In recent years, blood pumps have become the bridge to heart transplantation for patients with heart failure. Portability and wearability of blood pumps should be considered to ensure patient satisfaction in everyday life. To date, the focus has been on the development of portable and wearable peripheral components, little attention has been paid to the portable and wearable performance of the blood pump itself. This study reported a novel design of a wearable and portable extracorporeal centrifugal blood pump. Based on an in-house centrifugal maglev blood pump, the wearable and portable blood pump was designed with parallel inlet and outlet pipes to improve the wearable performance. A ring cavity was set at the inlet to convert the circumferential velocity of the inlet pipe to an axial velocity. The hydraulic and hemolytic performance of the baseline and portable blood pumps were analyzed and compared. Compared with the baseline pump, the hydrodynamic and hemolytic performance of the portable pump has been maintained without serious degradation. The results of this study will improve the life quality of patients with heart failure, and enhance the clinical benefits of artificial heart.

Introduction: The quality of life of patients with heart failure is influenced by several factors, one of which is spiritual factors. Tri Hita Karana (THK) is one of the concepts of the life order of the Balinese people, which basically teaches about maintaining harmony between humans and God (prahyangan), humans and humans (pawongan), humans with the environment (palemahan). This study aimed to determine the relationship between THK implementation on the quality of life of heart failure patients at Regional Hospital of Mangusada. Methods: A cross-sectional design study was used. In total 41 heart failure patients who visited Mangusada Hospital between April to May 2020, selected using a purposive sampling technique. The research instrument used was the attitude and behavior implementation of THK and the Minnesota Living with Heart Failure Questionnaire (MLHFQ). The Spearman rank test was used in this study. Results: There were a significant correlation between Tri Hita Karana's Implementation and Life Quality of Heart Failure Patients in Mangusada Regional Hospital with p-value (0.001), where p < α (0.05). Conclusion: The better the implementation of THK, the better the quality of life for patients with heart failure. The results of this study were expected to be used in providing education to patients by implementing THK to improve the quality of life for patients with heart failure.

Low hemolysis is an important factor for axial blood pumps that has been used in patients with heart failure. The structure of impellers plays a key role in the hemolytic properties of axial blood pumps. Axial blood pumps with various structure impellers exhibit different hemolytic characteristic. In the present study, we aimed to investigate the type of impellers structures in axial blood pumps that contain the best low hemolytic properties. Also, it is expensive and time-consuming to validate the axial blood pump's hemolytic property by in vivo experiments. Therefore, in the present study, the numerical method was applied to analyze the hemolytic property in a blood pump. Specifically, the hemolysis of the pump was calculated by using a forward Euler approach based on the changes in shear stress and related exposure times along the particle trace lines. The different vane structures and rotational speed that affect hemolysis were analyzed and compared. The results showed that long–short alternant vanes exhibited the best hemolytic property which could be utilized in the optimization design of axial blood pumps.

Exercise training (ET) is now recommended as an important component of a comprehensive approach to patients with heart failure (HF). Despite the existence of proven benefits of ET, many HF patients remain physically inactive. Introducing telerehabilitation (TR) may eliminate most of the factors that result in the currently low number of patients undergoing outpatient-based rehabilitation programmes and thus increase the percentage of those who will undergo cardiac rehabilitation. Despite the fact that TR is highly applicable and effective, there are few papers dedicated to the study of TR in HF patients. Until recently, only a couple of home rehabilitation-monitoring models have been presented, from the simplest, i.e. heart rate monitoring and transtelephonic electrocardiographic monitoring, through to the more advanced tele-electocardiogram (tele-ECG) monitoring (via a remote device) and realtime electrocardiographic and voice transtelephonic monitoring. It seems the last two are the most useful and reliable. Based on published studies, TR in HF patients could be equally effective as and provide similar improvements in health-related quality of life to standard outpatient cardiac rehabilitation. In addition, adherence to cardiac rehabilitation seems to be better during TR. Due to disease-related limitations, TR seems to be a viable alternative for comprehensive cardiac rehabilitation in HF patients. Further studies are needed to confirm the utility of this type of rehabilitation in routine clinical practice, including its cost-effectiveness. Because of the diversity of technological systems, it is necessary to create a platform to ensure compatibility between the devices used in telemedicine.

Patients with heart failure have difficulty in self-care management, as daily monitoring and recognition of symptoms do not readily trigger an action to avoid hospital admissions. The purpose of this study was to understand the impact of a nurse-led complex intervention on symptom recognition and fluid restriction. A latent growth model was designed to estimate the longitudinal effect of a nursing-led complex intervention on self-care management and quality-of-life changes in patients with heart failure and assessed by a pilot study performed on sixty-three patients (33 control, 30 intervention). Patients in the control group had a higher risk of hospitalisation (IRR 11.36; p < 0.001) and emergency admission (IRR 4.24; p < 0.001) at three-months follow-up. Analysis of the time scores demonstrated that the intervention group had a clear improvement in self-care behaviours (βSlope. Assignment_group = −0.881; p < 0.001) and in the quality of life (βSlope. Assignment_group = 1.739; p < 0.001). This study supports that a nurse-led programme on symptom recognition and fluid restriction can positively impact self-care behaviours and quality of life in patients with heart failure. This randomised controlled trial was retrospectively registered (NCT04892004).
