Evaluating the quality of YouTube videos on fibromyalgia (Preprint)

BACKGROUND YouTube is a popular American video-sharing platform that has been accessible to the public since 2005. Previous studies have shown that YouTube is potentially beneficial to medical education, but the quality of videos still need to be determined. OBJECTIVE The goal of this study is to understand the quality of the YouTube videos by evaluating the characteristics of physicians and the total number of views on videos regarding fibromyalgia. METHODS The term “fibromyalgia” was searched on the YouTube search engine by relevance, the default setting. Information from the first 100 videos were analyzed. A search was performed on Scopus to determine the h-index and fibromyalgia-related publication for any physician who was featured in the videos. RESULTS Of the top 100 videos, there were 64 academic videos, 18 vlogs, 5 interview videos, and 13 miscellaneous videos. Out of the 64 academic videos, 30 physicians, 7 Doctors of Philosophy (Ph.D.), 5 physical therapists, and 5 chiropractors were identified. The majority physicians have an adequate academic affiliation such as h-index and academic publications. CONCLUSIONS Residents and medical students will encounter a large number of academic videos on fibromyalgia on YouTube. This study suggests that many videos were posted for academic purposes and that the quality of the videos can be ensured to some degree. However, developing a better systemic evaluation of the quality of YouTube content is still necessary.

OBJECTIVE This study identifies physicians associated with the production of educational YouTube videos about shingles and analyzes those physicians based on their research background or academic affiliation, which may be good surrogates for video content quality. METHODS Using the YouTube search engine with default settings, the term “shingles” was searched on May 8, 2020. A cross-sectional study was performed using the first 50 search results. A search on Scopus for each identified physician was performed, and data regarding their research background and academic affiliation were recorded. RESULTS Of the 50 YouTube videos, 35 (70%) were categorized as academic. Of the 35 academic videos, 24 (71%) videos featured physicians, totaling 25 physicians overall. Out of these 25 physicians, 5 (20%) had at least 1 shingles-related publication and 8 (32%) had an h-index >10. A total of 21 (84%) physicians held an academic affiliation. CONCLUSIONS These results ensure to a certain degree the quality of the content in academic videos on YouTube for medical education. However, further evaluation is needed for this growing platform.

Abstract Objective: As the COVID-19 vaccine is introduced, it is critical to recognize that public opinion on vaccines is largely influenced by health communications, with YouTube being a major source of information and misinformation. This analysis graded the accuracy, quality, and reliability of the most viewed YouTube videos depicting COVID-19 and vaccinations over a six-month period. Methods: We collected hyperlinks for the 150 most viewed YouTube videos discussing COVID-19 from January through June 2020. Closed captioning data was searched for the term “vaccine,” yielding 32 videos. This sample was evaluated for quality, accuracy, and reliability using a rubric that incorporated existing instruments: Global Quality Scale (GQS), JAMA Benchmark Criteria, and DISCERN. Results: These 32 videos had 139,764,188 views at the time of data collection. The majority of videos received low scores, with network news sources receiving the lowest scores overall. Conclusions: The overall quality of COVID-19 YouTube videos related to vaccines may be low and raises a precautionary alert for the public consuming these videos and for healthcare providers working to provide the best information to their patients. Existing scoring tools may not capture the complexities of social media. New tools could allow for a better understanding of the modern landscape of health communications.

BACKGROUND YouTube (Alphabet Incorporated) has become the most popular video-sharing platform in the world. The Fukushima Daiichi Nuclear Power Plant (FDNPP) disaster resulted in public anxiety toward nuclear power and radiation worldwide. YouTube is an important source of information about the FDNPP disaster for the world. OBJECTIVE This study's objectives were to examine the characteristics of YouTube videos related to the FDNPP disaster, analyze the content and comments of videos with a quantitative method, and determine which features contribute to making a video popular with audiences. This study is the first to examine FDNPP disaster–related videos on YouTube. METHODS We searched for the term “Fukushima nuclear disaster” on YouTube on November 2, 2019. The first 60 eligible videos in the relevance, upload date, view count, and rating categories were recorded. Videos that were irrelevant, were non-English, had inappropriate words, were machine synthesized, and were &lt;3 minutes long were excluded. In total, 111 videos met the inclusion criteria. Parameters of the videos, including the number of subscribers, length, the number of days since the video was uploaded, region, video popularity (views, views/day, likes, likes/day, dislikes, dislikes/day, comments, comments/day), the tone of the videos, the top ten comments, affiliation, whether Japanese people participated in the video, whether the video recorder visited Fukushima, whether the video contained theoretical knowledge, and whether the video contained information about the recent situation in Fukushima, were recorded. By using criteria for content and technical design, two evaluators scored videos and grouped them into the useful (score: 11-14), slightly useful (score: 6-10), and useless (score: 0-5) video categories. RESULTS Of the 111 videos, 43 (38.7%) videos were useful, 43 (38.7%) were slightly useful, and 25 (22.5%) were useless. Useful videos had good visual and aural effects, provided vivid information on the Fukushima disaster, and had a mean score of 12 (SD 0.9). Useful videos had more views per day (<i>P</i>&lt;.001), likes per day (<i>P</i>&lt;.001), and comments per day (<i>P</i>=.02) than useless and slightly useful videos. The popularity of videos had a significant correlation with clear sounds (likes/day: <i>P</i>=.001; comments/day: <i>P</i>=.02), vivid information (likes/day: <i>P</i>&lt;.001; comments/day: <i>P</i>=.007), understanding content (likes/day: <i>P</i>=.001; comments/day: <i>P</i>=.04). There was no significant difference in likes per day (<i>P</i>=.72) and comments per day (<i>P</i>=.11) between negative and neutral- and mixed-tone videos. Videos about the recent situation in Fukushima had more likes and comments per day. Video recorders who personally visited Fukushima Prefecture had more subscribers and received more views and likes. CONCLUSIONS The possible features that made videos popular to the public included video quality, videos made in Fukushima, and information on the recent situation in Fukushima. During risk communication on new forms of media, health institutes should increase publicity and be more approachable to resonate with international audiences.

Content produced by caregivers of stroke survivors on the online video-sharing platform YouTube may be a good source of knowledge regarding caregivers’ unmet needs. We aimed to examine the content, quantity and quality of YouTube videos that target and discuss the needs and concerns of caregivers of stroke survivors. YouTube was systematically searched using six search strings, and the first 20 videos retrieved from each search were screened against the inclusion criteria. A pre-determined coding schedule was used to report the rate of unmet needs in each video. Twenty-six videos were included in the analysis. In total, 291 unmet needs were reported by caregivers of stroke survivors, an average of 11.2 unmet needs per video. The most common unmet needs domain was ‘Impact of Caregiving on Daily Activities’ (44%). Most videos were developed in the United States (61.5%) and featured spouses of stroke survivors (65.47%). Content produced by caregivers of stroke survivors on YouTube may be used as a tool for caregivers to provide and receive support through online communication. YouTube videos offer insight into the unmet needs of caregivers of stroke survivors and may be used as an additional resource for stroke services to support caregivers.

I read with great interest Dr. Rothman's manuscript, "The Quality of Medical Care," published in your section of Pediatrics in February, 1952. I am an admirer of Dr. Rothman, and his discussion of medical education was certainly a scholarly one. I would, however, like to present a difference of opinion regarding some statements in one paragraph of his manuscript. In this paragraph, he decries the fact that internes have so much time off and are getting married and having families while still in training. He infers that medical education has suffered by this change. I would like to quote some of this paragraph: ". . . at the turn of the century, the house staff had little or no time off and very little or no financial remuneration. Living quarters were provided within the hospital; matrimony was postponed . . . towards the end of the decade preceding the second world war, it became customary in the majority of the hospitals for the house staff to have every other night off, and every other week end off. Coincident with this change, the yearly stipend increased, and what with the war and some unknown factor, marriage became the rule rather than the exception.
