Presence and characterization of methicillin-resistant Staphylococcus aureus co-carrying the multidrug resistance genes cfr and lsa(E) in retail food in China

Staphylococcus aureus is an organism of great public health importance. It is widely studied because it is virulent, causes life threatening disease and has ability to adapt to diverse environmental conditions and so develops resistance to antibiotics easily. As a result, there is a need for surveillance of its antibiotic resistance and resistance genes. The susceptibility and molecular characterization of methicillin resistant Staphylococcus aureus recovered from urine samples of healthy students were undertaken. Standard procedures were employed for isolation, identification, susceptibility, and polymerase chain reaction analyses. Out of 217 samples collected, 73 were confirmed Staphylococcus aureus. Most of the isolates were susceptible to ciprofloxacin and vancomycin followed by gentamicin and co-trimoxazole and least susceptible to penicillin, cefotaxime, ofloxacin and cefoxitin. Thirty-two (32) isolates were resistant to 5 antibiotics while 3 isolates were resistant to the 11 antibiotics used in this study. Sixteen phenotypically methicillin resistant isolates contained mecA gene while ten of the isolates also showed the presence of mecB gene. The characteristic Sa442 and nuc genes of Staphylococcus aureus and the presence of spa gene confirmed MRSA. Continous surveillance for antibiotic resistance and resistance genes is paramount at local, regional and national levels. Surveillance data will assist in implementing interventions. ABSTRACT Sequential methicillin-resistant Staphylococcus aureus (MRSA) isolates from patients following attempted mupirocin nasal decolonization showed an increase in mupirocin resistance (MR) from 6.6% to 20%. MR isolates from patients who failed decolonization yielded indistinguishable spa types and carried multiple antimicrobial and antiseptic resistance genes, which may guide infection control and prevention.

Methicillin-resistant Staphylococcus aureus (MRSA) is mostly found in Thailand in the hospital as a nosocomial pathogen. This study aimed to report the genetic characterization of a clinical community-acquired MRSA (CA-MRSA) isolate collected from hospitalized patients in Thailand. Among 26 MRSA isolates, S. aureus no. S17 preliminarily displayed the presence of a staphylococcal cassette chromosome mec (SCCmec) type IV pattern. The bacterial genomic DNA was subjected to whole-genome sequencing. Panton–Valentine leukocidin (PVL) production, virulence toxins, and antibiotic resistance genes were identified, and multi-locus sequence typing (MLST) and spa typing were performed. The strain was matched by sequence to MLST type 2885 and spa type t13880. This strain carried type IV SCCmec with no PVL production. Five acquired antimicrobial resistance genes, namely blaZ, mecA, Inu(A), tet(K), and dfrG conferring resistance to β-lactams, lincosamides, tetracycline, and trimethoprim, were identified. The detected toxins were exfoliative toxin A, gamma-hemolysin, leukocidin D, and leukocidin E. Moreover, there were differences in seven regions in CR-MRSA no. S17 compared to CA-MRSA type 300. In summary, we have reported the ST2885-SCCmec IV CA-MRSA clinical strain in Thailand for the first time, highlighting the problem of methicillin resistance in community settings and the consideration in choosing appropriate antibiotic therapy.
