Lonkan tekonivelleikkausten sekä leikkauksen jälkeistä fysioterapiaa tarvitsevien määrä kasvaa väestön ikääntyessä. Kuntoutumisen kannalta fysioterapeuttinen ohjaus on tärkeää. Tämän tutkimuksen tarkoituksena oli tarkastella fysioterapeuttien käsityksiä lonkan tekonivelleikkauksen jälkeisestä fysioterapeuttisesta ohjauksesta. Laadullisen tutkimuksen aineistona oli fysioterapeuttisten ohjaustilanteiden (n=7) kuvanauhojen pohjalta tehdyt recall-haastattelut. Aineiston fenomenografisessa analyysissä muodostettiin kolme hierarkkista kuvauskategoriaa, jotka olivat suppeammasta laajempaan: asiantuntijalähtöinen fysioterapeuttinen ohjaus, potilaan yksilöllisyyttä arvostava fysioterapeuttinen ohjaus sekä fysioterapeuttinen ohjaus osana potilaan kuntoutumista. Nämä kategoriat sisälsivät kuusi ohjaustilanteita kuvaavaa teemaa ja niiden variaatiot: potilaan osallistuminen, kehollinen ohjaus, eettisyys ohjaussuhteessa, fysioterapeutin asiantuntijuus, yhteistyö ja terveydenhuoltojärjestelmän eettisyys. Fysioterapeuttisen ohjauksen onnistumisen kannalta kriittiset tekijät fysioterapeuttien ohjauskäsityksissä olivat potilaskeskeisyys, moniammatillinen yhteistyö ja kuntoutumisprosessin jatkuvuus. Abstract Physiotherapists’ perceptions of patient counseling after total hip arthroplasty (THA) The number of total hip arthroplasties is growing due to the fact that THA is a cost-effective procedure moving people quickly from poor to good functional capacity. The aim of the qualitative study was to explore physiotherapists’ perceptions of post-operative patient counseling. The data of this study consisted of seven physiotherapy situations with inpatients in hospital setting. Data were collected by open recall-interviews based on videotapes of patient counseling situations. The data were analyzed according to phenomenographic approach, which revealed three hierarchical categories: therapist-centered patient counseling, patient counseling which values patient’ individuality and patient counseling as part of individual rehabilitation process. Six themes that varied hierarchically in three categories describing different ways of understanding patient counseling were found: patients’ active participation, counseling for movement awareness, ethical counseling relationship, physiotherapy expertise, cooperation, and ethicality of national health care system. The critical variation between categories manifested as patient-centeredness, multiprofessional cooperation and the continuity of rehabilitation process.

The hundreds thousands total hip arthroplasties (THA) annually  performed around the world with the number of revision procedures  estimated around 10%. Dual-energy X-ray absorptiometry (DEXA)  allows to control projectional bone mineral density (BMD) around hip stem dynamically with minor radiation exposure and in a cost-effective way. In routine practice there is a monthly interval between investigations that does not allow to evaluate activity and trends in  short weekly oscillations of bone metabolism in peri-prothetic areas.  Projectional BMD oscillations in peri-prosthetic zone after total hip  arthroplasty in chrono-biological manner evalua ted for the first  time. Therefore chronobiologic approach opens new opportunities for prognosis of tendencies in structural and functional skeleton  reorganization in peri-prothetic zone after THA but there is need in  further research that could give more insight.

AbstractHip joint arthroplasty is considered to be a safe, successful and cost-effective procedure to restore hip joint functionality. However, a successful hip joint arthroplasty affects different stakeholders e.g. patient and surgeon and depends on various influencing factors within the product life cycle as well as the applied technological opportunities. Due to the complex dependencies between influencing factors, technological opportunities and stakeholders, this contribution introduces an IPD-based approach to improve the quality of total hip arthroplasties.

AbstractThe aim of this study was to assess a large, single-surgeon dataset of uncemented total hip arthroplasties that had been templated using software. This assessment compared preoperative templated predictions with what was implanted with regard to (1) femoral prosthesis size and (2) acetabular cup size. The operation notes for two types of uncemented total hip arthroplasty performed by a single surgeon between January 9, 2008, and March 21, 2019, were assessed. The data were refined so that only those that were fully templated on a correctly calibrated pelvic radiograph were included. This provided a total of 153 cases. The predicted software template was compared with actual implanted component sizes. With regard to the femoral stems, the templating software was exactly correct in 45.75% of cases and was accurate to within one size in 87.59% of cases. With regard to the acetabular component, the templating software was exactly correct in 52.94% of cases and was accurate to within one size (2 mm) in 86.94% of cases. Templating software offers an accurate prediction of the size of uncemented femoral and acetabular components. In addition, the benefits of templating include cognitive rehearsal, insight into potentially challenging aspects of surgery, and highlighting intraoperative issues where there is a large intraoperative deviation from what is templated. This study demonstrates the congruence and reproducibility of templating software when compared with other smaller studies performed in the literature.
