Environmental control measures help eliminate triggers that initiate the allergic reaction and reduce the conditions that sustain it. The most frequent offenders are the ubiquitous dust mites, which are ideally suited to living in the home environment. Control measures consist of minimizing their reservoirs in the patient's bedding, carpets, and upholstered furniture; decreasing humidity; and using acaricides. Cats are the source of another important indoor allergen that becomes and stays airborne as small particies. Removal of the animal (or washing it weekly) and the use of high-efficiency particulate air filters for air ducts and vacuum cleaners are useful in reducing dust mite and cat allergens. Dogs and rodents also produce allergens offensive to the sensitized patient and should be removed or relegated outdoors. Cockroaches have become an increasingly prevalent source of allergens responsible for asthma and rhinitis. Their removal is the focus of research, because current control measures are usually inadequate. Molds, which thrive in any moist environment, produce allergens. Closed windows prevent further influx of outdoor molds and pollens, whereas those harbored indoors, including those residing on plants, should be eliminated.

House dust mites produce potent allergens that exacerbate asthma in sensitized patients, whom are recommended to practice allergen avoidance within their home environment. We tested the effect of activated charcoal impregnated fibers on house dust mite survival. One hundred live adult house dust mites (Dermatophagoides pteronyssinus) were added to eight culture dishes preequilibrated at room temperature () and 70% humidity () containing house dust mite food and active charcoal fibers. At 10 minute intervals, live and dead house dust mites were counted. All house dust mites instantly attached to the activated charcoal fibers and started to shrink almost immediately. There were no live house dust mites present as early as 40 minutes in some dishes while after 190 minutes all house dust mites were dead. In conclusion, activated charcoal fibers, if incorporated into bedding items, have the potential to control house dust mites in the indoor environment.

The authors conclude that major risk factors to develop recurrent wheezing in school-age children include the exposure to such aeroallergens as house dust mite, animal dander, particularly cat, and mold spores in addition to ETS. They feel that a reduction in indoor aeroallergens and ETS could reduce recurrent wheezing by 65% and they make a great case for implementing strong environmental control measures, praticularly in those with a strong family history of asthma.

The water quality in the 12 priority watercourses of Hong Kong was appraised in respect of the various environmental control measures being undertaken. It was noted that water quality in Lam Tsuen River had been significantly improved since 1983. This is attributed to recent resumption of unsewered lands for town development, training of river basin to increase flow, and declaration of the catchment as a “Water Control Zone” under the Water Pollution Control Ordinance. In contrast with the other heavily polluted watercourses to which little abatement measures were implemented, the water qualities of Shing Mun River and Tuen Mun River were slightly upgraded because of the efforts to rectify unauthorized industrial discharges back to foul sewer and provision of interceptors and sewers to villages. The 10 year Livestock Waste Control Scheme enforced on 24 June 1989 was found in parallel with BOD and suspended solids decrease in watercourses. To assess its effectiveness, however, a longer term monitoring is required to get a conclusive result.

Despite dramatic reductions in malaria cases in the catchment area of Macha Hospital, Choma District, Southern Province in Zambia, prevalence has remained near 1–2% by RDT for the past several years. To investigate residual malaria transmission in the area, this study focuses on the relative abundance, foraging behavior, and phylogenetic relationships of Anopheles squamosus specimens. In 2011, higher than expected rates of anthropophily were observed among “zoophilic” An. squamosus, a species that had sporadically been found to contain Plasmodium falciparum sporozoites. The importance of An. squamosus in the region was reaffirmed in 2016 when P. falciparum sporozoites were detected in numerous An. squamosus specimens. This study analyzed Centers for Disease Control (CDC) light trap collections of adult mosquitoes from two collection schemes: one performed as part of a reactive-test-and-treat program and the second performed along a geographical transect. Morphological identification, molecular verification of anopheline species, and blood meal source were determined on individual samples. Data from these collections supported earlier studies demonstrating An. squamosus to be primarily exophagic and zoophilic, allowing them to evade current control measures. The phylogenetic relationships generated from the specimens in this study illustrate the existence of well supported clade structure among An. squamosus specimens, which further emphasizes the importance of molecular identification of vectors. The primarily exophagic behavior of An. squamosus in these collections also highlights that indoor vector control strategies will not be sufficient for elimination of malaria in southern Zambia.

Background: Transmission of healthcare-associated pathogens such as Clostridioides difficile and methicillin-resistant Staphylococcus aureus (MRSA) is a persistent problem in healthcare facilities despite current control measures. A better understanding of the routes of pathogen transmission is needed to develop effective control measures. Methods: We conducted an observational cohort study in an acute-care hospital to identify the timing and route of transfer of pathogens to rooms of newly admitted patients with negative MRSA nares results and no known carriage of other healthcare-associated pathogens. Rooms were thoroughly cleaned and disinfected prior to patient admission. Interactions of patients with personnel and portable equipment were observed, and serial cultures for pathogens were collected from the skin of patients and from surfaces, including those observed to come in contact with personnel and equipment. For MRSA, spa typing was used to determine relatedness of patient and environmental isolates. Results: For the 17 patients enrolled, 1 or more environmental cultures became positive for MRSA in rooms of 10 patients (59%), for C. difficile in rooms of 2 patients (12%) and for vancomycin-resistant enterococci (VRE) in rooms of 2 patients (12%). The patients interacted with an average of 2.4 personnel and 0.6 portable devices per hour of observation. As shown in Figure 1, MRSA contamination of the floor occurred rapidly as personnel entered the room. In a subset of patients, MRSA was subsequently recovered from patients’ socks and bedding and ultimately from the high-touch surfaces in the room (tray table, call button, bedrail). For several patients, MRSA isolates recovered from the floor had the same spa type as isolates subsequently recovered from other sites (eg, socks, bedding, and/or high touch surfaces). The direct transfer of healthcare-associated pathogens from personnel or equipment to high-touch surfaces was not detected. Conclusions: Healthcare-associated pathogens rapidly accumulate on the floor of patient rooms and can be transferred to the socks and bedding of patients and to high-touch surfaces. Healthcare facility floors may be an underappreciated source of pathogen dissemination not addressed by current infection control measures.Funding: NoneDisclosures: None

The successful management of upper respiratory tract allergy is based on a triad of interdependent approaches that, with rare exceptions, must all be considered in every allergic patient. They consist of allergen avoidance with environmental control measures, pharmacotherapy with use of a step-care approach, and immunotherapy. Environmental control measures should be used to prevent events that trigger and sustain the condition. Once started, the allergic reaction includes the release of mediators of inflammation that produce well-known symptoms of allergic rhinitis. Treatment should consist of a step-wise, rational approach that includes site-based therapy with one or more drugs acting at different sites. The drugs used are antihistamines, decongestants, cromolyn sodium, and corticosteroids. Immunotherapy should be considered at any step, because it offers the only curative approach.
