Background: Infantile colic is characterized by prolonged periods of inconsolable, incessant crying and persistent fussing in an otherwise healthy infant. It is a self-limiting condition, but causes significant stress to mothers. AIM: To observe the role of Lactobacillus reuteriDSM 17938 in reducing crying time in colicky infants in routine clinical practice. Methods: This was a prospective observational multicentric clinic-based study. Each practitioner included approximately 30 infants < 5 months of age with infantile colic who were prescribed L. reuteri DSM 17938 for a period of 21 days. There were four physical consultations and two telephonic consultations. The parents were given a daily diary to record the duration of crying and fussing episodes and a questionnaire was administered during the consultations. Results: A total of 120 infants with a mean age of 56.9 ± 34.2 days were included in this 28-day study. The mean crying time as reported by the parents in the subject diary reduced from 248.2 ± 101.2 min, 95% CI: 229.45, 266.94 at baseline to 45.6 ± 79.1 min 95% CI: 31.02, 60.31 at study end (P < 0.01). The clinical response (defined as reduction of 50% in crying time) was observed in 85% of subjects at study end. The fussiness and parental perception of colic recorded during the consultations were reduced by 66% and 72%, respectively, at study end. The maternal depression scores were reduced to 63% at study end. Conclusion: L. reuteri DSM 17938 was associated with a significant reduction in crying time in colicky infants, and showed improvement in maternal depression.

AbstractLearning is a complex phenomenon. Contemporary theories of education underline active participation of learners in their learning processes. One of the key arguments supporting this approach is the learner’s simultaneous and unconscious development of their ability of “learning to learn”. This ability belongs to the soft skills highly valued by employers today. For Mathematics Education, it means that teachers have to go beyond making calculations and memorizing formulas. We have to teach the subject in its social context. When the students start understanding the relationship between real-life problems and the role of numbers and formulas for their solutions, their learning becomes a part of their tacit knowledge. Below we explain the theoretical background of our approach and provide examples of such activities.

Background. Although infliximab (IFX) has been available since 2005, there are very little data on the long-term drug survival of infliximab in real-life. Objective. Our aim was to identify and describe psoriasis patients treated with IFX for longer than 6 years. Methods. Psoriasis patients treated with IFX for longer than 6 years were retrospectively included. Demographic and clinical data were collected in May 2018. Results. Between January 2005 and December 2012, 43 patients were maintained on IFX for 6 years or longer. IFX was introduced as a 4.5 line of systemic therapy. The mean duration of IFX treatment was 8.5 years (6–12). In May 2018, 30 patients (70%) were still maintained on IFX at 4–6 mg/kg every 8–10 weeks with an efficiency of about 100%. IFX was stopped in 13 patients (30%) mainly for loss of efficacy in 6 patients (46%). Three patients developed solid cancer including bladder cancer, lung cancer, and prostate cancer. Limitation. Retrospective study. Conclusion. We report the efficacy and safety of IFX maintained for up to 12 years in psoriasis patients. The long-term use of IFX was associated with a high BMI confirming the critical role of weight-based dosing for this drug.

Biomedical studies often involve an event that occurs to individuals at different times and has a significant influence on individual trajectories of response variables over time. We propose a statistical model to capture the mean trajectory alteration caused by not only the occurrence of the event but also the subject-specific time of the event. The proposed model provides a post-event mean trajectory smoothly connected with the pre-event mean trajectory by allowing the model parameters associated with the post-event mean trajectory to vary over time of the event. A goodness-of-fit test is considered to investigate how well the proposed model is fit to the data. Hypothesis tests are also developed to assess the influence of the subject-specific time of event on the mean trajectory. Theoretical and simulation studies confirm that the proposed tests choose the correctly specified model consistently and examine the effect of the subject-specific time of event successfully. The proposed model and tests are also illustrated by the analysis of two real-life data from a biomarker study for HIV patients along with their own time of treatment initiation and a body fatness study in girls with different age of menarche.

The goal of many mathematicians since the dawn of time has been to apply mathematics to practical applications and to also derive the mathematics behind many everyday things. Although we seldom have such pursuits, many everyday things have been affected by the principles of math. And things like the internet, run on fundamental mathematics principles, and hardly any credit is given to the subject. This paper aims to uncover contributions of everyday Math in the working of the internet. From encryption and decryption all the way to how search engines to index various web pages online, if one looks hard enough, concepts related to mathematics are bound to pop up. This paper also sheds light on various concepts taught in higher education that are often forgotten and only treated as something solely scholastic, but in reality, has a lot of applications in real life.

The article analyzes the possibilities of using the DIKW-model in relation to educational proposals. The analysis brings us back to understanding the concepts of "data", "information", "knowledge" and wisdom, where the goal of teaching is to obtain knowledge, and the learning process itself leads to wisdom. The definitions of these basic concepts are considered, which will prevent their misinterpretation and substitution. It will give an opportunity to develop a model of training, which in the future will form the required specialist. The structure of this model is considered, the connections between its components are highlighted, which are of paramount importance for the development of an effective educational proposal within the framework of innovative education and increase of independent work of the higher education seeker, where the role of the teacher changes. The preparedness for real life, forming the necessary competences and teaching the student as a specialist and personality become one of the primary tasks. First, to move to the concept of "learning", it is necessary to specify the definitions of "information" and "knowledge" to understand in which case the presentation of information on the subject will lead to the formation of knowledge and in which it will remain information. We understand the need to form a future specialist’s "knowledge" and opportunities related to the category of "wisdom" under the introduction of the DIKW methodology in the educational environment. "Knowledge" cannot be replaced by an information flow or a data flow. This is a process of multiple processing of information, data, related knowledge and the copyright of the studied information is not an indicator of effectiveness. The indicator of effectiveness was defining as the ability to make the decision that is most appropriate in a given situation.

Purpose: To describe the anatomical and functional outcomes in patients with exudative age-related macular degeneration (AMD) undergoing ranibizumab therapy in real-life practice. Methods: This is a retrospective analysis of patients with exudative AMD treated with ranibizumab. Visual acuity (VA) and optic coherence tomography characteristics at baseline and at the end of the follow-up, clinical forms of the disease, delay between diagnosis and treatment as well as the number of follow-up visits and of intravitreal injections were collected. Results: One hundred and seventy-nine patients (220 eyes) were followed up during a mean of 24 months. The mean delay between diagnosis and treatment was 20.3 days (SD ±16.8). VA stabilization was observed in 46.4% of eyes, 21.7% of eyes gained ≥15 ETDRS (Early Treatment Diabetic Retinopathy Study) letters and 31.9% lost ≥15 ETDRS letters. The mean central retinal thickness decreased from 380.6 μm at baseline to 295.6 µm at the final examination. A lower baseline VA score was associated with a greater gain of letters (OR 1.04, 95% CI 1.02-1.06; p < 0.001). Conclusion: Shortening the delays in diagnosis appears to be a key point in real-life situations.

The lack of female participation in antipsychotic trials for schizophrenia poses an important issue regarding its applicability, with direct and real-life repercussions to clinical practice. Here, our aim is to systematically review the sampling sex bias among randomized clinical trials (RCTs) of second-generation antipsychotics—namely risperidone, olanzapine, quetiapine, ziprasidone, and aripiprazole—as an update to a previous 2005 review. We searched MEDLINE and the Cochrane database for studies published through 7 September 2020 that assessed adult samples of at least 50 subjects with a diagnosis of schizophrenia, schizophrenia spectrum disorder, or broad psychosis, in order to investigate the percentage of women recruited and associated factors. Our review included 148 RCTs, published from 1993 to 2020, encompassing 43,961 subjects. Overall, the mean proportion of women was 34%, but only 17 trials included 50% or more females. Younger samples, studies conducted in North America, pharmaceutical funding and presence of specific exclusion criteria for women (i.e., pregnancy, breast-feeding or lack of reliable contraceptive) were associated with a lower prevalence of women in the trials. Considering the possible different effects of antipsychotics in both sexes, and our lack of knowledge on the subject due to sampling bias, it is imperative to expand actions aimed at bridging this gap.

Previous studies have shown that, while color has the property to increase the production of responses to the Rorschach, such an effect is not present in children’s protocols. Based on a review of the literature, the author highlights the characteristic of high reactive demand of the subject to color stimulus and traces its evolution to the preoperational period, where color nominations and the incongruent use of color, especially of pure color, are frequent. With the entry in the period of concrete operations such responses become rare or even disappear. In turn, the mean number of responses to color cards decreases between 5 to 6 and 11 years, and their form quality goes down as well. The cause of this change is the emergence and development of logical thinking. Given the intensity of color stimulus, the child does not yet have the equipment that will allow her to later integrate color and object as they really are.
