To improve the independence of brain impairment patients in ADL, we sought to identify influential parameters from information commonly collected in hospitals, prioritize the factors, and specify the degree to which those factors are necessary. In total, 64 patients with hemiplegia, who had been admitted to the one of the authors, were examined using various evaluation tools. Afterwards, we checked the difference between the capable group and the incapable group with an independent t-test or chi-squared test to determine the significant factors, and we prioritized the significant factors with Spearman’s rho test. The degree of their necessity was determined with the ROC curve. Standing balancing ability and hip joint and knee extensor strength are necessary in most ADL except for eating. In order to independently perform most ADL except eating and one gait cycle, the strength of the knee extensor and hip flexor was required to exceed grade 3 on the MRC scale. However, one gait cycle was possible even if the strength of the hip joint and knee extensors rated lower than 3 on the MRC scale. Additionally, upper limb motor recovery was required up to the distal parts in the order of bathing, dressing, and grooming.

This study’s purpose was to investigate whether a 10-week low-intensity strength-training program could improve strength of the knee extensors and functional ability. Participants 65 years and older with low knee-extensor muscle strength were randomized into an exercise (n = 11) and a control group (n = 10). Knee-extensor strength and functional ability were measured before and after the program and again 6 months later. Knee-extensor strength (Nm) increased by 54% (13% in the control) by the end of the training program (F = 13.02, p = .01), and most of this improvement was still present 6 months later. The program had a beneficial effect on functional tasks, especially the time taken to rise from a chair in combination with a 3-m walk (F = 3.99, p = .03) and self-reported ability related to lower extremity performance (F = 6.97, p = .02). It seems that this program could contribute to improving functional ability in frail older people.

Carnitine deficiency is prevalent in patients undergoing hemodialysis, and it could result in lowered muscle strength. So far, the effect of treatment with levocarnitine on lower limb muscle strength has not been well described. This observational study examined the association between treatment with levocarnitine with the change in knee extensor strength (KES) in hemodialysis patients. Eligible patients were selected from the participants enrolled in a prospective cohort study for whom muscle strength was measured annually. We identified 104 eligible patients for this analysis. During the one-year period between 2014 to 2015, 67 patients were treated with intravenous levocarnitine (1000 mg per shot, thrice weekly), whereas 37 patients were not. The change in KES was significantly higher (p = 0.01) in the carnitine group [0.02 (0.01–0.04) kgf/kg] as compared to the non-carnitine group [−0.02 (−0.04 to 0.01) kgf/kg]. Multivariable-adjusted regression analysis showed the positive association between the change in KES and the treatment with levocarnitine remained significant after adjustment for the baseline KES and other potential confounders. Thus, treatment with intravenous levocarnitine was independently and positively associated with the change in KES among hemodialysis patients. Further clinical trials are needed to provide more solid evidence.

The purpose of this study was to determine the effect of bicycle exercise on knee-muscle strength and gait speed in 8 male participants with cerebrovascular accident (CVA). Isokinetic knee-extensor and -flexor strength were measured in both concentric- and eccentric-contraction modes. Fifty-foot walking tests were used for gait speed. After only 4 weeks of stationary recumbent cycling (12 sessions), participants improved eccentric muscle strength of the knee extensors, bilaterally. Walking-speed improvements approached but did not achieve significance with training. Improvement in concentric muscle strength of the knee extensors was observed in the involved limb, although most participants demonstrated a nonsignificant increase in muscle strength in the contralateral limb, as well. No improvements were demonstrated in the knee-flexor muscles. Thus, bicycle exercise serves to improve knee-extensor strength. In addition, these strength improvements might have implications for better control of walking in terms of bilateral improvement of eccentric muscle strength.

Sagittal plane inclination of the trunk and tibia have been shown to independently modulate hip and knee extensor moments during squatting. The purpose of this study was to determine if the difference between sagittal plane trunk and tibia inclinations can be used to approximate the relative demand of the hip and knee extensors across a range of squatting conditions. Kinematic and kinetic data were obtained from 16 participants during 8 squat conditions in which trunk and tibia inclination were manipulated. The average hip/knee extensor moment ratio (HKR) was calculated during the lowering (eccentric) phase of each squat condition using inverse dynamics equations. Linear regression evaluated the association between the difference in trunk and tibia inclination at peak knee flexion and the average HKR. Across all squat conditions, the difference between trunk and tibia inclinations explained 70% of the variance in the average HKR (p < 0.001). The squat was knee extensor biased (HKR ≤ 1.0) when the tibia was at least 8 degrees more inclined than the trunk. The results of this study indicate that the relationship between sagittal plane trunk and tibia inclination can be used to estimate the relative demand of the hip and knee extensors.

Background: The combination of chlorhexidine (CHX) and fluoride is believed to enhance the effects of both constituent elements, and reduce their possible side effects. This study aimed to evaluate the effect of CHX containing sodium fluoride on dental plaque, gingival inflammation, and tooth discoloration. Methods: In this double-blind clinical study, 40 patients were selected and randomly divided into two groups. One group was given CHX 0.12%, and the other one was provided with sodium fluoride 0.05%-CHX 0.12% mouthwashes. Plaque index (PI), gingival index (GI), and discoloration index (DI) were measured at the beginning of the study and then after two weeks. Data were analyzed using chi-squared and independent t test. Results: PI and GI were significantly reduced in the group with CHX + sodium fluoride compared to the one with CHX (P<0.001); however, the difference between two groups in terms of DI was not statistically significant (P =0.08). Both groups showed complications, but their differences were not statistically significant (P=0.5). Conclusions: Mouth wash containing CHX + sodium fluoride was more effective in dental plaque control and gingival inflammation than the one only including CHX, although complications were not statistically significant between the two groups.

SummaryIn 4 collaborative trials, involving a varying number of hospital laboratories in the Stockholm area, the coagulation activity of different test materials was estimated with the one-stage prothrombin tests routinely used in the laboratories, viz. Normotest, Simplastin-A and Thrombotest. The test materials included different batches of a lyophilized reference plasma, deep-frozen specimens of diluted and undiluted normal plasmas, and fresh and deep-frozen specimens from patients on long-term oral anticoagulant therapy.Although a close relationship was found between different methods, Simplastin-A gave consistently lower values than Normotest, the difference being proportional to the estimated activity. The discrepancy was of about the same magnitude on all the test materials, and was probably due to a divergence between the manufacturers’ procedures used to set “normal percentage activity”, as well as to a varying ratio of measured activity to plasma concentration. The extent of discrepancy may vary with the batch-to-batch variation of thromboplastin reagents.The close agreement between results obtained on different test materials suggests that the investigated reference plasma could be used to calibrate the examined thromboplastin reagents, and to compare the degree of hypocoagulability estimated by the examined PIVKA-insensitive thromboplastin reagents.The assigned coagulation activity of different batches of the reference plasma agreed closely with experimentally obtained values. The stability of supplied batches was satisfactory as judged from the reproducibility of repeated measurements. The variability of test procedures was approximately the same on different test materials.

I am grateful to Abe, Fry, Min, Vongvipanond, and Yu (hereafter re¬ferred to as AFMVY) [1] for obliging me to reconsider my article [2] on the demand for money in Pakistan. Upon careful examination, I find that the AFMVY results are, in parts, misleading and that, on the whole, they add very little to those provided in my study. Nevertheless, the present exercise as well as the one by AFMVY is useful in that it furnishes us with an opportunity to view some of the fundamental problems involved in an empi¬rical analysis of the demand for money function in Pakistan. Based on their elaborate critique, AFMVY reformulate the two hypo¬theses—the substitution hypothesis and the complementarity hypothesis— underlying my study and provide us with some alternative estimates of the demand for money in Pakistan. Briefly their results, like those in my study, indicate that income and interest rates are important in deter¬mining the demand for money. However, unlike my results, they also suggest that the price variable is a highly significant determinant of the money demand function. Furthermore, while I found only a weak support for the complementarity between money demand and physical capital, the results obtained by AFMVY appear to yield a strong support for that rela¬tionship.1 The difference in results is only a natural consequence of alter¬native specifications of the theory and, therefore, I propose to devote most of this reply to the criticisms raised by AFMVY and the resulting reformulation of the two mypotheses.

The effects of leakage and blockage on the acoustic performance of particle filters have been examined by using one-dimensional acoustic analysis and experimental methods. First, the transfer matrix of a filter system connected to inlet and outlet pipes with conical sections is measured using a two-load method. Then, the transfer matrix of a particle filter only is extracted from the experiments by applying inverse matrices of the conical sections. In the analytical approaches, the one-dimensional acoustic model for the leakage between the filter and the housing is developed. The predicted transmission loss shows a good agreement with the experimental results. Compared to the baseline, the leakage between the filter and housing increases transmission loss at a certain frequency and its harmonics. In addition, the transmission loss for the system with a partially blocked filter is measured. The blockage of the filter also increases the transmission loss at higher frequencies. For the simplicity of experiments to identify the leakage and blockage, the reflection coefficients at the inlet of the filter system have been measured using two different downstream conditions: open pipe and highly absorptive terminations. The experiments show that with highly absorptive terminations, it is easier to see the difference between the baseline and the defects.

Education is one of the key goals of sustainable development (SD), which establishes the basis for the improvement of the people’s living conditions. In this logic a special role is played by universities that create an institutional framework for educating citizens on sustainable development, offering a new understanding of social problems. On the one hand, universities can create and promote knowledge about SD by their educational, expert and research activities, hence developing relevant values among people. On the other hand, universities can become an active agent in implementing the concept of SD by introducing it into its own academic activities. The article considers stakeholder approach as one of the approaches to the implementation of the concept of education for sustainable development (ESD). Therefore based on this approach, the article explores the benefits of the key stakeholders of the sustainable university. Low awareness among key stakeholders is one of the significant factors that hindering the implementation of the SD concept. Due to the lack of a sufficient research focused on studying the interests of the main stakeholders in the framework of ESD, this article is an attempt to narrow this gap.

Three new analytical methods were optimized and validated for the estimation of tigecycline (TGN) in its injection formulation. A difference UV spectroscopic, an area under the curve (AUC), and an ultrafast liquid chromatographic (UFLC) method were optimized for this purpose. The difference spectrophotometric method relied on the measurement of amplitude when equal concentration solutions of TGN in HCl are scanned against TGN in NaOH as reference. The measurements were done at 340 nm (maxima) and 410nm (minima). Further, the AUC under both the maxima and minima were measured at 335-345nm and 405-415nm, respectively. The liquid chromatographic method utilized a reversed-phase column (150mm×4.6mm, 5µm) with a mobile phase of methanol: 0.01M KH2PO4 buffer pH 3.5 (using orthophosphoric acid) in the ratio 80:20 %, v/v. The flow rate was 1.0ml/min, and diode array detection was done at 349nm. TGN eluted at 1.656min. All the methods were validated for linearity, precision, accuracy, stability, and robustness. The developed methods produced validation results within the satisfactory limits of ICH guidance. Further, these methods were applied to estimate the amount of TGN present in commercial lyophilized injection formulations, and the results were compared using the One-Way ANOVA test. Overall, the methods are rapid, simple, and reliable for routine quality control of TGN in the bulk and pharmaceutical dosage form.
