This research aims to help develop aviation safety policies for the general aviation industry, especially for flight training schools. The analytical hierarchy process (AHP), fuzzy AHP, and fuzzy integral methods were used to find variables that impact aviation safety for training pilots in Korea and the United States using survey participants’ experience and perceptions. The results represent the circumstances of aviation safety in the real world where single pilot resource management, especially situational awareness, is crucial. The authors find that integral fuzzy AHP provides more explicit considerations, making up for the ambiguity of the linguistic responses caused by the AHP and fuzzy AHP.

Introduction: Medical errors are a frequent cause of morbidity and mortality in hospitals worldwide. Poor teamwork and communication skills rather than lack of technical knowledge have been identified as a common factor in numerous studies. Objective: To promote a safer medical practice through the adoption of Crew Resource Management (CRM) procedures directly adapted from the aviation industry for the first time in Argentina. Methods: Starting in 2009 we organized a hybrid team of two physicians/amateur aviators and two expert airline pilots with a significant background on safety analysis and CRM training . The setting for the initial experience was a teaching hospital with high volume cardiovascular and organ transplantation departments. CRM training has been offered at least once a year in a non compulsory manner to the entire spectrum of healthcare personnel, including medical and nursing students. Courses are structured in two day sessions divided in several chapters and workshops: Introduction, statistics, leadership, communication skills, and avoidance of risky behaviors. Slides, full text articles and reports are available for permanent consultation by personnel along with discussion forums at the Moodle based site of the program. Results: Satisfaction levels among attendees has consistently averaged 92%. Following training, briefings and debriefings have been implemented as standard procedures along with checklists both at Intensive Care, Operating Rooms and General Ward. No wrong side or wrong patient interventions have been reported over 3 years, while in 1/9500 surgical procedures a misidentification of a patient occurred with successful detection at the second crossed check on admission to the OR. Overdosing was reported in 1 every 4960 admissions, none fatal or disabling. Conclusions: Choosing a hybrid team of physicians and airline pilots rather than physicians or nurses alone as facilitators was a key factor to spark interest in teamwork factors and to avoid fear of punishment following reports. Despite cultural differences, CRM can be as feasible and successful in a Latin American hospital environment as previously shown in several centers in the United States.

As the future of the general aviation industry seems to be improving, a cultural paradigm shift may be imminent with the development of an advanced, revolutionary transportation system within the United States. One significant problem with this idea has been shown to be the fatigue structural certification of small, composite airplanes. By implementing a fatigue safe-life certification method for small general aviation aircraft, reduced certification program costs may be obtained while maintaining their reliability and capability. Although acceptable only when certifying aluminum aircraft, the current method has been modified here to incorporate the use of discontinuous composite materials on a defined small general aviation aircraft. Through this modification, a simplified method for the determination of the stress-cycle curve and a new “scatter factor” methodology based on the composite material properties have been developed. By using fiberglass reinforced polyester as an example composite material family, the safe-life of small general aviation aircraft relative to composite fatigue can be determined.

The recent hype in online purchasing has skyrocketed the importance of the electronic commerce (e-commerce) industry. One of the core segments of this industry is business-to-consumer (B2C) where businesses use their websites to sell products and services directly to consumers. Thus, it must be taken care of that B2C websites are designed in a way which can build a trustworthy and long-term relationship between businesses and consumers. Thus, this study assesses and prioritizes factors for designing a successful B2C e-commerce website. The study employs multi-criteria decision making (MCDM), and to minimize any ambiguity and greyness in the decision-making, it integrates fuzzy and grey respectively with the Analytical Hierarchy Process (AHP) and Technique for Order of Preference by Similarity to Ideal Solution (TOPSIS) to form FAHP and TOPSIS-Grey. Initially, the study conducts a thorough literature survey to screen important factors reported in past studies. Five main factors and nineteen sub-factors were selected for further prioritization. Later, FAHP prioritized factors based on their importance. Finally, based on the FAHP results, TOPSIS-Grey ranked five alternatives (e-commerce websites). FAHP revealed “service quality” as the most successful website designing factor, while TOPSIS-Grey reported “Website-3” as the most successful website, having incorporated the factors required to design a successful website.

PT. PT. PQR is a company engaged in the production of lightweight bricks. Lightweight bricks are produced by using the Autoclaved Aerated Concrete (AAC) system. From the observations in the company obtained results thatight brick products often experience damage due to falling from the pallet when moved using a hand lift. This is due to the distance between the production floor and the product warehouse, so operators have difficulty in the process of moving. The purpose of this study was to redesign the layout of the facility using the Fuzzy Analytical Hierarchy Process (Fuzzy AHP) with the Systematic Layout Planning (SLP) method to obtain an efficient design. The paired matrix weighting results showed that labor is a variable that has the highest weight of 0.2735. After an analysis was conducted, it was found that the closeness between departments was strongly influenced by the movement of labor (operator) between departments. The results of analysis, a relationship chart (ARC) activity was designed to get closeness between departments, then a new layout was designed based on the broad consideration of each department. Furthermore, the results of this design were compared with the initial design, such that obtained a distance reduction of 165.01m .

Data on 2015 [9] show that less than 50 percent of provincial and local level bridges in Central Java are in good condition. In the other hand, it has been reported that local bridges in the city of Surakarta have been deteriorated and damaged. The maintenance and rehabilitation action often done based solely on incidental reports without systematic planning. Analytical Hierarchy Process, Fuzzy Analytical Hierarchy Process, and Technique for Order Preference by Similarity to Ideal Solution (TOPSIS) were used to compare the priority scale of bridge management in Kecamatan (District) Banjarsari, Surakarta. Bridge Management System 1993 (BMS 1993) standard was used to quantify bridge damage condition by means of direct visual observation. Scores used in this 3 different analysis were determined by comparing aforementioned 2 criteria and 9 sub criteria. Accordingly, criteria and subcriteria defined in this paper were based on the standard. The criteria employed were bridge damage condition and average daily traffic (ADT). Bridge condition criteria were then divided further into sub criteria; i.e. river stream, safety support building, foundation, pier, girder system, parapet, floor system, expansion joint, and other property. From 11 bridges considered in this study, Maris Bridge has the highest pirority determined in 3 different methods. AHP and TOPSIS methods show Ringin Semar Bridge the lowest priority to maintain. On the other hand, Fuzzy AHP determines Balapan Bridge as the lowest order. Keyword: Bridge Management, BMS 1993, AHP, FAHP, TOPSIS.

Abstract The Advanced General Aviation Transport Experiments (AGATE) Consortium is a cost-sharing industry-university-government partnership initiated by NASA to create the technological basis for revitalization of the U.S. general aviation industry. It was founded in 1994 to develop affordable new technology as well as the industry standards and certification methods for airframe, cockpit, flight training systems, and airspace infrastructure for next generation single pilot, 4–6 place, near all-weather light airplanes. The AGATE consortium has more than 70 members from industry, universities, the FAA, and other government agencies. With respect to the advanced materials program within AGATE, the government-industry-academia program is directed toward the creation of material allowables that will be approved by the FAA for qualification of composite airframes — the first two being the Cirrus SR20 from Cirrus Design Corporation of Duluth, MN and the Lancair Columbia 300 from Pacific Aviation Composites of Bend, OR. These aircraft will be the first two all composite, four-seat AGATE-type airplanes to be certified in the United States. AGATE members Lancair, Cirrus Design, Cessna Aircraft, Raytheon Aircraft, Global Aircraft, Stoddard-Hamilton and Simula Technologies are contributing members in the program that promises to increase the level of sharing between competitors for the benefit of the entire general aviation industry. One major goal of the advanced materials program is produce FAA approved certification methods for the use of composite materials within the general aviation community. A recently published document entitled “Material Qualification Methodology for Epoxy-Based Prepreg Composite Material Systems” has been approved which describes an acceptable program to substantiate that the materials and processes employed meet FAA requirements for a selected material system. This is the first FAA public document which “standardizes” the procedure for qualifying a composite material system which follows guidelines set forth by the MIL-HDBK-17 committee. These requirements apply to the original material qualification. Once certified, changes to the material, process tooling, and/or facility require a review and repetition of some (or all) of these tests may be required. The plan gives specific information about the qualification program for epoxy-based pre-impregnated carbon or fiberglass unidirectional tape and pre-impregnated carbon or fiberglass woven fabric cured and processed at or above 240 degrees Fahrenheit. Specifically, this plan covers qualification methodology for no-bleed prepreg systems manufactured using vacuum bag molding. The properties of traditional materials — aluminum, steel, etc. — have long been accepted by the FAA in the design of airframes. These “allowables” mean the designer does not have to test every part to destruction, which is a very expensive, time-consuming process, but may use these allowable to substantiate the design using various forms of analysis. Due to company proprietary data restrictions, these “allowables” are generally not shared with other airframers which cause a repetition of qualification, sometimes for identical material. The AGATE database will attempt to standardize this data for a specific material system which will allow AGATE and non-AGATE companies to use the qualification data without having to completely repeat the full qualification [as depicted in Figure (1)]. The overall savings to the baseline qualification procedure should result in an “order of magnitude” savings in cost and over a factor of four savings in qualification time [as depicted in Figure (2)]. The overall goal of the program is to decrease to time and cost required for a new aircraft certification while still maintaining a high level of reliability and safety.

The group-buying online, a boom of e-commerce phenomenon, expands rapidly in China. Hence, the evaluation of group-buying website is essential for CEO to improve their website performance and for consumers to select the platform to purchase. However, the evaluation of group-buying website is a multiple-criteria decision making (MCDM) problem that is not easy to determine. For this reason, we propose analytical hierarchy process based approaches Fuzzy AHP, to evaluate the group-buying websites. In addition, a case study is presented to demonstrate how the approach can help in solving such problems in practice.
