Salivary gland neoplasm are rare and constitute about 3% of all head and neck neoplasms. Mucins are altered in pathological states and are stained by special stains like Periodic Acid Schiff, Alcian Blue and Mucicarmine. To study the histomorphology of resected salivary gland tumors and mucin staining pattern wherever indicated. Surgically resected specimens received at our tertiary care hospital and subjected to histopathological examination. Specimens were fixed in 10% formalin, processed and embedded in paraffin blocks, serially cut to get sections of 3-5 microns thickness. Stained with hematoxylin and eosin for all. Mucin stains were used wherever applicable. Total number of cases studied were 70. Out of which 46 were benign (65.7%) and 24 were malignant (34.3%). Among benign tumours, Pleomorphic adenoma was the commonest tumour (48.57), followed by Warthin tumor (7.14%), Basal cell adenoma (4.28%), Myoepithelioma (1.43%), Oncocytoma (1.43%), Hemangioma (1.43%), Sailolipoma (1.43%). The Mucoepidermoid carcinoma was the most common malignant tumor (17.14%) followed by Adenoid cystic carcinoma (5.71%), Acinic cell carcinoma (4.28%), Polymorphous adenocarcinoma (1.43%), Epithelial myoepithelial carcinoma, Squamous cell carcinoma (1.43%), Salioblastoma (1.43%), Lymphoma (1.43%). Parotid was most common site for both benign and malignant tumor. Females are affected more commonly than males. Mucin staining pattern was noted. Salivary gland tumors have complex range of morphological spectrum. Histopathological examination is the golden standard for diagnosis and mucin stain would add as an adjunct to the diagnosis.

BACKGROUND Salivary gland lesions constitute less than 1 % of tumours and about 4 % of all epithelial neoplasms of head and neck region. These comprise of a wide variety of benign, malignant and non-neoplastic lesions which exhibits a difference in histological behaviour. There are no reliable criteria to differentiate on clinical grounds the benign from malignant ones. So morphological evaluation is necessary. We aim to study the frequency of various salivary gland lesions in sialoadenectomy specimens and categorise them into neoplastic and nonneoplastic lesions. METHODS It is a record based retrospective 5-year study carried out in the Government Medical College, Thiruvananthapuram, Kerala, from January 2014 to December 2018. RESULTS In this study a total of 329 histopathologically proven cases of salivary gland lesions were included. Neoplastic lesions and non-neoplastic lesions constituted 78.42 % and 21.58 % respectively. Pleomorphic adenoma was the most common neoplasm (50.54 %) trailed by Warthin’s tumour (9.73 %). Most common malignant neoplasm encountered in our study was mucoepidermoid carcinoma (9.73 %) among which low grade tumours showed predominance. We observed significantly higher incidence of benign and malignant lesions in the 5 th to 6th decade while non neoplastic lesions were seen more in the 4th to 5th decade. Average age of the patients with salivary gland tumours was 46.12  SD 15.57. Majority of cases of salivary gland lesions in our study were from parotid gland (75.68 %) followed by submandibular gland 24.01 %. CONCLUSIONS Pleomorphic adenoma was the most common benign tumour in our study and mucoepidermoid carcinoma the most common malignant tumour. Neoplastic lesions showed a predominance over non neoplastic lesions. Histopathological examination is the mainstay for diagnosis and clinical management. Objective: This study is aimed to know the histopathological spectrum and outcome of surgery for salivary gland tumors presented in a tertiary care hospital of Pakistan. Study Design: Retrospective study. Setting: Tertiary Care Hospital of Pakistan. Period: July 2018 to June 2020. Material & Methods: After approval from Institutional Review Board. Total 73 patients were selected with preoperative diagnosis of salivary gland tumors with consecutive non probability sampling. All surgical procedures were done by specialist general surgeons who had experience of more than 10 years of head and neck surgery. Results: Male to female ratio was 2:1 with mean age of patients was 46 years. Among males 29 (59.1%) were benign salivary gland tumors while 20 (40.81%) were malignant salivary gland tumors. Among females 13 (54.16%) were benign and 11 (45.83%) were malignant salivary gland tumors Most common benign tumor was pleomorphic adenoma 32 (43.8%) while mucoepidermoid carcinoma was most common malignant tumor 17 (23.28%). Only 9 (12.32%) patients experienced nerve paresis. Conclusion: Male preponderance, a relatively younger age at presentation and single predominant benign tumor (pleomorphic adenoma) were the significant findings. Majority of them were found in parotid gland and found to be benign in nature. Majority of them were found in parotid gland and found to be benign in nature. Although fine needle aspiration cytology and magnetic resonance imaging provide some useful information about the nature of tumor but most of them will acquire a surgical excision in order to find the definitive diagnosis.

Introduction: Fine-needle aspiration cytology (FNAC) can be challenging to provide a precise diagnosis in salivary gland cytopathology due to diversity of lesions and cytomorphological convergence between the tumors and within the same tumor of salivary gland. The recently proposed Milan System for Reporting Salivary Gland Cytopathology (MSRSGC) provides a risk stratication-based classication system with an intrinsic risk of malignancy (ROM) for each diagnostic category, which aims to furnish useful information to the clinicians. This study was undertaken to evaluate the diagnostic utility and validity of MSRSGC. A 4-year retrospective Materials and Methods: study was conducted at a tertiary care hospital in Central Gujarat in Western India. A total of 90 cases of FNAC of salivary gland lesions for the period of January 2018 to March 2021 were reviewed. Patient clinical details, FNAC smears and histological slides where available were retrieved from the departmental records. Results: The cases belong to following categories: non-diagnostic (4.4%), non-neoplastic (10%), atypia of undetermined signicance (1.1%), benign neoplasm (68.9%), salivary gland neoplasm of uncertain malignant potential (8.9%), suspicious for malignancy (2.2%), and malignant (4.4%). Out of 90 cases, 62 cases (68.8%) had follow-up. The ROM were 7.6% for category IV—A and 50% for category IV-B. The sensitivity, specicity, positive predictive value, negative predictive value, and likelihood ratio for differentiating between benign and malignant lesions were recorded as 98.1%, 42.9%, 92.8%, 75.0%, and 1.750, respectively. Conclusion: Application of MSRSGC has immense value for standardization of reporting of salivary gland FNAC. The Milan system of reporting is a risk stratication system which can improve the overall effectiveness of reporting and care of patients.

Background: Granulomatous dermatosis shares the histological finding of granuloma formation; it is usually formed because of the persistence of a non-degradable product of active hypersensitivity. The identical histological picture may be produced by several causes, which pose a diagnostic challenge to dermatopathologist, Present study aims at classifying cutaneous granulomatous dermatosis based on the morphology and aetiology of granulomas, and to highlight its significance for specific clinical diagnosis.Methods: A retrospective analysis of skin biopsy was done and cases of cutaneous granulomatous lesions diagnosed on histopathological examination were retrieved for a period of 8 years. Clinical data and diagnosis were retrieved from hospital records. Hematoxylin and eosin stained paraffin sections were reviewed. The morphological pattern of granuloma was classified into sarcoidal, necrotizing, necrobiotic and suppurative granulomas and further aetiological evaluation for the granulomatous dermatosis were done using various special stains like Periodic Acid Schiff stain, Fite-Farraco stain, Gomori methenamine silver stain and acid-fast bacilli stain.Results: A total of 228 cases of cutaneous granulomatous lesion were retrieved; out of these 93cases (40.79%) were sarcoidal granuloma type, 83cases (36.40%) were of suppurative granulomas, 29 cases (12.72%) were of necrobiotic granulomas, 20 cases (8.77%) were necrotizing granuloma and 3 cases (1.32%) had granulomatous dermatitis with vasculitis. Infective aetiology was the commonest cause for granulomatous dermatosis (57.89%), mainly by leprosy, tuberculosis and various fungal infection.Conclusions: Granulomatous dermatosis has significant overlap in histopathological picture of various granulomatous reactions. Morphology alone is seldom specific and cannot be used as diagnostic tool. It is better understood based adequate clinical data, morphology of granuloma, special stains and laboratory workup in arriving at a etiology specific diagnosis for definitive clinical management.

Objectives: The aim of this study was to analyze the risk of malignancy in salivary gland tumors on the basis of the Milan System for Reporting Salivary Gland Cytopathology. Methods: A retrospective review was performed of the charts of patients with salivary gland tumors in whom the final diagnosis was confirmed by surgical excision. Preoperative fine needle aspiration results were categorized according to the Milan System for Reporting Salivary Gland Cytopathology: non-diagnostic (category I), nonneoplastic (category II), atypia of undetermined significance (category III), neoplasm (category IV), suspicious for malignancy (category V), and malignant (category VI). Fine needle aspiration and final diagnosis were compared, and the risk of malignancy and operative/oncological outcomes were analyzed. Results: A total of 288 patients were enrolled in this study. Postoperative histopathologic salivary gland malignancies were found in 30 (10.4%) patients. Risk of malignancy was 7.1%, 0%, 48.0%, 4.8%, 88.7%, and 100% in categories I, II, III, IV, V, and VI, respectively. The most common malignant tumor in category III was salivary duct carcinoma (37.5%), followed by acinic cell carcinoma (25.0%), mucoepidermoid carcinoma (25.0%), and squamous cell carcinoma (12.5%). The 5-year survival rate of patients with malignant tumors showed no statistical difference between category III and category V/VI ( P = .140). Risk of malignancy was 88.9% and 100% in category V and VI, respectively. Conclusions: A half of atypia of undetermined significance (category III) cases were malignant. Once diagnosed, the prognosis of malignant tumor in category III was similar with that in category V/VI.

Background: Salivary gland tumors can arise from either the major salivary glands (parotid, submandibular and sublingual) or the minor salivary glands which are located throughout the submucosa of the upper aerodigestive tract. They can show a striking range of morphological diversity between different tumor types and sometimes within an individual tumor mass. Aim of the study was to study the role of fine needle aspiration cytology (FNAC) in the diagnosis of salivary gland tumors.Methods: This study was conducted comprising 34 patients attending the department of general surgery at Rajiv Gandhi government general hospital, MMC from January 2019 to December 2020 period of 24 months. A standard protocol was followed consisting of detailed history and physical examination, radiological evaluation followed by fine needle cytological examination. Histopathological examination was done in all the excised tumors for final diagnosis.Results: FNAC was done only in 20 out of 29 parotid tumors of which 12 were benign, 5 were malignant and 3 were inconclusive. In the present study superficial parotidectomy was done in 24 cases. Total parotidectomy was done in 3 out of 5 cases. Two patients refused surgery. Following surgery, 5 patients developed transient facial nerve paralysis and one patient developed permanent facial nerve paralysisConclusions: In tumors of the parotid gland, post-operative facial nerve palsy was rarely noticed. The best means of reducing iatrogenic facial nerve injury in parotid surgery remains an understanding of the anatomy coupled with a gentle technique.

Objectives: Holmium: YAG laser has gained its popularity throughout the years and is used to treat sialolithiasis, which helps to overcome the limitations of traditional sialendoscopic lithotripsy for larger-sized salivary stones. However, little information is available regarding factors predicting the success rate of Holmium: YAG laser intraductal lithotripsy. The purpose of this study is to investigate the factors affecting the success rates of Holmium: YAG laser lithotripsy for salivary stones treatment in a tertiary care hospital. Methods: A retrospective study conducted in patients receiving sialolithiasis surgery under sialendoscopy from May 2013 to March 2015 at Mackay Memorial Hospital, Taiwan. Data on various factors, including patients’ age, gender, glands, size of largest stone, multiple stones (≥2 stones), location of the stone (distal duct, middle duct, proximal duct, and hilum), and operative time. The success of the surgery defined as patients without any complaints such as swelling or tenderness. Logistic regression and Fisher exact tests were employed to examine these factors on the success rate. Results: Fifty-four patients who received sialendoscopy surgery with a mean age of 35.74 years old recruited. Logistic regression identified the operation time exceeding 210 minutes showed 23.497 folds higher odd ratio of having a result of operation failure ( P < .05). Conclusion: The prolonged operation time is the sole independent factor affecting the successful outcome for salivary gland intraductal laser lithotripsy. We recommend operative time be no more than 210 minutes to increase the success rate in salivary gland Holmium: YAG laser intraductal lithotripsy.
