Abstract Background: Diabetes mellitus and depression are both considered the most prevalent diseases of the recent century. On the other hand, it is estimated that the rate of depression is nearly doubled in diabetic patients compared with the general population.Method: A total of 89 patients, who were admitted to the hospital because of the complications of the disease, were voluntarily and conveniently participated in the study. The participants were given the questionnaires of Beck Depression Inventory-2nd-edition and Beck Scale for Suicide Ideation. They were divided into two age groups: 18-50 years old and 50-75 years old. Control group, consisting of 89 individuals without any underlying disease, were randomly selected from the patients’ visitors, matched with the participants according to the age group and gender. Statistical analysis was performed between the two groups based on scores obtained. Results: Beck Depression Inventory Scale score was significantly higher, regardless of age, in the patient group compared to the control group. However, there was no significant difference between the scores of Beck Scale for Suicide Ideation between the patient group and the control group. In the patient group, it was found that among the variables, female gender, and diabetic foot were associated with higher severity of depression symptoms. Conclusion: It is vitally essential for diabetic patients to undergo periodic psychiatric evaluation. Early diagnosis of depression could help them to better cope with their mental health issue, and as a result, it will decrease risk of suicidal ideation and other consequences.

Oxidized low density lipoprotein (ox-LDL) is a product of oxidative stress. In this cross-sectional study, we compared the ox-LDL concentrations in diabetic patients with normoalbuminuria (n=28), microalbuminuria (n=28), and macroalbuminuria (n=28) with their first degree relatives (n=28) and healthy control people (n=31). They were selected by consecutive patient selection method. The ox-LDL level was assayed using ELISA. We measured blood pressure, lipid profile, fasting plasma glucose (FPG), and HbA1c in all groups. There was no significant difference in ox-LDL concentrations among normoalbuminuric, microalbuminuric, and macroalbuminuric diabetic groups. In diabetic patients with micro- and macroalbuminuria, ox-LDL concentration was higher than their first degree relatives (P=0.04andP=0.03) and control group (P=0.001andP=0.03, resp.). In normoalbuminuric diabetic persons, ox-LDL concentration was just higher than that of healthy people (P=0.02). There was no statistically significant difference in ox-LDL levels between normoalbuminuric diabetic patients and their first degree relatives. In conclusion, the presence and progression of albuminuria in diabetic patients are not related to ox-LDL concentration and genetic predisposition influences the plasma OX-LDL level. Larger sample size is needed to confirm this conclusion in future studies.

Objective: The aim of the study was to assess netrin-1 levels in carbon monoxide (CO) poisoning to determine its relationship with poisoning severity and neurotoxicity. Methods: This is a cross-sectional prospective study. The patients older than 18 years with CO poisoning were included. The patients were categorized into two groups on the basis of neurological involvement. Both the patient and the control groups were sampled for netrin-1 at 0th hour, and the patient group only was sampled for netrin-1 at 4th hour. Results: A total of 84 patients and 50 healthy controls were enrolled. The median 0th hour netrin-1 level of the patient group (765.1 pg/mL (619.8-983.1) was significantly higher than the control group (484 pg/mL (376-1031.6)) ( p < 0.001). There was also a significant difference between the 0th hour and 4th hour netrin-1 (888.9 pg/mL (700.3–1175.5)) levels in the patient group ( p < 0.001).There was no significant statistical difference between patients with and without neurological involvement ( p = 0.62) and between those who underwent hyperbaric oxygen therapy (HBOT) and those who did not ( p = 0.76) with respect to 4th hour netrin-1 levels. Conclusion: The significantly higher netrin-1 levels in patients with CO poisoning, suggests that netrin-1 is elevated as a stress marker. Although there is no significant difference in netrin-1 levels in patients with neurological impairment in CO poisonings, netrin-1 may show subclinically neurological effects. Hence, we believe that netrin-1 cannot be used as a marker of poisoning severity.

Purpose: Incontinence is a condition that can cause significant problems that can affect patients’ quality of social, emotional, psychological and sexual life. The aim of this study was to evaluate the level of anxiety, health anxiety, depression, and somatosensory amplification in patients with urge incontinence. Materials and Methods: The study group consisted of 58 patients that met the inclusion criteria. The control group consisted of 67 volunteer participants that did not have physical or psychiatric illness and incontinence complaints. All participants filled out sociodemographic data form, Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), Somatosensory Amplification Scale (SSAS), and Health Anxiety Inventory (HAI). Results: The mean duration of incontinence in patients with urge incontinence was 16.55 ± 10.03 months. The mean age in urge incontinence group and the control group were 40.98 ± 9.58 and 39.1 ± 7.89 years, respectively. The mean values of SSAS, HAI, and BAI scores in the incontinence group were significantly higher than the control group (p <0.001), but there was no significant difference between the groups in terms of BDI scores. The linear regression analysis indicated that HAI and BAI significantly affected SSAS (p = 0.025 and 0.019, respectively). Conclusions: Anxiety, health anxiety and somatosensory amplification are more common in patients who report urge incontinence. For these reasons, we believe that psychiatric evaluation should be included in the diagnosis and treatment process of patients presenting with urgency and incontinence symptoms. Dermatoglypic patterns are extensively investigated to apply in disease-related risk assessment due to an obvious association between morphological and genetic characteristics. In the current study, we aimed to determine whether the fingerprint and palmar patterns vary between case population with schizophrenia and general population. A cross sectional study was conducted in people diagnosed with schizophrenia (cases) and a control population between 2016 and 2019. In this study, 252 people were participated. Ink and paper method was used to evaluate the difference of fingerprints palm prints between patients with schizophrenia and participants in control group.93 participants were analyzed in schizophrenic group and 142 participants were investigated in the control group. The percentage of arches on the right ring finger was significantly different between the schizophrenic patient group and control group (p = 0.011). The whorl pattern type (U-W-U-W-W-W-W-U-W-U) was dominantly observed in both of the schizophrenic patient group and control group. A-B ridge count in schizophrenic patient group and control group produced a markedly significant difference (p<0.05). Interestingly, a strong significant difference was produced in comparing of A-B ridge count in catatonic schizophrenia group with residual schizophrenia group (p<0.005). In comparison, index of pattern intensity in control group was slightly higher than that in schizophrenic patient group. Taking together, these results showed that the dermatoglypic characteristics might be a valuable tool to describe the nature of schizophrenia and its clinical subtypes and further studies are needed in clinical application.

Iron chelation therapy with deferoxamine (DFO),the current standard for the treatment of iron overload in patients with betathalassemia,requires regular subcutaneous or intravenous infusions. This can lead to reduced quality of life and poor adherence,resulting in increased morbidity and mortality in iron-overloaded patients with beta-thalassemia. Deferasirox (DFX) is an orally administered iron chelator that has been approved for use in many countries. The requirement of an effective,well tolerated iron chelator with a less demanding mode of administration has led to the development of deferasirox. The present study was aimed to compare the satisfaction and compliance with deferoxamine versus deferasirox (Exjade®),a novel oral iron chelator in patients with transfusion - dependent beta- thalassemia. A cross-sectional,single-center investigation study was carried out in the Thalassemia Center of Ibn-Atheer Teaching Hospital in Nineveh province,Iraq. One hundred and eight thalassemic patients aged between 2- 20 years old having received multiple blood transfusions and a serum ferritin greater than 1500 ng/ml. Patients were randomised into two groups. Group 1 received deferoxamine at a dose of 20-50mg/kg/day and group 2 received deferasirox at the dose of 10-30 mg/kg/day. Another 56 apparently healthy volunteers were used as a control group. The assessment of chelation was done during the period between November 2013 and February 2014 by measurement of serum ferritin. Satisfaction and compliance was assessed by using a special questionnaire prepared by the researcher. Out of the 108 thalassemic patients enrolled there was no discontinuation in treatment with the two drugs under study. The serum ferritin did not change significantly in any of the chelation groups. In comparison with the patients who were treated with DFO,those receiving DFX reported a significantly higher rate of compliance and satisfaction (P < 0.05). However,no significant difference was observed between the two groups regarding their satisfaction (P > 0.05).Compliance with deferasirox (50 %) was more than that with deferoxamine (20 %). Satisfaction with deferoxamine was significantly lower than deferasirox (p= 0.00).

Background: The aim of the present study is to describe the occurrence of Blastocystis sp. detection among asymptomatic subjects and patients with irritable bowel syndrome in order to evaluate the potential association between irritable bowel syndrome and the parasitic infection. Methods: Cross-sectional study where adult patients with irritable bowel syndrome diagnosed according to Rome IV criteria were included. A control group was formed by asymptomatic subjects older than 18 years. Exclusion criteria were: immunosuppressive condition or having received any drug with demonstrated activity against Blastocystis sp. within the last 6 months before study inclusion. Epidemiological and clinical information was collected from all included participants. Two stool samples were obtained from all participants: one sample for microscopic examination and one sample for Blastocystis sp. PCR detection. Blastocystis sp. infection was defined by the positivity of any of the diagnostic techniques. Results: Seventy-two participants were included (36 asymptomatic subjects and 36 patients with irritable bowel syndrome). Thirty-five (48.6%) were men, and median age of participants was 34 (IQR 29–49) years. The overall rate of Blastocystis sp. carriage was 27.8% (20/72). The prevalence assessed through microscopic examination was 22.2% (16/72), while the prevalence measured by PCR was 15.3% (11/72). When comparing the presence of Blastocystis sp. between asymptomatic subjects and IBS patients, we did not find any statistically significant difference (36.1% vs. 19.4% respectively, p = 0.114). Conclusions: regarding the occurrence of Blastocystis sp., no differences were found between asymptomatic participants and patients with irritable bowel disease irrespective of the diagnostic technique performed.

Introduction: Metabolic syndrome includes a range of disorders that increase the risk of cardiovascular disease and diabetes mellitus. In this study, we examined the serum level of vitamin D3 in diabetic individuals with metabolic syndrome compared with non-diabetic individuals without metabolic syndrome and the association of serum vitamin D3 levels with metabolic syndrome and atherogenic factor (LDL/HDL). Material and Methods: In a case-control study, we included 110 women with metabolic syndrome according to ATP III criteria and 127 healthy women as a control group. Serum concentration of total cholesterol, LDL-C, FBS, HDL-C and serum triglyceride determined by enzymatic method and colorimetric and, serum level 25-(OH) vitamin D determined by ELISA. Results: It was found that the two healthy and metabolic groups were significantly different in terms of total cholesterol levels, LDL and triglyceride levels, HDL, VLDL, FBS, atherogenic index (LDL/HDL) and vitamin D levels (p<0.05). All participants in the control group and the patient and the whole study population were divided into two categories of insufficient and sufficient based on their measured serum concentrations of 25-(OH) vitamin D. There was a significant difference between the group with insufficient levels of vitamin D in comparison with the group with sufficient levels of vitamin D in terms of total cholesterol, LDL and triglyceride levels, HDL, VLDL, FBS and atherogenic index (LDL/HDL) (p=0.000). Conclusion: The present results showed that there is a significant relationship between level 25-(OH) D and atherogenic index (LDL/HDL) and the incidence of metabolic syndrome.

<p><strong>Objectives.</strong> Depression is associated with diabetes mellitus and affects treatment goals negatively. We aimed to determine the prevalence of depression and identify its socio-demographic or clinical correlates among patients with diabetes mellitus attending an out-patient clinic in Nigeria.</p><p><strong>Methods.</strong> Two hundred consecutively recruited diabetes patients (index group) were compared with a similar number of apparently healthy controls in a cross-sectional survey. In both groups, in addition to obtaining socio-demographic details, depression was diagnosed using the Schedule for the Clinical Assessment in Neuropsychiatry (SCAN), while the Beck Depression Inventory (BDI) was used to assess depression symptom severity. <strong></strong></p><p><strong>Results.</strong> Sixty (30%) diabetes patients met a SCAN diagnosis for clinical depression, compared with 19 (9.5%) in the control group. Having a smaller income and more children were significantly correlated with higher depression symptoms on the BDI.</p><p><strong>Conclusion.</strong> Depression is highly co-morbid with diabetes mellitus. The care of individuals with diabetes mellitus should include the screening and possible treatment for depression in order to achieve and sustain treatment goals.</p>

Lunula is the white, half-moon shaped area seen in proximal ends of some nails. Though a few studies have described the nail changes that can occur in association with HIV infection, none of these paid much attention to lunula. Aims and Objectives. To study the lunula in fingernails among HIV infected patients. Materials and Methods. An observational, cross-sectional study to record presence of lunula in 168 HIV-positive patients and compare it with age and sex matched 168 healthy HIV-negative control. Anolunula (absence of lunula) in HIV-positive patients was correlated with CD4 counts, stages of HIV infection, time since patient was diagnosed as HIV-positive, and status of antiretroviral therapy. Results. Anolunula was present in significantly more fingernails in HIV-positive patients compared to HIV-negative controls. There was a highly significant difference for total anolunula (anolunula in all fingernails) in study and control group. Incidence of total anolunula was directly proportional to the stage of HIV infection, increasing progressively as the HIV infection advances from stage 1 to stage 4. Conclusion. Absence of lunula is related to not only HIV infection per se but also the stages of HIV infection.

Abstract. To explore suppressor T lymphocyte function in Graves' disease, studies were performed in one group of patients in the hyperthyroid state and in another group in the euthyroid state after treatment. Peripheral blood lymphocytes were cultured for 1–7 days., Pokeweed mitogen (PWM; 1.25 μg/ml) was added at the initiation of the cultures or after 24 h. The degree of lymphocyte activation was assessed by measurements of the cellular uptake of [3H]thymidine and expressed in counts per minute (cpm). The suppressor lymphocyte function was estimated by a quotient between the maximum cpm values from cultures with and without pre-incubation. For the hyperthyroid group (n = 15) the quotient was 1.00 ± 0.07 (mean ± sem), for the euthyroid patient group (n = 21) 1.12 ± 0.05 and for the healthy control group (n = 21) 1.37 ± 0.08. There was a significant difference between the quotients for the control group and the hyperthyroid (P < 0.01) as well as the euthyroid (P < 0.05) patient group. The quotients for the two groups of patients did not differ significantly. In conclusion, the present study supports the view of a defect in suppressor T lymphocyte function in patients with Graves' disease in the hyperthyroid state and indicates that this defect can persist in the euthyroid state after treatment.
