Abstract Background Ethiopia embarked on combating malaria with an aim to eliminate malaria from low transmission districts by 2030. A continuous monitoring of malaria prevalence in areas under elimination settings is important to evaluate the status of malaria transmission and the effectiveness of the currently existing malaria intervention strategies. The aim of this study was to assess the prevalence of malaria and associated risk factors in selected areas of Dembiya district. Methods A cross-sectional parasitological and retrospective survey was conducted in the two localities of Dembiya District, selected based on their long standing history of implementing malaria prevention and elimination strategies. Thin and thick blood smears collected from 735 randomly selected individuals between October and December, 2018 were microscopically examined for malaria parasites. Six years (2012–2017) retrospective malaria data was collected from the medical records of the health centres. Structured questionnaires were prepared to collect information about the socio-economic data of the population. Logistic regression analysis was used to determine a key risk factor explaining the prevalence of malaria. The data were analysed using SPSS version 20 and p ≤ 0.05 were considered statistically significant. Results The 6-year retrospective malaria prevalence trend indicates an overall malaria prevalence of 22.4%, out of which Plasmodium falciparum was the dominant species. From a total of 735 slides examined for the presence of malaria parasites, 3.5% (n = 26) were positive for malaria parasites, in which P. falciparum was more prevalent (n = 17; 2.3%), Plasmodium vivax (n = 5; 0.7%), and mixed infections (n = 4; 0.5%). Males were 2.6 times more likely to be infected with malaria than females (AOR = 2.6; 95% CI 1.0, 6.4), and individuals with frequent outdoor activity were 16.4 times more vulnerable than individuals with limited outdoor activities (AOR = 16.4, 95% CI 1.8, 147.9). Furthermore, awareness about malaria transmission was significantly associated with the prevalence of malaria. Conclusions Malaria is still a public health problem in Dembiya district irrespective of the past and existing vector control interventions. Therefore, the authorities should work on designing alternative intervention strategies targeting outdoor malaria transmission and improving community awareness about malaria transmission and control methods in the study area. For this, continuous monitoring of vectors’ susceptibility, density, and behaviour is very important in such areas.

Background. Visceral leishmaniasis (VL, also called kala-azar) is a public health problem in Ethiopia, especially in sesame and sorghum growing areas. Compared to other populations, labor migrants are the most exposed. Knowing the seroprevalence ofLeishmania donovaniand associated risk factors is essential to design appropriate control measures. The main aim of this study was to assess the seroprevalence of asymptomaticL. donovaniamong laborers and associated risk factors in agricultural camps of West Armachiho district, Northwest Ethiopia. Therefore, this study was conducted to know the seroprevalence and associated risk factors ofL. donovaniinfection.Method. A cross-sectional study was conducted among 185 laborers from October to December 2017. A simple random sampling technique was used to select study participants from selected agricultural camps. After obtaining written informed consent, data were collected using a structured pretested Amharic version questionnaire using the interview technique. A single finger prick blood sample was collected from the study participants and the blood samples were subjected to the serological diagnostic method using the rk39 kit. The multivariable logistic regression model was used to identify risk factors associated withL. donovaniinfection.Result. Among 185 participants examined using rk39, 14 (7.6%) were seroreactive forL. donovani.Leishmania donovaniinfection had a statistically significant association with sleeping underBalanitestrees (AOR: 4.36, 95%CI: 1.186-16.06), presence of domestic animals near sleeping place (AOR: 4.68, 95% CI: 1.25-17.56), and lack of knowledge about VL transmission (AOR: 3.79, 95% CI: 1.07-13.47).Conclusion. Seroprevalence of asymptomaticL. donovaniamong laborers in agricultural camps of West Armachiho was low. Prevention measures and health education about risk factors that expose toL. donovaniinfection for the laborers are essential to prevent the spread of the disease.

AbstractVitamin D deficiency is an epidemic public health problem worldwide. It is common in the Middle East and is more severe in women. This cross-sectional study was conducted to assess vitamin D deficiency and associated risk factors in women living in Riyadh, Saudi Arabia. Serum 25-hydroxyvitamin D (25(OH)D) was measured in 166 women aged 30–65 years. Socio-demographic, lifestyle and health status characteristics, as well as intake of selected dietary supplements, were collected. Weight and height were measured. Vitamin D deficiency (25(OH)D < 20 ng/mL) was reported in 60.2% of participants. Mean of serum 25(OH)D was 20.7 ng/mL. Older age and taking the supplements of vitamin D, multi-vitamins or calcium were identified as factors that associated with a lower risk of hypovitaminosis D. A national strategy is needed to control a hypovitaminosis D crisis in Saudi Arabia. This could be accomplished by raising public awareness regarding vitamin D, regulating and enhancing vitamin D fortification and supplementation and screening vitamin D status among women at high risk.

Abstract Background: Although much progress has been made in reducing malaria morbidity and mortality worldwide in the last decade, malaria still remains the third leading cause of death and still considered as major public health problem. Objectives : The main objective of this study was to assess malaria as a public health problem in and around sugar cane plantation area of Arjo Didhessa sugar factory, Western Ethiopia. Methods: A community based cross sectional study supplemented with clinical retrospective data, which included 452 study subjects was employed and the study period extended from May 2016 up to November of 2017. A standardized questionnaire was used to assess malaria risk factors and blood samples were received from all study participants and further subjected to Giemsa staining for determination of malaria prevalence. Data was analyzed by SPSS version 20.Malaria risk factors were identified by multivariate logistic regression at significance level of P < 0.05 . Results: The overall malaria prevalence was 3.1%; Plasmodium vivax being the main type of malaria parasite. Overnight outdoor sleeping and improper utilization of mosquito bed nets were found to be statistically significant as malaria risk factors in the community. In the retrospective studies of five years, the peak malaria cases (13.84%) were reported in 2013 and less cases (1.24%) in 2017. Conclusion: The prevalence of malaria observed in the area is still higher as compared to national prevalence of malaria. Therefore; we recommend further strengthening of malaria prevention and control strategies. Additionally, educative training opportunities must be provided for workers in the plantation area on malaria prevention and control.

ABSTRACTBackgroundDespite high coverage and successes in malaria control strategies, some areas of Tanzania have indicated stagnantion or revesal of malaria burden. In malaria research, most studies are designed to assess drivers of malaria transmission focusing only on one dimension, single location while very few studies assess multiple components and their interactions at once. This article describes the protocol used to assess intrinsic and extrinsic drivers of persistent malaria transmission (hotsposts) in four regions from northwestern (Geita and Kigoma) and southern (Ruvuma and Mtwara) Tanzania.MethodsThis cross-sectional study was conducted between July and November 2017 in eight districts (two from each region). Based on the health facilities records, two villages were selected from each district. The study assessed five components individually and their linkages: socio-economic and malaria risk factors, parasitological, entomological, socio-anthropological and health system factors. Households (HHs) and household members were enumerated, socio-economic status and risk factors associated with malaria transmission were assessed. A total of 120 HHs were sampled from each village where malaria testing using rapid test and microscopy were done and blood spots on filter papers for genetic studies were collected. Heads of HHs were interviewed to capture information on knowledge, attitude, practice and beliefs towards malaria and its control. Assessment of adult mosquitoes in 25 HHs in each village and complimented with assessment of immature mosquitoes through larvae was conducted. The performance of the health system was assessed with respect to Information on availability, accessibility, affordability and quality of malaria prevention and case management services were collected from these health facilities.DiscussionThe proposed analysis plan and results from this study are expected to determine factors potentially responsible for persistence of malaria (hotspots) in the study areas. Rather than the traditional methodology of focusing on one metric, the approach proposed here triangulates observations from all five components, highlighting understanding of potential drivers while studying their complex interactions and map spatial heterogeneity. This study will provide an important framework and data which will guide future studies and malaria surveillance in Tanzania and other malaria endemic countries.

Background Syphilis and HIV can be transmitted from pregnant women to their children and they remain a public health problem in Africa. Our study aimed to determine the trends of seroprevalence of HIV/syphilis co-infection and syphilis infection overtime through the national surveillance system in Cameroon and to explore associated risk factors. Methods We conducted cross-sectional studies of HIV and syphilis, targeting each year 7000 first antenatal care (ANC-1) attendees at the same sites during the 2009, 2012 and 2017 sentinel surveillance surveys. Pregnant women were enrolled at their ANC-1, sociodemographic and clinical information were collected. HIV and Syphilis test were performed by serial algorithm as per the national guidelines. Trends were assessed for HIV, syphilis and HIV/syphilis by estimating seroprevalence from cross-sectional studies. Associated risk factors were explored using multinomial logistic regression with 4 outcomes: HIV/syphilis co-infection, HIV infection only, syphilis infection only and no infection. Results Overall, 6 632, 6 521 and 6 859 pregnant women were enrolled in 2009, 2012 and 2017 respectively. In 2017, a total of 3 901 pregnant women enrolled were tested for syphilis. Almost half of them (47.9%) were living in urban area and were aged less than 25 years (44.7%). While HIV epidemic was on a decline (from 7.6% (95% CI: 6.99–8.28) in 2009 to 5.7% (95% CI: 4.93–6.4) in 2017), a huge significant increase of syphilis prevalence was observed (from 0.6% (95% CI:0.40–0.80) in 2009 to 5.7% (95% CI:4.93–6.40) in 2017). Pregnant women residing in rural areas were more likely to be infected with syphilis than those living in the urban area (aOR = 1.8 [95% CI: 1.3–2.4]). Unmarried pregnant women were three time more likely to be infected by HIV/Syphilis Co-infection than married, cohabiting, widow or divorced pregnant women (aOR = 2.8 [95% CI: 1.3–2.4]). Furthermore; living in Northern region was associated with a lower risk of being infected with HIV (aOR = 0.6 [95% CI: 0.5–0.9]) and Syphilis infection (aOR = 0.6 [95% CI: 0.4–0.9]). Conclusion The epidemiological dynamics of syphilis suggests a growing burden of syphilis infection in the general population of Cameroon. Our findings support the fact that while emphasizing strategies to fight HIV, huge efforts should also be made for strategies to prevent and fight syphilis infection especially among HIV positive women, in rural area, and southern regions.

Background: Hypertension is one of the non-communicable diseases, which is an important global public health problem and also the most leading cause for cardiovascular disease in both developed and developing countries. The aim and objectives of the study were to estimate the prevalence of hypertension and to assess the associated risk factors among adults of rural population.Methods: A community based cross sectional study was done on 225 study subjects, using systemic random sampling method at Kancheepuram district, Tamil Nadu. Results: The study included 225 participants of which 53% were male and 47% were female. The overall prevalence of hypertension among the study population was 26.2%. Risk among male was greater than female (OR=1.390). Factors like age, body mass index, diet, family history of hypertension had significant association (p<0.05) with hypertension. Conclusions: Regular screening for hypertension among adults should be done in the general OPD as well as in health camps so that they have a knowledge regarding their own blood pressure status. Among those who already have hypertension, anti -hypertensive drugs, timely blood pressure check-up, screening for cardio vascular diseases and self-care management of hypertension need to be promoted.

Abstract Background- Malaria is one of the leading causes of morbidity and mortality particularly in sub-Saharan Africa including Ethiopia. Approximately, 68% of its population is at a risk of malaria infection and nearly three-fourth of the Ethiopia landmass is considered as malarious. Most of the malaria related researches are being conducted in the rural areas; however, urban malaria should be also targeted without which malaria elimination is impossible. Therefore, the aim of this study was to assess the magnitude of malaria, prevalence of P. falciparum and P. vivax and its associated risk factors during low transmission season (April to July, 2018) in Batu town, East Shoa, Oromia Regional state, Ethiopia.Methods – Finger pricked blood samples were collected from 356 febrile individuals visiting health facilities in Batu town from April to July, 2018 supplemented with questionnaire survey to assess socio-demographic and risk factors associated with malaria infection.Results – In the current study an overall prevalence of 17.13% (61/356) malaria infection was observed of which 50.8% (31/61) were positive for P. vivax, 45.95% (28/61) were positive for P. falciparum and 3.3% (2/61) were mixed infections of P. falciparum and P. vivax. Male individuals (32 (9%)) were more infected with malaria than females (29 (8.2%)). Sex had no statistical significance association with malaria (χ2 = 2.026, P = 0.156). Logistic regression analysis revealed that individuals who possessed ITN (OR = 0.38, 95% CI (0.194, 0.743) and P = 0.005) and whose houses were sprayed with insecticides (OR = 0.18, 95% CI (0.097, 0.34) and P < 0.001) were found less likely to get malaria infection. Individuals living closer to stagnant water had more chance (almost three-fold times) of malaria infection than those who were found at a distant from stagnant water (OR= 2.97, 95% CI (1.68, 5.26) and P < 0.001).Conclusion- The current study revealed that malaria is still public health problem in Batu town, which might indicate the same in other urban areas of the country. Therefore, in order to get the national picture about the prevalence of urban malaria and its associated risk factors, we recommend large scale investigation of the situation.

Chronic kidney disease (CKD) is a global major public health problem. Almost all of previous studies evaluating the prevalence of CKD focused on adults, while studies among the elderly were relatively rare, especially in China. The aim of this study was to investigate the prevalence and associated risk factors of CKD among the elderly in Qingdao, China. This was a cross-sectional study with 38,038 inhabitants (aged 60–109) randomly recruited in Qingdao, China. All participants were required to complete a questionnaire for their demographic characteristics. Blood and urine samples of participants were collected, and the albumin and creatinine levels were measured for albuminuria and estimated glomerular filtration rate (eGFR) assessment. The associations between risk factors and indicators of kidney damage were analyzed by logistic regression. A total of 34,588 inhabitants completed the survey. The overall prevalence of CKD was 11.41% (95% confidence interval (CI): 11.07–11.74%) in the elders from Qingdao in 2016. The prevalence of albuminuria and low eGFR (<60 mL/min per 1·73 m²) were 8.47% (95% CI: 8.17–8.76%) and 3.98% (95% CI: 3.78–4.19%), respectively. Older age, hypertension, diabetes, anemia, hyperuricemia, hyperhomocysteinemia, hypertriglyceridemia, obesity, and LDL-C ≥ 4.1 mmol/L were independently associated with the presence of CKD. In conclusion, common chronic non-communicable diseases, including hypertension, diabetes, obesity, hyperhomocysteinemia, hyperuricemia, and hypertriglyceridemia, were associated with greater prevalence of CKD.

Abstract Background Hookworm is a major public health problem in Ethiopia. The prevalence of Hookworm and associated factors among podoconiosis patients and healthy residents has not been studied.Objective to determine the prevalence of Hookworm and associated risk factors among podoconiosis patients and healthy residents.Methods A cross-sectional study was conducted at Debre Elias from January to February 2018. One hundred ninety two participants were included. Stool sample was collected for microscopy, kato-katz and formalin ether concentration technique. Logistic regressions were performed to assess association between variables. P-value < 0.05 was used as statistically significant.Result The prevalence of Hookworm among podoconiosis patients was 84.4% and 75% among control. Hookworm infection was relatively higher among podoconiosis patients 52.9%. Higher age group, illiterate and farmers more affected with high proportion of podoconiosis patients. Being a formers, no history of stool examination and no shoe wearing habit were significantly associated with hookworm infection (AOR = 4.1; 95% CI= (1.65- 10.11)), (AOR = 3.03; 95% CI= (1.11-8.25)) and (AOR= 3.9; 95% CI= (1.11-14.24) respectively.
