O cenário adverso da saúde no país, agravado pela fragilização do Sistema Único de Saúde (SUS), que, a despeito de tal contexto, tenta fazer frente à maior tragédia sanitária de nossa história recente, tem neste documento uma posição para reflexão da Rede de Pesquisa em Atenção Primária à Saúde (APS) da ABRASCO, acerca da Eficiência e Gestão Pública no contexto da APS.  Estas notas abordam o contexto da APS e o agravamento com a pandemia da COVID-19, reforçando a importância do debate sobre subfinanciamento com recursos públicos e visa a potencializar os objetivos e valores sociais que perpassam a APS universal e de qualidade. Abordam também a compreensão do conceito de eficiência e sua adoção no contexto da APS, com foco na geração de serviços e cuidados com os recursos humanos, materiais, físicos e financeiros disponíveis às entregas e resultados decorrentes.  Nessa perspectiva, será abordada a premissa de que mensurar a eficiência é uma maneira capaz de estimular resultados ótimos com os recursos disponíveis.  No caso da APS, são mensuráveis os efeitos e impactos sobre os demais níveis de atenção. Ainda, a discussão sobre a Gestão Pública no contexto da APS é realizada considerando os diferentes formatos possíveis e seus efeitos à luz dos preceitos preconizados para uma saúde pública e universal. Por último, estas notas apresentam uma posição para o debate na Rede APS. Palavras-chave: APS, Eficiência, Gestão Pública   Abstract The adverse health scenario in the country, aggravated by the weakening of the Unified Health System (SUS), which, despite such a context, tries to face the greatest health tragedy in our recent history, has in this document a position for reflection by the Research Network in Primary Health Care (PHC) of ABRASCO, about Efficiency and Public Management in the context of PHC. These notes address the context of PHC and the worsening with the COVID-19 pandemic, reinforcing the importance of the debate on underfunding with public resources and aiming to enhance the social goals and values ​​that permeate universal and quality PHC. They also address the understanding of the concept of efficiency and its adoption in the context of PHC, focusing on the generation of services and care with human, material, physical and financial resources available for deliveries and resulting results. From this perspective, the premise that measuring efficiency is a way capable of stimulating optimal results with available resources will be addressed. In the case of PHC, the effects and impacts on other levels of care are measurable. Still, the discussion on Public Management in the context of PHC is carried out considering the different possible formats and their effects in light of the precepts advocated for universal and public health. Finally, these notes present a position for debate in the Rede APS. Keywords: PHC, efficiency, public management.

RESUMO Objetivo: analisar os reflexos do processo do planejamento na assistência prestada em duas unidades da atenção básica de saúde. Método: trata-se de um estudo qualitativo, descritivo, tipo pesquisa de campo, tendo, como cenários, uma Unidade Básica de Saúde e uma Policlínica. Coletaram-se os dados em 2017, utilizando-se, como instrumento, um roteiro de entrevista semiestruturado, com quatro enfermeiros. Apresentaram-se os resultados por meio da Análise de Conteúdo. Resultados: evidenciou-se a categoria “Os reflexos do processo do planejamento na assistência prestada na atenção básica em saúde”.  Conclusão: demonstrou-se o conhecimento, por parte dos enfermeiros, sobre o planejamento ao analisar os discursos, refletindo a preocupação do envolvimento de todos para alcançar as metas institucionais. Possibilitou-se, ao ser empregado pelos enfermeiros, refletir sobre a importância da sua utilização para otimizar os serviços de Enfermagem e das instituições onde estes profissionais estão inseridos, porém, a existência de fragilidades, tais como a falta de recursos humanos, materiais, além da falta de reconhecimento de seu trabalho, dificultam, algumas vezes, o alcance dos objetivos. Method: this is a qualitative study, descriptive, field-research type, whose scenarios were a Basic Health Unit and a Polyclinic. Data were collected in 2017, using as instrument a semistructured interviews with four nurses. The results were presented by means of content analysis. Results: the category “Consequences of the planning process in the assistance provided in primary health care” emerged. Conclusion: the nurses demonstrated knowledge about planning when analyzing their speeches, reflecting the concern about the involvement of everyone to achieve the institutional goals. When used by nurses, it allowed reflecting on the importance of its use to optimize the nursing services and institutions where these professionals are inserted, however, the existence of weaknesses, such as the lack of human, material resources, in addition to the lack of recognition of their work, sometimes hinder achieving the objectives.Descriptors: Nurses; Planning; Health Services; Primary Health Care; Administration and Organization; Health Facilities.RESUMEN Objetivo: analizar los efectos del proceso de planificación en la asistencia que se presta en dos unidades de atención básica de la salud. Los datos fueron recolectados en el 2017, usando como instrumento una entrevista semiestructurada con cuatro enfermeras. Los resultados fueron presentados por medio de análisis de contenido. Resultados: se evidenció la categoría “Reflexiones del proceso de planificación en la asistencia prestada en la atención primaria de la salud”. Conclusión: Se demostró el conocimiento, por parte de enfermeras, acerca de la planificación cuando se analizaron los discursos, reflejando la preocupación de la participación de todos para lograr los objetivos institucionales. Se utilizado por las enfermeras, reflexiona sobre la importancia de su uso para optimizar los servicios de enfermería e instituciones donde estos profesionales se insertan, sin embargo, la existencia de deficiencias, como la falta de recursos humanos, materiales, además de la falta de reconocimiento de su trabajo, hacen difícil, a veces, lograr los objetivos. The aim of the study was to describe the Programa Academia da Cidade do Recife (PAC-REC) action plan during the social isolation requirement due to the COVID-19 pandemic. As temporary measures to confront the COVID-19, the City of Recife has published a contingency  plan, which suspended the non-essential services, including the presential operations of PAC-REC. Considering the program's commitment to the Unified Health System and to the Primary Health Care the PAC's board committee developed an action plan, restructuring its potential through the reorganization of remote activities to be offered to their users. The action plan has six stages encompassing planning (first and second), development (third and fourth), monitoring and evaluation (fifth and sixth). Despite the changes in the epidemiological scenario and the format of actions, the PAC-REC is maintaining its Health Promotion actions throughout its territory.

OBJECTIVE: To identify limiting factors in the management of pharmaceutical services in the primary health care provided by the Brazilian Unified Health System (SUS). METHODS: This study was based on the data from the Pesquisa Nacional sobre Acesso, Utilização e Promoção do Uso Racional de Medicamentos no Brasil (PNAUM – National Survey on Access, Use and Promotion of Rational Use of Medicines), and it was conducted by interviews with professionals responsible for pharmaceutical services in Brazilian cities, in 2015. To identify the management limiting factors, we considered the organizational, operational, and sustainability indicators of the management. For the analyses, we included the weights and structure of analysis plan for complex samples. The results were expressed by frequencies and measures of central tendency with 95% confidence interval, considering the Brazilian geographic regions. RESULTS: We identified the following limiting factors: lack of pharmaceutical services in the Municipal Health Secretariat organization chart (24%) and in the health plan (18%); lack of participation of managers in the Health Board and the absence of reference to this topic in the agenda of meetings (58.4%); lack of financial autonomy (61.5%) and lack of knowledge on the available values (81.7%); lack of adoption of operational procedures (about 50%) for selection, scheduling, and acquisition; and the fact that most professionals evaluate the organization of pharmaceutical services as good and great (58.8%), despite the worrisome indicators. CONCLUSIONS: Pharmaceutical services management is currently supported by a legal and political framework that should guide and contribute to improve the pharmaceutical services in the Brazilian Unified Health System primary health care. However, there is a mismatch between the goals established by these guidelines and what is actually happening

OBJECTIVE: To evaluate the access to medicines in primary health care of the Brazilian Unified Health System (SUS), from the patients’ perspective. METHODS: This is a cross-sectional study that used data from the Pesquisa Nacional sobre Acesso, Utilização e Promoção do Uso Racional de Medicamentos – Services, 2015 (PNAUM – National Survey on Access, Use and Promotion of Rational Use of Medicines), conducted by interviews with 8,591 patients in cities of the five regions of Brazil. Evaluation of access to medicines used concepts proposed by Penshansky and Thomas (1981), according to the dimensions: availability, accessibility, accommodation, acceptability, and affordability. Each dimension was evaluated by its own indicators. RESULTS: For the “availability” dimension, 59.8% of patients reported having full access to medicines, without significant difference between regions. For “accessibility,” 60% of patients declared that the basic health unit (UBS) was not far from their house, 83% said it was very easy/easy to get to the UBS, and most patients reported that they go walking (64.5%). For “accommodation,” UBS was evaluated as very good/good for the items “comfort” (74.2%) and “cleanliness” (90.9%), and 70.8% of patients reported that they do not wait to receive their medicines, although the average waiting time was 32.9 minutes. For “acceptability,” 93.1% of patients reported to be served with respect and courtesy by the staff of the dispensing units and 90.5% declared that the units’ service was very good/good. For “affordability,” 13% of patients reported not being able to buy something important to cover expenses with health problems, and 41.8% of participants pointed out the expense with medicines. CONCLUSIONS: Results show 70%–90% compliance, which is compatible with developed countries. However, access to medicines remains a challenge, because it is still heavily compromised by the low availability of essential medicines in public health units, showing that it does not occur universally, equally, and decisively to the population
