A 12 years 6 months old Indian male presented with a chief complaint of irregular upper and lower front teeth. Patient exhibited a mild convex profile on Class l skeletal base with slightly decreased vertical proportions. This was complicated by severe upper and lower labial segment crowding. Also the molar relationship was half unit class ll bilaterally. Since the patient did not want to go for extraction of premolars, treatment involved use of Pendulum appliance along with upper and lower pre-adjusted edgewise appliance (0.022x0.028” slot) with MBT prescription. Various elastics and overlay wires were used along with proximal stripping for the correction of severe crowding in upper and lower anterior teeth.: Clinically Angle’s Class I occlusion was achieved bilaterally with good intercuspal relationship. Vertical growth continued throughout the treatment.

Abstract Introduction Objective of this study was to investigate the incidence and severity of apical root resorptions (ARR) during orthodontic treatment with aligners. Materials and methods The sample comprised 100 patients (17–75 years of age) with a class I occlusion and anterior crowding before treatment, treated exclusively with aligners (Invisalign®, Align Technologies, Santa Clara, CA, USA). The following teeth were assessed: upper and lower anterior teeth and first molars. Root and crown lengths of a total of 1600 teeth were measured twice in pre- and post-treatment panoramic radiographs. Afterwards, relative changes of the root length during treatment were calculated by a root-crown-ratio taking pre- and post-treatment root and crown lengths into consideration. A reduction of this ratio was considered as a shortening of the initial root length. Additionally, tooth movements of the front teeth were assessed by lateral cephalograms and the 3-dimensonal set up of each patient. Results All patients had a reduction of the pre-treatment root length with a minimum of two teeth. On average 7.36 teeth per patient were affected. 54% of 1600 measured teeth showed no measurable root reduction. A reduction of >0%-10% of the pre-treatment root length was found in 27.75%, a distinct reduction of >10%-20% in 11.94%. 6.31% of all teeth were affected with a considerable reduction of >20%. We found no statistically significant correlation between relative root length changes and the individual tooth, gender, age or sagittal and vertical orthodontic tooth movement; except for extrusion of upper front teeth, which was considered as not clinical relevant due to the small amount of mean 4% ARR. Conclusions The present study is the first analyzing ARR in patients with a fully implemented orthodontic treatment with aligners (i.e. resolving anterior crowding). The variety was high and no clinical relevant influence factor could be detected. A minimum of two teeth with a root length reduction was found in every patient. On average, 7.36 teeth per patient were affected.

Mandibular incisor extraction for orthodontic treatment is considered an unusual treatment option because of the limited number of patients that meet the criteria for such treatment. Accurate diagnosis and treatment planning is essential to achieve the desired results. Adult orthodontic patients are increasingly motivated by esthetic considerations and reject the idea of conventional fixed appliances. In recent years, Invisalign appliances have gained tremendous attention for orthodontic treatment of adult patients to meet their esthetic demands. In this case report, a case of Class I malocclusion was treated with mandibular incisor extraction using the Invisalign appliance system. Successful tooth alignment of both arches was achieved. The use of Invisalign appliance is an effective treatment option in adult patients with Class I malocclusion that requires incisor extraction due to moderate to severe mandibular anterior crowding.

Background: Bimaxillary and bidental protrusion Class I Angle malocclusions have a characteristic convex facial profile and protrusion lips due to the labial inclination of the anterior teeth. Extraction of the first four premolars is the most common choice for orthodontic treatment of these cases when all the permanent teeth are complete and in good condition. Orthodontic treatment can be performed using the Begg or Straightwire techniques. Purpose: This study aims to investigate the difference in the effect of orthodontic treatment with Begg and Straightwire appliances on molar position, occlusal plane, and anterior and posterior facial height. Methods: Sixty pairs of lateral cephalograms before and after the treatment of patients with bimaxillary and bidental protrusive Angle malocclusion Class I, aged 18–35 years old, who underwent orthodontic treatment using the Begg and Straightwire techniques with the extraction of all first premolars that met the inclusion criteria. Data analysis was performed using two-way repeated analysis of variance (p<0.05) and Pearson correlation (p<0.05). Results: Molar position, occlusal plane angle, and anterior and posterior facial heights increased significantly after the Begg technique treatment and decreased significantly after the Straightwire technique treatment (p<0.05), but there were no significant differences between the four variables in the two techniques (p>0.05). Medium correlation was found between variables in both the Begg and Straightwire techniques. Conclusion: Molars were extruded and mesialized and the occlusal plane angle and height of the anterior and posterior faces increased after the Begg appliances treatment. The molars moved mesially and occlusally and there was a decrease in the occlusal plane angle, as well as the height of the anterior and posterior faces, after treatment with the Straightwire appliances. However, there was no difference between the two techniques.

The variety of etiological factors contributing to the development of distal bite various form, as well as the structures that are affected by the disorders determine the deegre of complexity of the malocclusion itself, and thus the choice of therapeutic procedure by which it will be treated. An 11-year-old female patient was diagnosed with this type of malocclusion with a certain degree of presence of other irregularities. After a detailed diagnostic procedure, and determining the type of growth, we approach the therapeutic procedure in two phases. By applying the funcional therapy in the first phase, we correct the vertical growth type, and then in the second phase of therapy using a fixed technique we achieve harmonious intermaxillary relations, an adequate molar and canine Class I relationship as well, thus improving the aesthetics of the patient's face.

Gigitan terbuka merupakan maloklusi yang bercirikan tidak terdapatnya tutup menutup gigi rahang atas dan bawah, dapat terjadi pada regio anterior maupun posterior dan dapat melibatkan dental maupun skeletal. Maloklusi ini memerlukan ketelitian dalam penentuan diagnosis dan perawatan untuk mendapatkan hasil perawatan yang baik dan kestabilan jangka panjang. Tujuan penulisan studi kasus ini adalah untuk menginformasikan manajemen pasien dengan gigitan terbuka tipe skeletal. Pasien pria berumur 19 tahun datang ke Klinik Ortodonsia FKG UGM dengan keluhan utama gigi depan yang tidak rata dan tidak dapat digunakan untuk menggigit. Pemeriksaan klinis menunjukkan pasien memiliki kebiasaan menelan dengan menjulurkan lidah. Pemeriksaan model studi menunjukkan maloklusi Angle kelas I dengan gigitan terbuka anterior dari regio premolar kedua kanan ke kiri sebesar 10,7 mm disertai malposisi gigi individual dan pergeseran garis tengah rahang bawah ke kiri. Pemeriksaan sefalometri menunjukkan relasi skeletal kelas II dengan retrusif bimaksila, rotasi mandibula searah jarum jam dan gigitan terbuka skeletal. Pasien menolak tindakan bedah ortognatik sehingga dilakukan perawatan ortodontik kamuflase. Perawatan diawali dengan latihan miofungsional untuk melatih cara penelanan yang benar dilanjutkan dengan perawatan ortodontik teknik straightwire dengan pencabutan empat gigi molar pertama. Penutupan gigitan terbuka menggunakan elastic box anterior. Hasil evaluasi menunjukkanpengurangan besar gigitan terbuka dari 10,7 mm menjadi 1,25 mm. Kesimpulannya elastic box anterior dapat digunakan untuk mengoreksi gigitan terbuka yang etiologinya melibatkan intrusi gigi anterior.  Skeletal Anteroposterior Open Bite Treatment with Straight Wire Technique. Open bite is a malocclusion with characteristic no overlapping between maxillar and mandibular teeth. This malocclusion may occur in anterior or posterior region and involved dental or skeletal. This malocclusion needed precise diagnosis and treatment to get a good treatment result and long term stability. The aim of this case report was to inform management of patient with skeletal open bite. A 19 years old male came to orthodontic clinic Faculty of Dentistry Gadjah Mada University with the chief complaint anterior crowding, and anterior teeth cannot be used to bite. Clinical finding showed patient had tongue thrusting habit. Study model analysis showed class I Angle malocclusion with 10.7 mm anterior open bite from right second premolar to left second premolar, with individual teeth malposition and mandibular midline shifting to the left. Cephalometric finding showed class II skeletal relationship with bimaxillar retrusive, clockwise mandibular rotation and skeletal open bite. This patient refused orthognatic surgery, so he received camouflage orthodontic treatment. This treatment was started with monofunctional exercise to correct the swallowing action then continued with straight wire orthodontic treatment with four first molar extractions. Anterior box elastic was used to close the bite. Evaluation result showed open bite was decreased from 10.7 mm to 1.25 mm. The conclusion was anterior box elastic could be used in open bite correction that involved anterior teeth intrusion as an etiology.

ABSTRACT Introduction New design of brackets have been introduced in the field of orthodontics in the recent past, and one of such advancement is the self-ligating brackets. These brackets are said to have less friction, with a shorter period of treatment. Better patient acceptance and good treatment results are some of the other advantages offered by self-ligating brackets. Hence, we comparatively evaluated root resorption of anterior teeth by self-ligating and conventional preadjusted brackets in cases of severe anterior crowding in class I patients. Materials and methods The present study was carried out at the department of orthodontics in a dental institution and included 140 patients that presented with the chief complaint of Angle class I malocclusion with crowding of more than 6 mm in the anterior tooth region. Two groups were formed with 70 patients in each group. In one group self-ligating brackets were used, while in other group conventional brackets were used. Malmgren's method was sued for evaluating the root resorption score ranging from 0 to 4. All the results were analyzed by the Statistical Package for the Social Sciences software. Chi-square test, nonpaired t-test, and paired t-test were used to evaluate the level of significance. Results The mean age of the patients in groups I and II was 13.92 and 13.81 years respectively. While comparing age of the patients and time duration of the treatment, no significant results were obtained. While comparing the root resorptions at various time intervals in groups I and II patients, significant results were obtained. While comparing the root resorptions in between groups I and II patients, no statistically significant results were obtained (p-value > 0.05). Conclusion Statistically similar amount of resorption is seen in patients in both the groups either on self-ligating brackets or on conventional preadjusted brackets during treatment of Angle class I patients with severe anterior crowding. Clinical significance In treating anterior crowding patients with Angle class I patients, choice of brackets has no effect on the amount of root resorption. Introduction: The purpose of this study was to determine the difference in the ratio of maxillary anterior teeth as the results of Edgewise Standard fixed orthodontic treatment with and without extraction with golden proportion values. Methods: This study is using a retrospective analitist descriptive method. Sample divided at two groups of 13 patients with extraction of first premolar and 19 patients without first premolar extraction. The study population was patients who had completed the Edgewise Standard fixed orthodontic treatment at the Orthodontics Specialist Professional Program of the Faculty of Dentistry Universitas Padjadjaran at 2000-2009. The study was conducted using frontal intraoral photometry before and af-ter treatment with 3R photo size, then measured with electronic digital calliper software. Results: Evaluation is done by measuring the maxillary anterior teeth ratio using phimatrix software on frontal intra oral photometry of the post standard edgewise orthodontics patients with Class I malocclusion to the golden proportion, then proceed with statistical t-test. Conclusion: The conclusion of this study shows that the maxillary anterior teeth post standard edgewise orthodontic treatment in class I dento-skeletal patients with or without the first premolar extraction whose treated in orthodontic resident’s clinic Padjadjaran University shows a significant differences with the golden proportion.

Ektopik kaninus seringkali dijumpai dalam praktek bidang ortodontik. Sebagian orang yang merasa terganggu dengan keadaan ini akan datang ke dokter gigi untuk mendapatkan perawatan. Perawatan ortodontik dilakukan untuk mengoreksi gigi yang ektopik dan memperbaiki fungsi estetik. Pada perawatan kasus ektopik kaninus ini, pencabutan gigi premolar kedua dilakukan karena tidak diperlukan perubahan profil. Teknik Begg merupakan teknik ortodontik yang menggunakan gaya ringan dengan kawat busur berpenampang bulat. Kawat busur akan bergerak bebas tanpa friksi dan menghasilkan gerak tipping mahkota gigi. Tujuan dari studi kasus ini adalah untuk memaparkan perawatan kaninus ektopik dalam tahapan teknik Begg. Pasien perempuan usia 19 tahun mengeluhkan gigi depan yang gingsul dan berjejal. Hasil pemeriksaan objektif menunjukkan ektopik pada gigi 13, 23 dan 33, overbite 5 mm, crowding anterior mandibula, dan crossbite anterior pada gigi 22 dan 33. Maloklusi kelas I skeletal dengan protrusif bimaksiler dan protrusif bidental, ektopik kaninus maksila bilateral, ektopik kaninus mandibula unilateral, deep bite, crowding anterior mandibula dan crossbite anterior. Dilakukan perawatan ortodontik cekat teknik Begg multiloop dengan pencabutan 15, 25, 36 dan 46. Sembilan bulan setelah perawatan, crossbite anterior, dan gigi 13, 23 dan 33 yang ektopik telah terkoreksi. Crowding anterior mandibula telah mengalami perbaikan dan perawatan masih berlanjut hingga saat ini. Perawatan teknik Begg multiloop dengan pencabutan gigi premolar kedua merupakan alternatif perawatan untuk koreksi ektopik kaninus, jika tidak diperlukan perubahan profil wajah pasien. ABSTRACT: Ectopic Canines Treatment Using Begg Technique with Second Premolar Extraction. Ectopic canines are often found in the field of orthodontic practice. People who are annoyed with this situation usually come to an orthodontist to seek for treatment. Orthodontic treatment has been performed to correct ectopic teeth and improve the function of aesthetics. In this case of ectopic canines, a second premolars tooth was extracted because profile changes are not required. Begg orthodontic technique is a technique that uses light forces by using round archwire. Archwire will move freely without friction and produce a tipping movement of dental crowns. A 19 year old female patient complained of ectopic and crowding anterior teeth. The objective examinations find ectopic of 13, 23 and 33, overbite: 5 mm, anterior mandibular crowding, and anterior crossbite of 22 and 33. Class I skeletal malocclusion, bimaxillar protrusive, bidental protrusive, bilateral ectopic canine maxilla and lateral ectopic canine mandibula, deep bite, anterior crowding and anterior crossbite. A fixed orthodontic treatment was performed by multiloop Begg technique with tooth extraction of 15, 25, 36, and 46. 9 months after treatment, anterior crossbite and ectopic 13, 23, 33 have been corrected by using multiloop Begg technique. Crowding in the lower arch has improved compared to initial condition and treatment still continues to this day. Multiloop Begg technique with second premolars extraction is an alternative treatment for ectopic canines correction if patient’s facial profile changes are not required.

This paper concerns orthodontic treatment of a 17 years old Bangladeshi female with a class I malocclusion along with anterior crowding in the mandibular arch. Orthodontic treatment carried out with preadjusted Roth type (018 slot) fixed brackets with labial flaring of the mandibular incisors to accomplish the treatment. The esthetics and occlusion were maintained after retention. In cases requiring extraction of four first premolars, ensuring maintenance of adequate posterior anchorage and proper inclination of the teeth adjacent to the extraction sites during space closure can be difficult. The Invisalign G6 first premolar extraction solution has been developed to specifically address these problems. This case report describes the treatment of a 30-year-old woman whose main concerns were related to her protrusive and malaligned anterior teeth. Clinical findings revealed, among others, bimaxillary anterior protrusion and moderate-to-severe anterior crowding. A treatment approach involving extraction of the four first premolars was chosen, followed by orthodontic treatment with the Invisalign G6 first premolar extraction solution and Invisalign aligners. At the completion of 19.5 months of aligner treatment, the patient’s anterior teeth were retracted and uprighted, resulting in an improvement in her lip profile. Normal overjet and overbite were also achieved along with alleviation of the anterior crowding. The patient was extremely happy with the treatment results.
