Eating Behavior and its Disorders

This chapter examines the most representative pathological behaviors of the so-called feeding and eating disorders, with a particular attention to the psychopathological meaning of the different behaviors. These behaviors, including restrictive diet, binge eating, purging, or obsessive control of body shape are generally adopted to define the broad category of eating disorders. In particular, it is considered the trans-nosographic and dimensional approach to the eating disorder psychopathology, as most of the symptoms described in this chapter can be observed also in different psychopathological conditions. The chapter considers the subjective transformation in patients of the original meaning of food and alimentation into something negative and dangerous, adopting a phenomenological approach to eating disorders.

IntroductionThe eating disorder are not only isolated power problems. They interweave issues and pathologies in patients often difficult to approach and have crimping going to reach a solution and get to the real problem of the patient. As it has been observed in studies if they have established relationships between patients with an impulsive nature and traumatized regarding the presentation of eating disorders.ObjectivesOur study aims to establish the relationship between eating disorder such as anorexia and bulimia with factors such as impulsivity and suffered traumas.Materials and methodsIt has used a sample (n = 57) and patients spent three scales “Barratt Impulsiveness Scale” (BIS-11), “Questionnaire traumatic experiences” (TQ) and the BSQ, scale measuring dissatisfaction disorders eating behavior. The aim is to see the relationship between anorexia and bulimia regarding trauma and impulsivity.ResultsIt is intended to check if having character traits of impulsivity or miss having lived or suffered traumatic experiences such as the unexpected death of a family member, rape, kidnapping… establishes a relationship in patients who develop conduct disorders food.ConclusionThis study aims to see if there is a correlation between eating disorder and factors such as traumatic experiences and an influential feature in this pathology such as the impulsivity through a sample of patients (n = 57) over 18 years.Disclosure of interestThe authors have not supplied their declaration of competing interest.

Objective: To examine eating disorder attitudes and psychopathology among female university students in Australia and Thailand. Method: Participants were 110 Caucasian Australians, 130 Asian Australians and 101 Thais in Thailand. The instruments included the Eating Attitudes Test (EAT) and the Eating Disorders Inventory (EDI). Results: Eating disorder attitudes and psychopathology scores in the Thai group were found to be highest. The Asian Australian group did not have significantly higher scores on the EAT-26 than the Caucasian Australian group, but had higher scores in some subscales of the EDI-2. That the Thai group had the highest scores in susceptibility to developing an eating disorder and eating disorder psychopathology may be partially explained in sociocultural terms, with pressure to be thin more extreme in Thailand than in Australia. The evidence suggested that unhealthy eating disorder psychopathology is not limited to Western societies but is already present in Thai and other Asian societies.

Background: Enhanced Cognitive Behaviour Therapy (CBT-E) (Fairburn, Cooper and Shafran, 2003) was developed as a treatment approach for eating disorders focusing on both core psychopathology and additional maintenance mechanisms. Aims: To evaluate treatment outcomes associated with CBT-E in a NHS Eating Disorders Service for adults with bulimia and atypical eating disorders and to make comparisons with a previously published randomized controlled trial (Fairburn et al., 2009) and “real world” evaluation (Byrne, Fursland, Allen and Watson, 2011). Method: Participants were referred to the eating disorder service between 2002 and 2011. They were aged between 18–65 years, registered with a General Practitioner within the catchment area, and had experienced symptoms fulfilling criteria for BN or EDNOS for a minimum of 6 months. Results: CBT-E was commenced by 272 patients, with 135 completing treatment. Overall, treatment was associated with significant improvements in eating disorder and associated psychopathology, for both treatment completers and the intention to treat sample. Conclusions: Findings support dissemination of CBT-E in this context, with significant improvements in eating disorder psychopathology. Improvements to global EDE-Q scores were higher for treatment completers and lower for the intention to treat sample, compared to previous studies (Fairburn et al., 2009; Byrne et al., 2011). Level of attrition was found at 40.8% and non-completion of treatment was associated with higher levels of anxiety. Potential explanations for these findings are discussed.

The intensity and effort of bodybuilding training suggest an overinvestment in body shape and physical appearance, which has been suggested to be a risk factor for developing eating disorders. The purpose of this study was to investigate the prevalence of eating disorder tendencies among a sample of collegiate male bodybuilders (BB,n= 68) and controls (C,n= 50) (nonbodybuilders), using the Eating Disorders Inventory 2 (EDI-2).Ttests were used to test the hypothesis that bodybuilders' scores would be higher than those of controls. The mean scores on the EDI-2 did not indicate the presence of eating disorder tendencies for either group. Controls scored significantly higher than bodybuilders on the Body Dissatisfaction scale. Results indicate that when the EDI-2 is used, college-age male bodybuilders are not shown to be more likely to have eating disorders than a group of college-age male controls.

The transition from DSM-IV to DSM-5 relaxed diagnostic criteria for anorexia nervosa and bulimia nervosa, and recognised a third eating disorder, binge eating disorder. However, a large proportion of cases remain in the ill-defined category of ‘other specified feeding and eating disorders’. We sought to investigate the utility of a proposed solution to classify this group further, subdividing based on the dominant clinical feature: binge eating/purging or restraint. Cluster analysis failed to identify clusters in a treatment-seeking sample based on symptoms of restraint, binge eating, purging and over-evaluation of shape and weight. Further investigation of this highly heterogeneous group is required.

Eating disorders are widespread illnesses with significant impact. There is growing concern about how those at risk of eating disorders overuse online resources to their detriment. We conducted a pre-registered systematic review and meta-analysis of studies examining Problematic Usage of the Internet (PUI) and eating disorders. The meta-analysis comprised n=32,295 participants, in which PUI was correlated with significant eating disorder psychopathology Pearson r=0.22 (s.e.=0.04, p<0.001), body dissatisfaction r=0.16 (s.e.=0.02, p<0.001), drive-for-thinness r=0.16 (s.e.=0.04, p<0.001) and dietary restraint r=0.18 (s.e.=0.03). Effects were not moderated by gender, PUI facet or study quality. Results are in support of PUI impacting significantly on vulnerable populations towards the development or maintenance of eating disorder psychopathology; males may be equally vulnerable to these potential effects. Prospective and experimental studies in the field suggest that small but significant effects exist and may have accumulative influence over time and across all age groups. Those findings are important to expand our understanding of PUI as a multifaceted concept and its impact on multiple levels of ascertainment of eating disorder psychopathology.

L’anoressia nervosa rischia di venir fraintesa, etichettandola come mero disturbo neurobiologico e affrontandola con tattiche assistenziali, che mirano semplicemente alla rapida correzione del peso. Un approccio fenomenologico coglie invece nel disturbo alimentare una strategia di liberazione, per quanto rischiosa e piena di contraddizioni. Nel presente articolo abbiamo indicato tre dimensioni etiche ed estetiche di questa pericolosa trasformazione di sé. L’anoressica scolpisce il corpo in forme dissonanti, al modo di una body artist; ella scrive nella carne la propria storia alla luce di un mito affettivo, che la guida come un racconto esemplare di formazione; infine l’anoressica prova per il cibo il disgusto che ella vive nei confronti di relazioni mancate di cui ha ancora disperata fame. La persona che soffre tenta di dirigere perfezionisticamente, attraverso la malattia, una trasformazione individuale, imbattendosi in un mondo di oggetti trasfigurati (fra cui i cibi, le sostanze alimentari), un mondo simile a quello istituito dall’arte contemporanea. Per questi motivi, il lavoro medico-psicologico condotto sui disturbi alimentari è più efficace quando si posseggono competenze in ambito umanistico, particolarmente di ordine etico ed estetico. ---------- Anorexia nervosa risks being misunderstood by labelling it merely as a neurobiological disorder or by tackling it only with behavioral advice, in order to rapidly achieve some weight gain. On the contrary, a phenomenological approach recognizes in an eating disorder also an ethical strategy of liberation, although it may well be risky and full of contradictions. This article indicates three ethical and aesthetic dimensions of this dangerous transformation of self-image. Anorexia sculpts the body in dissonant forms, in the way of a body artist; it writes in the flesh a suffering story in the light of an affective myth, which guides the patient towards an ideal of mature development. Finally, sick people feel the same disgust for food that they experience with regard to missed or damaged relationships of which they still desperately hunger for. Through the illness, an attempt is made to manage in a perfectionist way the bodily transformation, but the result is that they come upon a disquieting world of transfigured objects, a world similar to that established by some contemporary art movements. For these reasons, the medical-psychological work carried out on eating disorders requires competence in the field of medical humanities and particularly in the sphere of ethical and aesthetic education.

Feeding and eating disorders are defined by persistent disturbance of eating (or behaviors related to eating) with subsequent changes in consumption or absorption of nutrition that are detrimental to physical health and social functioning. The following eating disorders are described in the DSM-5: anorexia nervosa, bulimia nervosa, binge eating disorder, pica, rumination disorder, avoidant/restrictive food intake disorder (ARFID), other specified feeding or eating disorder (OSFED), and unspecified feeding or eating disorder (USFED). ARFID, OSFED, USFED, rumination disorder, and binge eating disorder are new additions to the manual and are first described in the DSM-5. The DSM-5 also provides severity specifiers—mild, moderate, severe, and extreme—for the diagnoses of bulimia nervosa and anorexia nervosa. This review describes the eating disorders enumerated in the DSM-5 and provides information regarding their genesis and course. Background: Patients in the postoperative period following bariatric surgery are at risk of developing eating disorders. This study aims to analyze the relation between bariatric surgery and the development and recurrence of eating disorders. Material and methods: A literature review was carried out on 15 November 2020. Fourteen studies that met the eligibility criteria were included for qualitative synthesis, and 7 studies for meta-analysis. Results: The prevalence of eating disorders in the postoperative period was 7.83%, based on the 7 studies in the meta-analysis. Binge eating disorder alone was 3.81%, which was the most significant factor, and addressed in 6 of these studies. Conclusion: The investigated studies have significant methodological limitations in assessing the relation between bariatric surgery and eating disorders, since they mostly present data on prevalence. This narrative review provides an overview of the epidemiology of binge eating disorder (BED), highlighting the medical history of this disorder and its entry as an independent condition in the Feeding and Eating Disorders section of the recently publishedDiagnostic and Statistical Manual of Mental Disorders, Fifth Edition. Estimates of prevalence are provided, as well as recognition that the female to male ratio is lower in BED than in other eating disorders. Evidence is also provided of the most common comorbidities of BED, including mood and anxiety disorders and a range of addiction disorders. In addition, discussion of the viewpoint that BED itself may be an addiction — at least in severe cases — is presented. Although the genetic study of BED is still in its infancy, current research is reviewed with a focus on certain neurotransmitter genes that regulate brain reward mechanisms. To date, a focal point of this research has been on the dopamine and the μ-opioid receptor genes. Preliminary evidence suggests that a predisposing risk factor for BED may be a heightened sensitivity to reward, which could manifest as a strong dopamine signal in the brain’s striatal region. Caution is encouraged, however, in the interpretation of current findings, since samples are relatively small in much of the research. To date, no genome-wide association studies have focused exclusively on BED.
