Personalized approach to the administration of haloperidol in patients with acute alcoholic hallucinatie (literature review)

To date, it is known that haloperidol is used to treat productive psychopathological symptoms in acute alcoholic hallucinosis, but its use is associated with a high risk of developing undesirable drug reactions (NLR). A number of isoenzymes of the cytochrome P-450 family take part in the metabolism of haloperidol. The biotransformation of haloperidol occurs with the participation of the CYP2D6 isoenzyme encoded by the gene of the same name. The CYP2D6 gene is highly polymorphic, and this polymorphism can lead to a change in the activity of the encoded isoenzyme. Changes in the rate of biotransformation of haloperidol may affect the profile of its effectiveness and safety. This review is aimed at analyzing the information accumulated in the literature on the role of genetic factors in the formation of an individual response to haloperidol therapy in patients with acute alcoholic hallucinosis.

Results of the study related to polymorphism of ACE gene (rs1799752)‎, integrin αIIbβ3, and CSK gene (rs1378942) influencing development of arterial hypertension in young patients with metabolic syndrome are presented. Hypertension as a component of the metabolic syndrome was detected in 15.0% of young patients. Prevalence of mutant alleles of the studied genes among the examined patients was quite high, so homozygous DD genotype was found in 21.6%, and mutant D allele of the ACE gene in 47.4%. A high risk of hypertension in patients with MS was detected in carriers of the T allele of the CSK (rs1378942) gene – 54.8%, which was most often observed in a combination of polymorphic ACE and CSK gene loci (p = 0.0053).

The issue of serum concentrations of tumor markers in hemodialyzed patients is a widely discussed one due to the fact that these patients are usually at high risk of developing concurrent malignancies on one hand and due to the fact that half-time of these markers is strongly influenced by an impaired renal function. This is a literature review focusing on the role of determination of tumor markers such as CA 125, CA 15-3, AFP in hemodialyzed patients.

As early as December 2019 in the province of Hubei, China, contamination of patients with pneumonia of an unknown etiology occurred. These patients presented with symptoms such as coughing, sore throat, malaise, diarrhea, high fever, and dyspnea. This emerging disease was named COVID-19 due to being part of the group of coronaviruses (CoVs) belonging to the subfamily Orthocoronavirinae, in the Coronaviridae family and in the Nidovirales order. COVID-19 is most commonly transmitted through speech, coughing, sneezing, and salivary sputum. Because dental professionals work closely with the oral cavity, it is imperative that infection prevention controls are strictly adhered to. It is important that the dental profession treats patients while also limiting the possible contamination through the production of aerosol in the dental environment. Furthermore, the dental professional also has a key role in raising awareness and guidance amongst the population concerning COVID-19 related biosafety measures. This literature review aims to inform dental professionals about the COVID-19 pandemic and to present the implications of the virus to the dentist. Dental professionals are considered to be at high risk for contracting SARS-CoV-2.

Despite the crucial role of uncertainty in entrepreneurship and management studies, the characteristics of uncertainty, its relationship with risk, and the sources of uncertainty affecting firms’ activities still require investigation. Extant literature lacks systematization, and concepts like risk and uncertainty are interchangeably used. The issue of uncertainty is present in very different fields of research but with different nuances and sometimes conflicting approaches. This work contextualises the concept of uncertainty in strategic management and entrepreneurship, because in both fields actors make decisions impacting on the survival and long run performance of their organisations. These decisions take place in a context shrouded in either high risk or uncertainty. We systematically analyse 96 papers and 28 books, and purposely integrate the economics, entrepreneurship, and organizational perspectives into our review. We contribute to extant knowledge by systematizing the literature about uncertainty. Firstly, we disentangle its construct from the construct of risk. Secondly, we reveal its complex nature, as a context at the interface between the external and the internal environment. Finally, we link this conceptualisation of uncertainty to the formation of opportunities in the processes of strategic management and entrepreneurship.

Abstract Background Seizures due to posterior fossa lesions is an uncommon phenomenon. In this study systemic literature review was done to i) study the incidence of seizures in posterior fossa lesions ii) determine factors associated with high risk for seizures and iii) ascertain role of prophylactic AEDs in such cases. Methods Systemic literature review was done, for the MeSH terms “posterior cranial fossa” AND “seizures” AND “anticonvulsants”. All original research articles, case reports and systematic reviews pertaining to seizures or use of anticonvulsants in posterior fossa lesions were considered for inclusion. Results A total of 79 cases of posterior fossa lesions, identified from 8 studies, were included for analysis. Incidence of seizures in posterior fossa lesions ranged from 1.8–5% in various studies. The highest incidence for seizures was reported with medulloblastoma, cerebellar haemorrhage and during microvascular decompression for cases of neurovascular conflict. The most significant risk factor for seizures in post-operative period was use to ventricular shunt or ventriculostomy. AEDs were administered symptomatically after the occurrence of seizures in 78 cases except for the use of prophylactic AEDs in a single case of posterior cranial fossa lipoma. Conclusion Seizures in association with posterior fossa lesions are rare and associated with a poor prognosis. Clinical detection can often be difficult and EEG helps in early diagnosis and treatment. Further studies are needed to confirm the role of prophylactic AEDs in high-risk cases.
