised to (i) receive the RNT-targeting app immediately for 6 weeks (ii) a waiting list control who receive the app after 6 weeks. In total, the study will aim to recruit 204 participants, with no current diagnosis of major depression, bipolar disorder or psychosis, across the UK. Assessments take place at baseline (pre-randomisation), 6 and 12 weeks post-randomisation. Primary endpoint and outcome for the study is level of rumination assessed on the Rumination Response Styles Questionnaire at 6 weeks. Worry, depressive symptoms, anxiety symptoms and well-being are secondary outcomes. Compliance, adverse events and potentially mediating variables will be carefully monitored. Discussion This trial aims to better understand the benefits of tackling RNT via an mobile phone app intervention in young people. This prevention mechanism trial will establish whether targeting worry and rumination directly via an app provides a feasible approach to prevent depression and anxiety, with scope to become a widescale public health strategy for preventing poor mental health and promoting well-being in young people. Abstract Background Promoting well-being and preventing poor mental health in young people is a major global priority. Building emotional competence (EC) skills via a mobile app may be an effective, scalable and acceptable way to do this. However, few large-scale controlled trials have examined the efficacy of mobile apps in promoting mental health in young people; none have tailored the app to individual profiles. Method/design The Emotional Competence for Well-Being in Young Adults cohort multiple randomised controlled trial (cmRCT) involves a longitudinal prospective cohort to examine well-being, mental health and EC in 16–22 year olds across 12 months. Within the cohort, eligible participants are entered to either the PREVENT trial (if selected EC scores at baseline within worst-performing quartile) or to the PROMOTE trial (if selected EC scores not within worst-performing quartile). In both trials, participants are randomised (i) to continue with usual practice, repeated assessments and a self-monitoring app; (ii) to additionally receive generic cognitive-behavioural therapy self-help in app; (iii) to additionally receive personalised EC self-help in app. In total, 2142 participants aged 16 to 22 years, with no current or past history of major depression, bipolar disorder or psychosis will be recruited across UK, Germany, Spain, and Belgium. Assessments take place at baseline (pre-randomisation), 1, 3 and 12 months post-randomisation. Primary endpoint and outcome for PREVENT is level of depression symptoms on the Patient Health Questionnaire-9 at 3 months; primary endpoint and outcome for PROMOTE is emotional well-being assessed on the Warwick-Edinburgh Mental Wellbeing Scale at 3 months. Depressive symptoms, anxiety, well-being, health-related quality of life, functioning and cost-effectiveness are secondary outcomes. Compliance, adverse events and potentially mediating variables will be carefully monitored. Conclusions The trial aims to provide a better understanding of the causal role of learning EC skills using interventions delivered via mobile phone apps with respect to promoting well-being and preventing poor mental health in young people. This knowledge will be used to develop and disseminate innovative evidence-based, feasible, and effective Mobile-health public health strategies for preventing poor mental health and promoting well-being. AbstractBackgroundAdolescents and young adults have been greatly affected by quarantine measures during the coronavirus-19 pandemic. Quantitative evidence suggests that many young people have struggled with their mental health throughout “lockdown”, but little is understood about the qualitative impact of social distancing restrictions on mental health, wellbeing and social life. We therefore sought to elicit the views and experiences of adolescents and young adults living in the UK during the pandemic.MethodsSemi-structured qualitative interviews were undertaken with 37 participants aged 13-24.ResultsWe identified 4 superordinate themes most commonly described by participants about their experiences during the pandemic, including: a) missing social contact during lockdown, b) disruption to education, c) changes to social relationships, and d) improved wellbeing during lockdown. Although we identified some positive experiences during the pandemic, including an increased awareness of mental health and stronger relationship ties, many said they struggled with loneliness, a decline in mental health, and anxiety about socialising after the pandemic.ConclusionsFindings suggest that some young people may have felt less stigma talking about their mental health now compared to before the COVID-19 pandemic. However, many are worried about how the pandemic has affected their education and social connections and may require additional psychological, practical and social support. Our findings highlight the important role that education providers play in providing a source of information and support to adolescents and young adults during times of uncertainty.

IntroductionThe diagnosis, management and services available for mental disorders are of growing concern and controversy in the UK. Transitional care between child and adult services and the interface between primary and secondary/ specialist services is often disjointed and thresholds for referral to Child and Adolescent Mental Health Services are high. Objectives and ApproachRoutinely collected healthcare datasets and data linkage were used to identify patterns of healthcare utilisation by young people and young adults with mental health disorders across the four UK Nations. We explored the extent to which routinely collected datasets can contribute to an assessment of the health needs and the quality of care that children and young people with mental health disorders receive. Data was requested from the national data providers in each country. A series of descriptive analyses were performed and methods were developed for cross- national comparisons to be made (e.g. Four Nation Person Spell). ResultsIt is feasible to explore healthcare utilisation across the four countries of the UK using routine data. However the recording, availability and access varied considerably between countries, making meaningful comparisons challenging. Descriptive analyses showed strong deprivation gradients in the diagnoses and care provided for young people and young adults with mental health disorders. Depression and anxiety were the most commonly recorded mental health conditions in primary care. In secondary care drug/alcohol disorders and self-harm were the most commonly recorded. Re-admissions to emergency departments were higher for those admitted for self-harm or psychiatric conditions. Conclusion/ImplicationsRoutine data has the potential to make a difference to care. However collection and access needs to be standardised in order to improve efficiency and effectiveness in improving the care for children and young people with mental health disorders. MQ has funded an Adolescent Data Platform to facilitate this.

BackgroundMindfulness-based approaches for adults are effective at enhancing mental health, but few controlled trials have evaluated their effectiveness among young people.AimsTo assess the acceptability and efficacy of a schools-based universal mindfulness intervention to enhance mental health and well-being.MethodA total of 522 young people aged 12–16 in 12 secondary schools either participated in the Mindfulness in Schools Programme (intervention) or took part in the usual school curriculum (control).ResultsRates of acceptability were high. Relative to the controls, and after adjusting for baseline imbalances, children who participated in the intervention reported fewer depressive symptoms post-treatment (P = 0.004) and at follow-up (P = 0.005) and lower stress (P = 0.05) and greater well-being (P = 0.05) at follow-up. The degree to which students in the intervention group practised the mindfulness skills was associated with better well-being (P<0.001) and less stress (P = 0.03) at 3-month follow-up.ConclusionsThe findings provide promising evidence of the programme's acceptability and efficacy.

A brief follow on report (from Report 1, see https://psyarxiv.com/uq4rn/). This report presents data on parents and their children's well being as a result of the COVID-19 pandemic from our adult survey study. In addition to presenting additional data showing a potentially significant increase in anxiety and depression in young people aged 13-24, as a consequence of COVID-19. Data collection for our Adult Study (Wave 2) took place between 22nd April and was ended on Friday, May 1st, here we report headline figures for the impact of Covid-19 on parents and their children. We have described our methods in a separate report (https://psyarxiv.com/wxe2n) and released two reports on our mental health outcomes from wave 1 (https://psyarxiv.com/hb6nq, https://psyarxiv.com/ydvc7).

IntroductionResearch indicates that stressors introduced by the COVID-19 pandemic have negatively impacted mental health, particularly among young people.1 Time spent on digital technology (e.g., social media, smartphones) has also increased2 as schools, workplaces, and social gathering sites have closed, thus intensifying pre-pandemic concerns regarding the putative effects of digital technology use (DTU) on mental health. Indeed, recent academic and newspaper articles have both directly and indirectly asserted that increased DTU is a source of the heightened psychological distress observed during the pandemic.3–5 However, these claims are dubious for two primary reasons. First, these articles rely on self-report measures of DTU, which are inaccurate6 and prone to systematic bias.7 Second, since the pandemic has impacted both mental health and DTU for many, the observed association between the two may be attributable to a shared common cause, rather than causality. Thus, we investigated the longitudinal associations between objectively measured DTU and mental health while accounting for important COVID-19-related effects.MethodsThis study was approved by the University of Pittsburgh and followed Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) reporting guidelines. In this four-wave panel study, participants were recruited from Prolific (https://www.prolific.co/), an online participant-recruitment platform. Waves of data collection were launched on August 14, September 12, October 14, and November 9 of 2020. Eligible participants were U.S. residents, 18-35 years old, iPhone users, and had ≥ 10 previous submissions on Prolific with approval rating ≥95%. At each wave, participants uploaded screenshots of their “Screen Time” application (which passively tracks device usage) and completed self-reports of mental health (depression, anxiety, suicidal ideation [SI]), COVID-19-related stressors, and perceived COVID-19-related impact on well-being and DTU (Table 1). We extracted three elements from the “Screen Time” screenshots: (1) total screen time, (2) total time spent on social media, and (3) total number of pickups. We estimated separate random-intercept multilevel models for each mental health outcome using Mplus. Predictors were entered hierarchically in blocks (see eTable 3 in Supplement) to assess ΔR2 at the within- and between-person levels. See Supplement for methodological details. ResultsA total of 384 young adults participated in this study (Mage = 24.5, SDage = 5.1; 57% female; 54% white; 48% Bachelor’s degree education or above). Overall, participants averaged 47.5 hours of Screen Time, 677 pickups, and 15.5 hours of social media over the past week. On average, participants reported experiencing between 4 and 5 pandemic-related stressors per wave. Mean depression and anxiety t-scores were 54.6 and 56.7, respectively, and nearly 29% of participants reported past-week SI at least once. See eTable 1 for summary statistics of sample demographics and primary variables.Results of the multilevel analyses revealed that objectively-measured total screen time and social media use were unrelated to within- or between-person differences in mental health, while between-person difference in pickups was negatively associated with depression (see Figure 1). Together, the objective DTU variables explained, at most, 2.8% of the within- or between-person variance in any of the mental health outcomes (eTable 3 in Supplement). COVID-related impacts on well-being had the largest effects across models—accounting for about 45% and 10%, respectively, of the between- and within-person variance in depression and anxiety, and 21%/28% of the between/within variance in SI. DiscussionAmong a sample of young adults, a population with particularly high rates of DTU8 and COVID-19-related distress,1 we found that objectively-measured DTU did not contribute to increases in depression, anxiety, or SI—refuting the popular notion that increases in DTU may be contributing to young peoples’ psychological distress during the pandemic. Rather, depression, anxiety, and SI were driven mostly by young peoples’ reports of the pandemic’s impact on their well-being. The convenience-based sample, retrospective (past week) assessments of mental health outcomes, and single-item measures of COVID-19-related impacts are limitations of the study. Nevertheless, results indicate that current speculations about the direct harms of DTU on mental health may be unfounded and risk diverting attention from a more likely cause: pandemic-related stressors.

Abstract The mental health of young people is a pressing concern in global development. However, there is little research on how young adults report their own mental health. The interview data gathered in this study (n = 74) explored young adults’ well-being during the transition period from care to independent living under an English local authority and in Finland. Participatory action research methods were employed. The interview schedule included 71 open and closed questions, and was analysed by content and summarised using the SPSS software application and Excel tables. The themes concerning mental health and social relationships were divided into three categories: ‘They have been there for me’, ‘My friends are the only ones’ and ‘They just guided me’. Participants who felt they had supportive social networks also felt their mental well-being and security to be better than those who did not. Overall, the findings demonstrated that good, significant social relations provided a sense of security but did not guarantee a positive mental outlook. Exploring young adults’ own evaluations of their social networks provides social work practitioners with sensitive information with which to find ways for young people to support their mental health in their own terms.

AbstractGambling is a common activity amongst young adults in the UK, and was a behavior of interest during the early mitigation against COVID-19 (first lockdown). The Avon Longitudinal Study of Parents and Children (ALSPAC) was used to investigate attitudes, moods and behavior during lockdown in England. ALSPAC participants were invited to complete online questionnaires in May 2020, including a set of questions about frequency of gambling and gambling activities which had been asked three years previously. Mental health and wellbeing data and alcohol use were also collected as part of lockdown questionnaires. Gambling questions were completed by 2632 young adults, 71% female, with a mean age of 27.8 years. Overall, gambling frequency reduced during lockdown for both males and females, but more males engaged in regular (weekly) gambling. Gambling activities became more restricted compared to previous reports, but online gambling (e.g. online poker, bingo, casino games) was more frequent. Previous gambling behaviour predicted gambling frequency during lockdown. No associations were apparent between gambling frequency and measures of mental health and well-being. Heavy alcohol use was strongly linked with regular gambling during lockdown. Gamblers were more than twice as likely as non-gamblers to have experienced financial difficulties pre-COVID, but gambling frequency was not related to employment status during lockdown. Online gambling increased during lockdown, whilst offline gambling activities decreased in frequency. A small minority of regular weekly gamblers, who tended to be male and heavy users of alcohol, participated in a wide range of online and offline gambling activities.

AbstractObjectiveThis study examined the COVID-19 risk perceptions and mental health of university students on returning to campus in the midst of the COVID-19 pandemic.MethodsAn online survey was completed during the first four weeks of the academic year (October 2020) by 897 university students. The survey included demographics and measures of experiences of COVID-19 testing, self-isolation, shielding, perceived risk, mental health and indices capturing related psychological responses to the pandemic.ResultsWe observed higher levels of depression and anxiety, but not stress, in students compared with pre- pandemic normative data, but lower than levels reported earlier in the pandemic in other similar cohorts. Depression, anxiety and stress were independently associated with greater loneliness and reduced positive mood. Greater worry about COVID-19 was also independently associated with anxiety and stress. Female students and those with pre-existing mental health disorders were at greatest risk of poor mental health outcomes.ConclusionAlthough students perceived themselves at only moderate risk of COVID-19, the prevalence of depression and anxiety among university students should remain a concern. Universities should provide adequate support for students’ mental health during term-time. Interventions to reduced loneliness and worry, and improve mood, may benefit students’ overall mental well-being.
