SummaryCurrent guidelines recommend optimised algorithms for diagnosis of suspected deep-vein thrombosis (DVT). There is little data to determine to what extent real-world health care adheres to guidelines, and which outcome in terms of diagnostic efficiency and safety is achieved. This registry involved patients with clinically suspected DVT of the leg recruited in German ambulatory care between October and December 2005. Registry items were: diagnostic methods applied; diagnostic categories at day 1; and venous thromboembolic events up to 90 days in patients without firmly established DVT. A total of 4,976 patients were recruited in 326 centres. Venous ultrasonography was performed in 4,770 patients (96%), D-dimer assay in 1,773 patients (36%) and venography in 288 patients (6%). At day 1, DVT was confirmed in 1,388 patients (28%), and ruled out in 3,389 patients (68%), and work-up was inconclusive in 199 patients (4%).The rate of venous thromboembolism at 90 days was 0.34% (95% confidence interval [CI]: 0.09 to 0.88) in patients in whom the diagnosis of DVT had been ruled out, and 2.50% (95% CI: 0.69 to 6.28) in patients with inconclusive diagnostic workup. This nationwide evaluation in German ambulatory care revealed that the diagnostic work-up for suspected DVT did not adhere to current guidelines. However, the overall diagnostic safety was excellent, although there is potential for improvement in a well defined minority of patients.The TULIPA registry was funded by GlaxoSmithKline GmbH und Co KG, Munich.

The projects in this chapter introduce MIDI recording, sequencing, and editing techniques for composition and songwriting. The projects include strategies for enabling students to quickly generate a large amount of original musical material quickly. Beginner songwriters and composers find empty DAW sessions to be daunting, so it is essential to build their confidence. Therefore, the chapter gives techniques for quickly generating melodic and drum sequences, duplicating them, and creating variations on them. This approach also has the benefit of mirroring “real-world” professional production practice. The projects cover drum programming and arranging, as well as writing chord sequences, basslines, and short melodies. A pragmatic instrument taxonomy of “drums” and “not-drums” is used, and the typical instrumental layers in contemporary electronic music genres are explained. Also presented is a practical approach to relevant music theory concepts. Finally, three popular genres—future bass, house music, and trap—are used to introduce musical form.

For the first time, a detailed study has been carried out on the real world of people suffering from high blood pressure, involving the doctors who are directly in contact with them, who shoulder the responsibility of the diagnosis and make the decisions about treatment. Data were collected from fifty-eight patients diagnosed and monitored in the clinics by twenty-nine specialist physicians who supplied information about the diagnostic work-up, the follow-up and the therapy. The data were collected using an online questionnaire and were processed statistically. The result is a cross-section of interesting and original results, featuring a number of peaks of excellence and various troughs of negative criticality, which call for reflection and for decisions on action to be taken. The figures are hence useful for both clinicians and epidemiologists and all those who are involved with medical governance.

ObjectiveTo test the current progressive multifocal leukoencephalopathy (PML) diagnostic criteria by applying them to patients previously diagnosed with natalizumab (NTZ)-associated PML in a real-world clinical setting.MethodsPatients from the Dutch-Belgian NTZ-PML cohort (n=28) were reviewed at the time of first diagnostic work-up and during follow-up, using the PML diagnostic criteria as proposed in a consensus statement from the American Academy of Neurology.ResultsAt first diagnostic work-up, 18 patients (64.3%) met the criteria for high diagnostic certainty for PML (‘definite PML’ or ‘probable PML’). During follow-up, this increased to 20 patients (71.4%) as JC virus DNA was detected in cerebrospinal fluid of two additional patients. Nonetheless, 28.6% of patients were still classified as ‘possible PML’ or ‘not PML’ (6 (21.5%) and 2 (7.1%) patients, respectively) despite a very high suspicion for PML based on lesion evolution and signs of PML-immune reconstitution inflammatory syndrome on MRI, and development of compatible symptoms.ConclusionsThe current case definition of PML has low sensitivity for diagnosis of NTZ-PML in a real-world clinical setting in which MRI is frequently used for PML screening. This may delay diagnosis and appropriate management of PML, and may complicate a valid estimation of PML incidence during NTZ therapy.
