An Experimental Study on Timing in Tracheal Stenosis Surgery

Abstract Background TNF-α, IL-6, and TGF-β are important bio mediators of the inflammatory process. This experimental study has investigated inflammatory biomarkers' efficacy to determine the appropriate period for anastomosis surgery in tracheal stenosis cases. Methods First, a pilot study was performed to determine the mean stenosis ratio (SR) after the surgical anastomosis. The trial was planned on 44 rats in four groups based on the pilot study's data. Tracheal inflammation and stenosis were created in each rat by using micro scissors. In rats of groups I, II, III, and IV, respectively, tracheal resection and anastomosis surgery were applied on the 2nd, 4th, 6th, 8th weeks after the damage. The animals were euthanized 8 weeks later, followed by histopathological assessment and analysis of TNF-α, IL-6, and TGF-β as biochemical markers. Results Mean SR of the trachea were measured as 21.9 ± 6.0%, 24.1 ± 10.4%, 25.8 ± 9.1%, and 19.6 ± 9.2% for Groups I to IV, respectively. While Group III had the worst SR, Group IV had the best ratio (p = 0.03). Group II had the highest values for the biochemical markers tested. We observed a statistically significant correlation between only histopathological changes and TNF-α from among the biochemical markers tested (p = 0.02). It was found that high TNF-α levels were in a relationship with higher SR (p = 0.01). Conclusion Tracheal anastomosis for post-traumatic stenosis is likely to be less successful during the 4th and 6th weeks after injury. High TNF-α levels are potentially predictive of lower surgical success. These results need to be confirmed by human studies.

A case of tracheal stenosis in the cervical portion of the trachea was encountered in a 5.5-month-old St. Bernard-Ro􀄴weiler cross dog. Breathing difficulty was seen in the clinical examination and presence of an obvious narrowing between the 3rd–5th cervical tracheal rings was determined in the radiological examination. Under general anesthesia the portion with stenosis was resected and the healthy trachea ends were anastomosed using the split cartilage technique. In the postoperative period the breathing difficulty disappeared and there was no development of a new stenosis in the anastomosis region. In the late period check-up the patient was seen to lead a healthy life.

Nitrogen fixation (acetylene reduction) associated with various communities in the intertidal sand and mud flat was measured in situ. Areas which were colonized by algae, plants, and animals generally had significantly higher activities than areas which were visibly uncolonized. The highest activities were measured at sites colonized by a mixed bloom of Oscillatoria–Euglena–photosynthetic bacteria. These areas occupied only about 1% of the surface of the sediments, yet contributed nearly 50% of the biologically fixed nitrogen. Enteromorpha communities also exhibited relatively high activities. Sites were grouped according to the statistical significance of the mean of their activities: group I: uncolonized sand, mud and gravel flats, Eelgrass (Zostera) communities, and snail (Amphibola) beds, with an estimated contribution of 1 g N ha−1 day−1; group II: Cockel (Chione) beds, 3g N ha−1 day−1; group III: Salicornia, Ulva, Juncus, and Enteromorpha, 10 g N ha−1 day−1; and group IV: Oscillatoria–Euglena–photosynthetic bacteria association, 200 g N ha−1 day−1. Enteromorpha and Oscillatoria–Euglena–photosynthetic bacteria communities showed significantly lower rates of nitrogen fixation in the dark than in the light.

AbstractBackgroundWe present a group of infants and children with pulmonary arterial sling and tracheal stenosis. In some of the patients, the anomalously located pulmonary artery had previously been reimplanted, but without simultaneous repair of the trachea.MethodsFrom 1992 to 2007, we reimplanted the left pulmonary artery in 13 children with a pulmonary arterial sling. Their median age was 8 months, with a range from 1 to 72 months. We also performed tracheal resection with end-to-end anastomosis, or complex tracheal reconstructions. In 5 patients, the reoperation was indicated because of persistent tracheal stenosis not treated initially at first correction of the arterial sling. All patients presented with stridor and respiratory distress. Cardiac catheterization, bronchoscopy and multidetecting computer tomography angiography were performed in all cases prior to the operation. All operations were performed under cardiopulmonary bypass.ResultsThere was no operative or late mortality. The patients were extubated under bronchoscopic control. The mean period of intubation was 18 plus or minus 8 days, and the average follow-up was 8 plus or minus 4 years. The patients showed no signs of tracheal re-stenosis clinically or on bronchoscopy. The group of the patients under reoperations, however, required longer periods of intubation and hospitalization.ConclusionOur experience demonstrates that, in patients with a pulmonary arterial sling, any associated tracheal stenosis should be explored at the initial operation, since decompression of the trachea by reimplanting the anomalously located pulmonary artery fails to provide relief. The funnel trachea, if present, undergoes progressive stenosis, and will require surgical repair. The use of cardiopulmonary bypass permitted extensive mobilization of the tracheobronchial tree, and allowed us to perform a tension-free anastomotic reconstruction of the trachea.

Abstract Background The use of laryngeal mask airway (LMA) ventilation in surgeries to be performed in upper tracheal stenosis has been reported in the case series. However, there is no generally accepted standardized approach for the use of LMA. In this study, LMA usage areas and advantages of trachea surgery were examined. Methods The records of 21 patients who underwent tracheal surgery using LMA ventilation between March 2016 and May 2020 were evaluated retrospectively. The patient data were analyzed according to age, gender, mean follow-up time, surgical indication, mean tracheal resection length, anastomosis duration, mean oxygen saturation, mean end-tidal CO2 levels, and postoperative complications. Results Four patients were female and 17 were male, their median age was 43 (11–72 range) and the mean follow-up time was 17.6 months. The most common surgical indication was postintubation tracheal stenosis. The mean tracheal resection length was 26.6 mm and the mean anastomosis duration was 11.3 minutes. The mean pulse oximetry and mean end-tidal CO2 during laryngeal mask ventilation was 97.6% ± 2.1 and 38.1 ± 2.8 mm Hg, respectively. Postoperative complications were higher in patients with comorbidities. Conclusion LMA-assisted tracheal surgery is a method that can be used safely as a standard technique in the surgery of benign and malignant diseases of both the upper and lower airway performed on pediatric patients, patients with tracheostomy, and suitable patients with tracheoesophageal fistula.

Xestospongia sp. is one of marine sponge belongs to demosponges class that mainly found in Southeast Sulawesi and the secondary metabolites contained in Xestospongia sp. suspected to have immunomodulatory activity. A previous study exhibited the immunomodulatory of Xestospongia sp. ethanolic extract (XEE) at dose of 300 and 400 mg/Kg BW by affecting the phagocytic activity of macrophages. Thus, this study aims to investigate the effect of XEE towards interferon-gamma (IFN-γ) and tumor necrosis factor-alpha (TNF-α) at dose of 300 and 400 mg/Kg BW. Wistar male rats are divided into 4 groups (n=6) randomly and treated for 7 days orally each as follow: group I (XEE dose of 300 mg/KgBW); group II (XEE dose of 400 mg/KgBW); group III (0.5% NaCMC); and group IV (commercial phylantii extract). On day 8, animals were infected with Staphylococcus aureus and left for 1 hour. Animals were sacrificed and the blood was drawn by cardiac puncture (3 mL), followed by analyzed under ELISA Kit for IFN-γ and TNF-α. Collected data were analyzed statistically using SPSS®. The IFN-γ levels obtained were 350.113; 392.970; 118.416; and 61.958 ρg/mL, respectively and the TNF-α were 2808; 1308; 778; and 845.5 ρg/mL, respectively. According to results obtained, both doses of XEE are affecting the IFN-γ and TNF-α levels (p<0.05) compared to group III as negative control, and group IV as positive control. As conclusion, XEE of both doses is increasing IFN-γ and TNF-α levels of animals that respond to phagocytic activity

Background: Cisplatin is a potent antineoplastic agent widely used for a variety of malignancies. However, it has many dose-limiting side effects such as neurotoxicity, cytotoxicity and ototoxicity. The aim of our study was to determine the effectiveness of astaxanthin (ASX) as a cytoprotective agent against cisplatin-induced cytotoxicity in the submandibular glands of rats.Methods: Thirty-six adult male Wistar albino rats were divided into six groups as follows: group I: saline control; group II: 75 mg/kg/day ASX; group III: 16 mg/kg cisplatin; group IV: 25 mg/kg/day ASX + cisplatin; group V: 75 mg/kg/day ASX + cisplatin; group VI: olive oil + cisplatin. In all groups, submandibular gland histopathological and histochemical investigations were done using a light microscope. Every rat section was semi-quantitatively scored. Neutrophil infiltration density, myoepithelial cell density in the degeneration area, degenerative granular duct cell density, degenerative seromucous acinus cell density, and changes in the content of the secretory granules of seromucous acini and granular ducts of the parenchyma and stroma were calculated.Results: The results of the analysis of the mean acinus area of the submandibular gland revealed that there was a significant decrease in cisplatin group rats when compared to control rats (p<0.05, p=0.00). However, there was no significant difference between-group IV, V and control group in terms of mean acinus area. (p=0.541, p=0.773). The results of the analysis of the mean ducts area of the submandibular gland also showed that there were significant increased group III compared to control rats (p<0.05, p=0.031). There was no significant difference between-group IV, V and control group in terms of mean ducts area (p>0.05, p=0.921). Similarities were observed in the mean ducts area with the group IV and the group V (p>0.05, p=0.571).Conclusions: These results suggest the possibility that the clinical use of ASX could reduce or prevent damage to the salivary gland of patients receiving cisplatin chemotherapy.

The effects of two doses of cloprostenol and two doses of flurogestone acetate sponge on the onset of oestrus, and embryo recovery and quality were evaluated. Thirty-two indigenous ewes (Wera breed) were allocated into four groups of eight. All ewes were synchronised with 100 µg (Group I) or 175 µg (Group II) cloprostenol injection, 9 days apart, or insertion of intravaginal sponges containing 30 mg (Group III) or 45 mg (Group IV) flurogestone acetate (FGA) for 12 days. The ewes were superovulated with 600 IU PMSG intramuscularly 10 days after the second cloprostenol injection or immediately after sponge removal on Day 12. After the detection of oestrus, the ewes were mated naturally at 6 and 12 h, and some ewes were inseminated laparoscopically. Embryos were recovered surgically 5 or 6 days after service. All ewes exhibited oestrus. The onset of oestrus occurred significantly (P < 0.05) earlier in FGA-treated (50.0 ± 1.5 and 48.0 ± 0.00 h) than in cloprostenol-treated groups. There were no significant differences (P > 0.05) in the mean time of onset of oestrus (50.0 ± 1.5 and 48.0 ± 0.00 h) between the two doses of cloprostenol. The mean number of corpora lutea (8.1 ± 1.26) and embryos recovered (6.1 ± 1.00) was significantly (P < 0.05) higher in ewes treated with 45 mg FGA than in ewes treated with cloprostenol. Embryo recovery rate was significantly (P < 0.05) higher in ewes treated with 45 mg FGA (75.4%) than in other groups (41.7% and 51.6% in 100 μg and 175 μg cloprostenol- and 52.7% in 30 mg FGA-treated groups, respectively). Fertilisation rate was 93.3% in ewes given 100 µg cloprostenol, whereas other groups showed 100% fertilisation rate. The highest percentage (100%) of Grade 1 embryos was in FGA groups. In conclusion, despite FGA protocol presenting superior results, cloprostenol protocol was equally efficient in synchronising oestrus. The embryo recovery rate was better after 45 mg FGA than 30 mg FGA or after either dose of cloprostenol.

Question: Can exercise dose be established for bone weakness treatment? Design: Randomized experimental study. Participants: Thirty two adult (12 weeks) Wistar rats (16 males and 16 females) weighed [Formula: see text][Formula: see text]gm. Intervention: Rats were tail suspended for 2 weeks and then randomly divided into four groups; Group I ([Formula: see text]) sacrificed after suspension, Group II ([Formula: see text]) normal cage activities. Group III: ([Formula: see text]) level surface treadmill running. Group IV: ([Formula: see text]) uphill treadmill at angle of 15∘. The running speed was 12[Formula: see text]m/min for a period of 30[Formula: see text]min daily at frequency of 5 days/week for 6 weeks. Outcome measures: Histomorphometric analysis of distal femurs of right hind limbs to measure bone architectural parameters (cortical thickness, Tb.Th, Tb.Nm, Tb.Sp, Tb.Ar, Bone marrow area and density and TBA). Results: Significant improvements were shown in bone architecture in distal femur of Group IV ([Formula: see text]) compared to both Group I and Group II. No significant improvement in distal femur in Group III ([Formula: see text]) compared to both Group I and Group II. The percentage of change was found to be 62–96% between Group IV and Group II and 72– 82% between Group IV and Group I. Conclusion: The results suggested that uphill treadmill running exercises were effective to induce changes in bone tissue in the form of changing trabecular dimensions and density. The induced structural improvement was site specific and dose dependent.
