In pediatric practice, much attention is paid to natural components used for the prevention and treatment of respiratory infections. Nasal drops Tamba Tuya (Adonis LLC, Russia) contain extracts of peloid from lake Tambukan, thuja, essential oils of fir, lavender, vitamin E, dexpanthenol, dihydroquercetin, glycerol, olive oil, and peach oil. These components are effective for the prevention and treatment of rhinitis, adenoiditis, bacterial complications, as well as for prevention of adverse reactions from decongestants. We have developed a method of its application that has not only local, but also systemic effects in children. Step 1: nasal rinsing with saline solutions; step 2: instilling Tamba Tuya drops (1–2 drops in each nostril) three times a day; step 3: infriction of the drops into the skin on the temples for 2–3 minutes as aromatherapy, which ensures penetration of the drug components through the skin into the blood with a systemic effect. Minimum course duration: 2 weeks. The use of the drops from the first days of respiratory infections in 30 children aged 4 to 7 years in the experimental group (30 children were in the control group) ensured milder (76.3% compared to 60% in the control group) and shorter disease (by 2 days). The majority of participants (83.3%) from the experimental group demonstrated rapid resolution of rhinitis symptoms and lower need for decongestants. A total of 86.6% of children in the experimental group and 53.3% of controls showed an improvement in emotional and behavioral responses, including activity, adaptability, intensity, mood, and distraction. 【Abstract】Objective: Toanalyze the efficacy of combined drug therapy for elderly patients with coronary heart disease and hypertension.METHODS:Sixty-six elderly patients with coronary heart disease and hypertension were enrolled from December 2017 to November 2018. They were randomly divided into two groups, 33 patients in each group. Patients in the experimental group received nifedipine. In combination with enalapril, patients enrolled in the control group received nifedipine monotherapy.RESULTS:Compared with the control group, the total effective rate, Serum Nitric Oxide (Serum NO) after treatment, CRP after treatment, HCY after treatment, and blood pressure after treatment were significantly improved (P<0.05). Serum NO and treatment before treatment in the 2 groups. There was no significant difference in pre-CRP, pre-treatment HCY, pre-treatment blood pressure, and adverse reactions during treatment (P>0.05).Conclusion: Theelderly patients with coronary heart disease and hypertension are treated with nifedipine and enalapril.

Background: To explore the application effects of whole course high-quality nursing on patients with liver cancer during radiotherapy. Methods: One hundred and five patients with liver cancer who received radiotherapy in Liaocheng People’s Hospital, Liaocheng, China from March 2010 to February 2012 were selected and divided into a control group (42 cases with routine nursing) and an experimental group (63 cases with whole course high-quality nursing). The two groups of patients were observed and compared in terms of clinical data and the Activity of Daily Living (ADL) score before and after nursing (1 course of treatment/6 week). The Self-Rating Anxiety Scale (SAS) and Self-Rating Depression Scale (SDS) were used to evaluate and compare the patients’ anxiety and depression before and after nursing. Adverse reactions during radiotherapy and nursing satisfactory of patients were observed and compared between the two groups. Results: After nursing, the ADL score and the nursing satisfaction in the experimental group was significantly higher than that in the control group (P<0.05). The SDS and SAS scores in the 2 groups before nursing were significantly higher than those after nursing (P<0.05). The total incidence of adverse reactions in the control group was significantly higher than that in the experimental group (P<0.05). Conclusion: Whole course high-quality nursing can improve the negative emotions, quality of life and nursing satisfaction, and reduce adverse reactions of patients with liver cancer during radiotherapy.

Probiotics exert beneficial effects on their host health by creating microbial balance in the digestive system. The role of some probiotic strains in strengthening the immune system and reducing the risk of diseases, especially respiratory infections, has been proven in previous studies. Aim: The aim of this study was to evaluate the effect of probiotic supplementation containing Bacillus coagulans on the Runner athletes immune system. In this study, the effect of Bacillus coagulans probiotic on immunoglobulins A, M and monocytes count 60 male athlete sprints Evaluates that which were randomly divided into two groups of 30.For 3 months, the experimental group received a daily glass of probiotic juice containing 109 cfu / ml containing probiotic supplement and the control group received plain and no supplemental juice. During the study period, once every 2 weeks, One day after exercise (running 200 meters), blood samples were taken from all participants Then In the collected samples, IgA, IgM and lymphocytes were evaluated. Consumption of probiotic juice containing 2 × 109 f cfu/ml Bacillus coagulans probiotic supplement showed a significant difference in the amount of IgA, IgM and Lymphocyte between the experimental group and the control group. The results of this study showed that the consumption of juice containing probiotic supplement Bacillus coagulans can increase the level of immune factors IgM, IgA, lymphocytes and prevent the occurrence of diseases, especially respiratory infections, by improving the function of the immune system.

Objective: To evaluate the clinical effects of apatinib combined with DOS regimen in the neoadjuvant chemotherapy for locally advanced gastric cancer (LAGC). Methods: Eighty patients with LAGC admitted to Baoding first Central Hospital from January 2018 to October 2020 were randomly divided into two groups (n=40, respectively). The control group received DOS chemotherapy regimen alone. The experiment group additionally orally took apatinib mesylate tablets. The changes in CEA, CA19-9 and other tumor markers, RO resection rate, incidence of operative complications, adverse reactions, and other indicators were compared between the two groups. Results: The overall response rate (ORR) of the experimental group was 72.5%, which was significantly better than that of the control group (50%) (p=0.03). After the treatment, the CEA and CA19-9 in the experiment group were significantly lower than those in the control group (p=0.00). The Ro resection rate was 77.5% in the experiment group and 57.5% in the control group (p=0.03). The operation time was shortened and amount of bleeding decreased in the experiment group, and the differences were statistically significant (p=0.00). The incidence of surgical complications in the experimental group was 17.5%, significantly lower than that in the control group (37.5%) (p=0.04). Conclusion: Apatinib combined with DOS regimen is effective for patients with LAGC without significantly increasing adverse reactions. Meanwhile, tumor markers are reduced significantly. Objective. The purpose of the study was to investigate the effect of entecavir combined with adefovir dipivoxil on clinical efficacy and TNF-α and IL-6 levels in patients with hepatitis B cirrhosis. Methods. A total of 100 patients with hepatitis B cirrhosis admitted to our hospital between January 2018 and June 2019 were randomly selected and divided into the control group (n = 50) and experimental group (n = 50) according to the order of admission. Among them, the control group patients were treated with entecavir, while the patients in the experimental group received entecavir combined with adefovir dipivoxil. After that, the effective rate of treatment, the incidence of adverse reactions, liver function indexes, liver fibrosis condition, and TNF-α and IL-6 expression levels were all compared between the two groups. Results. The effective rate of treatment in the experimental group was significantly higher than that in the control group, with statistical significance ( p < 0.001 ); the incidence of adverse reactions of the patients in the experimental group was significantly lower than that in the control group, with statistical significance ( p < 0.001 ); the liver function indexes in the experimental group were significantly better than those in the control group, with statistical significance ( p < 0.001 ); the number of patients with liver fibrosis in the experimental group was significantly less than that in the control group, with statistical significance ( p < 0.001 ); the TNF-α and IL-6 expression levels in the experimental group were significantly lower than those in the control group, with statistical significance ( p < 0.001 ). Conclusion. Entecavir combined with adefovir dipivoxil in the treatment of hepatitis B cirrhosis can effectively improve the therapeutic effect and reduce the serum inflammatory factor levels, with high safety, which is worthy of application and popularization.

Objective. To evaluate the efficacy and safety of Tongning Gel (TNG) compared to placebo-controlled (PC) for knee osteoarthritis (KOA). Methods. A multicentre, randomized, double-blinded, parallel, placebo-controlled, clinical trial was performed in 576 patients (432 patients in the TNG group, 144 patients in the PC group), and 1 in the experimental group withdrew due to nonuse of drug. Patients were randomized to receive TNG or PC applied to knee skin at 3g per time, 2 times per day, which lasted for 3 weeks. The Western Ontario and McMaster Universities Arthritis Index (WOMAC) pain score was used to evaluate the primary efficacy of TNG and WOMAC stiffness and physical function and total scores were used to evaluate the secondary efficacy of TNG. All participants who received at least one dose of study drug were included in the safety analysis. This trial has been registered in Chinese Clinical Trial Registry (no. CTR20131276). Results. Primary efficiency outcome: there were significant differences in the decreased value of WOMAC pain score between two groups (P<0.05), and the decreased value of WOMAC pain score in the TNG group were better than those in the PC group (P<0.05). Secondary efficiency outcome: the WOMAC total score, WOMAC stiffness score, WOMAC physical function score, and the decrease of the above indexes of the two groups of patients after treatment were statistically significant (P<0.05), and the improvement of the above indexes in the TNG group was better than that of the PC group (P<0.05). Safety Evaluation. A total of 42 adverse events were reported by 29 patients: 25 adverse events reported by 16 patients (3.71%) in the experimental group and 17 adverse events were reported by 13 patients (9.03%) in the control group. And 8 adverse reactions were reported by 6 patients including 2 adverse reactions by 2 patients (0.46%) in the experimental group and 6 adverse reactions by 4 patients (2.78%) in the control group. Two cases of significant adverse events occurred in the experimental group. Both groups had one serious adverse event, respectively, which were not relevant to the intervention. Conclusion. These results of the trial demonstrate that TNG is superior to placebo in the treatment of patients with KOA, and TNG can improve other symptoms of KOA, such as stiffness and physical function. TNG is safe for the treatment of knee osteoarthritis as a whole.

To study the effect of standardized pain management combined with clinical teaching on the adverse reactions and nursing efficacy in patients with rotator cuff injury after surgery. 104 patients with rotator cuff injury treated in our hospital (April 2019-April 2020) were chosen as study subjects and randomly split into experimental group (n=52) and control group (n=52). After all the patients received arthroscopic revision surgery, the experimental and control group received standardized pain management combined with clinical teaching and routine pain management postoperatively, respectively. After that, the effect of different pain management modes on the adverse reactions and nursing efficacy in patients with rotator cuff injury after surgery was analyzed by comparing various indexes between two groups. No obvious differences in sex ratio, average age, mean BMI, injury severity, injury cause, education level and residence were found between two groups (P > 0.05); no obvious differences in numeric rating scale (NRS) scores at T1 were found between the two groups (P > 0.05), and the NRS scores at T2 and T3 in experimental group were obviously lower compared with control group (P < 0.001); no obvious differences in the angle of shoulder abduction, anterior flexion and extension were found between the two groups before intervention (P > 0.05), and angle of shoulder abduction, anterior flexion and extension in experimental group at 3 and 6 months after surgery was obviously greater compared with control group (P < 0.001); no obvious differences in the constant shoulder scores were found between the two groups before the intervention (P > 0.05), and the constant shoulder scores in experimental group at 3 and 6 months after surgery were obviously higher compared with control group (P < 0.001); the nursing satisfaction in experimental group was obviously higher compared with control group (P < 0.05); the clinical effective rate in experimental group was obviously higher compared with control group (P < 0.05); the incidence of adverse reactions in experimental group after surgery was obviously lower compared with control group (P < 0.05). Standardized pain management combined with clinical teaching in treating rotator cuff injury of patients can significantly reduce shoulder pain severity and improve shoulder mobility, with significant efficacy and high safety, which is worthy of application and promotion.

In this study, the authors explored the use of music during hemodialysis (HD) as a complementary therapy to improve overall well-being in elderly patients. The authors recruited a convenience sample of 88 patients on maintenance HD from a teaching hospital in northern Taiwan and randomly assigned them to either an experimental group ( n = 44) or a control group ( n = 44). In the first week, participants in the experimental group created their own music playlists. During the second week, these participants listened to music from their own playlists during each HD session (three times/week). The authors evaluated the effects of music as therapy by assessing its impact on perceived stressors and adverse reactions during HD (HD Adverse Reactions Self-Assessment Scale and HD Stressor Scale [HSS]) and measuring changes in physiological indices during the course of the music listening. After 1 week of the use of music as therapy during HD, the authors noted significant reductions in the frequency and severity of adverse reactions during dialysis and in scores on the HSS, p < .001. The authors also observed significantly decreased respiratory rate and significantly increased finger temperature and oxygen saturation, p < .001, during the same period. In conclusion, listening to music during HD may promote overall patient well-being. It may thus serve as a complementary form of therapy that facilitates care and delivery of adequate dialysis and thus improves overall patient well-being in the long run.

Objective To investigate the safety and efficacy of acitinib mesylate combined with chemotherapy in the treatment of patients with gastroesophageal junction adenocarcinoma. Methods A total of 119 patients with gastroesophageal junction adenocarcinoma were enrolled and randomized into an experimental group (n = 60) and a control group (n = 59). Both groups were treated with a combination of taxane, irinotecan and fluorouracil, while the experimental group also received acitinib mesylate. The clinical efficacy, survival time and adverse reactions of patients in two groups were recorded and analyzed. Results The total remission rate in the experimental group and the control group was 15.79% and 3.23%, respectively; the disease control rate was 73.68% and 54.84%, respectively; and progression-free survival was 3.72 months (1–13.5 months) and 3.04 months (1–6 months), respectively. Overall survival was 13.66 months (5–24 months) and 10.08 months (6.5–19.5 months), in the experimental group and the control group, respectively. In addition, the incidence of adverse events in the experimental group was significantly lower than that in the control group. Conclusion Apatinib mesylate combined with chemotherapy for the treatment of patients with gastroesophageal junction adenocarcinoma was safe and effective, with improved survival benefit compared with control.

Pain is the most common syndrome in rheumatology. NSAIDs are often used in its treatment. But they have limitations associated with side effects. An advantage of topical drugs and physical factors (electrophoresis, phonophoresis) combination is the use of low doses of the drug and a decrease in time, to achieve a therapeutic effect, reduce the risk of adverse reactions, increase the concentration of the drug in the inflammation focus, prolonged action (creating a drug depot in tissues). Ketoprofen gel (“Artrocol gel”) is one of the representatives of this pharmacological group. Physicochemical properties help ketoprofen to penetrate easily through the skin and create a therapeutic concentration in the underlying tissues. Objective. To assess the effectiveness of ketoprofen gel (“Arthrocol gel”) electrophonophoresis in the treatment of pain in rheumatological patients. Materials and methods. The study was carried out on the basis of Physiotherapy Department of Vinnytsia Regional Clinical Hospital named after M. I. Pirogov. All patients treated according to the unified clinical protocols were divided into experimental group (EG) and control group (CG). Patients of EG (98 persons) underwent electrophonophoresis with ketoprofen gel once a day for a week. In CG (43 persons) ketoprofen gel was used without electrophonophoresis. The intensity of the pain syndrome is monitored using a visual analog scale (VAS) of Huskisson at rest and during movement. A week later, the patient and the doctor evaluated satisfaction with the results of treatment on the scale: not effective, insufficient, satisfactory, good, excellent. Results. There was a persistent decrease in the intensity of pain syndrome in patients of both groups. The VAS scores were lower in the EG starting from the 3rd day of treatment. The intensity of the acute pain syndrome after 1-week treatment at rest (t = 4.71, p < 0.001) and during the movement (t = 3.84,  p < 0.001) more decrease in EG. Satisfaction with the results of treatment was higher in EG.
