The provision of nutrition care process (NCP) in diabetes mellitus (DM) patients is very important in determining the patient's diet to control blood glucose and to prevent complications. This study aimed to determine the diff erences in levels of intake and blood glucose levels before and after the implementation of NCP for type 2 DM (T2DM) inpatients of RSUD Dr. Saiful Anwar Malang. The design of this study was cross sectional, using secondary data sources from 32 patient medical records consisting of data on age, gender, nutritional status, complications of disease, nutritional diagnosis, nutritional intervention, energy intake, and blood glucose level. The analysis test used was the T-test dependent test on the normal data distribution and the Wilxocon test on the abnormal data distribution. The results of this study indicate a diff erence in the level of after and before intake (p = 0.020) with an increase in the average intake of 65.75 ± 18.23% to 75.50 ± 17.69% of the total energy needs. The analysis of blood glucose before and after showed p = 0.023, which means that there were diff erences in blood glucose before and after the NCP implementation. Blood glucose results showed an average decrease of 205 ± 93.85 mg/dl to 155.9 ± 50.53 mg/dl. The results of this study showed that there were diff erences in levels of energy intake and blood glucose levels before and after the provision of NCP by dietitians/nutritionists.

Diabetes Mellitus is a group of heterogeneous disorders characterized by elevated blood glucose levels or hyperglycemia. Counseling to people with Diabetes Mellitus about nutrition and healthy lifestyle is a method to raise awareness of Diabetes Mellitus patients to change food intake so as to improve blood glucose levels. The purpose of this study is to determine the effect of nutrition counseling on nutrient intake and blood glucose levels in patients with type 2 Diabetes Mellitus in the work area of ​​Motoling Puskesmas. The research type is pre experiment design with one group pretest and posttest approach. The population in this study were 55 outpatient Diabetes Mellitus patients at Motoling Puskesmas, a sample of 23 outpatient Diabetes Mellitus Type 2 patients at Motoling Puskesmas. Nutrition counseling using media leaflets is given four times a month. Data Food intake was taken by using food recall 24 hours, data of glucose level of patient was taken by doing autocheck blood sugar test and for characteristic data of respondent using questioner. To determine the difference of glucose level with carbohydrate and fat intake before and after nutritional counseling was analyzed by using paired test of T test and Wilcoxon test. The results showed that there was a significant difference between carbohydrate intake before and after nutritional counseling (p = 0.002 <α 0.05). Average carbohydrate intake before counseling 359.52 g and after counseling 290.09 g a decrease of 69.43 g. For fat intake showed no significant difference in carbohydrate intake before and after nutritional counseling (p = 0.009 <α 0.05). Average fat intake before counseling 128.61 g and after counseling 75.04 g decreased 53.56 g. While blood glucose levels showed no significant difference in carbohydrate intake before and after nutritional counseling (p = 0.000 <α 0.05). Mean blood glucose levels before counseling 255 mg / dl to 202.39 mg / dl decreased 52.60 mg / dl. Conclusion, there is a significant difference between nutrient intake and blood glucose levels in patients with Diabetes Mellitus type 2 in the work area of Motoling Puskesmas.

AbstrakDiabetes Melitus (DM) adalah penyakit metabolik kronik yang ditandai dengan meningkatnya kadar glukosa darah. Kepatuhan pengobatan yang rendah terhadap terapi pengobatan pada pasien DM tipe 2 dapat mengakibatkan peningkatan resiko biaya pengobatan serta peningkatan komplikasi penyakit. Salah satu cara untuk meningkatkan kepatuhan terhadap pengobatan dapat dilakukan dengan pemberian edukasi. Edukasi dapat dilakukan dengan berbagai cara salah satunya dengan pemberian booklet. Penelitian ini bertujuan untuk mengetahui tingkat kepatuhan minum obat pasien DM tipe 2 sebelum dan setelah pemberian booklet dilihat dari kadar Glukosa Darah Sewaktu (GDS) dan skor Morisky Medication Adherence Scale-8 (MMAS-8) dan mengetahui hubungan antara kadar GDS dan skor MMAS-8 terhadap tingkat kepatuhan minum obat pasien DM tipe 2 di Puskesmas Wua-Wua Kota Kendari Tahun 2018. Jenis penelitian ini adalah penelitian quasi eksperimental dengan sampel sebanyak 40 pasien DM tipe 2 yang dibagi menjadi 2 kelompok yaitu 20 pasien kelompok kontrol dan 20 pasien kelompok intervensi. Kadar glukosa darah sewaktu dan skor MMAS-8 diukur sebelum dan setelah empat minggu pemberian intervensi. Hasil penelitian menunjukkan bahwa terdapat perbedaan tingkat kepatuhan minum obat pasien DM tipe 2 di Puskesmas Wua-Wua Kota Kendari sebelum dan setelah pemberian booklet. Berdasarkan kadar GDS, kelompok kontrol sebelum dan setelah pemberian booklet yaitu 296,2 mg/dL turun menjadi 261,85 mg/dL, sedangkan kelompok intervensi yaitu 275,7 mg/dL turun menjadi 198,1 mg/dL. Berdasarkan skor MMAS-8 pada kelompok kontrol sebelum dan setelah pemberian booklet yaitu 3,4 menjadi 2,35 sedangkan kelompok intervensi yaitu 4,45 menjadi 0,8. Hasil uji korelasi Spearman menunjukkan bahwa terdapat hubungan yang kuat (r=0,783) antara kadar glukosa darah sewaktu dengan skor MMAS-8 pada pasien DM tipe 2 dalam menggambarkan tingkat kepatuhan pasien dengan taraf kepercayaan p=0,000 (p<0,05), dimana nilai p menunjukkan bahwa edukasi DM melalui pemberian booklet efektif membantu meningkatkan kepatuhan pasien yang dapat dilihat dari skor MMAS-8 rendah dan kadar GDS yang rendah.Kata kunci: DM Tipe 2,  MMAS-8, Booklet, Kepatuhan AbstractDiabetes mellitus (DM) is chronic metabolic disease characterized by increased blood glucose levels. Low medication compliance for treatment therapy in patients with type 2 diabetes can result in increased risk of treatment costs and increased disease complications. One way to improve adherence of treatment can be done by providing education. Education can be done in various ways one of them is by giving booklets. This study aims to determine the level of compliance for taking medication of patients with type 2 DM before and after the administration of the booklet seen from the level of Blood Glucose While (GDS) and Morisky Medication Adherence Scale-8 (MMAS-8) scores and determine the relationship between GDS levels and MMAS-scores 8 to the level of compliance for taking medication of patients with type 2 DM in Wua-Wua Health Center, Kendari City in 2018. This research is quasi-experimental study type with 40 patients with type 2 DM divided into 2 groups: 20 control group and 20 intervention group. Blood glucose levels and MMAS-8 scores were measured before and after four weeks of intervention. The results showed that there were differences in the level of compliance for taking medication of type 2 DM patients at Wua-Wua Health Center in Kendari City before and after the booklet was given. Based on the level of GDS, the control group before and after the administration of the booklet was 296.2 mg / dL down to 261.85 mg / dL, while the intervention group that was 275.7 mg / dL fell to 198.1 mg / dL. Based on MMAS-8 scores in the control group before and after the administration of the booklet, it was 3.4 to 2.35 while the intervention group was 4.45 to 0.8. The Spearman correlation test results showed that there was strong relationship (r = 0.783) between the blood glucose levels when with MMAS-8 scores in type 2 DM patients in describing the level of compliance of patients with level p = 0,000 (p <0.05), p-value indicates that DM education through effective booklet administration trust helps improve patient compliance which can be seen from low MMAS-8 scores and low GDS levels.Keywords: DM Type 2, MMAS-8, Booklet, Compliance

The prevention of Diabetic Mellitus (DM) and its complications is the main aim of this study, in addition to the training of lotion foot care application and the development of small scale industry. The research team delivered knowledge in the form of training on Diabetic Mellitus, healthy food, treatment and prevention of complications, and small-scale production of cosmetic products. The aim of this study was to determine the correlation between training on diabetic and lotion foot care application as preventive measures against diabetic complications on the patient's blood glucose levels in the community of residents in Banyuurip Jaya, Surabaya. It was expected from this training that the knowledge of the residents increases and people living with diabetic undergo lifestyle changes and therefore blood sugar levels can be controlled. The parameters measured in this research were blood glucose levels, the anti diabetic drug types consumed, and compliance on diabetics. This study used the data taken from 60 patients with DM over a period of one month. Questionnaires and log books was used to retrieve data and changes in blood glucose levels in diabetic patients. The results showed the demographic data of patients with type 2 diabetic of 85% female and 15% male, with the range of patients aged of 61-70 years of 46.67% and had history of diabetic (90%). The history of drugs consumed by respondents was anti diabetic drugs such as metformin (40%), glimepiride (33.37%) and insulin (6.67%). In addition, the increased knowledge of DM patients after being given the training compared to before training was shown in several questions in the questionnaire. A statistical analysis using t-test analyzed a correlation between training provided in order to enhance understanding of the patient, as well as correlation with blood glucose levels. A paired T-test showed that there was a relationship between the knowledge of trainees before and after training (p less than 0.05). An interesting result was that there was no relationship between blood glucose levels before and after training provided (p> 0.05).

O trabalho tem como objetivo analisar os estudos que avaliaram os efeitos dos programas de treinamento aeróbio, força e combinado nos níveis de glicose sanguínea em indivíduos com diabetes do tipo 2. Foi utilizado o método de revisão sistemática, sendo utilizada a base de dados PubMed. As palavras chaves utilizadas para pesquisa foram training and diabetes. Foram identificados 484 artigos originais. Apenas 17 estudos respeitaram os critérios de inclusão. Os resultados evidenciam que os programas de treinamento aeróbio diminuíram os níveis de glicose. O programa de treinamento de força também foi favorável à diminuição dos níveis de glicose sanguínea. Já o programa de treinamento combinado não demonstrou efeitos favoráveis no controle da glicose sanguínea. Conclui-se que o programa de treinamento aeróbio e de força ajudam a controlar os níveis de glicose sanguínea em indivíduos com diabetes do tipo 2. Palavras-chave: diabetes mellitus, treinamento, glicose.ABSTRACTThe study aims to analyze the studies that evaluated the effects of aerobic, strength and combined programs training in blood glucose levels in people with type 2 diabetes. We used a systematic review method and is used to PubMed database. The key words used for searching were training and diabetes. We identified 484 original articles. Only 17 studies complied with the inclusion criteria. The results show that aerobic training programs decreased glucose levels. The strength training program was also favorable to decrease in blood glucose levels. But the combined training program has not shown favorable effects on blood glucose control. We conclude that the aerobic training and strength helps control blood glucose levels in individuals with type 2 diabetes. Keywords: diabetes mellitus, training, glucose.

Abstract Background Ethnobotany knowledge in a community has shaped local wisdom in utilizing plants to treat diseases, such as the use of Malaka (Phyllanthus emblica) flesh to treat type 2 diabetes. This study presented evidence that the phenolic extract of the Malaka flesh could reduce blood sugar levels in the diabetic induced rats. Methods The phenolic extract of the P. emblica was administrated to the glucose-induced rats of the Wistar strain Rattus norvegicus for 14 days of treatment where the Metformin was used as a positive control. The data generated were analyzed by the two-way ANOVA Software related to the blood glucose level and by SAS Software related to the histopathological studies at a significant 95% confidence. Results The phenolic extract with concentrations of 100 and 200 mg/kg body weight could reduce blood glucose levels in diabetic rats. The post hoc Dunnet test showed that the administration of the extract to the rats with a concentration of 100 mg/kg body weight demonstrated a very significant decrease in blood glucose levels and repaired damaged cells better than administering the extract at a concentration of 200 mg/kg weight body. Conclusion The evidence indicated that the phenolic extract of the Malaka flesh can be utilized as anti type 2 Diabetes mellitus without damaging other organs.

BACKGROUND The use of remote data capture for monitoring blood glucose and supporting digital apps is becoming the norm in diabetes care. One common goal of such apps is to increase user awareness and engagement with their day-to-day health-related behaviors (digital engagement) in order to improve diabetes outcomes. However, we lack a deep understanding of the complicated association between digital engagement and diabetes outcomes. OBJECTIVE This study investigated the association between digital engagement (operationalized as tagging of behaviors alongside glucose measurements) and the monthly average blood glucose level in persons with type 2 diabetes during the first year of managing their diabetes with a digital chronic disease management platform. We hypothesize that during the first 6 months, blood glucose levels will drop faster and further in patients with increased digital engagement and that difference in outcomes will persist for the remainder of the year. Finally, we hypothesize that disaggregated between- and within-person variabilities in digital engagement will predict individual-level changes in blood glucose levels. METHODS This retrospective real-world analysis followed 998 people with type 2 diabetes who regularly tracked their blood glucose levels with the Dario digital therapeutics platform for chronic diseases. Subjects included “nontaggers” (users who rarely or never used app features to notice and track mealtime, food, exercise, mood, and location, n=585) and “taggers” (users who used these features, n=413) representing increased digital engagement. Within- and between-person variabilities in tagging behavior were disaggregated to reveal the association between tagging behavior and blood glucose levels. The associations between an individual’s tagging behavior in a given month and the monthly average blood glucose level in the following month were analyzed for quasicausal effects. A generalized mixed piecewise statistical framework was applied throughout. RESULTS Analysis revealed significant improvement in the monthly average blood glucose level during the first 6 months (<i>t</i>=−10.01, <i>P</i>&lt;.001), which was maintained during the following 6 months (<i>t</i>=−1.54, <i>P</i>=.12). Moreover, taggers demonstrated a significantly steeper improvement in the initial period relative to nontaggers (<i>t</i>=2.15, <i>P</i>=.03). Additional findings included a within-user quasicausal nonlinear link between tagging behavior and glucose control improvement with a 1-month lag. More specifically, increased tagging behavior in any given month resulted in a 43% improvement in glucose levels in the next month up to a person-specific average in tagging intensity (<i>t</i>=−11.02, <i>P</i>&lt;.001). Above that within-person mean level of digital engagement, glucose levels remained stable but did not show additional improvement with increased tagging (<i>t</i>=0.82, <i>P</i>=.41). When assessed alongside within-person effects, between-person changes in tagging behavior were not associated with changes in monthly average glucose levels (<i>t</i>=1.30, <i>P</i>=.20). CONCLUSIONS This study sheds light on the source of the association between user engagement with a diabetes tracking app and the clinical condition, highlighting the importance of within-person changes versus between-person differences. Our findings underscore the need for and provide a basis for a personalized approach to digital health.

Insulin secretion is impaired in type 2 diabetes (T2D). The insulin and glucose responses to central autonomic activation induced by excitation of brain medullary TRH receptors were studied in T2D Goto-Kakizaki (GK) rats. Blood glucose levels in normally fed, pentobarbital-anesthetized GK and nondiabetic Wistar rats were 193 and 119 mg/100 ml in males and 214 and 131 mg/100 ml in females. Intracisternal injection (ic) of the stable TRH analog RX 77368 (10 ng) induced significantly higher insulin response in both genders of overnight-fasted GK rats compared with Wistar rats and slightly increased blood glucose in female Wistar rats but significantly decreased it from 193 to 145 mg/100 ml in female GK rats. RX 77368 (50 ng) ic induced markedly greater glucose and relatively weaker insulin responses in male GK rats than Wistar rats. Bilateral vagotomy blocked ic RX 77368-induced insulin secretion, whereas adrenalectomy abolished its hyperglycemic effect. In adrenalectomized male GK but not Wistar rats, ic RX 77368 (50 ng) dramatically increased serum insulin levels by 6.5-fold and decreased blood glucose levels from 154 to 98 mg/100 ml; these changes were prevented by vagotomy. GK rats had higher basal pancreatic insulin II mRNA levels but a lower response to ic RX 77368 (50 ng) compared with Wistar rats. These results indicate that central-vagal activation-induced insulin secretion is susceptible in T2D GK rats. However, the dominant sympathetic-adrenal response to medullary TRH plays a suppressing role on vagal-mediated insulin secretion. This unbalanced vago-sympathetic activation by medullary TRH may contribute to the impaired insulin secretion in T2D.
