In order to assess the mortality and lethality rates of newborns in obstetric organizations of the North-Western Federal District in 2013–2019, a comparative analysis of official statistics data was carried out. It was found that in the North-Western Federal District in the period from 2013 to 2019, there was an almost annual decrease in newborn mortality rates. The overall decrease in mortality in maternity care organizations of the Federal District (from 2,5% to 1,9%) was mainly due to a decrease in mortality in obstetric hospitals of the first and second levels (respectively from 3,2% to 1,2% and from 1,7% to 0,7%), while most children died in perinatal centers, where the mortality rate has not changed in recent years (2013 – 4,1%; 2019 – 4,0%). The study showed a decrease in mortality rates in the subjects of the Russian Federation that are part of the federal district, except for the city of St. Petersburg. The average hospital lethality rate of newborns in maternity care organizations of the North-Western Federal District during 2013–2019 was in the range of 1,9%–2,0%, did not change significantly and corresponded to the national average. However, the level of hospital lethality significantly differed in individual subjects of the district – in more than half of them, the level of hospital lethality exceeded the average, while in others it was significantly lower. Thus, the decrease in the mortality and lethality rates of newborns in maternity care organizations indicates an increase in the quality of medical care for children in the North-Western Federal District. dicates an increase in the quality of medical care for children in the North-Western Federal District.

Abstract Combined heat and power plants (CHPP) are an important component of the energy balance in many countries. However, of all types of power plants, CHPPs have the largest impact on the environment. In recent years, the urban ecology has become one of the key indicators that determine the quality of life of city residents. The article evaluates the impact of the CHPPs in Ulan-Ude on the example of CHPP-1. The main sources of environmental pollution, and, accordingly, the zone of exposure of CHPP-1’ pollutants have been identified. Virtually the entire territory of the city of Ulan-Ude falls under this zone, but since northwestern winds prevail in this area, the north-western part of the city is most affected. Based on the results of a detailed dispersion calculation, we defined the following substances which should be monitored: nitrogen dioxide, sulfur dioxide, mineral oil, and inorganic dust (70-20% SiO2 and coal dust). The concentration of these substances in the air is significant.

In the previous article, we presented an analysis of the prevalence and quality of registration of patients with malignant neoplasms of the skin (C44) in Russia and the North-Western Federal District, showed the quality of accounting and revealed the features of the dynamics of localization and histological structure. In this report, for the first time in Russia, our attention is focused on the nature of evaluating the effectiveness of anti – cancer measures based on the main criterion-calculating the survival rates of patients with malignant skin neoplasms at the level of the North-Western Federal District, whose level of "rough" and standardized indicators of morbidity and mortality of the population practically does not differ from the average Russian one. Calculations of all types of survival rates of patients with malignant skin neoplasms (C44) at the federal district level became possible after the organization in February 2019 of a single database (DB) of the cancer registry of the North-Western Federal District, numbering more than 1 million cases of ZNO, including more than 20,000 with malignant melanoma of the skin (2.0%). The created database allows for in-depth development of the dynamics of the localization and histological structure for any part of the localization structure (lip, ear, trunk, etc. parts of the body), which is not available for conducting on the basis of state reporting forms formed before receiving all the information characterizing the state of the oncological service, not to mention that when analyzing the activities of the oncological service, databases of population cancer registers are not used and calculations of the main criterion for its assessment – the calculation of patient survival.

Objective. Analysis of main indicators of the quality of medical care for children with cancer in Ural Federal District.Materials and methods. The study analyzed operational reports for 2017 obtained from the public health care executive authorities of the 6 Russian Federation Subjects constituting Ural Federal District: Kurgan Oblast, Tyumen Oblast, Sverdlovsk Oblast, Chelyabinsk Oblast, Khanty‑Mansi Autonomous Okrug, and Yamal‑Nenets Autonomous Okrug.Results. There were 3 paediatric oncology departments. No paediatric oncology departments were available in 3 Subjects (50 %), one Subject (20 %) had no paediatric oncology hospital beds. There were 192 paediatric oncology hospital beds in total (0.7 per 10,000 children aged 0‑17 years). No paediatric oncology hospital beds were available in one Subject (20 %) (Yamal‑Nenets Autonomous Okrug). The average hospital bed occupancy was 343 days a year. The number of physicians providing medical care for children with cancer was 59, and 35 of them (59.3 %, 0.1 per 10,000 children aged 0‑17 years) had a paediatric oncologist certificate. No paediatric oncologists or paediatric oncology hospital beds were available in one Subject (20 %) (Yamal‑Nenets Autonomous Okrug). The 2017 incidence rate in Ural Federal District was 14, the prevalence rate was 147.2 (per 100,000 patients aged 0‑17 years). The mortality rate was 2.5 per 100,000 patients aged 0‑17 years, the one‑year mortality rate was 5 %. There were 46 patients diagnosed through active case finding (12.1 %). A total of 94 patients (24.8 %) were referred to medical centres of Federal subordination. Four primary patients (1 %) left the Russian Federation for treatment.Conclusion. Apparent registration flaws (low incidence) and the lack of reliable follow‑up data (mortality assessment is difficult) can be overcome by the introduction of electronic registries. Audit of medical records is necessary for reliable assessment of how the general demand in paediatric oncology hospital beds is met and of the percentage of patients referred for treatment to medical centres of Federal subordination. The long‑standing problems of lack of paediatric oncologists and low percentage of patients diagnosed through active case finding should be tackled through reform of the medical education system.

Background. Improving the quality of medical care is based on a coordinated policy at the federal, regional and municipal levels in the field of health. The implementation of this goal is based on a regular assessment of the current state of the healthcare system in the Russian Federation.Objective. Our aim was to analyse of the main indicators characterizing medical care for children with cancer in the South Federal District.Methods. The operative reports for 2017 of the executive authorities in the sphere of health protection of 8 subjects of the Russian Federation that are part of the South Federal District of the Russian Federation have been analyzed.Results. The number of children were 3 216 797 people (aged 0–17 years), the number of children’s oncological beds is 272 (0.8 per 10 thousand children aged 0–17 years), the average number of days of berth employment in the year is 289.9 bed days. In 4 (50%) subjects of the department of pediatric oncology are absent, in 1 (12.5%) — there are no children’s oncological beds. The number of doctors providing medical care to children with cancer is 50, of them 42 (84%, 0.1 per 10 thousand children aged 0–17 years) have a certificate of a pediatric oncologist. In 2 (25%) subjects, there are no pediatric oncologists. Incidence of malignant tumors was 13.9 (per 100,000 children aged 0–17 years), prevalence was 86.5 (per 100,000 children aged 0–17 years), mortality was 2.5 (per 100,000 children aged 0–17 years), a one-year mortality rate of 4.7%. 8% of patients were actively detected. 104 (23.2%) of primary patients were sent to medical organizations of federal subordination, 4 (0.9%) of primary patients left the territory of the Russian Federation.Conclusion. The analysis revealed a number of defects: incorrect information contained in the reports, low incidence (detectability) of patients, including during planned preventive examinations, staff shortages and non-rational use of bed facilities.

The relevance of the study of individual psychological characteristics of employees of criminal investigation departments in transport in the North-Western Federal district of the Ministry of Internal Affairs of Russia (hereinafter referred to as the NWFD) in relation to indicators of professional deformation is due to the specific features of their official activities. Despite a significant amount of research on the phenomenon of professional deformity of police officers, there are few thoroughly developed and completed works on the prevention of professional deformities of police officers. The purpose of our research was to study the individual psychological characteristics of employees of criminal investigation departments in connection with the risk of professional deformation. The results of the empirical study were processed by correlation and factor analysis (49 parameters). The results of the initial analysis showed that the overall assessment of job satisfaction among employees of the studied departments is average with a downward trend. The results of the study on «professional burnout» revealed that a number of employees surveyed are close to emotional exhaustion. It is established that empathic abilities, social intelligence, and constructive coping strategies play an important role in the structure of individual psychological characteristics of police officers. Low ability of employees to learn behavior determines non-constructive strategies and models for coping with stressful situations, which leads to deformation of relationships with other people, i.e. to professional deformation. The obtained data can be used as the basis for the program of psychoprophylaxis of professional deformation of criminal investigation units in transport in the northwestern Federal district.

The article reveals methodological approaches to evaluating the effectiveness of the use of resources of medi-cal organizations in order to improve the availability and quality of medical care based on the application of the methodology of performance audit; a methodological approach to the use of individual elements of the efficiency audit methodology for evaluating the performance of medical organizations and the effectiveness of the use of available resources is proposed.
