Introduction: House officers are reported to feel less confident in working, especially in the initial part of their training. Among the factors that make them feel unprepared are in terms of adequate knowledge, clinical skills and dealing with workload. The impact of less confident house officers (HO) can lead to detrimental effects.Objectives: This study aims to look into the confidence and readiness levels of medical graduates prior to starting a HO preparatory course and factors associate.Methods: This is a cross sectional questionnaire study done among medical graduates between April – September 2018. This is part of a bigger study looking into the effectiveness of this course, which was initially organised under the Islamic Medical Association of Malaysia (IMAM) and later privatized to Medicorp (a medical training company for junior doctors). 238 participants who fulfilled the inclusion and exclusion criteria, completed the adapted IMU Competency survey pre and post the course. Their level of confidence and readiness to work was scored out of a Likert scale of 1 to 5. The higher the score, the higher the confidence or readiness to work.Results: We found significant association of confidence levels pre-course with ethnic it y (p=0.038). The scores were 2.0 (IQR=2.0), 2.0 (IQR=2.0), 3.0 (IQR=1.0), 2.5 (IQR=1.25) for Malay, Chinese, Indian and others respectively. We report significant associations with level of confidence (p=0.03) and readiness (p=0.08) not only prior, but levels of confidence (p<0.001) and readiness (p<0.001) after the course in association with place of study. Significant association was found for levels of confidence (p=0.048) and readiness (p=0.07) according to gender for scores post-course which were both 3.0 (IQR=1.0) and 4.0 (IQR=1.0) for females and males respectively.Conclusion: Confidence level to start work were higher for the Indian ethnicity before the HOpreparatory- course. AbstractIntroductionCoronavirus 2019 (COVID-19) has strike all nations hard since the end of year 2019, Malaysia unable to escape the fate as well. Healthcare system, financial growth, industrial development and educational programme are stunted. Inevitably, professional training and education are affected which include the medical training of medical interns.MethodsThis is a cross-sectional, pilot study to determine the impact of the pandemic on University Malaya Medical Centre (UMMC) medical interns. A survey which comprises 37-items was used. Data are analysed by Ordinal Logistic Regression Analysis.ResultsMedical interns feel that they lack clinical skills (p = 0.005) and need more exposure in surgical operations (p =0.029). Some are satisfied with the introduction of triage (p = 0.024), online teaching (p = 0.005) and bedside teaching (p=0.023). Most of them think they are fit and ready to handle the pandemic (p = 0.012 and 0.025 respectively) except first year medical interns (p = 0.029). Some feel like their time are wasted (p <0.05) as they are involved in many non-clinical activities (p = 0.003).ConclusionIn summary, COVID-19 has a great impact on medical training amongst medical interns. Alternative measures should be taken to minimize the interruption in training of our future leaders in medical field.

Background: Shahid Beheshti General Medical Degree Curriculum (2004) is the first big reform program in Iran with more than 10 major educational interventions. Objectives: We aimed to evaluate the graduates’ viewpoints about the program. Methods: This study is a descriptive cross-sectional graduate survey in 2018, in which 194 medical graduates from 2011 to 2014 participated with a response rate of 51.3%. The authors prepared a questionnaire with a four-point Likert scale based on the stakeholders’ viewpoints. The questionnaire contained 121 questions in seven domains: Job status, undertaking social responsibility, self-improvement, competency, effects of interventions, adequacy of educational facilities, the impact of faculty members, and reform book effectiveness. In data analysis, the authors combined the positive ratings of "I agree" and "I strongly agree" and negative ratings of "I disagree" and "I strongly disagree.” Results: More than 88% of the respondents were specialists, medical residents, or were preparing for the medical residency entrance exam. More than 94% of the graduates evaluated themselves as competent in clinical skills, health promotion, and self-improvement, but 48.9% were competent in health advocacy, 43.3% in rehabilitation, 38.6% in the knowledge of diseases, and 29.4% in cognitive skills. The organ system approach (99%) and the approach to the presentation (87.1%) were the most favorable aspects of the program; but, communication skills education (60.8%), problem-solving sessions (64.3%), and professionalism education (66%) were the weak aspects. Bedside clinical teaching (83%) was more effective than outpatient education (79.4%) and didactic education (74.2%). Among the participants, 64.4% assessed the educational facilities as desirable, and 78.4% of them, the educational hours. Besides, 77.8% of the respondents agreed with the positive effect of clinical faculty members, and 45.9% agreed with the basic faculty members. Moreover, a difference was observed between the effects of student textbooks (80.4%) and study guides (60.3%) on enhancing students' learning. Conclusions: Overall, the program has shown effectiveness in many areas; however, it has a few weaknesses that should be considered in curriculum revision.

Ever-developing changes to the working hours of junior doctors by the European Working Time Directive, the junior doctor contract of 2019 and most recently the COVID-19 pandemic have impacted the professional identity of doctors. There has been little investigation into its influence on the multifaceted aspects of postgraduate medical training, which feeds into how trainees consider themselves professionally and the concept of professional identity or ‘being a doctor’. A review of the medical, socio-political and educational literature reveals that the impact on the professional identity development of trainees is influenced by several perspectives from the trainee, trainer and the public. Gross reduction in working hours has no doubt decreased the raw volume of clinical experiences. However, to counteract this, smarter learning processes have evolved, including narrative reflection, supervised learning events, and a greater awareness of coaching and training among trainers.

Background: The evidence surrounding the impact of COVID-19 on medical learners remains anecdotal and highly speculative despite the anticipated impact and potential consequences of the current pandemic on medical training. The purpose of this study was to explore the extent that COVID-19 initially impacted medical learners around the world and examine global trends and patterns across geographic regions and levels of training. Methods: A cross-sectional survey of medical learners was conducted between March 25–June 14, 2020, shortly after the World Health Organization declared COVID-19 a pandemic. Results: 6492 learners completed the survey from 140 countries. Most medical schools removed learners from the clinical environment and adopted online learning, but students reported concerns about the quality of their learning, training progression, and milestone fulfillment. Residents reported they could be better utilized and expressed concerns about their career timeline. Trainees generally felt under-utilized and wanted to be engaged clinically in meaningful ways; however, some felt that contributing to healthcare during a pandemic was beyond the scope of a learner. Significant differences were detected between levels of training and geographic regions for satisfaction with organizational responses as well as the impact of COVID-19 learner wellness and state-trait anxiety. Conclusions: The disruption to the status quo of medical education is perceived by learners across all levels and geographic regions to have negatively affected their training and well-being, particularly amongst postgraduate trainees. These results provide initial empirical insights into the areas that warrant future research as well as consideration for current and future policy planning.

Objective To investigate trainee doctors’ and trainers’ perceptions of the validity of the Annual Review of Competence Progression (ARCP) using Messick’s conceptualisation of construct validity. Design Qualitative semi-structured focus groups and interviews with trainees and trainers. Setting Postgraduate medical training in London, Kent Surrey and Sussex, Yorkshire and Humber, and Wales in November/December 2015. Part of a larger study about the fairness of postgraduate medical training. Participants Ninety-six trainees and 41 trainers, comprising UK and international medical graduates from Foundation, General Practice, Medicine, Obstetrics and Gynaecology, Psychiatry, Radiology, and Surgery, at all levels of training. Main outcome measures Trainee and trainer perceptions of the validity of the ARCP as an assessment tool. Results Participants recognised the need for assessment, but were generally dissatisfied with ARCPs, especially UK graduate trainees. Participants criticised the perceived tick-box nature of ARCPs as measuring clerical rather than clinical ability, and which they found detrimental to learning. Trainees described being able to populate their e-portfolios with just positive feedback; they also experienced difficulty getting assessments signed off by supervisors. ARCPs were perceived as poor at identifying struggling trainees and/or as discouraging excellence by focussing on minimal competency. Positive experiences of ARCPs arose when trainees could discuss their progress with interested supervisors. Conclusions Trainee and trainer criticisms of ARCPs can be conceptualised as evidence that ARCPs lack validity as an assessment tool. Ongoing reforms to workplace-based assessments could address negative perceptions of the ‘tick-box’ elements, encourage constructive input from seniors and allow trainees to demonstrate excellence as well as minimal competency, while keeping patients safe.

Abstract BackgroundNowadays, COVID-19 constitutes a significant portion of the global burden of the diseases. Self-efficacy beliefs in disease management refer to individuals' confidence and certainty in their ability to successfully perform specific health-related behaviors. The aims of the study were: (i) to develop the COVID-19 Self-Efficacy Scale (COVID-19SES), and (ii) to investigate the impact of self-efficacy on COVID-19.MethodsThis was a descriptive cross-sectional study. A convenience sample of 66 medical students was recruited. The COVID-19SES was administered to the students.ResultsOnly 9 students (13.6%) chose statement 1 of the COVID-19SES, indicating a lack of knowledge and low self-efficacy; 9 students (22.73%) chose statements 3 and 4, indicating adequate knowledge but low self-efficacy. Of the students, 42 (63.6%) endorsed the statement 5, indicating adequate knowledge and high self-efficacy. Only 9 students (13.6%) chose statement 1 of the COVID-19SES, indicating a lack of knowledge and low self-efficacy; 9 students (22.73%) chose statements 3 and 4, indicating adequate knowledge but low self-efficacy. Of the students, 42 (63.6%) endorsed the statement 5, indicating adequate knowledge and high self-efficacy. ConclusionsThis study provides evidence for the usefulness of the COVID-19SES for assessing self-efficacy in students. Twenty-four students (36.4%) reported low COVID-19 self-efficacy, and education is necessary to promote self-efficacy in health-related matters during the COVID-19 pandemic.

Abstract Background The coronavirus disease 2019 (COVID-19) caused by a novel coronavirus (SARS-CoV-2) has spread across the world at unprecedented speed and gained worldwide attention. The pandemic proved to hold an impact to humankind including dental students in all aspects of life. Dental students’ performances may indirectly be affected following the preventive measures in containing the disease. This study aims to evaluate the impact of COVID-19 pandemic on physical, mental, financial health and academic concern among dental students in Malaysia. Methods The current research implemented a cross sectional study among dental students in Malaysia. Assessment of the impact of COVID-19 on dental education was done by the distribution of a set of online survey consisting of 28 questions to dental students (n = 353) from public and private universities in Malaysia. The questionnaires include sociodemographic backgrounds and assessment on the 4 main domains. Results A total number of 353 respondents was recorded and 76.2% comprised of female. 59.7% were clinical students and 40.3% were preclinical students. 55.8% of the respondents attended hybrid mode of study. 78.0%, 76.5% and 91.8% students were concerned about their own emotional, physical health and the amount of clinical skill they acquire respectively. Year 3 students were found to be more concerned about their mental and financial health concern (p < 0.05). Conclusions COVID-19 pandemic had indeed significantly affected Malaysian dental students mainly due to fear of the quality of online learning and the amount of clinical skills acquired.

Objectives: To determine stress grades of doctors working in tertiary care  hospitals by using a standard measuring tool i.e. Kessler 10 psychological distress scale along with finding out different coping methods adopted by doctors during the COVID-19 pandemic. Study Design: Cross Sectional study. Setting: Tertiary Care Hospitals of Khyber Pakhtunkhwa (KPK), Pakistan. Period: 15 March 2020 to 20 June 2020. Material & Methods: A structured questionnaire was sent online to the junior and senior doctors working during the pandemic. The data was automatically collected with the help of Google docs and then analysed using SPSS 26. Results: Of the 219 doctors who completed the questionnaire 142 were males (64.8%) and 77 were females (35.2%). Interestingly, analysis of the score revealed that 38.8% of the doctors were found likely to be well, 16.4% of the doctors were likely to have mild disease and 20.5% were likely to have a moderate disorder while 24.2% were likely to have a severe disorder. Further analysis showed that the mean of  Kessler’s score was greater for junior doctors as  compared to Consultants which was found to be statistically significant (p=0.044). Conclusion: The impact of Corona virus disease (COVID-19) on the stress levels of health workers warrants considerable attention as it was declared a pandemic. Assessing the psychological impact and ways to tackle them will not only help the health care workers during this pandemic but will be beneficial in future pandemics as well.

Abstract Background The COVID-19 pandemic has brought unprecedented changes to medical education. However, no data are available regarding the impact the pandemic may have on medical training in Mexico. The aim of our study was to evaluate and identify the medical school students’ perceptions of the changes in their clinical training due to the pandemic in Mexico. Methods This was a cross-sectional study where a previous validated online survey was translated and adapted by medical education experts and applied to senior medical students from March to April of 2021. The 16-item questionnaire was distributed online combining dichotomous, multiple-choice, and 5-point Likert response scale questions. Descriptive and multivariate analyses were performed to compare the student’s perceptions between public and private schools. Results A total of 671 responses were included in the study period. Most participants were from public schools (81%) and female (61%). Almost every respondent (94%) indicated it was necessary to obtain COVID-19 education, yet only half (54%) received such training. Students in private schools were less likely to have their clinical instruction canceled (53% vs. 77%, p = 0.001) and more likely to have access to virtual instruction (46% vs. 22%, p = 0.001) when compared to students from public schools. Four out of every five students considered their training inferior to that of previous generations, and most students (82%) would consider repeating their final year of clinical training. Conclusions The impact of the COVID-19 on medical education in Mexico has been significant. Most final-year medical students have been affected by the cancellation of their in-person clinical instruction, for which the majority would consider repeating their final year of training. Efforts to counterbalance this lack of clinical experience with virtual or simulation instruction are needed.

ObjectivesThough dissertation is mandatory for postgraduates (PG), it is unknown if adequate knowledge on plagiarism exists at that level. Thus, we intended to study the knowledge and attitude towards plagiarism among junior doctors in India.DesignCross-sectional studySettingPG medical residents and Junior faculty from various teaching institutions across south India.ParticipantsA total of N=786 doctors filled the questionnaires of which approximately 42.7% were from government medical colleges (GMCs) and the rest from private institutions.MethodsParticipants were given a pretested semistructured questionnaire which contained: (1) demographic details; (2) a quiz developed by Indiana University, USA to assess knowledge and (3) Attitudes towards Plagiarism Questionnaire (ATPQ).Outcome measuresThe Primary outcome measure was knowledge about plagiarism. The secondary outcome measure was ATPQ scores.ResultsA total of N=786 resident doctors and junior faculty from across 11 institutions participated in this study. Of this, 42.7% were from GMCs and 60.6% were women. The mean (SD) knowledge score was 4.43 (1.99) out of 10. The factors (adjusted OR; 95% CI; p value) that emerged as significant predictors of knowledge were number of years in profession (−0.181; −0.299 to –0.062; 0.003), no previous publication (0.298; 0.099 to 0.498; 0.003) and working in a GMC (0.400; 0.106 to 0.694; 0.008). The overall mean (SD) scores of the three attitude components were: Permissive attitudes—37.33 (5.33), critical attitudes —20.32 (4.82) and subjective norms—31.05 (4.58), all of which corresponded to the moderate category.ConclusionParticipants lacked adequate knowledge on how to avoid plagiarism suggesting a need for a revamp in medical education curriculum in India by incorporating research and publication ethics.
