The role of thoracic surgery in the management of hyperhidrosis is well-known and thoracoscopic sympathetic interruption is commonly accepted as being the most effective treatment. However, some concerns still remain regarding the potential to develop compensatory hyperidrosis (CH), the most troublesome and frequent side effect after surgery and its management. Compensatory hyperidrosis prevention may be achieved by identifying subjects at higher risk and/or targeting nerve interruption level on the base of single patient characteristics gathered during the preoperative survey. Furthermore, the surgical treatment may consist of different techniques aimed at reversing the effects of previous sympathetic interruption. To predict CH after sympathectomy, the most interesting proposals in recent literature are a temporary thoracoscopic sympathetic block and the introduction of new and targeted preoperative surveys. If the role of nerve clipping technique vs. the definitive cutting is still intensely under debated, new approaches have been recently proposed to reduce the incidence of CH. In particular, extended sympathicotomy has been described as an alternative to overcome severe forms. Last, among the techniques developed to reverse sympathetic interruption effect, diffuse sympathicotomy (DS) and microsurgical sympathetic trunk reconstruction represent advances in this field. An all-round review of these topics is strongly needed. Our aim is to cover all the above issues point by point. Although sympathectomy represents a small part of thoracic surgery, we believe that it is worthy of interest because of the profound effect that complications for a benign condition can have on patients.

This chapter addresses a very simple question: is it possible to frame coinage in the Early Middle Ages? The answer will be certainly yes, but will also acknowledge that we lack considerable amounts of relevant data potentially available through state-of-the-art methodologies. One problem is, though, that many times we do not really know the relevant questions we can pose on coins; another is that we still have not figured out the social role of coinage in the aftermath of the Roman Empire. This chapter shows a number of things that could only be known thanks to the analysis of coins. And as its title suggests it will also include some reflections on greed and generosity.

After reviewing some of the recent literature on non-causal and mathematical explanation, this chapter develops an argument as to why renormalization group (RG) methods should be seen as providing non-causal, yet physical, information about certain kinds of systems/phenomena. The argument centres on the structural character of RG explanations and the relationship between RG and probability theory. These features are crucial for the claim that the non-causal status of RG explanations involves something different from simply ignoring or “averaging over” microphysical details—the kind of explanations common to statistical mechanics. The chapter concludes with a discussion of the role of RG in treating dynamical systems and how that role exemplifies the structural aspects of RG explanations which in turn exemplifies the non-causal features.

In a natural gravel-bed stream, the bed that has an organized roughness structure created by the streamflow is called the water-worked gravel bed (WGB). Such a bed is entirely different from that created in a laboratory by depositing and spreading gravels in the experimental flume, called the screeded gravel bed (SGB). In this paper, a review on the state-of-the-art research on WGBs is presented, highlighting the role of water-work in determining the bed topographical structures and the turbulence characteristics in the flow. In doing so, various methods used to analyze the bed topographical structures are described. Besides, the effects of the water-work on the turbulent flow characteristics, such as streamwise velocity, Reynolds and form-induced stresses, conditional turbulent events and secondary currents in WGBs are discussed. Further, the results form WGBs and SGBs are compared critically. The comparative study infers that a WGB exhibits a higher roughness than an SGB. Consequently, the former has a higher magnitude of turbulence parameters than the latter. Finally, as a future scope of research, laboratory experiments should be conducted in WGBs rather than in SGBs to have an appropriate representation of the flow field close to a natural stream.

Health literacy is a critically important determinant of health and is influenced by access to supportive social networks and services. Global investment in education throughout the life course is required to support health literacy development. The aim of this review is to characterize the role, responsibilities, and the optimal setting for the emergent role of a Health Literacy Mediator (HLM). A scoping review of recent literature was conducted. The review revealed a lack of consensus on who should be teaching health literacy, and variability in confidence when teaching health literacy. Professionals reported facing barriers such as a lack of time, a lack of knowledge, and recognized that the health literacy needs of children worldwide are not being met. Further research into the role of HLM is required to determine who is best suited to this role and what their responsibilities will be to ensure consistent health literacy education.

AbstractSince at least the 1980s, the role of adversariality in argumentation has been extensively discussed within different domains. Prima facie, there seem to be two extreme positions on this issue: argumentation should (ideally at least) never be adversarial, as we should always aim for cooperative argumentative engagement; argumentation should be and in fact is always adversarial, given that adversariality (when suitably conceptualized) is an intrinsic property of argumentation. I here defend the view that specific instances of argumentation are (and should be) adversarial or cooperative to different degrees. What determines whether an argumentative situation should be primarily adversarial or primarily cooperative are contextual features and background conditions external to the argumentative situation itself, in particular the extent to which the parties involved have prior conflicting or else convergent interests. To further develop this claim, I consider three teloi that are frequently associated with argumentation: the epistemic telos, the consensus-building telos, and the conflict management telos. I start with a brief discussion of the concepts of adversariality, cooperation, and conflict in general. I then sketch the main lines of the debates in the recent literature on adversariality in argumentation. Next, I discuss the three teloi of argumentation listed above in turn, emphasizing the roles of adversariality and cooperation for each of them.

Exosomes (EXOs) are nano-sized informative shuttles acting as endogenous mediators of cell-to-cell communication. Their innate ability to target specific cells and deliver functional cargo is recently claimed as a promising theranostic strategy. The glycan profile, actively involved in the EXO biogenesis, release, sorting and function, is highly cell type-specific and frequently altered in pathological conditions. Therefore, the modulation of EXO glyco-composition has recently been considered an attractive tool in the design of novel therapeutics. In addition to the available approaches involving conventional glyco-engineering, soft technology is becoming more and more attractive for better exploiting EXO glycan tasks and optimizing EXO delivery platforms. This review, first, explores the main functions of EXO glycans and associates the potential implications of the reported new findings across the nanomedicine applications. The state-of-the-art of the last decade concerning the role of natural polysaccharides—as targeting molecules and in 3D soft structure manufacture matrices—is then analysed and highlighted, as an advancing EXO biofunction toolkit. The promising results, integrating the biopolymers area to the EXO-based bio-nanofabrication and bio-nanotechnology field, lay the foundation for further investigation and offer a new perspective in drug delivery and personalized medicine progress.

AbstractThis paper argues that open-mindedness is a corrective virtue. It serves as a corrective to the epistemic vice of confirmation bias. Specifically, open-mindedness is the epistemically virtuous disposition to resist the negative effects of confirmation bias on our ability to reason well and to evaluate evidence and arguments. As part of the defense and presentation of our account, we explore four discussions of open-mindedness in the recent literature. All four approaches have strengths and shed light on aspects of the virtue of open-mindedness. Each mentions various symptoms of confirmation bias and some explore aspects of the corrective role of open-mindedness. However, ours is the first to explicitly identify open-mindedness as a corrective virtue to the specific epistemic vice of confirmation bias. We show how the corrective account also permits a response to the concern that open-mindedness might not actually count as a virtue.

Chitosan-based edible coatings represent an eco-friendly and biologically safe preservative tool to reduce qualitative decay of fresh and ready-to-eat fruits during post-harvest life due to their lack of toxicity, biodegradability, film-forming properties, and antimicrobial actions. Chitosan-based coatings modulate or control oxidative stress maintaining in different manner the appropriate balance of reactive oxygen species (ROS) in fruit cells, by the interplay of pathways and enzymes involved in ROS production and the scavenging mechanisms which essentially constitute the basic ROS cycle. This review is carried out with the aim to provide comprehensive and updated over-view of the state of the art related to the effects of chitosan-based edible coatings on anti-oxidant systems, enzymatic and non-enzymatic, evaluating the induced oxidative damages during storage in whole and ready-to-eat fruits. All these aspects are broadly reviewed in this review, with particular emphasis on the literature published during the last five years.

Non-small cell lung cancers (NSCLC) are different today, due to the increased use of screening programs and of innovative systemic therapies, leading to the diagnosis of earlier and pre-invasive tumors, and of more advanced and controlled metastatic tumors. Surgery for NSCLC remains the cornerstone treatment when it can be performed. The role of surgery and surgeons has also evolved because surgeons not only perform the initial curative lung cancer resection but they also accompany and follow-up patients from pre-operative rehabilitation, to treatment for recurrences. Surgery is personalized, according to cancer characteristics, including cancer extensions, from pre-invasive and local tumors to locally advanced, metastatic disease, or residual disease after medical treatment, anticipating recurrences, and patients’ characteristics. Surgical management is constantly evolving to offer the best oncologic resection adapted to each NSCLC stage. Today, NSCLC can be considered as a chronic disease and surgery is a valuable tool for the diagnosis and treatment of recurrences, and in palliative conditions to relieve dyspnea and improve patients’ comfort.
