During the nursing administration process, including preparation, management, and documentation, there is a high proportion of work interruption that results in nursing administration errors that consequently affect the safety of patients, increase the occurrence of nursing risks, and cause adverse reactions. To investigate the impact of nursing on drug withdrawal disputes in obstetrics and gynecology and reduce medical disputes are of great importance. In this study, documents related to medical disputes are collected and linked to relevant professional information systems for data collection and consultation. The patients are divided into groups and treated with different nursing methods, and then the correlation between nursing methods and disputes in different groups is computed. The results show that, based on the proposed nursing process, medical disputes caused by the cessation of nursing can be effectively reduced. Compared with traditional nursing methods, when the proposed nursing procedures are followed, the dispute rate is reduced by more than 10% and the possibility of serious disputes is also reduced with a reduction rate of about 15%. This shows that, based on the proposed nursing process, reengineering can play an effective preventive role in stopping obstetrics and gynecology nursing disputes.

AbstractIn the early 2000s, Japan instituted the Great Heisei Consolidation, a national strategy to promote large-scale municipal mergers. This study analyzes the impact that this strategy could have on watershed management. We select the Lake Kasumigaura Basin, the second largest lake in Japan, for the case study and construct a dynamic expanded input–output model to simulate the ecological system around the Lake, the socio-environmental changes over the period, and their mutual dependency for the period 2012–2020. In the model, we regulate and control the following water pollutants: total nitrogen, total phosphorus, and chemical oxygen demand. The results show that a trade-off between economic activity and the environment can be avoided within a specific range of pollution reduction, given that the prefectural government implements optimal water environment policies, assuming that other factors constraining economic growth exist. Additionally, municipal mergers are found to significantly reduce the budget required to improve the water environment, but merger budget efficiency varies nonlinearly with the reduction rate. Furthermore, despite the increase in financial efficiency from the merger, the efficiency of installing domestic wastewater treatment systems decreases drastically beyond a certain pollution reduction level and eventually reaches a limit. Further reductions require direct regulatory instruments in addition to economic policies, along with limiting the output of each industry. Most studies on municipal mergers apply a political, administrative, or financial perspective; few evaluate the quantitative impact of municipal mergers on the environment and environmental policy implications. This study addresses these gaps.

Gut microbiota plays an important role in human health. It may promote carcinogenesis and is related to several diseases of the gastrointestinal tract. This study of microbial dysbiosis in the etiology of colorectal adenoma aimed to investigate the possible causative role of microbiota in the adenoma–carcinoma sequence and its possible preventive role. A systematic, PRISMA-guided review was performed. The PubMed database was searched using “adenoma microbiota” and selecting original articles between January 2010 and May 2020 independently screened. A higher prevalence of Proteobacteria, Fusobacteria, and Bacteroidetes phyla was observed in the fecal luminal and mucosa-associated microbiota of patients with adenoma. However, other studies provided evidence of depletion of Clostridium, Faecalibacterium, Bacteroides and Romboutsia. Results on the relationship between adenoma endoscopic resection and microbiota were inconsistent. In conclusion, none of the analyzed studies developed a predictive model that could differentiate adenoma from non-adenoma patients, and therefore, to prevent cancer progression. The impact of adenoma’s endoscopic resection on microbiota was investigated, but the results were inconclusive. Further research in the field is required.

Based on forced vibration tests for high-performance concrete (HPC), the influence of bridge vibration induced by traveling vehicle on compressive strength and durability of HPC has been studied. It is concluded that 1 d and 2 d compressive strength of HPC decreased significantly, and the maximum reduction rate is 9.1%, while 28 d compressive strength of HPC had a slight lower with a 3% maximal drop under the action of two simple harmonic vibrations with 2 Hz, 3 mm amplitude, and 4 Hz, 3 mm amplitude. Moreover, the vibration had a slight effect on the compressive strength of HPC when the simple harmonic vibration had 4 Hz and 1 mm amplitude; it is indicated that the amplitude exerts a more prominent influence on the earlier compressive strength with the comparison of the frequency. In addition, the impact of simple harmonic vibration on durability of HPC can be ignored; this shows the self-healing function of concrete resulting from later hydration reaction. Thus, the research achievements mentioned above can contribute to learning the laws by which bridge vibration affects the properties of concrete and provide technical support for the design and construction of the bridge deck pavement maintenance.

The  presence  of  air  pollution  in  ambient  air  is  closely  related  to  the incidence  of  adverse reactions affecting human health. One of harmful pollutants and potentially major cause health problems is sulfur dioxide (SO 2 ). The number of vehicles that are passing and queuing on the crossroads  because  of  traffic light can  affect  the  concentration  of  SO 2 .  Besides,  in  these locations  there  are a lot of road users  which  are  potentially  exposed  by  contaminants, so information about the concentration of SO 2  is important to know. This study aimed to investigate the  impact  of  meteorological  factors  and  the  number  of vehicles  on  SO 2   concentrations. Impinger was used for air sampling, and pararosaniline method was used for determining SO 2  concentration. Sampling and calculation  of the number of passing vehicles were performed 3 times ie in the morning, afternoon and evening. Based on the results of the study, the highest concentrations of SO 2  were on the range of 15-21 mg/Nm3.

Objective: The aim of this meta-analysis was to discuss evidence supporting the efficacy of adjuvant human papillomavirus (HPV) vaccination in reducing the risk of recurrent cervical intraepithelial neoplasia (CIN) 2 or greater after surgical treatment. Methods: A systematic literature search was performed for studies reporting the impact of HPV vaccination on reducing the risk of recurrence of CIN 2+ after surgical excision. Results were reported as mean differences or pooled odds ratios (OR) with 95% confidence intervals (95% CI). Results: Eleven studies met the inclusion criteria and were selected for analysis. In total, 21,310 patients were included: 4039 (19%) received peri-operational adjuvant HPV vaccination while 17,271 (81%) received surgery alone. The recurrence of CIN 2+ after treatment was significantly lower in the vaccinated compared with the unvaccinated group (OR 0.35; 95% CI 0.21–0.56; p < 0.0001). The recurrence of CIN 1+ after treatment was significantly lower in the vaccinated compared with the unvaccinated group (OR 0.51; 95% CI 0.31–0.83; p = 0.006). A non-significant trend of reduction rate of HPV persistence was observed in the vaccinated compared with the unvaccinated cohorts (OR was 0.84; 95% CI 0.61–1.15; p = 0.28). Conclusions: HPV vaccination, in adjuvant setting, is associated with a reduced risk of recurrent CIN 1+ and CIN 2+ after surgical treatment.

ABSTRACT Vacuum cooling is a common practice in the California leafy green industry. This study addressed the impact of vacuum cooling on the infiltration of Escherichia coli O157:H7 into lettuce as part of the risk assessment responding to the E. coli O157:H7 outbreaks associated with leafy green produce from California. Vacuum cooling significantly increased the infiltration of E. coli O157:H7 into the lettuce tissue (2.65E+06 CFU/g) compared to the nonvacuumed condition (1.98E+05 CFU/g). A stringent surface sterilization and quadruple washing could not eliminate the internalized bacteria from lettuce. It appeared that vacuuming forcibly changed the structure of lettuce tissue such as the stomata, suggesting a possible mechanism of E. coli O157:H7 internalization. Vacuuming also caused a lower reduction rate of E. coli O157:H7 in stored lettuce leaves than that for the nonvacuumed condition.

Background. Malaria and schistosomiasis remain life-threatening public health problems in sub-Saharan Africa. The infection pattern related to age indicates that preschool and school-age children are at the highest risk of malaria and schistosomiasis. Both parasitic infections, separately or combined, may have negative impacts on the haemoglobin concentration levels. The existing data revealed that artemisinin derivatives commonly used to cure malaria present also in antischistosomal activities. The current study investigated the impact of Artesunate-Amodiaquine (AS-AQ) on schistosomiasis when administered to treat malaria in rural area of Lemfu, DRC. Methodology. A prospective longitudinal study including 171 coinfected children screened for anaemia, Schistosoma mansoni, and Plasmodium falciparum infections. The egg reduction rate and haemoglobin concentration were assessed four weeks after the treatment with AS-AQ, of all coinfected children of this series. Results. One hundred and twenty-five (74.4%) out of 168 coinfected children treated and present during the assessment were found stool negative for S. mansoni eggs. Out of 43 (25.6%) children who remained positives, 37 (22%) showed a partial reduction of eggs amount, and no reduction was noted in 3.6% of coinfected. The mean of haemoglobin concentration and the prevalence of anaemia were, respectively, 10.74±1.5g/dl , 11.2±1.3g/dl, and 64.8%, 51.8%, respectively, before and after treatment, p<0.001. Conclusion. The AS-AQ commonly used against Plasmodium allowed curing S. mansoni in coinfected children and increasing the Hb level. For the future, the randomized and multicentric clinical trials are needed for a better understanding of the effectiveness of AS-AQ against Schistosoma spp. The trial registration number was 3487183.

Because of the problem of local scour caused by the change of the flow structure caused by the water-resistance of the column bridge pier, the theoretical analysis, and indoor water tank test were used to study the effect of installing a new anti-scouring device in front of the bridge pier on the local scour reduction effect; the influence of the main design parameters such as the height of the protective device, the angle of the protective device and the distance from the protective device to the bridge pier on the local scour of the bridge pier was selected, and the optimal parameter design combination was selected. The test results show that: under the same water flow conditions, the maximum scour depth reduction rate of the measuring point under the protection of the protective device is 48.4% to 74.2% compared with the unprotected scour; the reduction rate of the bridge pier is relative to the relative height of the device and the device equivalent. The angle and the distance between the device and the bridge pier are related, and the shock reduction rate decreases with the increase of the flow intensity. In the test range, the ratio of the device height to the water depth is 2/3, the device angle is 60 °, and the distance from the bridge pier is 3. When the diameter of the pier is doubled, the effect of reducing the impact on the pier is the best.

Abstract Background Previous studies have proven that the closure of live poultry markets (LPMs) was an effective intervention to reduce human risk of avian influenza A (H7N9) infection, but evidence is limited on the impact of scale and duration of LPMs closure on the transmission of H7N9. Method Five cities (i.e., Shanghai, Suzhou, Shenzhen, Guangzhou and Hangzhou) with the largest number of H7N9 cases in mainland China from 2013-2017 were selected in this study. Data on laboratory-confirmed H7N9 human cases in those five cities were obtained from the Chinese National Influenza Centre. The detailed information of LPMs closure (i.e., area and duration) was obtained from the Ministry of Agriculture. We used a generalized linear model with a Poisson link to estimate the effect of LPMs closure, reported as relative risk reduction (RRR). We used classification and regression trees (CARTs) to select and quantify the dominant factor of H7N9 infection. Results All five cities implemented the LPMs closure, and the risk of H7N9 infection decreased significantly after LPMs closure with RRR ranging from 0.80-0.93. Respectively, a long-term LPMs closure for 10-13 weeks elicited a sustained and highly significant risk reduction of H7N9 infection (RRR = 0.98). Short-time LPMs closure with 2 weeks in every epidemic did not reduce the risk of H7N9 infection (p>0.05). Partially closed LPMs in some suburbs contributed only 35% for reduction rate (RRR=0.35). Shenzhen implemented partial closure for first 3 epidemics (p>0.05) and all closure in the latest 2 epidemic waves (RRR=0.64). Conclusion Our findings suggest that LPMs all closure in whole city can be a highly effective measure comparing with partial closure (i.e. only urban closure, suburb and country remain open). Extend the duration of closure and consider permanently closing the LPMs will help improve the control effect. The effect of LPMs closure is greater than that of meteorology on H7N9 transmission.
