Assessment of Shift Work and Its Health Related Behaviors Among Employees of Petrochemical Company, Saudi Arabia, 2021 (Preprint)

BACKGROUND Shift work has been reported to lead to adverse health effects and is receiving increased attention. OBJECTIVE The study aimed to understand the relationship between shift work schedules and health behaviours among day-shift workers and rotating day-evening-shift workers and to detect the difference between day-shift work or fixed day-shift and rotating day-evening-shift employees. METHODS A cross-sectional study was conducted among employees of Petrochemical Company during 2021. A self-administered questionnaire was used to collect the data. It consisted of two parts demographic data and General Health Questionnaire (GHQ-28). RESULTS Out of 126 employees, 58 (46.0%) were from group age 41-50 years, 55 (43.7%) were overweight and 41 (32.5%) were obese, 38 (30.2%) were smokers, 99 (78.6%) worked in fixed shift. Two thirds 94 (74.6%) reported 6-8 sleeping hours per day, where 41 (32.5%) reported that they were satisfactory about overall quality of sleep and 40 (31.7%) were somewhat unsatisfactory, 62 (49.2%) reported “playing regular moderate to vigorous exercise”. There was no significant relation between shifts type and general health status (p =0.122). There was a significant association between shift types and the following ; age, BMI, education, nationality, household income, Working experience, Chronic diseases, Frequency of shift work, and Overall amount of sleep per day (p<0.0001, p0.017, p=0.027, p<0.0001, p0.042, p=0.002, p=0.024, p<0.0001, and p=0.044). CONCLUSIONS In the current study, rotating shifts were associated with extra sleeping hours, normal weight and having exercise. There was no significant differences in general health between workers in different shift schedules. Further studies need to be conducted to gather more information in order to decrease the additional health burden allied to non-standard work-schedules.

The link between shift work and cardiovascular disease (CVD) has been extensively studied being lifestyle behaviours, sleep disruption and circadian misalignment the key mechanisms involved. Social Jetlag (SJL) has been proposed as a proxy for circadian misalignment in epidemiological studies, once it takes into account  individual’s chronotype and working schedules. Therefore we hypothesize that, among  workers under fixed atypical work schedules, those with a greater SJL have a higher CVD risk. Methods A cross-sectional observational study was conducted among blue-collar workers of one retail company. Fixed working schedules were early morning, late evening, and night work. Sociodemographic, occupational, lifestyle and sleep data were collected through questionnaire. SJL was quantified by the difference for mid-sleep points on work- and free-days. Even though SJL is a continuous variable, 3 categories have been used (≤2h; 2-4h; ≥4h). Blood pressure (BP) and the total cholesterol (TC) were assessed. The CVD risk was estimated according to the relative risk SCORE chart. A relative risk≥3 was considered "high CVD risk". Descriptive statistics and bivariate analysis according to the CVD risk (high vs other) was performed. The relationship between SJL and high CVD risk was analysed through logistic binary regression using generalized linear models adjusted for age, sex, education, Body Mass Index, consumptions, sleep duration and quality plus work schedule and seniority. Results Of the 301 workers, 56.1% were male with a mean age of 33.0 ± 9.4years. Average SJL was 1:57 ± 1:38hours with the majority of workers experiencing ≤2h (59.4%) and 8% (n = 24) more than 4h. Less than a half had hypercholesterolemia (48.8%), overweight (37.9%)or hypertensive values (10.6%), however 50.5% were currently smokers. We found a significant trend for hypertension (p = 0.006) and smoking prevalence (p = 0.043) among ordinal SJL categories. A relative "high CVD risk" was found in 20.3% of the sample (n = 61). These workers were significantly older (p &lt; 0.001), less educated (p = 0.003) and slept less hours on workdays (p = 0.021). In the multiple regression analysis, SJL was an independent risk factor for a "high CVD risk" (p = 0.029).The odds of having a "high CVD risk" increased almost thirty per cent  per each additional hour of SJL (OR = 1.29; 95% CI:1.03-1.63), even after adjusting for sociodemographic, lifestyle, sleep and working features. Conclusions We found compelling evidence that a greater SJL was associated with a bigger chance of high CVD risk. From this innovative perspective, the focus is not just on the working schedule itself but also on the worker’s chronotype. These findings suggest that interventions aimed to reduce Social Jetlag, especially in extreme chronotypes and working schedules, poses a great opportunity to minimize the cardiovascular health impact of shift work.

This study aimed to evaluate the association between shift work and dry eye disease (DED) in the general population. The 2011 Korea Health Panel (KHP) was used. Chi-square test and multivariate logistic regression were used to assess the relationship between shift work and DED. Stratification analysis was conducted by sex and age. Overall, the odds ratio (OR) of DED according to shift work did not showed significant results (adjusted OR = 1.230, 95% CI 0.758–1.901). When findings were stratified based on age older or younger than 40 years, the OR of DED increased to 2.85 (95% CI: 1.25–5.90) in shift workers under 40 years of age. Our results show an association between shift work and DED in a group of younger subjects.

Introducción: El personal de enfermería es uno de los grupos más afectados por la actual pandemia por COVID-19. Se han reportado problemas de salud mental en esta población, sin embargo, también es importante estudiar la resiliencia, para identificar sus fortalezas. El objetivo de este estudio fue estudiar la resiliencia en enfermeros mexicanos y los factores sociodemográficos y de salud mental asociados a ella. Se llevó a cabo un estudio transversal.Método: Se envió vía online un cuestionario sociodemográfico y relacionado a COVID-19, así como el Inventario de Resiliencia y el Cuestionario General de Salud-28. Se incluyeron 556 enfermeros, la mayoría fueron mujeres (80%), solteras (60.8%) y con edades entre 26-35 años (38.3%).Resultados: Se encontraron niveles bajos de resiliencia en los enfermeros más jóvenes (p<0.001, ɳ2=0.05), solteros (p<0.001, ɳ2=0.02) y con menor nivel educativo (p=0.001, ɳ2=0.02). Los predictores de resiliencia fueron la búsqueda de información sobre salud mental (β =-0.152, p <0.001), nivel educativo más alto (β = 0.142, p<0.001), niveles bajos de depresión (β=-0.307, p<0.001) y bajos niveles de disfunción social (β =-0.261, p<0.001).Conclusión: Estos hallazgos permiten identificar los factores asociados a la resiliencia en los enfermeros y cómo estos juegan un rol muy importante en su salud mental. Asimismo, estos datos permiten la identificación de grupos con mayor riesgo psicosocial, con la finalidad de guiar estrategias en salud mental orientadas a aumentar la resiliencia. Introduction: Nursing personnel are one of the groups which have been most affected by the current COVID-19 pandemic. Although mental health problems have been reported in this population, it is important to study resilience, in order to identify its strengths. The purpose was to study resilience in Mexican nurses and the mental health and sociodemographic factors associated with it. A cross-sectional study was used. Method: A sociodemographic and COVID-19 related questionnaire, the Resilience Inventory and the General Health Questionnaire-28, was sent via online. 556 nurses were included, the majority were women (80%), single (60.8%), aged between 26-35 years (38.3%). Results: Lower resilience was found among nurses who were younger (p<0.001, ɳ2=0.05), single (p<0.001, ɳ2=0.02) and with lower levels of education (p=0.001, ɳ2=0.02). Predictors of resilience included the search for mental health information (β =-0.152, p <0.001), higher education (β = 0.142, p<0.001), low levels of depression (β=-0.307, p<0.001) and low levels of social dysfunction (β =-0.261, p<0.001). Conclusion: This findings allowed to identify the factors which are associated with resilience among nurses and how this plays an important role in their mental. Likewise, this data allows for the identification of high psychosocial risk groups, to better guide mental health strategies aimed at increasing resilience. Da mesma forma, esses dados permitem identificar grupos de maior risco psicossocial, a fim de nortear estratégias de saúde mental que visem aumentar a resiliência.

Introduction: The body's response to noise pollution is very similar to the way the body responds to stress, which can lead to poor health over time. In this regard, this study was conducted to determine the general health of the elderly and its relationship with traffic noise pollution in Ardakan. Methods: This descriptive cross-sectional study was conducted in 1399. Participants were 200 elderly people in Ardakan and were included in the study by random sampling. Data were collected through sound measurement with SVANTEK device, General Health Questionnaire 28 (GHQ-28). After completing the questionnaires and measurements, the collected data were entered into SPSS-19 software and descriptive statistics as well as analytical statistics including: Chi-square, regression and analysis of variance were used to analyze it. Results: The overall average sound level equivalent in decibels at all measuring stations was within the standard range. There was a significant relationship between noise annoyance and the dimensions of "anxiety symptoms and sleep disorders" and "depressive symptoms" of general health (p <0.001). Conclusion: People with more voice annoyance have more anxiety, sleep disorders and depression. Existence of noise pollution levels in the allowable or standard level can lead to the observation of no significant relationship between traffic noise pollution levels and general health. Reducing anxiety, depression and sleep disorders in the elderly is possible with interventions to reduce voice irritation in them.

Sleep disorders are prevalent occupational health problems among shift workers, especially healthcare workers with long shifts. Serotonin is a neurotransmitter related to circadian variations accompanied by shift work. A cross-sectional study was performed on 73 nurses at a tertiary hospital in Cairo, Egypt, to assess sleep quality among shift work nurses (SWNs), to determine blood serotonin level, and its relation to shift work and sleep quality. A demographic and occupational history questionnaire, Pittsburgh Sleep Quality Index (PSQI) questionnaire, and measurement of blood serotonin were carried out to the studied group. The data were analyzed using SPSS 25, and descriptive statistics, unpaired t-test, ANOVA, Kruskal–Wallis Test, Chi-square, Spearman correlation, and multivariate regression analysis were utilized. The results showed that the mean PSQI global score was significantly higher among SWNs than non-shift work nurses (NSWNs) and was the highest (10.32 ± 3.56 and 10.22 ± 2.4, respectively) among rotatory and fixed night shift nurses. Blood serotonin showed highly significant differences between SWNs over NSWNs ( p = 0.001), and mostly reduced among rotatory and fixed night shift nurses (66.7% and 65%, respectively). Moreover, there were highly significant differences in serotonin levels between poor and good sleep quality nurses ( p < 0.001), and most of the poor sleep quality nurses (62.7%) had low serotonin levels. Abnormal serotonin level (odds = 246.5) and working years (odds = 1.2) were statistically significant predictors of poor sleep quality. In conclusion, SWNs, especially rotating and night shift nurses, suffer from poor sleep quality associated with abnormal levels of blood serotonin.

Background: Health literacy (HL) is considered a key concept to positively address relevant decisions concerning physical and mental health. According to an integrated model of a European Consortium, the process to access, understand, appraise and apply health information is at the centre of practising HL. Aim: In this study, we examine HL in a population with an early onset of a mental disorder (MD). Methods: Results are based on a cross-sectional survey among people with MD ( n = 310) who sought help at an early detection centre for MD in Cologne, Dresden or Munich. Help-seekers filled out the European Health Literacy Survey questionnaire (HLS-EU-Q) on perception-based HL, socio-demographic data and general health status. Psychopathology was assessed separately by trained specialists. Data are compared with a representative sample of the German population. Results: Overall, HL was lower in a sample with MD compared with the general population. Disease-specific limitations were present in accessing, appraising and applying health information, whereas understanding was perceived fairly easy. Statistical analysis of limited HL revealed correlations with the diagnosis of affective disorders and anxiety disorders, an increase of depressive symptoms as well as the presence of more than one MD. In line with these findings, low levels of HL were associated with a worse general health status. Conclusion: In a population with MD, accessing, appraising and applying health information seemed to be particularly challenging. Therefore, educational programmes that mainly focus on increasing knowledge might not be sufficient for improving the HL in people with MD. Further research should concentrate on context-specific HL to foster behavioural change and improve overall health.

Shift work is associated with adverse health outcomes. The aim of this study was to explore the effects of shift work on circadian activity rhythms (CARs) and objective and subjective sleep quality in nurses. Female day-shift ( n = 16), evening-shift ( n = 6), and night-shift ( n = 13) nurses wore a wrist actigraph to monitor the activity. We used cosinor analysis and time–frequency analysis to study CARs. Night-shift nurses exhibited the lowest values of circadian rhythm amplitude, acrophase, autocorrelation, and mean of the circadian relative power (CRP), whereas evening-shift workers exhibited the greatest standard deviation of the CRP among the three shift groups. That is, night-shift nurses had less robust CARs and evening-shift nurses had greater variations in CARs compared with nurses who worked other shifts. Our results highlight the importance of assessing CARs to prevent the adverse effects of shift work on nurses’ health.

Abstract Objectives: To explore the relationship between work-related stress and obesity among female shift workers. Additionally, we also aimed to test the interaction between shift work and work-related stress in this association. Design: A cross-sectional study was conducted among Brazilian female shift workers. Work-related stress was assessed through a demand–control questionnaire (Job Stress Scale). Work-related stress was defined by the presence of high psychological demands and low control at work. The obesity cases were defined as those with a BMI of 30 kg/m2 or more. Multivariate Poisson regression with robust variance was used to obtain the prevalence ratios (PR) and their respective 95 % CI. Setting: A group of industries located in southern Brazil in 2017. Participants: Four hundred and twenty female workers aged 18–59 years. Results: The overall prevalence of obesity was 30 % (95 % CI: 25·6, 34·4), and the presence of work-related stress was identified in 24 % (95 % CI: 19·9, 28·1) of the sample. We found an indication of interaction between work-related stress and night shift work on obesity (P = 0·026). After adjusting for confounding factors, work-related stress was associated with a 71 % greater probability of obesity (PR = 1·71; 95 % CI: 1·02, 2·87; P = 0·042) among female night shift workers. Conclusions: In this study, we revealed that exposure to work-related stress and night shift work were associated with obesity among female shift workers. Furthermore, the prevalence of obesity was high among female shift workers.

Abstract Background Approximately 17% of the European Union workforce is engaged in shift work. Shift work has been associated with a number of chronic conditions, including obesity and obesity-related metabolic diseases. The aim of this study was to explore the dietary and lifestyle behaviours of shift workers with a healthy vs. overweight/obese body mass index (BMI). Methods A cross-sectional study was conducted on 1080 shift workers using a 15-min, telephone-administered questionnaire developed from qualitative research on Irish shift workers and national dietary intake data. Demographic and work-related factors, as well as dietary and lifestyle behaviours were recorded. BMI was calculated using self-reported height and weight. Univariate and multivariate logistic regression methods were used to analyze data according to BMI category. Results Over 40% of shift workers were classified as overweight or obese. Multivariate analysis indicated that being male [P &lt; 0.001, aOR = 2.102, 95% CI (1.62–2.73)] and middle- or older-aged were independently associated with overweight and obesity [P &lt; 0.001, aOR = 2.44 95% CI (1.84–3.24) and P &lt; 0.001, aOR = 2.9 95% CI (1.94–4.35), respectively]. Having a medium–high consumption of fried foods was independently associated with overweight and obesity [aOR = 1.38, 95% CI (1.06–1.8)]. Conclusions Similar to the general population, overweight and obesity were strongly associated with male sex and middle- or older-age. Male shift workers may benefit from targeted dietary and lifestyle advice specifically focused on limiting fried foods to help protect against overweight and obesity.
