Background: Neovascularization is one of the most important risk factors for unstable plaque. This study was designed to correlate plaque thickness, artery stenosis and levels of serum C-reactive protein with the degree of intraplaque enhancement determined by contrast-enhanced ultrasound. Patients and methods: Contrast-enhanced ultrasound was performed on 72 carotid atherosclerotic plaques in 48 patients. Contrast enhancement within the plaque was categorized as grade 1, 2 or 3. Maximum plaque thickness was measured in short-axis view. Carotid artery stenosis was categorized as mild, moderate or severe. Results: Plaque contrast enhancement was not associated with the degree of artery stenosis or with plaque thickness. Serum C-reactive protein levels were positively correlated with the number of new vessels in the plaque. C-reactive protein levels increased in the three groups(Grade 1: 3.72±1.79mg/L; Grade 2: 7.88±4.24 mg/L; Grade 3: 11.02±3.52 mg/L), with significant differences among them (F=10.14, P<0.01), and significant differences between each two groups (P<0.05). Spearman’s rank correlation analysis showed that serum C-reactive protein levels were positively correlated with the degree of carotid plaque enhancement (Rs =0.69, P<0.01). Conclusions: The combination of C-reactive protein levels and intraplaque neovascularization detected by contrast-enhanced ultrasound may allow more accurate evaluation of plaque stability.

SummaryEvent-related potentials (ERP) are regarded as age dependent. However, it is not known whether this is an intrinsic property of ERP or an extrinsic factor. We designed a setting in which ERP were evoked using a modified oddball paradigm with highly differentiable and detectable target and non-target stimuli. A total of 98 probands were enrolled in this study. We evaluated the latency and amplitude of the P3 component of visually evoked ERP. The mean P3 latency was 294 ± 28 ms and was not related to age (r = –0.089; p = 0.382; Spearman-rank-correlation). The P3 amplitude was related to age in the total sample (r = –0.323; p = 0.001; Spearmanrank-correlation) but not in the probands under the age of 60 years. There were no significant differences regarding sex. Our findings suggest that ERP are not age dependent if highly differentiable and detectable stimuli are used. This should be considered when normal values of ERP are created for clinical use.

<p>Documentation is an activity of recording, reporting or recording an event and activities carried out in the form of providing services that are considered important and valuable. One factor that can influence documentation is the nurse's workload. The purpose of this study is to identify the relationship between nurses' workload and the application of documentation in the Hj. Anna Lasmanah Banjarnegara. This  research is quantitative with a cross sectional approach descriptive correlation design. Samples were taken with a total sampling of 65 nurses. Instruments to measure documentation using observation sheets. While the nurse workload instrument uses a questionnaire sheet. The analysis technique uses Spearman Rank correlation. Based on the research results of the workload of a nurse in the hospital room , most of them are in the weight category, as many as 46 respondents (70.8%). Application of nursing care documentation in the hospital room Hj. Anna Lasmanah Banjarnegara, most of them are respondents in the incomplete category as many as 63 respondents (96.9%). There is a significant relationship between nurse workload with the application of documentation, this is evidenced by the results of the Spearman Rank correlation bivariate analysis, which is r = 0.688 with p = 0.000 &lt;0.05. It is hoped that management will motivate nurses to complete the documentation of nursing care</p>

Background : The clot burden score (CBS) is a scoring system used in acute ischemic stroke (AIS) to predict patient outcome and guide treatment decision. However, CBS is not routinely practiced in many institutions. This study aimed to investigate the feasibility of CBS as a relevant predictor of good clinical outcome in AIS cases. Methods:  A retrospective data collection and review of AIS patients in a teaching hospital was done from June 2010 until June 2015. Patients were selected following the inclusion and exclusion criteria. These patients were followed up after 90 days of discharge. The Modified Rankin scale (mRS) was used to assess their outcome (functional status). Linear regression Spearman Rank correlation was performed between the CBS and mRS. The quality performance of the correlations was evaluated using Receiver operating characteristic (ROC) curves. Results: A total of 89 patients with AIS were analysed, 67.4% (n=60) male and 32.6% (n=29) female. Twenty-nine (29) patients (33.7%) had a CBS ?6, 6 patients (6.7%) had CBS <6, while 53 patients (59.6%) were deemed clot free. Ninety (90) days post insult, clinical assessment showed that 57 (67.6%) patients were functionally independent, 27 (30.3%) patients functionally dependent, and 5 (5.6%) patients were deceased. Data analysis reported a significant negative correlation (r= -0.611, p<0.001). ROC curves analysis showed an area under the curve of 0.81 at the cut-off point of 6.5. This showed that a CBS of more than 6 predicted a good mRS clinical outcome in AIS patients; with sensitivity of 98.2%, specificity of 53.1%, positive predictive value (PPV) of 76%, and negative predictive value (NPV) of 21%. Conclusion: CBS is a useful additional variable for the management of AIS cases, and should be incorporated into the routine radiological reporting for acute ischemic stroke (AIS) cases.

Background: Nursing educators should train nursing students to pursue physical, psychological, spiritual, and social health promotion. The purpose of this study was to explore relationships between nursing students’ meaning of life, positive beliefs, and well-being. Methods: A cross-sectional correlational study with a quantitative approach was adopted. Purposive sampling was used. A total of 170 nursing students voluntarily participated in this study. A 56-item questionnaire was used to examine nursing students’ meaning of life (1-25 items), positive beliefs (1-11 items), and well-being (1-20 items). The content validity index (CVI) of the study questionnaire was established as 0.95 by seven expert scholars. The reliability values for the three parts of the measure were as follows: meaning of life, Cronbach’s α 0.96; positive beliefs, Cronbach’s α 0.93; and well-being, Cronbach’s α 0.95. Percentages, frequencies, means, SDs, Kruskal-Wallis one-way analysis of variance by rank, Spearman’s rank correlation, one-way analysis of variance, Spearman’s rho correlation, and regression analysis were used for the data analysis. Results: Nursing students had the following mean scores: meaning of life with 4.02 (SD 0.56); positive beliefs with 3.92 (SD 0.62); and well-being with 3.95 (SD 0.57). The results indicate that for all nursing students, meaning of life was positively correlated with positive beliefs, r=0.83 (P<.01); similarly, all nursing students had positive beliefs that were positively correlated with meaning of life, r=0.83 (P<.01). In the results of the study, the nursing students’ background, meaning of life and positive beliefs explained 63% of the variance in well-being (Adjusted R2 squared =0.63, F=33.41, P<.001). Conclusions: Nursing students’ sense of meaning of life and positive beliefs may impact their well-being. Therefore, nursing educators can promote meaning of life and positive beliefs to nursing students as a way to increase their well-being for physical, psychological, spiritual, and social health promotion.

ABSTRACT An important factor for the success of a CSR program is effective communication. Communication will be effective if it has an impact. If the information is conveyed based on the needs, then the communication will be effective. This study aims to analyze the factors which are related to the effectiveness of CSR communication. This study used a census method to approach 37 participants who received CSR programs. The Data analysis used the Spearman rank correlation for the statistical tests. The results showed that there was a correlation between factors that have the strength of CSR companion communication (level of attractiveness of the companion, quality of message content, and sources of information) which have real communication at the level of understanding of the participants of the Kertajaya Creative Destination (KCD) CSR program. In addition, there is a real correlation the factors that have the strength of CSR companion communication (the level of credibility of the companion, the source information, and the level of the recipient) and having communication at the level of attitudes of participants in the Kertajaya Creative Destination (KCD) CSR program. However, there is no real correlation between CSR companion communication factors and participant actions.Keywords :communication effectiveness, CSR, elements of communication. ABSTRAK Faktor penting dari keberhasilan program CSR adalah komunikasi yang efektif. Komunikasi dikatakan efektif jika menimbulkan dampak. Bila informasi tersampaikan sesuai dengan kebutuhan, maka komunikasi yang dijalankan efektif. Penelitian ini bertujuan untuk menganalisis efektivitas komunikasi pendamping CSR. Penelitian ini menggunakan pendekatan sensus terhadap 37 orang peserta penerima program CSR. Analisis data menggunakan uji statistik korelasi rank Spearman. Hasil penelitian menunjukkan bahwa terdapat hubungan nyata antara faktor efektivitas komunikasi pendamping CSR (derajat daya tarik pendamping, kualaitas isi pesan, dan sumber informasi)  dengan efektivitas komunikasi pada tingkat pemahaman peserta program CSR Kertajaya Creative Destination (KCD). Selain itu, terdapat hubungan nyata antara faktor efektivitas komunikasi pendamping CSR (tingkat kredibilitas pendamping, sumber informasi, dan tingkat penerima) dengan efektivitas komunikasi pada tingkat sikap peserta program CSR Kertajaya Creative Destination (KCD). Namun, tidak terdapat hubungan nyata antara faktor efektivitas komunikasi pendamping CSR dengan tindakan peserta. Kata Kunci : CSR, efektivitas komunikasi, unsur-unsur komunikasi.

Objective. Multi-patient care is important among medical trainees in an emergency department (ED). While resident efficiency is a typically measured metric, multi-patient care involves both efficiency and diagnostic / treatment accuracy. Multi-patient care ability is difficult to assess, though simulation is a potential alternative. Our objective was to generate validity evidence for a serious game in assessing multi-patient care skills among a variety of learners. Methods. This was a cross-sectional validation study using a digital serious game VitalSignsTM simulating multi-patient care within a pediatric ED. Subjects completed 5 virtual “shifts,” triaging, stabilizing, and discharging or admitting patients within a fixed time period; patients arrived at cascading intervals with pre-programmed deterioration if neglected. Predictor variables included generic multi-tasking ability, video game experience, medical knowledge, and clinical efficiency with real patients. Outcome metrics in 3 domains measured diagnostic accuracy (i.e. critical orders, diagnoses), efficiency (i.e. number of patients, time-to-order) and critical thinking (number of differential diagnoses); MANOVA determined differences between novice learners and expected expert physicians. Spearman Rank correlation determined associations between levels of expertise. Results. Ninety-five subjects’ gameplays were analyzed. Diagnostic accuracy and efficiency distinguished skill level between residency trained (residents, fellows and attendings) and pre-residency trained (medical students and undergraduate) subjects, particularly for critical orders, patients seen, and correct diagnoses (p < 0.003). There were moderate to strong correlations between the game’s diagnostic accuracy and efficiency metrics compared to level of training, including patients seen (rho = 0.47, p < 0.001); critical orders (rho = 0.80, p < 0.001); time-to-order (rho = −0.24, p = 0.025); and correct diagnoses (rho = 0.69, p < 0.001). Video game experience also correlated with patients seen (rho = 0.24, p = 0.003). Conclusion. A digital serious game depicting a busy virtual ED can distinguish between expected experts in multi-patient care at the pre- vs. post-residency level. Further study can focus on whether the game appropriately assesses skill acquisition during residency.

Abstract Background Brain arteriovenous malformations (bAVM) may rupture causing disability or death. BAVM vessels are characterized by abnormally high flow that in general triggers expansive vessel remodeling mediated by cyclo-oxygenase-2 (COX2), the target of non-steroidal anti-inflammatory drugs. We investigated whether COX2 is expressed in bAVMs and whether it associates with inflammation and haemorrhage in these lesions. Methods Tissue was obtained from surgery of 139 bAVMs and 21 normal Circle of Willis samples. The samples were studied with immunohistochemistry and real-time quantitative polymerase chain reaction (RT-PCR). Clinical data was collected from patient records. Results COX2 expression was found in 78% (109/139) of the bAVMs and localized to the vessels’ lumen or medial layer in 70% (95/135) of the bAVMs. Receptors for prostaglandin E2, a COX2-derived mediator of vascular remodeling, were found in the endothelial and smooth muscle cells and perivascular inflammatory cells of bAVMs. COX2 was expressed by infiltrating inflammatory cells and correlated with the extent of inflammation (r = .231, p = .007, Spearman rank correlation). COX2 expression did not associate with haemorrhage. Conclusion COX2 is induced in bAVMs, and possibly participates in the regulation of vessel wall remodelling and ongoing inflammation. Role of COX2 signalling in the pathobiology and clinical course of bAVMs merits further studies.
