This study aimed to compare the acute effect of a proprioceptive exercise session and a non-specific exercise session on knee position sense, and the static and dynamic balance of athletes. Sixty male athletes (19.4 ± 1.2 years) participated in a within-subjects repeated-measures study. Knee position sense in closed kinetic chain, and static (BESS test) and dynamic balance (Y-balance test) were measured before and after two exercise sessions, consisting of 10 min of non-specific exercise in a cycle-ergometer or proprioceptive exercise with an unstable platform. Overall, both exercise sessions significantly improved knee position sense, BESS score, and YBT composite score, and no differences were detected between proprioceptive and non-specific sessions (knee position sense, −6.9 ± 65.2% vs. −11.5 ± 75.0%, p = 0.680; BESS, −19.3 ± 47.7% vs. −29.03 ± 23.5%, p = 0.121; YBT, 2.6 ± 2.7% vs. 2.2 ± 2.2%, p = 0.305). Twenty athletes did not improve knee position sense after the exercise session (non-responders). When analyzing only the exercise responders, both sessions improved knee position sense, but the improvement was greater after the proprioceptive exercise session (56.4 ± 25.6% vs. 43.8 ± 18.9%, p = 0.023). In conclusion, a single proprioceptive, as well as non-specific, exercise session increased knee position sense and balance. The proprioceptive exercise seems to be more effective in improving joint position sense when considering only athletes who respond to the intervention.

The purpose of this study was to determine if a difference exists in the reproducibility of knee joint flexion angles in an open versus a closed kinetic chain. Thirty generally healthy subjects (12 males, 18 females; mean age 23.8 years) participated. Subjects actively reproduced small, medium, and large knee flexion angles (with target angles of 15°, 45°, and 75°, respectively) in an open and a closed kinetic chain while being videotaped. Goniometric measurements were taken from the videotape of initial and reproduced joint angles. Data were analyzed using ANOVA with repeated measures on kinetic chain test position and joint angle. Subjects more accurately reproduced knee flexion angles in a closed kinetic chain position. The main effect for angle and the interaction of angle and test position were nonsignificant. The results indicate that knee joint position is more accurately reproduced in closed kinetic chain. Closed kinetic chain testing is also a more functional assessment of joint position sense, and thus closed kinetic chain assessment of lower extremity joint position sense is recommended.

Context: Rounded shoulder posture is a common problem in the athletic population. Recently Kinesio tape has been utilized to improve balance, proprioception, and posture. However, the literature has been unable to provide definitive answers on the efficacy of Kinesio tape use. Objective: To determine the immediate effect of the limb rotational Kinesio tape application on the dynamic balance and proprioception of the shoulder measured by the Y-Balance Upper Quarter Test (YBT-UQ) in male collegiate athletes. Design: Cross-sectional. Setting: Sports medicine research laboratory.Participants: Nineteen healthy male collegiate National Association of Intercollegiate Athletics athletes (including rodeo, baseball, football, and soccer) with a mean age of 19.8 (1.4) years. Interventions: Subjects were randomized into Kinesio tape and non-Kinesio tape groups. The limb rotational Kinesio tape application was applied to the Kinesio tape group, while the non-Kinesio tape group received no intervention. Each group performed the YBT-UQ, which requires reaching in 3 directions in a push-up position, before and after the randomized intervention on a single day. Main Outcome Measures: The variables of interest included the maximum reach distance in each of the 3 directions and the composite score for both trials between the Kinesio tape and non-Kinesio tape groups. Each score was normalized against the subject’s limb length. Results: No statistically significant improvements in any YBT-UQ scores were observed following either the Kinesio tape or non-Kinesio tape intervention. Conclusions: Applying the limb rotational Kinesio tape technique did not improve immediate YBT-UQ scores in a male collegiate athletic population with rounded shoulder posture. The use of Kinesio tape to improve immediate closed kinetic chain function in male collegiate athletes with rounded shoulder posture cannot be supported.

Context:Fatigue of the shoulder rotator muscles may negatively affect joint position sense (JPS) and ultimately lead to injury.Objective:Recovery of shoulder JPS after muscle fatigue.Design:A repeated-measures study.Setting:Musculoskeletal research laboratory.Patients:Thirteen subjects participated in joint position error tests and isokinetic concentric strength assessment in shoulder rotation, before and after rotator muscle fatigue.Interventions:Local muscle fatigue was induced using isokinetic concentric contractions of the shoulder rotator muscles.Main Outcome Measurements:Shoulder rotator strength and JPS error signals were measured before fatigue, immediately after fatigue, and every ten minutes thereafter for one hour.Results:Before shoulder rotation muscle fatigue, the accuracy of shoulder JPS was 2.79 ± 1.67 degrees. After muscle fatigue, the accuracy decreased to 6.39 ± 2.90 degrees. Shoulder JPS was influenced up to 40 minutes after muscle fatigue, but shoulder strength was only affected for 10 minutes after muscle fatigue.Conclusions:Proprioceptive recovery was slower than strength following fatigue of the shoulder rotators.

When the upper extremity is injured, open kinetic chain (OKC) exercises are primarily used to increase strength and restore functional ability—the goals of rehabilitation. It is also imperative, however, that the receptors responsible for static and dynamic stabilization of the joint be trained. This can be done with closed kinetic chain (CKC) exercises. The purposes of this study were to investigate the effect of a 4-week CKC training program on the neuromuscular control of the upper extremity and to determine whether there was a significant difference between skill-dominant limb and nondominant limb stability indices. Thirty-two physically active participants (14 men, 18 women) were tested on the FASTEX 4 weeks apart. The training group's scores significantly improved, whereas the control group's scores remained the same. It was concluded that the CKC training significantly improved the training group's ability to remain stable. The results suggest that CKC training can increase the accuracy of joint position sense because of increased stimulation of the mechanoreceptors.

Closed kinetic chain exercise is a common component of lower extremity rehabilitation. It has virtually replaced open kinetic chain exercise in the treatment of some conditions. In this paper, anatomy and physiology as they relate to closed chain exercise are examined to elucidate its unique contributions to rehabilitation. Claims made about the specificity, functionality, and safety of closed kinetic chain exercise are discussed. Muscle action, the stretch–shortening cycle, joint position sense, and clinical cases are used to illustrate the distinct role of closed kinetic chain exercise in rehabilitation.

Context:Proper functioning of the intrinsic foot musculature (IFM) is essential in maintaining the integrity of the medial longitudinal arch (MLA). Improper functioning of the IFM leads to excessive pronation of the foot, which has been linked to various pathologies. Therefore, training the IFM to avoid excessive pronation may help prevent some of these pathologies; however, it is not clear how to train these muscles optimally.Objective:To investigate the effects of 2 different types of IFM training on the height of the MLA and static- and dynamic-balance task performance.Design:Randomized controlled trial, repeated-measures mixed-model design.Setting:University biomechanics laboratory for testing and a home-based training program.Participants:24 healthy, university-age volunteers (3 groups of 8) with no history of major lower limb pathology or balance impairment.Interventions:One experimental group performed 4 wk of the short-foot exercise (SFE) and the other performed 4 wk of the towel-curl exercise (TCE). Participants were asked to perform 100 repetitions of their exercise per day.Main Outcome Measures:Navicular height during weight bearing, the total range of movement of the center of pressure (COP) in the mediolateral (ML) direction for a static-balance test and a dynamic-balance test.Results:There were no differences in the navicular height or static-balance tests. For the dynamic-balance test, all groups decreased the ML COP movement on the dominant limb by a small amount (~5 mm); however, the SFE group was able to decrease COP movement much more than the TCE group in the nondominant limb.Conclusions:The SFE appeared to train the IFM more effectively than the TCE; however, there were differing results between the dominant and nondominant legs. These imbalances need to be taken into consideration by clinicians.

Abstract Background: Assessment of shoulder proprioception outside the laboratory in the absence of specialized equipment remains a challenge with field-based tests often lacking good reliability, validity and feasibility. This study aimed to enhance the knowledge base surrounding the assessment of shoulder proprioception and investigated the effect of fatigue on shoulder joint position sense (JPS) amongst amateur male handball players.Method: 27 healthy recreationally active participants and 13 amateur male handball players undertook two sessions of active JPS tests using laser pen technology and a calibrated 2-dimensional target to assess test-retest reliability. The active JPS test was then utilized on the subgroup of handball players who were subjected to five bouts of a repeated throwing task in order to investigate the effect of local fatigue on shoulder JPS. Results: The intraclass correlation coefficient for the active JPS test was 0.78 (95% CI = [0.57; 0.89]). Standard error of measurement between trials was 0.70° (range 0.57°-0.90°). For the throwing task, repeated measures analysis of variance revealed a significant interaction for arm x bout (F5=2.74, p=0.028) and a significant effect for arm (F1=5.85, p=0.034). Post hoc analysis showed a significant difference between throwing arm and non-throwing arm after throwing bout one (p=0.036), three (p=0.026) and four (p=0.041). Conclusion: Assessment of the active JPS test showed acceptable reliability and measurement error. Repeated throwing to fatigue decreased shoulder JPS in amateur male handball players which indicate validity of the test to identify reduced proprioception.

To investigate acutely the effects of different stretching exercises on football players on the basic motoric properties (strength, flexibility, speed, balance and explosive force). 10 volunteer football players with an average age of 17 participated in the study. Three different measurements protocols were used in the study. The first measurement was performed without stretching exercise, the second measurement was performed after the static stretching exercise and the third measurement was performed after the dynamic stretching exercise. All measurements were carried out at Manisa Tarık Almış sports facilities. To determine the performances of volunteers; back strength, leg strength, hand grip strength, flexibility, 20 m. sprint and flamingo balance test. The Repeated Measures ANOVA test and the Bonferroni Test were used using the SPSS 23.0 package program to analyze the relationship between the performance of the study group and performance tests on different days and different protocols. When the three different methods applied to participants were compared; there is a statistically significant difference between sit and reach, speed and balance tests. As a result, there is a positive effect of stretching exercises on flexibility, sprint and dynamic balance skills. ÖzetFutbolcularda farklı germe egzersizlerinin temel motorik özellikler (kuvvet, esneklik, sürat, denge ve patlayıcı kuvvet) üzerine etkilerini akut olarak incelemektir. Çalışmaya yaş ortalamaları 17 olan 10 gönüllü futbolcu katılmıştır. Çalışmada 3 farklı ölçüm protokolü kullanılmıştır. Birinci ölçüm germe egzersizi yapmadan, ikinci ölçüm statik germe egzersizinden sonra ve üçüncü ölçüm dinamik germe egzersizinden sonra gerçekleştirildi. Tüm ölçümler Manisa Tarık Almış Spor Tesislerinde yapılmıştır. Katılımcıların performanslarını belirlemek için sırt kuvveti, bacak kuvveti, el kavrama kuvveti, esneklik, 20 m. sprint, flamingo denge testi uygulanmıştır. Araştırma grubunun farklı günlerde ve farklı germe egzersizlerini takiben uygulanan performans testlerinin aralarındaki ilişkiyi analiz etmek için SPSS 23.0 paket programından, Repeated Measures Testi ve Bonferroni Testi kullanılmıştır. Katılımcılara uygulanan three farklı ölçüm metodu karşılaştırıldığında; otur-eriş, sürat ve denge testleri arasında istatistiksel olarak anlamlı farklılık vardır (p<0.05). Sonuç olarak, germe egzersizlerinin esneklik, sürat ve denge üzerinde olumlu etkisi vardır.

Context: Athletes with intellectual disability (ID) have a high risk of injury while participating in various sports. Warm-up (WU) is the most preventive measure to reduce injuries in sports. Objective: To investigate the effects of dynamic stretching WU (DS-WU) and plyometric WU (PL-WU) on dynamic balance in athletes with ID. Design: Crossover study. Setting: Research laboratory. Participants: A total of 12 athletes with ID (age 24.5 [3.22] y, height 165.7 [8.4] cm, weight 61.5 [7.1] kg, intelligence quotient 61.1 [3.5]). Main Outcome Measures: Dynamic balance was assessed using the Star Excursion Balance Test (SEBT) at pre-WU, post-WU, and 15 minutes post-WU for both the DS-WU and the PL-WU. A 2-way analysis of variance (3 sessions × 2 WU methods) with repeated-measures was used in this study. Results: Following the DS-WU, participants demonstrated significant improvements in the SEBT composite score post-WU (89.12% [5.54%] vs 87.04% [5.35%]; P < .01) and at 15 minutes post-WU (89.55% [5.28%] vs 87.04%, P < .01) compared with pre-WU. However, no significant difference between these two post-WU scores (post-WU and 15 min post-WU) was found. For the PL-WU, participants demonstrated a significant decrease in the SEBT composite score at post-WU (85.95% [5.49%] vs 87.02% [5.73%]; P < .05); however, these scores increased significantly at 15 minutes post-WU (88.60% [5.42%] vs 87.02% [5.49%]; P < .05) compared with that at pre-WU. The SEBT composite scores are significantly higher in the DS-WU than in the PL-WU at both post-WU sessions (P < .05). Conclusion: Both DS-WU and PL-WU could improve dynamic balance and may be recommended as WUs in athletes with ID; however, particular caution should be exercised immediately after the PL-WU.

The primary objective of the present study was to compare the effect of a neoprene sleeve on knee joint position sense during a sitting open kinetic chain test and a supine closed kinetic chain test. Young (24 2 years old), healthy subjects (18 men and 18 women) performed knee joint angle replication tests during open kinetic chain knee extension (sitting) and closed kinetic chain leg press (supine with an axial load of 15% body weight) before and after application of a neoprene sleeve over the dominant knee. The improvement in ability to replicate joint angles after application of the sleeve (sleeve effect) was significantly less during the supine closed kinetic chain test (0.3° 1.4°) than during the sitting open kinetic chain test (1.2° 1.1°). The sleeve effect was inversely related to subjects' performance without the sleeve during both the sitting open kinetic chain and supine closed kinetic chain tests, suggesting that some people may derive greater benefit from the sleeve than others. Although the sleeve effects were small, particularly during the supine closed kinetic chain test, 72% of subjects felt that the sleeve improved their overall test performance. Future research is needed to establish the functional relevance of the small sleeve effects observed and to identify the characteristics of people who might derive greatest benefit from sleeve use.
