BackgroundMedical misconduct is an international problem. It is judged according to whether a doctor has endangered the health of the public. Little is known about the risk factors associated with medical misconduct. To inform patient safety, we undertook a focused mapping review and synthesis (FMRS) of tribunal reports retrieved from the Medical Practitioners Tribunal Service (MPTS).MethodsA four-phase FMRS was undertaken: (1) identification and retrieval of 1-year tribunal transcripts from the MPTS (focus), (2) analysis of transcripts to identify patterns mapped to ‘a priori’ risk factors (mapping), (3) peer review of the data (calibration) and (4) creation of a risk profile (synthesis).ResultsOut of the 351 investigative tribunals, 249 (70.94%) resulted in a guilty verdict. 82.73% of all guilty verdicts led to the removal of the doctor from practice. Through the identification of four a priori risk factors, we developed a model of risk associated with medical misconduct: (1) being male, (2) primary medical qualification (PMQ) outside of the UK, (3) working within general practice and surgical specialties, and (4) having passed PMQ more than 20 years ago. Notable ‘unconfirmed’ factors, such as locum work, PMQ achieved outside of the European Union, increasing age and lack of clinical guidance are also relevant to what is deemed professional behaviour and what is not.ConclusionsThe findings can inform debates about patient safety and lay the groundwork for further research into medical misconduct. Prospective studies should focus on confirming the contributory factors and relationship between these four a priori risk factors for medical misconduct: being male, PMQ outside of the UK, increasing age and working in general practice or surgical settings.

The electrocardiogram (ECG) is a graphic representation of the heart's electrical activity. Although it has some limitations as a diagnostic or prognostic tool, it contains a wealth of information necessary for the proper care of a patient with a potential cardiovascular disease. Understanding the ECG changes among athletes would allow medical practitioners to distinguish between normal physiological adaptations and abnormal changes. However, there is limited data on the ECG characteristics among multi-ethnic athletes such as in Malaysia. This study aimed to determine the ECG characteristics and its associated factors among Malaysian national athletes. Malaysian national athletes annual pre-participation medical records were retrieved. Information on sociodemographic, sports and medical history including the 12-lead resting ECG tracings were extracted. ECG were assessed and categorised into normal, physiological adaptation changes, and abnormal ECG using the standardised ‘Seattle criteria’. Differences in ECG characteristics between genders, ethnic background, and type of sports was investigated. Additionally, factors associated with the ECG characteristics were assessed using multiple logistic regression. Majority of Malaysian national athletes had physiological adaptation ECG changes (61%). The most frequent changes were early repolarization, sinus bradycardia and isolated left ventricular hypertrophy. We found significantly higher prevalence of physiological adaptation changes among men (÷2(2,371) = 18.9; p = 0.001) and athletes of Chinese ethnicity (both genders) (÷2(2,356) = 13.8; p = 0.002). Factors associated with physiological ECG changes were men (OR=2.67; 95% CI= 1.68, 4.27; p<0.001) and Chinese ethnicity (OR=2.92; 95% CI=1.68, 4.27; p=0.039). Most athletes had physiological adaptation ECG changes which were significantly associated with male gender and Chinese ethnicity. This information would facilitate the development of a specific guideline in interpreting ECG among Malaysian athletes.

The present study, explore the factors associated with anaemia among antenatal mothers at selected hospitals , Ernakulam was undertaken during 2020 in partial fulfillment of the requirement for the Ph. D under Himalayan university. Objective: to explore the factors associated with anemia among antenatal mothers and associate the explorative factors with demographic variables. Design: non experimental descriptive research design .Setting: medical Centre. Sample: 64 antenatal mothers with anemia. Sampling technique: Probability cluster sample. Conceptual Frame Work: Pender’s health promotion model (1996). Outcome Measures: Explorative factors of anemia among antenatal mothers. Results: 80 percentages of antenatal mothers belong to medical Centre were having anemia. 48(75 percent) of antenatal mothers were having mild anemia and 16(25 percent) were having moderate anemia. Most of the antenatal mothers with anemia were having poor obstetrical factors related to anemia. Majority of the antenatal mothers were having poor nutrition. Most of the antenatal mothers were not practicing good hygiene and cooking practice. Finding of the study revealed that antenatal mothers have average knowledge related to anemia. There is an association between knowledge with the monthly income of the family, number of children and parity, obstetrical factors with age and monthly income of family, illness factors with age, education, monthly income of family, number of children, parity and food habits, nutritional factors with education and number of children. Conclusion: The result of the study support that there is a positive association between explorative factors of anemia with demographic characteristics. It says the need of mass health teaching to antenatal mothers regarding iron deficiency anemia. Health education was given to antenatal mothers regarding anemia using flash cards.
