The prevalence of dysfunction of the temporomandibular joint, especially in people aged 18-60 years, reached 95-98 % among all dental applications. The course of the pathology is usually hidden, with periodic recurrences and has a long nature, which is accompanied by a decrease in overall quality of life. Treatment of this pathology of the temporomandibular joint is a set of complex therapeutic, orthopedic and psychological measures. The literature describes many ways to treat temporomandibular joint dysfunction, one of the modem ones is the use of occlusal splints, which allow to change the position of the mandible, diagnose and eliminate musculoskeletal dysfunction of the temporomandibular joint. The aim of the study was to determine the effectiveness of treatment of musculoskeletal dysfunction of the temporomandibular joint with occlusal splints according to axiography. 274 patients aged 18 to 65 years were diagnosed with temporomandibular joint pain syndrome before and after treatment. All patients with signs of temporomandibular joint dysfunction before treatment had a violation of the trajectory of the mandible (deviation is 68.7 %, dyslexia is 31.3 %). When opening and closing the mouth, asymmetrical shifts of the lower jaw to the sides of more than 2 mm (deviation from the midline is more than 2 mm) were observed. After treatment with occlusal splints there was an improvement in the trajectory of opening and closing the mouth: the number of patients with a violation of the trajectory decreased by 89.1 %, and the displacement of the mandible during opening and closing the mouth in 92.4 % of patients decreased on average to 0.9 mm. When analyzing the movements of the mandible in the sagittal plane in 79 % of cases, deviations of the trajectory of the mandible were detected. After treatment with occlusive muscle relaxation splints, elimination of violations of the trajectory of the mandible in the transverse plane was noted in 93.4 % of cases, reduction of displacement to 0.9 mm in 78.1 % of patients. The trajectory of the mandible in the sagittal plane improved in 80.1 % of patients, normalization of the position of the mandible relative to the neuromuscular trajectory was achieved in 93.4 % of clinical cases. According to the analysis of parameters, such treatment should be considered effective. Objective.The objective of this study was to evaluate the regression of temporomandibular pain as a result of intra-articular injections of platelet-rich plasma (PRP) to patients with temporomandibular joint dysfunction previously subjected to prosthetic treatment.Materials and Methods.The baseline study material consisted of 10 patients, both males and females, aged 28 to 53 years, previously treated due to painful temporomandibular joint dysfunction using occlusal splints. All patients were carried out to a specialist functional assessment of the dysfunction using the Polish version of the RDC/TMD questionnaire axis I and II. Intra-articular injections were preceded by a preparation of PRP. The injection sites were determined by the method used during arthroscopic surgical procedures. Following aspiration, 0.5 mL of plasma was injected into each temporomandibular joint.Results.The comparison of the intensity of pain during all examinations suggests a beneficial effect of the procedure being performed as the mean VAS score was 6.5 at examination I, 2.8 at examination II, and 0.6 at examination III.Conclusion.Application of the intra-articular injections of platelet-rich plasma into the temporomandibular joints has a positive impact on the reduction of the intensity of pain experienced by patients treated for temporomandibular joint dysfunction.

Background and aims.The objective of this study was to evaluate the accuracy of the CADIAX Compact 2 axiograph in confirming the presumptive clinical diagnosis of temporomandibular dysfunction, established according to the muscular-articular clinical examination. Methods. A retrospective analytical study was carried out on 50 patients, aged between 21 and 62 years, 31 women and 19 men. A clinical diagnosis was established after masticatory muscles, temporomandibular joints and mandibular border limit movements were assessed. During the same day a computerized axiography (Cadiax Compact 2, Gamma, Vienna, Austria) was performed in order to confirm the diagnosis. Results. The study had a sensitivity of 100%, which means that the CADIAX device detected all clinically positive patients. On the other hand, the specificity of the study was 74%, representing the percentage of patients negatively diagnosed by CADIAX device out of the total number of patients clinically diagnosed as negative. The 88% accuracy shows the proportion in which the CADIAX device provides a diagnosis equal to the clinical one. Conclusions. Of all the methods for investigating temporomandibular joint dysfunction, axiography is less invasive, does not irradiate the patient, it is relatively easy to use and it offers functional information about the TMJ function.

Introduction: The temporomandibular joint disorders TMD and theirmanagement is a problem in dentistry and complaints associated to this problem are painin the area of joint and associated muscles along with limitations and clicking sounds duringmandibular opening and closing. One of the popular method of treatment is splint therapy.Study design: It was longitudinal intervention study. Objectives: The purpose of this study is toassess about oral splint as an effective treatment for TMD patients. Period: Total study time forthis study was about two years. Setting: Study was carried out in prosthodontics department ofSharif medical and dental college Lahore. Methodology: In the present study 40 patients wereincluded the study. All patients were suffering from some psycho-social issue. Their maximummouth opening MMO was measured and all of them were treated with occlusal splints. Results:There was significant improvement in MMO and reduction in pain.75% patients responded totreatment very well, however, 22.5% responded moderately and 2.5% patients did not respondto treatment. Conclusion: Splint therapy is an effective way to treat TMD. Splint therapy alongwith some psychological rehabilitation of patients which may produce better results.

This retrospective study investigated the effects of combining manual therapy and acupuncture on the pain and maximal mouth opening (MMO), which were associated with temporomandibular joint dysfunction (TMD). The 49 TMD patients (15 men, 34 women; mean age = 30.47 years, SD = 13.52 years) were treated with a combination of acupuncture and manual therapy two or three times a week at the hospital. The pain and maximal mouth opening were assessed before and after 1 and 4 weeks of treatment. The combination therapy produced significant changes in pain levels ( p < 0.001) and mouth opening ( p < 0.001). All pairwise non-parametric comparison showed a significant improvement in pain ( p < 0.05 for all pairs) and MMO ( p < 0.05 for all pairs). These findings suggest that combining manual therapy and acupuncture decreases the pain level and increases the MMO of TMD patients. However, future studies should further investigate the efficacy of combined treatment on TMD with more rigorous randomized clinical trials.

As a result of examination and questionnaire of 143 patients who applied for orthopedic treatment of defects of teeth and dental rows, it was found that signs of dysfunction of temporomandibular joint with preserved dental rows are diagnosed in 36—55% cases, and with defects of dental rows — in 45—90% cases. The absence of eighth teeth in the dental row does not significantly change the functional state of the dental-jaw system. A statistically significant frequency of signs of EHS dysfunction has been identified among individuals having terminal dentition defects.In patients with terminal dentition defects, each 3rd patient is diagnosed with cochleovestibular syndrome, and in each 2nd, sounds are determined in the area of VNHS when the lower jaw moves. It has been found that if 15—13 and 12—11 pairs of antagonist teeth are preserved, the signs of dysfunction are determined in 55—45% cases, if the number of teeth having antagonists is reduced to 10—5 (in 90.0% these are patients with preserved 7—8 pairs of antagonist teeth), then the frequency of dysfunction signs increases to 75.0% (t1-3=1.33; t2-3=2.00), in such patients significantly more often — in 75.0% of cases, mandibular deviation is diagnosed when opening and closing the mouth than in persons with a large number of preserved antagonist teeth, respectively 55.0% (t=2.66) and 45.0% (t=3.93) in 1 and 2 groups. Thus, the identification of such signs as crunching, clicking in the joints, hearing loss or tinnitus, suggests the presence of intra-articular disorders in such patients. The deviation of the jaw from its main trajectory when opening the mouth indicates the possible involvement of the masticators muscles in the pathological process. All this requires the dentist to carry out early diagnosis and timely orthopedic treatment.

The incidence of temporomandibular joint dysfunction in women reaches 80% of the total number of patients. The symptoms of temporomandibular joint dysfunction are varied and were first described by otorhinolaryngologist J.B. Costen. In his honor, the entire symptom complex of this disease is named "Costen's syndrome" in specialized literature and in the International Classification of Diseases of the tenth revision. The symptom complex includes joint pain, sometimes radiating to the neck, back of the head, temple, ear, clicks in the joint during movements of the lower jaw, trismus, hearing loss, dull pain in the middle and outside the ear, pain and burning sensation of the tongue, dry mouth; dizziness, pain on the side of the affected joint and even facial pains like trigeminal neuralgia. It is clinically difficult to isolate all these symptoms in patients with dysfunction of the temporomandibular joint, because its pathogenic and etiological manifestation is characterized in most cases only by one or several symptoms. Temporomandibular joint dysfunction is polyetiological, has a varied clinical picture and is directly dependent on the hormonal state, and especially the gynecological status. Much attention is drawn to the study of estrogen, which, in addition to regulating the functioning of the reproductive system in a woman's body, also performs a number of other important functions, including helping the joint to remain healthy - preventing calcium leaching, stimulating collagen recovery and the work of osteoblasts. The aim of the study was to determine the frequency of temporomandibular joint dysfunction in women along with hormonal changes and to trace their correlation. Materials and methods. The study involved 169 students of the third year at the Department of Propedeutics of Surgical Dentistry of the Faculty of Dentistry of the Ukrainian Medical Stomatological Academy. The research methods were based on conducting an anonymous survey. The questions were written for both men and women and were characterized by general dental status. Questions on gynecological status were asked separately for women. Results and discussion. The study group consisted of 169 people, including 89 women and 80 men aged 18 to 28 years. 61% of men and 26% of women did not have complicated dental status, among those surveyed. At the same time, 35% of men (of their total number) and 28% of women (of their total number) had orthodontic pathology. The presence of individual symptoms of temporomandibular joint dysfunction in men was observed in 6% of the total number of patients, in women - 19% (4% of them had already consulted a dentist, but did not receive adequate treatment). It is worth noting that patients with suspected temporomandibular joint pathology complained only of clicks when opening their mouths or chewing. Among 89 women studied, 8% had temporomandibular joint dysfunction, but did not have pathologies of the reproductive system. In 20% of women who did not have temporomandibular joint dysfunction, pathology of the reproductive system was noted. 11% had temporomandibular joint dysfunction and pathology of the reproductive system. 26% of women were clinically healthy. Out of 10 women with temporomandibular joint dysfunction who had a pathology of the reproductive system, but never had problems with the dentition, 10% of the women in the group (1% of all women examined) suffered from menstrual irregularities and had human papillomavirus in their anamnesis. 20% had a suspicion of temporomandibular joint pathology along with problems in their gynecological status and a history of orthodontic treatment, 10% had menstrual irregularities (algodismenorrhea) and were treated by a gynecologist with hormonal contraceptives for up to 1.5 years. 30% of women had temporomandibular joint pathology with problems in their gynecological status and previous therapeutic treatment (presence of fillings in the oral cavity). Of these, 20% have menstrual irregularities, and 10% have cervical erosion. 40% of the total number of women had temporomandibular joint pathology with orthodontic treatment in the past and fillings in the oral cavity along with problems in their gynecological status, where all had menstrual irregularities. 10% of them had metaplasia, erosion of the cervix and uterine polyp, 10% - erosion of the cervix. It should be noted that out of 89 women, 14 (17.5%) did not have any complaints about the condition of the joint, but had fillings and orthodontic treatment in the past, as well as pathology of the reproductive system. 11 of them (14%) had menstrual irregularities. In 3 (4%) patients out of the total number of women, there was an increased level of androgens, progesterone, estradiol and a history of treatment by a gynecologist. Based on the results obtained, it is possible to confirm the forced frequency of women visiting the dentist. Among the total number of women with temporomandibular joint dysfunction and pathology of the reproductive system, there was only 1 patient out of 89 persons without any dental interventions in the past. This does not give us a reason to associate gynecological status with the etiology of temporomandibular joint dysfunction. But in view of the greater prevalence of temporomandibular joint dysfunction along with gynecological pathologies, it gives us a basis for active further research on this topic.

Temporomandibular joint (TMJ) sounds are important and common physical signs of temporomandibular disorders (TMD). The aim of this study was to evaluate the influence of the effect of the use of occlusal bite splints (stabilizing and repositioning) on the sounds produced in the TMJ, by means of the electrovibratography (EVG). Thirty-one patients with TMD from the Dental School of Ribeirão Preto, University of São Paulo, Brazil were selected for this study. Group 1 (n=23) wore stabilizing bite splints and Group 2 (n=8) used anterior repositioning splints. Before and after treatment with occlusal splints both groups were analyzed using the SonoPAK Q/S recording system (BioResearch System, Inc.). The treatments with stabilizing bite splints were satisfactory, since they reduced the total amount of the sound energies (p<0.05), but the use of anterior repositioning splints for no more than 4 weeks produced significantly better results (p<0.01). The total amount of vibration energy involved in the vibrating movements of the TMJ showed significant improvement using anterior repositioning splints.
