Abstract Studies addressing the hospitalization of patients affected by COVID-19 have reported that length of hospital stay in intensive care unit varies between 12 and 22 days. Part of these patients require orotracheal intubation (OTI) and, in cases where OTI is prolonged and/or with complications, they consequently end up undergoing open surgical tracheostomy (OST). In most hospitals, a multidisciplinary team composed of physicians, nurses, physiotherapists and speech-language therapists is responsible for this process, which allows early removal of the tracheal cannula, making it safer for patients, with reduced risk of failure and complications. Although safety recommendations and procedures support the most diverse protocols of weaning from mechanical ventilation and tracheostomy, there is no universally accepted protocol for this transition. This is a complex issue that is necessary in the current scenario, because studies establishing weaning and decannulation criteria in patients with COVID-19 are urgent at this moment. Purpose: To describe the process of weaning from the tracheostomy in COVID-19 patients. Methods: This is a descriptive study presenting a series of case reports. It describes the decannulation process in five tracheostomized patients with COVID-19 (3 men and 2 women) aged 41 and 74 years who agreed to participate in the study and remained hospitalized in a private hospital in the city of XXXX. Results: The decannulation process of the five COVID-19 patients lasted up to 10 days (mean duration of 6.4 days).

This study analyzed the attitudes of nurses concerning the occurrence of errors in nursing procedures carried out in an Intensive Care Unit (ICU) based on the bioethics framework. This descriptive study with qualitative approach was carried out with 14 nurses from a private hospital in the city of São Paulo, Brazil. Results were analyzed according to Bardin’s proposal of content analysis. The resulting themes were: acknowledging one’s fallibility; acknowledging and reporting errors; hiding errors. The nurses’ reports are based on considerations through the lens of bioethics: taking responsibility for an error implies acknowledging one’s own vulnerabilities; acknowledging an error with responsibility implies ethical conditions in the relationships among those involved; and errors are in the context of a particular environment. This study enables re-thinking nursing practice based on bioethics, resorting to the analysis of errors focusing on the relationships between those involved.

This study aimed to: estimate the billing of nursing procedures at an intensive care unit and calculate how much of total ICU revenues are generated by nursing. An exploratory-descriptive, documentary research with a quantitative approach was carried out. The study was performed at a general ICU of a private hospital in the city of Sao Paulo. The sample consisted of 159 patients. It was concluded that the nursing procedures were responsible for 15.1% of total ICU revenues, which breaks down to an average 11.3% of revenues coming from nursing prescriptions and 3.8% from medical prescriptions. Demonstrating how much nursing contributes to hospital revenues is essential information for nursing managers, as it is an important argument to obtain resources and guarantee safe care.

Myasthenia gravis (MG) is an autoimmune disease marked by weakness of voluntary musculature. Medical and surgical therapy of adult myasthenia is well documented. There is little pediatric surgical evidence, only a few case reports being available. The aim of this paper is to verify whether the surgical and anesthesiological techniques can warrant an early and safe discharge from the operating room. The secondary aim is to assess the presence of perioperative indicators that can eventually be used as predictors of postoperative care. During the years 2006-2009, 10 pediatric patients were treated according to a surgical approach based on video assisted thoracoscopic extended thymectomy (VATET). Standard preoperative evaluation is integrated with functional respiratory tests. Anesthetic induction was made with propofol and fentanyl/remifentanyl and maintenance was obtained with sevoflurane/desflurane/propofol ± remifentanyl. A muscle relaxant was used in only one patient. Right or left double-lumen bronchial tube (Ruesch Bronchopart® Carlens) placement was performed. Six patients were transferred directly to the surgical ward while 4 were discharged to the intensive care unit (ICU); ICU stay was no longer than 24 h. Length of hospital stay was 4.4±0.51 days. No patient was readmitted to the hospital and no surgical complications were reported. Volatile and intravenous anesthetics do not affect ventilator weaning, extubation or the postoperative course. Paralyzing agents are not totally contraindicated, especially if short-lasting agents are used with neuromuscular monitoring devices and new reversal drugs. Perioperative evaluation of the myasthenic patient is mandatory to assess the need for postoperative respiratory support and also predict timely extubation with early transfer to the surgical department. Availability of new drugs and of reversal drugs, the current practice of mini-invasive surgical techniques, and the availability of post anesthesia care units are the keys to the safety and successful prognosis of patients affected by MG who undergo thymectomy.

Background: Research is a central component of neurosurgical training and practice and is increasingly viewed as a quintessential indicator of academic productivity. In this study, we focus on identifying the current status and challenges of neurosurgical research in Iraq. Methods: An online PubMed Medline database search was conducted to identify all articles published by Iraq-based neurosurgeons between 2003 and 2020. Information was extracted in relation to the following parameters: authors, year of publication, author’s affiliation, author’s specialty, article type, article citation, journal name, journal impact factor, and topic. This data were then tabulated and analyzed. Results: Between 2003 and 2021, a total of 52 PubMed indexed papers were published from Iraq. All publications have been clustered in the period of 2012–2020. From 2012 to 2016, only four papers were published, one per year. The number of publications increased from 2017 to 2021, with an average of 12 publications per year. The most common article type was “case reports” (n = 14). Neurotrauma (n = 10) and vascular neurosurgery (n = 10) were the two most common topics. Most of the studies came from the city of Bagdad (n = 46), with just nine studies coming from peripheral governorates. The Neurosurgery Teaching Hospital in Bagdad was the neurosurgery center with the highest research output (n = 38). Conclusion: The number of publications per year has been showing a, relatively, promising trend since 2012. However, to promote sustained growth in academic productivity, a strategic plan that acknowledges the political, financial, and health-system-related challenges are urgently needed.
