ABSTRACTThe aim of this review was to present and discuss current views on the treatment of condylar fractures (CFs). The authors addressed the following issues: the etiology, epidemiology and mechanisms of CFs; strategies and methods for the treatment of CFs. Moreover, the choice of surgical approach for the open treatment of CFs as well as techniques and materials used for fixation of CF are discussed.The PubMed database was used to search for relevant articles published between 2000 and 2018. The analysis referred to both original and review papers (including meta-analyses) that concerned adult patients. There are still differences in opinions among researchers regarding the choice of appropriate treatment – closed or open. There is no consensus among open treatment supporters in choosing the right surgical approach. The important question is which material to choose for osteosynthesis of the condylar fracture.Recent studies show satisfactory results in terms of stability of condylar osteosynthesis with the use of 3D plates. Further discussion is required on the choice of material for fixation of the condylar fracture and, in particular, on the possibility of using resorbable materials.

Mandibular condylar fractures in a growing child may cause long-term consequences. A conservative approach has been recommended to permit the remodelling of the condyles as well as to re-establishing the mandibular dynamics and oral functions. This case report presents a bilateral mandibular condylar fracture in an 11 years old girl treated with myofunctional appliances in combination with exercises. The successful results from this clinical case support a conservative approach when treating mandibular condylar fractures in a growing child instead of an open surgical approach.

Aim of study: To investigate the short- and long-term outcome of patients with isolated lateral malleolar fracture type B treated with a single hemicerclage out of metallic wire or PDS cord. Methods: Over an 8-year period 97 patients were treated with a single hemicerclage for lateral malleolar fracture type B and 89 were amenable to a follow-up after mean 39 months, including interview, clinical examination and X-ray controls. Results: The median operation time was 35 minutes (range 15-85 min). X-ray controls within the first two postoperative days revealed an anatomical restoration of the upper ankle joint in all but one patient. The complication rate was 8%: hematoma (2 patients), wound infection (2), Sudeck's dystrophy (2) and deep vein thrombosis (1). Full weight-bearing was tolerated at median 6.0 weeks (range 2-26 weeks). No secondary displacement, delayed union or consecutive arthrosis of the upper ankle joint was observed. All but one patient had restored symmetric joint mobility. Ninety-seven percent of patients were satisfied or very satisfied with the outcome. Following bone healing, hemicerclage removal was necessary in 19% of osteosyntheses with metallic wire and in none with PDS cord. Conclusion: The single hemicerclage is a novel, simple and reliable osteosynthesis technique for isolated lateral type B malleolar fractures and may be considered as an alternative to the osteosynthesis procedures currently in use.
