Introduction: This is the case report of child who was diagnosed with LCA reported with complaints of large-amplitude, slow-frequency, roving nystagmus, frequent tendency to press on his eyes, enopthalmos, with completely normal ophthalmoscopic examination with normal appearing optic nerve and retina and has a non recordable ERG; considering this condition under the lines of Vataja Nanatmaja Vyadhi treatment was structured to render effective visual rehabilitation which showed marked results. Methods: 4 years old male child who was diagnosed with lebers congenital amaurosis was brought by his parents to Eye OPD, GAMC, Bengaluru. The presentation of this case includes bibliographic review of the subject, presentation of a clinical case and description of the importance of Ayurvedic prespective of handling of these patients. Results: The child showed improvement in fixing for light and also nystagmus showed marked improvement and there were considerable behavioural changes observed. Discussion: It is important to deepen the environment of the disease to know the possible implications in Ayurvedic management, recognize the magnitude of visual disability that our patient presents for the establishment of the treatment plan and provide an integral care of excellence in an interdisciplinary way in favor of visual rehabilitation of our patients and also help to restore quality of life with no potential risks of side effects.

This paper explores the fate and transport of microbial contamination in a cold climate and coarse aquifers. A confirmed norovirus outbreak in a small rural water supply in the late summer of 2004, which is estimated to have infected over 100 people, is used as a case study. A septic system, 80 m upstream of the water intake, is considered to have contaminated drinking water. Water samples tested were negative for coliform and strongly positive for norovirus. Modelling predicts that a 4.8-log10 removal was possible in the 8 m thick vadose zone, while only a 0.7-log10 and 2.7-log10 removal in the aquifer for viruses and Escherichia coli, respectively. The model results support that the 80 m setback distance was inadequate and roughly 900 m aquifer transport distance was needed to achieve 9-log10 viral removal. Sensitivity analysis showed that the most influential parameters on model transport removal rate are grain size diameter and groundwater velocity, temperature and acidity. The results demonstrate a need for systematic evaluation of septic systems in rural areas in lesser studied coarse strata at low temperatures, thereby strengthening data used for regulatory requirements for more confident determination on safe setback distances.

Diabetic peripheral neuropathy (DPN) is a common cause of many lower-extremity complications. This case study illustrates the potential perils of pet ownership associated with diabetes and neuropathy. The case describes an incident resulting in traumatic digital amputations inflicted by a patient’s pet feline while she was sleeping. In presenting this case, the potential risks of pet ownership for patients with DPN are discussed along with a review of the relevant literature. (J Am Podiatr Med Assoc 103(5): 441–444, 2013)

Purpose The purpose of this paper is to outline the duties and powers of the Adult Support and Protection (Scotland) Act (ASPSA) 2007 and place them in the wider Scottish adult protection legislative framework. It considers the potential value of a standalone adult safeguarding statute. Design/methodology/approach The authors draw upon their research and practice expertise to consider the merits of the ASPSA 2007. They take a case study approach to explore its implementation in one particular Scottish local authority, drawing on the qualitative and quantitative data contained in its annual reports. Findings Skilled, knowledgeable and well-supported practitioners are key to effective screening, investigations and intervention. Protection orders are being used as intended for a very small number of cases. Research limitations/implications The lack of national statistical reports means that there is limited scope for comparison between the local and national data. Practical implications Adult support and protection requires ongoing investment of time and leadership in councils and other local agencies to instigate and maintain good practice. Aspects that require further attention are self-neglect; capacity and consent and residents in care homes who pose potential risks to other residents and staff. Social implications ASPSA 2007 has helped to raise awareness of adults at risk of harm within the local communities and as social issue more generally. Originality/value The authors provide a critical appraisal of the implementation of Scottish adult safeguarding legislation over the last six years. They consider similar developments in England and Wales and argue for comparative research to test these out. Finally, they signpost future directions for bridging separate policy streams.

Recent produce-related outbreaks have been receiving heightened media coverage, which has increased public concern toward the safety of fresh fruits and vegetables. In response, the microbial contamination of Ontario-grown fresh fruits and vegetables was evaluated by the Ontario Ministry of Agriculture, Food and Rural Affairs during the summer of 2004. Prior to this survey, information specific to the microbial contamination of Ontario-produced fruits and vegetables was limited. This nonregulatory survey had two objectives: (i) to obtain a general microbiological profile of selected fruits and vegetables produced in Ontario and (ii) to use the information and knowledge gained from this survey to direct and support future on-farm food safety research and food safety programs to manage potential risks. In all, 1,183 samples, including muskmelon (151), scallions and green onions (173), leaf lettuce (263), organic leaf lettuce (112), head lettuce (155), parsley (127), cilantro (61), and fresh market tomatoes (141), were collected and analyzed. Samples were analyzed for Salmonella, Shigella, and generic E. coli. Enrichment cultures positive for E. coli were further assessed for verotoxigenicity. One sample each of Roma tomato and organic leaf lettuce were positive for Salmonella, with no samples yielding Shigella or verotoxigenic E. coli. The E. coli prevalence was highest in parsley (13.4%), followed by organic leaf lettuce (11.6%), leaf lettuce (6.5%), scallions (6.4%), cilantro (4.9%), muskmelon (1.3%), head lettuce (0%), and fresh market tomatoes (0%). These findings, in combination with foodborne illness data, will help target those commodities that require more focused risk mitigation efforts.


Lately, value stream mapping (VSM) is integrated with tools and techniques that belong to other areas of knowledge such as risk management (RM). It is well known tool in showing the value, value stream and the flow which represents three of lean manufacturing (LM) principles. This integration, gives more benefit in covering two of VSM issues such as considering the variability and uncertainty of production processes. In this paper, a model named variable value stream mapping (V-VSM) that integrates the two was showed, explained and tested. The model helps to generate the VSM in a dynamic way with the identification of current and potential risks. These risks might happen in the future bringing a strong impact on not reaching the main objectives in the defined time and cost. The model has been tested by conducting a case study in food sector. A current state map was built using both models, traditional VSM and VVSM. The results showed the effect of variability and uncertainty on the total cycle time (CT) and lead time (LT) values, where the traditional VSM failed to show it by being a static tool. Comparing the results of both models show the differences in presenting the real state of manufacturing environment..

Water scarcity is already causing serious challenges nowadays and devastating water-related processes will certainly continue for decades to come. The emerging water crisis requires new political arrangements, new technological solutions and the development of new water supply chains. Among the possible political-diplomatic answers, water trade may be a viable option, as water abundant countries could distribute resources by commercializing their water surplus. Water trade, however, faces serious obstacles. Dependence of different countries on states that have water surplus, political leverage and the possible opportunity when external powers could control the essentially important human needs are all examples of potential risks. These threats hinder the development of interstate water trade cooperation and orient countries struggling with water shortages towards domestic solutions. This study attempts to examine the practice of water scarcity and water diplomacy, while also applies the Turkish experience as a case study for demonstrating constraints of water trade. The paper will hopefully shed light on the threats posed by global and regional water scarcity and draw attention to the opportunities and limitations of water commercialization. The aim of our study is to outline the issues related to the distribution and trade of water, furthermore to present the problems of water as a product. Our research focuses primarily on Turkey, which has also become involved in water trade, but the story still ended in failure.

A fundamental problem concerning medical waste disposal is the evaluation of the real and potential risks arising from waste with the focus on the risk of infection. Therefore, the optimization of medical waste routing from collection to disposal center can minimize the risk of infection. The routing of medical waste considers significant to determine potential routes and select the route with minimum distance. The management of the medical waste is important decision for environmental sustainability and includes the collection, transportation and disposal of these materials. In this paper, a geographic information system (GIS) solution approach is proposed to determine the best location of disposal center. Proposed approach is applied to medical waste transportation between 167 health institutions (collection centers) and predetermined 5 disposal centers through TRB1 region in Turkey, which consist of Malatya, Elaz??, Bingöl and Tunceli provinces. The results of case study are examined and suggestions for future research are provided.

Assessing risks related to urban soil contamination represents a key part of pollution management. The current research proposes a quantitative method that defines and highlights unacceptable risks. The applicability of the method is presented in a case study on several urban recreational areas from the city of Cluj-Napoca, Romania. Concentrations of As, Cu, Cd, Zn, Pb, Hg, Co, Ni and Mg were identified in a number of 48 soil samples from 12 intensively used recreational areas in Cluj-Napoca. The proposed risk assessment method is applied, and potential risks are calculated for all locations.
