Rehabilitation of infertile patients after reconstructive plastic surgery on the pelvic organs

The research aims at studying the effectiveness of plasmapheresis and ozonotherapy in treatment of women of reproductive age in early postoperative period after reconstructive-plastic operations. 110 patients with different factors of infertility who passed surgical laparoscopy were examined. All the patients were rehabilitated applying medicinal plasmapheresis (70 patients) and ozonotherapy (40 patients). The results of the study showed the advantages of these methods of treatment in comparison with traditional therapy (antibacterial and physiotherapy in comparative group, 72 patients). So, early rehabilitation of patients including medicinal plasmapheresis and ozonotherapy makes it possible to increase essentially the course of postoperative period, to stabilize adaptive reactions, to recover reproductive function in considerable amount of infertile women.

The objective: in this work represented part of materials of department rehabilitation of reproductive function for the examination and treatment of 250 patients with uterine fibrods in order to study and determine in detail the predictors of the disease, develop methods of treatment (conservative and surgical), as well as rehabilitation of the reproductive function, depending on the type and localization of the nodes. Materials and methods. The analysis of the anamnestic data, taking into account both objective and subjective data, analysis of anamnestic data of close relatives (maternal and paternal lines), concomitant genital and extragenital pathology is carried out. Results. The most numerical group of patients with uterine fibroids, according to our data, in the age groups of 26–35 years (32%) is 36–40 years (28.4%), which are the most socially and reproductively active. In this category, women with a high education of 33.6% and social status (30.4% and 22.4%) prevailed. The transferred inflammatory diseases of the genitals were noted in 48% of the examined, weighed gynecological anamnesis (abortions, miscarriages) were noted in 42.4%. Conclusion. Also for this category of patients is characterized by a high percentage of endometrial pathology – 60.8%, as well as concomitant adenomyosis – 34% and adhesion process of pelvic organs – 26.8%. Cervical pregnancy is a currently important medical and social problem, characterized by relatively high mortality and often leads to a loss of fertility in women of reproductive age. This clinical case demonstrates a possible approach of conservative therapy in conditions of excessive bleeding in women interested in the reservation of reproductive function, using high-tech minimally invasive methods of treatment in combination with chemotherapy.

The article presents a clinical case from the practice of a patient with craniopharyngioma. The man of reproductive age with diencephalic obesity (BMI 35 kg/m2), recurrent craniopharyngioma, a long history of endocrine disorders (panhypopituitarism, including secondary hypogonadism, with the corresponding sexual function disorders and the lack of sexual activity), with visual disorders and psychiatric symptoms in the early postoperative period after shunting surgery and reducing the volume of the craniopharyngioma cyst, erection conditions arose against the background of episodes of disorders of consciousness within the framework of sleep dissociation. After the operation (installation of the Ommaya system), the patient had a state of spontaneous erection lasting up to 30 minutes against the background of dream-oneiric states of impaired consciousness with erotic experiences. These states were observed for 3 nights, the patient remembered the experiences and events that occurred to him in a dream, and could tell about them to others on the next morning. The identity of the patient remained intact, he was active in the department, ordered in behavior; memory for current events and new information was intact. These disorders in the patient did not require specialized treatment, regressed independently on the 6th day of the postoperative period.

Purpose of the study. To study functional changes in the neuropsychological status of reproductive age patients with newly diagnosed breast cancer and to assess the possibility of using xenon-oxygen therapy to correct disorders.Patients and methods. This study included 60 reproductive age patients with newly diagnosed breast cancer who were undergoing surgical treatment at the Department of Bone, Skin, Soft Tissue and Breast Tumors of the National Medical Research Centre for Oncology of the Ministry of Health of Russia from 2018 to 2020. The main group included 30 patients with breast cancer diagnosed at the surgical stage of combined treatment, who underwent a rehabilitation course of xenon-oxygen therapy in the early postoperative period. The control group was formed from patients with the same diagnosis, without the use of this therapy. The functional state of the central nervous system in all patients was assessed by the parameters of the electroencephalography (EEG) bioelectrical activity. For the final assessment of the physiological and psychological state of the patients, a standardized questionnaires of the quality of life – ESAS, MOS-SF‑36, were used. Statistical data processing was performed using the Statistica 10 software package. Results. During the study, statistically significant differences were found in the assessment of subjective indicators, so in the group of patients using xenon-oxygen therapy, there was an improvement in well-being by 2.6 times, a decrease in depression by 2.3 times, a decrease in symptoms of nausea by 3 times, anxiety 1.9 times (p < 0.05). Against the background of an improvement in the psychophysiological state, the EEG showed a significant increase in the power of slow delta and theta rhythms, an increase in the power of the alpha rhythm and a decrease in the power of the beta rhythm, while in the patients of the control group only a decrease in the power of the beta rhythm was noted.Conclusion. The early postoperative period in patients with newly diagnosed breast cancer is characterized by the formation of a depressive symptom complex. The use of a course of xenon-oxygen therapy contributes to the normalization of the subjective feeling of physical and psychological health, increasing vital and social activity. Changes in the indicators of brain bioelectric activity and an improvement in psychophysiological state occur against the background of changes in brain activity caused by the normalizing effect of xenon.

Actuality. In the conditions of modern development of medicine constantly there is an improvement of existing and emergence of  new methods of treatment of oncological diseases. The use of high- tech methods of medical care for the treatment of cancer allows to  increase the survival rates in the detection and subsequent  treatment of malignant neoplasms. This circumstance causes the  appearance of patients with cancer desire not only to cure this  underlying disease, but also to preserve the quality of life that  preceded it. Often, patients of reproductive age wish to realize their  reproductive function. The article analyzes the possibilities of the  medical organization, when detecting breast cancer in a patient of  childbearing age, to use methods of treatment aimed at the implementation of the patient’s reproductive rights.Purpose of research. To perform the law on the protection of  public health, which provides patient before the start of anticancer  therapy kryokonservierung oocyte.Results. It is noted that such opportunities are limited by the current legislation that does not grant the right to persons with such  cancer at the expense of the budget to use the methods of assisted  reproduction and cryopreservation technologies to maintain their  reproductive potential. The article substantiates the need to  consolidate the possibility for women of childbearing age who have  breast cancer and want to realize their reproductive function after  treatment before chemo - and radiation therapy to receive and  cryopreservate with the subsequent storage of oocytes, embryos or ovarian tissue at the expense of the budget.

Genital endometriosis is a disease of women of reproductive age, accompanied by infertility in 50% [1]. Adenomyosis can be considered as an endometriosis of the uterus. Histologically, this process is represented by ectopic, non-tumor endometrial glands, and stroma surrounded by hypertrophic and hyperplastic myometrium [2]. Adenomyosis is accompanied by pelvic pain of varying intensity as well as menstrual disorders [1]. The disease is accompanied by significant violations of reproductive function (infertility, unsuccessful attempts at pregnancy and miscarriage, abnormal uterine bleeding). Adenomyosis can be accompanied by a violation of the function of adjacent organs (such as the bladder, rectum). Often, one of the clinical manifestations of adenomyosis is the development of sideropenic syndrome, which is also caused by the development of chronic post-hemorrhagic iron deficiency anemia. This is accompanied by a deterioration in the general condition of patients, a decrease in their ability to work. Despite a large number of publications in Russian and foreign scientific sources devoted to this problem, reproductive doctors and obstetricians-gynecologists often underestimate the role of adenomyosis in pregnancy planning using assisted reproductive technologies. Without interpreting the anamnesis data obtained through an active survey, doctors do not prescribe additional methods for diagnosing this pathology, which is not complex and expensive. To confirm the diagnosis, a transvaginal ultrasound examination of the pelvic organs during the premenstrual period is sufficient. In cases that are difficult to diagnose, the MRI method of the corresponding anatomical area can be used. Underestimation of the clinical picture and under-examination of the patient did not allow prescribing timely correction of the pathology and led to unsuccessful attempts to implement the generative function using assisted reproductive technologies. The conducted examination with clarification of the cause of IVF failures and the prescribed reasonable treatment made it possible to achieve regression of endometriosis foci in this clinical situation, followed by the patient's ability to realize generative function.
