Abstract   Diabetic cardiovascular autonomic neuropathy (CAN) encompasses damage to the autonomic nerve fibers innervating the heart and blood vessels, resulting in abnormalities in heart rate control and vascular dynamics. Clinical symptoms associated with CAN include resting tachycardia, postural hypotension, dizziness, syncope, exercise intolerance, silent myocardial infarction. The earliest finding of CAN, even at the subclinical stage, is a decrease in heart rate variability (HRV) Purpose To study CAN prevalence and understand the diagnostic capabilities of HRV - tests of diabetic cardiovascular autonomic neuropathy in children with type 1 diabetes. Methods 100 children with type 1 diabetes were assessed for CAN (age 15.2 yrs [9.0–17.8], duration 6.5 yrs [4.0–10.6], HbA1c 8.1% 6.3–9.7]). The degree of compensation of diabetes was estimated according to criteria ISPAD Consensus Guidelines, 2018. Optimal level (HbA1c <7.0%) of compensation was revealed in 62% of children (group I) and nonoptimal in 38% of children (group II). For evaluation of the CAN we used HRV - tests: RR 30:15 ratio, ΔRR quiet breath, ΔRR deep breath, BP response to standing. Results Diabetic cardiovascular autonomic neuropathy was diagnosed in 41 children: in group I – in 6 (29%) and in group II – 35 (71%) children (p=0.00001). The prevalence of DCN correlated with HbA1c level in children and adolescents (groups I and II): ΔRR quiet breath r=−0.47 & r=−0.9; ΔRR deep breath r=−0.65 & r=−0.85; RR30:15 r=−0.77 & r=−0.88 respectively. Prevalence of CAN increased parallel to type 1 diabetes duration (p<0.01). Conclusions Diabetic cardiovascular autonomic neuropathy in children with type 1 diabetes was diagnosed in 41%. Prevalence of diabetic cardiovascular autonomic neuropathy increases parallel to type 1 diabetes duration (p<0.01). In this study we found an association between HRV indexes and HbA1c level. This provides support for HRV - testing to assess diabetic cardiovascular autonomic neuropathy in children. Funding Acknowledgement Type of funding source: None

AbstractThis paper is an empirically-based theoretical contribution to the field of research that investigates the function of trust and re-enactment in psychotherapeutic interaction. We use an ecological, embodied approach that pays attention to how human interaction is constrained by multiple timescales (past, present and future). The analysis sheds light on how trust, here in terms of a therapeutic alliance, is enabled, performed and maintained in interaction through the work with embodied re-enactments of previous events. Specifically, we describe how this therapeutic work constitutes an emerging, situated opportunity for teaching/practising embodied emotion regulation in the form of a co-participated enaction of “taking a deep breath,” and we emphasise how embodied, co-participated re-enactment of past (dys)functional behaviours outside of therapy can be a resource for redirecting, teaching and reinforcing therapeutically relevant behaviours in the context of therapy presenting themselves as fruitful opportunities for facilitating incremental change. Further, psychotherapy serves as a useful case for demonstrating the relevance of such an embodied interaction approach far more generally.

Breathing retraining alters respiratory muscle recruitment and improves respiratory muscle performance. However, the effectiveness of Pursed Lip Breathing in reducing dyspnoea is controversial. The aims of this study are to determine if breathing retraining is influencing the Single Breath Count performed by dyspneic subjects and to assess the correlation between Single breath Count Test and Snider match test as the standard measures of bedside pulmonary function test in patients with dyspnoea. 100 Patients with dyspnoea were assessed in the baseline with snider match test and they were asked to perform a single breath count. 5 times weekly and for 4 weeks, the participants have been practicing Breathing Retraining. Pursed lip breathing (PLB) and diaphragmatic breathing (DB) are breathing retraining strategies to subjects with dyspnoea; Single Breath Count was measured by asking patients to take a deep breath and count as far as possible in their normal speaking voice without taking another breath. Counting was timed to a metronome set at 2 counts per second. Snider match test was conducted by blowing the matches at increasing the test distance from 3, 6, 9 inches. The result of this study will determine whether breathing retraining is safe and beneficial for people with dyspnoea.

Distributivity in natural language occurs in sentences such as John and Mary (each) took a deep breath, when a predicate that is combined with a plurality-denoting expression is understood as holding of each of the members of that plurality. Language provides ways to express distributivity overtly, with words such as English each, but also covertly, when no one word can be regarded as contributing it. Both overt and covert distributivity occur in a wide variety of constructions. This article reviews and synthesizes influential approaches to distributivity in formal semantics and includes pointers to some more recent approaches. Theories of distributivity can be distinguished on the basis of how they answer a number of interrelated questions: To what extent can distributivity be attributed to what we know about the world, as opposed to the meanings of words or silent operators? What is the relationship between distributivity and plurality? Does distributivity always reach down to the singular individuals in a plurality? If not, under what circumstances is distributivity over subgroups possible, and what is its relation to distributivity over individuals?
