BACKGROUND Aesthetic dentistry is making its own new position in current scenario. The aesthetic desire of patient has become need of an hour and it is essential for dental practitioner to understand the aesthetics of implants. For planning of implants in anterior region, overall general health of patient should be considered including systemic health, good oral hygiene practice and good compliance of patients. The peri-implant aesthetics is determined by two components - soft tissue and hard tissue, which includes bone and gingiva. Modifications refining the tissues around implant can enhance the aesthetics. Treatment protocol should include pre-planning of all functional and aesthetic considerations which should be discussed to patients as well prior to preceding treatment of implant. Judicious understanding of these factors can not only provide physical integrity of tissue but also prevent future aesthetics and implant failure. Implants in anterior region become challenging for clinicians to emulate what nature has provided. The harmonious relationship between functional stability and peri-implant soft tissue becomes an essential requisite for successful endosseous implant so as to attain proper facial aesthetics. Visualization of aesthetics related to implants is a complex phenomenon which requires utilization of multidisciplinary approach including prosthetic and periodontal consideration as the implant position as well implant abutment junction if placed at improper position can cause bone resorption. Compiling this, comprehensive knowledge regarding soft and hard tissue can help dentists to integrate a balance between the smile and face of an individual. Aim. People with attractive facial appearance are more easily integrated in society and are considered to be more capable, more intelligent and more responsible. The aesthetic aspect is influenced by expression, by facial symmetry, facial contours and teeth colour. In dental restorations, the correct determination of teeth colour is a critical step with extreme importance for successful treatment. From this perspective the practitioner must have experience, must have the ability to distinguish colours and must train through repetitive visual practical exercises that can have an impact on the perception of dental aesthetics. Materials and method. The study followed comparatively the evolution of teeth colour determination and its impact on dental aesthetics perception. It was conducted on a group of fifth year students from the Faculty of Dental Medicine, Bucharest using the Tooth Training Box colour determination exercise that is based on the 3-D Master colour key principle. Results. Using training exercises to determine colour participants developed a better understanding of the physical principles behind the procedure but also a significant increase in accuracy of their measurements and confidence in their colour determinations. Conclusions. Using an aesthetic evaluation protocol that translates teeth colour in a universal language for both practitioners as well as dental technicians has a significant impact in aesthetic dentistry.

Background.Although the survival of children with soft tissue sarcoma (STS) has improved considerably, the outcome of patients with metastatic disease, and those with primary tumours of the extremities or parameningeal sites remains disappointing. We describe the clinical outcome of an ifosfamide-based regimen with local therapy directed only to children who failed to achieve a complete response to initial chemotherapy.Patients and Methods.Twenty-one children with STS (16 rhabdomyosarcoma) who presented with unresectable tumours were treated with five courses of ifosfamide (9 g/m2) and etoposide (600 mg/m2). Patients who did not achieve a complete response then received local therapy. Chemotherapy with ifosfamide combined with etoposide, vincristine (1.5 mg/m2and doxorubicin (60 mg/m2) or vincristine and actinomycin D (1.5 mg/m2) was continued for one year.Results and Discussion.Objective responses to five courses of ifosfamide and etoposide were seen in all patients. Disease free survival (DFS) at a median follow up of 59 months was 57% (95% CI 29–75%). The DFS of children who received local therapy was 89% compared with 33% in those who received chemotherapy alone (p=0.027). Locoregional recurrences did not occur in children who received radiotherapy to the site of the primary tumour. Ifosfamide-based chemotherapy does not reduce the incidence of loco-regional recurrence in children who do not receive local therapy.

ABSTRACT Soft tissue laser, such as diode and Nd:YAG lasers were initially used in soft tissue lesions because of its increased success rate. It was because of the fact that these lasers were well-absorbed by chromophores, such as hemoglobin and melanin which are found abundant in the oral mucosa. The introduction of erbium family in 1990 comprising the Er:YAG and Er,Cr:YSGG lasers made the hard tissue laser a boon for dentistry. Erbium, chromium-doped yttrium, scandium, gallium and garnet (Er,Cr:YSGG) was introduced in 1997 for the surgical needs of clinical dentistry in general practice. The erbium belongs to the rare earth which is embedded in a host crystal. The actual lasing process takes place in the Er ion Er3+. Two host crystals consisting of yttrium, aluminum, and garnet (Y3A5O12) and yttrium, scandium, gallium, and garnet (Y3Sc2Ga3O12) are added to the erbium. The interest to use these hard tissue laser in the treatment of soft tissue lesion was because of the properties of these lasers which are well-absorbed by chromophore water apart from hydroxy appetite crystals. Erbium laser energy is absorbed by collagen, hydroxyapatite, and water components. It allows the laser to cut soft tissue, tooth structure, and bone. In the noncontact mode, the incision is scalpel-like, with very little hemostasis. In contact mode, it performs soft tissue sculpting with adequate hemostasis. The Er,Cr:YSGG is the world's most advanced dental laser, which is ideal all-tissue laser because all dental tissues contain water, for the multidisciplinary dentist who performs a broad spectrum of procedures. It delivers the highest level of clinician control, operating efficiency, flexibility in tip, and accessory selection. For optimal clinical results and patient comfort in hard and soft tissue procedures, the erbium lasers have set a new standard of clinical performance. The present case series aims to highlight the various soft tissue applications of Er,Cr:YSGG (Waterlase Biolase®, Biolase, Inc, San Clemente, California, USA) in pediatric patients. Introduction: In past few years, there is huge improvement in orthodontics. Different innovation in orthodontics made the procedure easier and less time consuming. Application of laser is one of them. Many types of dental lasers are currently available that can be efficiently used for soft and hard tissue applications in the field of orthodontics. Two types of lasers are there. One is hard tissue laser, and another is soft tissue laser. Laser therapy is advantageous because it often avoids bleeding, can be pain free, is non‑invasive and is relatively quick. The high cost is its primary disadvantage. The purpose of this article is to provide an overview regarding safe and proper use of soft-tissue lasers in orthodontics

Introdução: A aparência estética é uma das preocupações do ser humano, o que leva a uma busca constante por uma estética que expresse harmonia, naturalidade e expressividade. Sendo assim, os procedimentos restauradores têm contribuído positivamente para a resolução dos desafios odontológicos estéticos. Objetivo e relato de caso: Relatar um caso clínico com associação de clareamento dental, confecção de pino intrarradicular e coroa indireta em dissilicato de lítio em que foi devolvida ao paciente a estética desejada. Pacientede 23 anos de idade fraturou o incisivo central superior direito na infância; insatisfeito com a instabilidade de cor do dente, buscou novas alternativas de tratamento. Conclusão: A associação de diferentes tratamentos na Odontologia estética pode apresentar resultados satisfatórios para o restabelecimento estético e funcional de dentes anteriores.Introduction: The aesthetic appearance is one of the human being concerns, leading to constant search for an aesthetic that expresses harmony, naturalness and expressiveness. So, the restorative procedures have contributed positively to overcome the aesthetic dentistry challenges. Objective and case report: Report a clinical case in association to dental whitening, making of intraradicular pin and indirect crown of lithium disilicate when it was returned to the patient the desired aesthetic. A 23 years old patient fractured the right central incisor in the childhood, dissatisfied with the tooth color instability, he looked for new treatment alternatives. Conclusion: The association of different treatments in the aesthetic dentistry might present satisfactory results to the aesthetic and functional reestablishment of anterior teeth.
