Dental status of geriatric patients with disorder of cognitive functions and their medical and social characteristics

Aim: analysis of medical and social characteristics and assessment of the dental status of geriatric patients with cognitive impairment.Materials and methods. The study was carried out at the Department of Therapeutic Dentistry. We analyzed the responses and laboratory results of 43 patients aged 60–92 years. The main methods used in the study were: questioning (Fox test), sialometry, saliva pH determination, microbiological analysis (of the upper respiratory tract) for fungal microflora.Results obtained showed that an objective study of the secretory function of the salivary glands according to the method of M.M. Pozharitskaya found that the average rate was 0.2 ± 0.004 ml/min, and the average saliva pH was 5.7 ± 0.04. The medical and social portrait of a geriatric patient indicates that mainly patients with impaired cognitive functions seeking dental treatment and prophylaxis were women, whose average age was 76.03 ± 2.8 years, as a rule, they were representatives of the humanitarian and natural-scientific professions, 75% are married. According to the results of the Fox test, it was found that 60% of patients complained of dry mouth of a different nature. The perception of dry mouth was statistically significantly dependent on gender (P = 0.025) and the presence of chronic diseases (P = 0.004). Women (OR = 1.17, P = 0.026) with chronic diseases (OR = 2.97, P = 0.002) were more likely to suffer from dry mouth.

Voice is a work tool for many professional groups. Currently, cases of dysphonia of multiple origin consist a growing issue. Voice disorders may result from disturbed voice production process, congenital defects, post-traumatic conditions, chronic diseases or hormonal disorders. Chronic diseases causing voice disorders include laryngopharyngeal reflux disease and esophageal reflux disease. The chronic character of reflux causes the formation of numerous morphological changes of the larynx, including: hyperemia of the mucosa limited to arytenoid and intraarytenoid area, edema of the vocal folds, edema of the larynx mucosa. These changes contribute to voice disorders. Among the pathological changes of voice organ etiologically associated with reflux, the following disease units may be distinguished: reflux laryngitis, subglottic edema, contact ulceration, larynx granuloma, larynx and pharynx cancer. Many of disorders in the upper respiratory tract are etiologically related to reflux, e.g. dysphonia, grunting, coughing and dyspnoea.

Subject. At the present stage of society development cancer incidence remains among the most pressing medical and social problems. In the structure of oncopathology a special place is occupied by malignant neoplasms of lymphoid and hematopoietic tissues which include chronic myeloid leukemia (CML). In some patients, the disease can appear with dental symptoms associated with a decrease in local immunity of the oral cavity — the appearance of aphthae, plaque on the oral mucosa, angular cheilosis, bleeding gums, dry mouth. It is diagnostically significant that such oral symptoms may precede general clinical symptoms, that is, they can be considered as early symptoms of CML. In addition, their severity often prompts a patient with CML to see a dentist. Thus, the importance of the professional knowledge of the dentist in general clinical and dental symptomatology of CML, the correct routing of the patient for further clinical and laboratory examination, treatment and observation by a hematologist is undeniably high. The goal is to study and identify the features of the dental status of patients with chronic myeloid leukemia. Methodology. The work carried out a comprehensive assessment of the state of the oral cavity and the need for dental treatment of patients with chronic myeloid leukemia in which patients with preserved systemic status and patients with chronic myeloid leukemia took part. Results. According to the results of a complex dental examination and the calculation of the main dental indices, patients with CML were found to have serious disorders in the state of their oral health. Conclusions. Patients with CML showed a high need for various types of dental care, primarily in conservative oral cavity sanitation, periodontal treatment and rationalization of oral hygiene protocols (professional and individual).

The aim of the research is to study the dental status of children with concomitant somatic pathology, to establish the interrelated correlations.To reach the aim the clinical observation of 460 12-15-year-old children was conducted: I group – children with chronic gastroduodenitis (90 people); II – children with dystonia (100); III – children with chronic diseases of the upper respiratory tract (90 people); IV – children with diabetes type 1 (80), V – children with diffuse nontoxic goiter (100 people). Dental status of children was assessed by the indices OHI-S, PMA, CPI, prevalence and intensity of dental caries (DMF - Decayed, Missing, Filled), non-carious lesions of dental hard tissues and dentoalveolar anomalies and deformities. Conclusions. The highest intensity and prevalence of dental caries and periodontal tissue diseases is observed in case of gastroenterological pathology (96.7 and 90.0%, respectively). Dental caries complications are often observed in children with diabetes mellitus (62.5%) and diseases of the gastrointestinal tract (44.4%). Dentofacial anomalies and deformities mostly occur in children with endocrinopathy (66% approximately). Poor oral hygiene, as well as the percentage of carious and extracted teeth in the DMF index structure indicates the need to improve dental care for these children.Development of prevention and treatment differentiated methods of children with concomitant somatic and dental pathology is promising.

Introduction. Dental caries has been one of the oldest but still one of the most common oral diseases in humans. Even though it has been almost eradicated in developed countries due to increased fluoride use, adoption of proper oral hygiene habits and systematic implementation of preventive programs, caries remained one of leading oral diseases in developing countries. Epidemiological data about the prevalence of caries is very important for prediction of need for dental care, number of dental care providers as well as financial aspect. The aim of this study was to determine dental status and indicate dental treatment need in adults of the municipality of Foca, Republika Srpska (Bosnia and Herzegovina). Material and Methods. The study included 244 randomly selected residents (131 males and 113 females) from the municipality of Foca which were divided into three age groups: 20-29, 30-39 and 40-49 years. Dental status and need for dental treatment were determined in accordance to the criteria and methodology of the World Health Organization. Results. Mean value of DMFT index was 12.7 for the first age group, 16.4 for the second and 20.5 for the third age group of respondents. Dominant components in the first group were restored and carious teeth while in the third age group number of extracted teeth dominated. Also, in the third age group of respondents need for one and two or more surfaces fillings was significantly lower (p<0.01), but need for removable dentures was significantly higher (p<0.01) as compared to other two groups. Conclusion. These results indicated high value of DMFT index in adult population of the municipality of Foca.
