Abstract Stroke survivors often exhibit gait dysfunction which compromises self-efficacy and quality of life. Muscle Synergy Analysis (MSA), derived from electromyography (EMG), has been argued as a method to quantify the complexity of descending motor commands and serve as a direct correlate of neural function. However, controversy remains regarding this interpretation, specifically attribution of MSA as a neuromarker. Here we sought to determine the relationship between MSA and accepted neurophysiological parameters of motor efficacy in healthy controls, high (HFH) and low (LFH) functioning stroke survivors. Surface EMG was collected from twenty-four participants while walking at their self-selected speed. Concurrently, transcranial magnetic stimulation (TMS) was administered, during walking, to elicit motor evoked potentials (MEPs) in the plantarflexor muscles during the pre-swing phase of gait. MSA was able to differentiate control and LFH individuals. Conversely, motor neurophysiological parameters including soleus MEP area, revealed that MEP latency differentiated control and HFH individuals. Significant correlations were revealed between MSA and motor neurophysiological parameters adding evidence to our understanding of MSA as a correlate of neural function and highlighting the utility of combining MSA with other relevant outcomes to aid interpretation of this analysis technique.

Background: Stroke frequently causes deficits that may impact a person’s ability to drive. Little has been reported about the frequency with which patients return to driving following a stroke or the use of driver evaluation services to determine a patient’s capacity to drive safely. Objective: To examine the frequency of return to driving post-stroke, self-imposed limitations in driving, and the utilization of evaluative services in the decision to return to driving. Methods: : The STEP-South Carolina Project surveyed 162 stroke survivors one year after their event, assessing clinical and functional outcomes and patient perceptions of the magnitude of the impact of stroke on their quality of life (“no effect”, “some effect”, or “great effect”). Multivariate analyses (adjusted for NIHSS) were performed to determine the relationship between driving status, the use of evaluative services, self-imposed limitations to driving and the patient’s perceived effect of stroke of the ability to perform valued life activities. Results: Multivariate analyses showed that 51.2% (83/162) of survey respondents returned to driving following their stroke, with 59% of these (49/83) returning within one month. Only 5.6% of all respondents (9/162) received any formal driving evaluation, though 11% (9/83) of those who had returned to driving had reported their stroke had a great effect on their ability to performed valued life activities. Among those who reported “no effect” on their ability to perform valued life activities and returned to driving, 45.8% (38/83) limited their driving, though it is unclear whether formal evaluation would have supported such a decision. Conclusion: The resumption of driving following stroke is usually a decision made without formal evaluation. Stroke survivors, including those who self-impose restrictions on their driving, may benefit from formal evaluation before returning to the driver’s seat.

Introduction: In stroke survivors, variables associated with lower quality of life (QOL) include hemiplegia, lower functional status, degree of walking ability, speed of gait, and overall walking dysfunction. The iStride TM Gait Solution, a home-use gait treatment device, has been shown to improve gait speed and other functional parameters in stroke survivors. This analysis discusses the relationship found between gait speed parameters and self-reported QOL after treatment with the iStride TM Gait Solution. Methods: Nineteen subjects were treated with the iStride TM device in their home environment for a targeted 12 sessions over four weeks. QOL was measured using the Stroke Specific Quality of Life Scale (SS-QOL) and gait speed was measured using the 10 Meter Walk Test (10MWT) at comfortable pace. Outcome measures were assessed at baseline and one-week post-treatment. Results: Results showed a statistically significant improvement from baseline to one-week follow-up for 10MWT (p=0.0001) and SS-QOL (p=0.007). The relationship between these variables appeared to be more dependent on the % improvement of gait speed and the ending gait speed being above the mean baseline speed (0.575 m/s) than the absolute improvement in gait speed. For example, subjects starting below the mean gait speed improved 23 points (14.8%) on SS-QOL (gait speed improvement = 0.22m/s) compared to 11.7 points (8%) on SS-QOL (gait speed improvement = 0.33 m/s). In addition, subjects that improved from the home ambulator category to limited community ambulator improved an average of 15.3 points (10.2%) on SS-QOL while subjects improving from limited community ambulator to full community ambulator improved only 4.8 points (4.6%) SS-QOL, despite a 0.16 m/s larger gait speed improvement. Conclusions: The results of this analysis indicate that larger gains in QOL may be achieved by focusing on patients reaching a gait speed above approximately 0.575 m/s. Our findings also support the importance of helping home ambulator stroke survivors achieve limited community ambulator status. These findings may guide clinicians who desire to improve the QOL of their patients to select effective treatment methods targeting gait speed improvement.
