Quality of life in patients with untreated age-related hearing loss

AbstractBackground:Hearing loss commonly affects quality of life in the elderly, yet is often neglected.Objectives:To investigate the impact of untreated age-related hearing loss on the quality of life of elderly individuals, and to assess the usefulness of quality of life questionnaires as screening tools for significant hearing loss.Methods:We recruited 80 patients aged 50 years or more with untreated hearing impairment. The Short Form 36 Health Survey and the Hearing Handicap Inventory for the Elderly Screening Version questionnaire were administered.Results:There was no significant association between severity of hearing impairment and Short Form 36 Health Survey scores. However, dose-graded correlation was observed between severity of hearing loss and Hearing Handicap Inventory for the Elderly Screening Version questionnaire scores (p < 0.001). A score for the latter questionnaire of more than 8 was 72.8 per cent sensitive and 71.4 per cent specific in detecting clinically significant hearing loss of at least 40 dB (receiver operating characteristic = 0.83).Conclusion:The Short Form 36 Health Survey, a generic measure, lacked specificity and sensitivity in detecting clinically significant hearing loss. However, significant hearing impairment was reflected in the Hearing Handicap Inventory for the Elderly Screening Version questionnaire scores, suggesting that this is a good, disease-specific screening tool. A combination of functional (i.e. the Hearing Handicap Inventory for the Elderly Screening Version questionnaire) and physiological (i.e. audiometric) assessment is recommended to investigate hearing loss in elderly individuals.

Abstract OBJECTIVE The aim of the present study was to characterise the relation between quality of life (QOL) and the four major complaints (hearing loss, tinnitus, vertigo and unsteadiness) caused by unilateral vestibular schwannoma (versus) in a cohort of well-characterised untreated patients. METHODS One hundred and ninety-nine consecutive patients (91 men, 108 women) with a mean age of 56.9 years were studied prospectively during the 4-year period 2001–2004. The average time span from symptom onset to the radiological diagnosis was 4.2 years. The patients were subject to a standardised examination including MRI, evaluation of hearing acuity, balance function by stabilometry, and a visual analogue scale (VAS) self-evaluation of tinnitus and vertigo. Furthermore, the patients responded to two questionnaires, Short-Form 36 (SF-36) and Glasgow Benefit Inventory (GBI)). A reference population was recruited from 80 adult persons who visited Haukeland University Hospital as non-patients or non-staff members. All data were recorded prospectively in a customised case report form. Statistical analysis was done with SPSS software. RESULTS The response rates of the Short-Form 36 and Glasgow Benefit Inventoryquestionnaires were 91.5 and 89.9%, respectively. According to the SF-36 questionnaire the patients scored significantly below that of expected norms with exception of physical function and mental health. Patients report negative benefit on the general and physical sections of the GBI questionnaire. Regression analysis showed that vertigo had a strong negative impact on QOL, whilst unilateral hearing loss and tinnitus had less impact on QOL. CONCLUSION Vertigo is the symptom causing the most pronounced negative effect on QOL in patients with versus The more frequent versus-symptoms unilateral hearing loss and tinnitus seem to be less important in the patients' perception of QOL evaluated by the questionnaires used in this study. If vertigo could be relieved by treatment, this symptom should be more in focus when discussing treatment options in small to medium sized versus

Age related hearing loss (ARHL) is one of the commonest health conditions of the elderly people which have an important relation with the cognition. Long standing hearing deprivation leads to decline of the cognitive performance. This has impact on quality of communication and result in social isolation, depression and enhances the dementia. Cognitive decline may be misdiagnosed or over-diagnosed when the sensory abilities of the patients are not properly evaluated. Adequate intervention by use of hearing aid or cochlear implant improves the communication, cognitive function, social, emotion function and positively impact on the quality of life. With rise of the elderly population and concomitant increase of ARHL with associated cognitive impairment, it is imperative to discuss this morbid clinical entity in present scenario. Cognitive decline in elderly age have a profound impact on the affected person, on caregivers and society. The financial costs for cognitive impairment in ARHL are also major source of concern for the society. In this review article, we focus on the epidemiology, pathophysiology, hypotheses of etiological mechanisms between the ARHL and cognitive decline or impairment, impact of cognitive impairment on quality of life and prevention.

Background The Lymphoedema Quality-of-Life Questionnaire is a validated disease-specific instrument to measure the impact of lymphoedema on patients’ lives. In this study, we tested its psychometric properties and validated the use of the questionnaire in its Dutch translation. Methods We obtained the answers to a standardised questionnaire, including Lymphoedema Quality-of-Life Questionnaire and Short-Form (36) Health Survey, twice at an interval of 2 weeks in 60 patients with lower limb lymphoedema. Feasibility was tested on the basis of missing responses and response distribution. Structure was studied using factor analysis. The reliability of the Lymphoedema Quality-of-Life Questionnaire was assessed using Crohnbach’s α and test-retest reliability. Construct validity was tested by correlating Lymphoedema Quality-of-Life Questionnaire scores with the Short-Form (36) Health Survey scores. Results The response rate was 88.2%. One of the 22 items missed >10% of responses; another showed a borderline ceiling effect. Internal consistency was good and test-retest reliability was excellent. The Lymphoedema Quality-of-Life Questionnaire correlated well with the physical component of the Short-Form (36) Health Survey and moderately with the mental component, suggesting that its construct validity was good. Conclusion The Dutch Lymphoedema Quality-of-Life Questionnaire can be used for health-related quality-of-life research in lower limb lymphoedema patients.

Abstract: Hypertension is the most common illness experienced by residents in Indonesia. Hypertension is known as high blood pressure resulting from various factors. Hypertension is a serious illness and most often occur to the elderly, which greatly affect their quality of life. Focus on this research is finding the influence of elderly aerobic exercise on hypertension patients’ quality of life in BPLU senja cerah paniki bawah Manado. This research is a field experiment with pre-post one group test plan. To test the significance of this study, statistic test such as t paired with level of error of 5% or 0,05 is applied. Elderly aerobic exercise is instructed to 30 elderly patients with hypertension as respondents in this study. Experiment is performed by comparing the quality of life scores, measured by filling up a short- form 36 questioners by hypertension patients before and after elderly aerobic exercise. There is a significant effect of elderly aerobic exercise towards the quality of life of hypertension patients in BPLU senja cerah paniki bawah, where the value of significance is 0,00. This means there is an influence of quality of life before and after elderly aerobic exercise, where an increase in quality of life score emerges. Hipertensi dikenal sebagai penyakit darah tinggi akibat dari berbagai macam faktor. Hipertensi ini merupakan penyakit serius dan paling sering terjadi terutama pada lansia yang sangat mempengaruhi kualitas hidup mereka. Fokus pada penelitian ini yaitu untuk mencari pengaruh senam bugar lansia terhadap kualitas hidup penderita hipertensi pada BPLU Senja Cerah Paniki bawah Manado. Jenis penelitian ini adalah eksperimental lapangan dengan rancangan pre-post one group test. Untuk menguji signifikan dari penelitian ini maka digunakan uji statistik berupa uji t berpasangan dengan taraf kesalahan yaitu 5 % atau 0,05. Senam bugar lansia dilakukan oleh 30 responden yang menderita hipertensi. Eksperimen dilakukan dengan membandingkan skor kualitas hidup yang diukur dengan  pengisian short-form 36 yang dilakukan lansia penderita hipertensi sebelum dan sesudah senam bugar lansia. Terdapat pengaruh yang signifikan antara senam bugar lansia terhadap kualitas hidup pasien penderita hipertensi di BPLU Senja Cerah Paniki Bawah dimana nilai signifikannya 0,000. Artinya ada pengaruh kualitas hidup sebelum dan sesudah senam buagr lansia, dimana terjadi peningkatan skor kualitas hidup. Kata Kunci: senam bugar lansia, kualitas hidup, hipertensi

Abstract BACKGROUND: Symptomatic patients with a brainstem cavernoma are treated surgically with increasing frequency. Generally, the patient's benefit from this difficult surgical intervention is quantified by the assessment of neurological symptoms. OBJECTIVE: To document the beneficial effect of surgery in a larger patient population by assessing the postoperative quality of life (QoL). METHODS: In a series of 71 surgically treated patients, a detailed neurological status was assessed by Patzold Rating and Karnofsky Performance Status Scale. Patients rated their QoL with the Short Form 36 Health Survey. To document the effect of surgery on QoL, we devised a supplementary questionnaire. The last 24 patients completed Short Form 36 Health Survey pre- and postoperatively. RESULTS: Karnofsky Performance Status Scale improved in 44 of 71 surgical patients (62%), remained unchanged in 19 (27%), and deteriorated in 8 (11%) individuals. Patzold Rating showed a more detailed picture of the neurological symptoms. It correlated significantly with Karnofsky Performance Status Scale, which underscores its usefulness for patients with brainstem lesions. In the Short Form 36 Health Survey score, the Mental Component Summary improved with surgery (paired test, P = .015). In addition, 58 individuals (82%) declared a clear subjective benefit of surgery. CONCLUSION: The results of this large series support the notion that microsurgical removal of a brainstem cavernoma represents an effective therapy in experienced hands and is generally associated with good clinical outcome, both neurologically and in terms of QoL.
