Methods: Experimental phase: Performed a partial nephrectomy off clamp in pig model followed by cauterization of lidocaine gel 2% with different power (control, 30W, 50W and 100W) in the kidney resection bed to evaluate efficacy and deep injury extension. Clinical phase: 20 patients submitted to laparoscopic or partial nephrectomy for low risk RENAL score were utilized greased lidocaine gel 2% with 50W in cautery scalpel to hemostasis of renal parenchima to validate efficacy and safety. Results: Experimental study shows that this technique is effective and promote better hemostasis with 50W and 100W, with deep injury of less than 3 mm. Clinical study confirm efficacy, good control of hemorrage, few complications and no transfusion. Minimal changes in hematocrit, haemoglobin and creatinine were observed. Conclusion: In this preliminary experience the use of this new alternative to hemostasis for low risk partial nephrectomy was satisfactory and with good intra and postoperative results. The best advantages were safety in terms of the depth thermal injury, low cost and absence of artifacts over the resection area observed at CT scan postoperatively.

Territorial systems are characterized by the relative longevity and stability of interactions between neighbouring individuals. Two abilities of signal receivers that can be seen as adaptations increasing the efficiency of territory defence will be discussed: identifying neighbouring individuals and ranging (i.e. determining the distance to) signallers. The costs involved in such discriminations will also be outlined. Although signalling has been traditionally considered as occurring between two individuals (signaller and receiver), long-range signals will be received by many individuals. In territorial systems a group of neighbours could be considered as a communication network: consisting at its simplest of a signaller and a number of receivers. The scope for low cost, low-risk information gathering in such networks by eavesdropping will be discussed with particular reference to territorial songbirds and electric fish.

Showering preoperatively with chlorhexidine gluconate is an issue that continues to promote debate; however, many studies demonstrate evidence of surgical site infection risk reduction. Methodological issues have been present in many of the studies used to compile guidelines and there has been a lack of standardisation of processes for application of the active agents in papers pre-2009. This review and commentary paper highlights the potential for enhancing compliance with this low-risk and low-cost intervention and provides some guidance for enhancing implementation of preoperative showering with both chlorhexidine in solution and impregnated wipes.

Background: This study aimed to identify the clinical results after thyrotropin suppression therapy (TST) cessation and evaluated clinical factors associated with successful TST cessation. Methods: Patients who underwent lobectomy due to low-risk papillary thyroid carcinoma (PTC) were included in this study. We compared clinical characteristics and outcomes between patients who succeeded to stop TST and failed to stop TST. Results: A total of 363 patients were included in the study. One hundred and ninety-three patients (53.2%, 193/363) succeeded to stop TST. The independent associated factors for successful TST cessation were the preoperative thyroid-stimulating hormone (TSH) level and the maintenance period of TST. Patients with low TSH level showed a higher success rate for levothyroxine (LT4) cessation than patients with high TSH level (1.79 ± 1.08 and 2.76 ± 1.82 mU/L, p < 0.001). Patients who failed to discontinue TST showed a longer maintenance period of TST than patients who succeeded to discontinue TST (54.09 ± 17.44 and 37.58 ± 17.68 months, p < 0.001). Conclusions: Preoperative TSH level and maintenance period of TST are important factors for successful cessation of TST. If TST cessation is planned for patients who are taking LT4 after lobectomy, a higher success rate of TST cessation is expected with low preoperative TSH level and early cessation of LT4.

Electrodeposition, which features low cost, easy scale-up, good control in the composition and great flexible substrate compatibility, is a favorable technique for producing thin films. This paper reviews the use of the electrodeposition technique for the fabrication of several representative chalcogenides that have been widely used in photovoltaic devices. The review focuses on narrating the mechanisms for the formation of films and the key factors that affect the morphology, composition, crystal structure and electric and photovoltaic properties of the films. The review ends with a remark section addressing some of the key issues in the electrodeposition method towards creating high quality chalcogenide films.

Tractor front-end loaders are an essential part of the equipment used on farms. At present, there are an important number of small- and medium-sized companies involved in the manufacturing of this equipment. These companies rely heavily on experience for innovative designs, as in the vast majority of cases they lack access to adequate methodology for the optimal design of new front-end loaders. The study conducted has developed a methodology to design tractor front-end loaders with a view of obtaining their accurate design during the bucket loading process. The methodology comprises two phases: the first phase involves a numerical analysis of the structural behaviour of the front-end loader components by means of the Finite Element Method; the second phase, the experimental phase, makes use of low-cost sensors, in particular, strain gauges, to analyse existing strains at selected points in the front-end loader structure. The experimental results obtained by means of low-cost sensors fitted onto the front-end loader allow analysing the existing strains at the points measured, as well as validate the numerical model developed. This methodology is validated by applying it to a commercial front-end loader, more specifically to model 430E2 of the company Maquinaria Agrícola El León S.A (Spain).

Organizations that care for people with diabetes have increasingly adopted telemedicine-based diabetic retinopathy screening (TMDRS) as a way to increase adherence to recommended retinal exams. Recently, handheld retinal cameras have emerged as a low-cost, lightweight alternative to traditional bulky tabletop retinal cameras. Few published clinical trials have been performed on handheld retinal cameras. Peer-reviewed articles about commercially available handheld retinal cameras have concluded that they are a usable alternative for TMDRS, however, the clinical results presented in these articles do not meet criteria published by the United Kingdom Diabetic Eye Screening Programme and the American Academy of Ophthalmology. The future will likely remedy the shortcomings of currently available handheld retinal cameras, and will create more opportunities for preventing diabetic blindness.
