A limitation of this chapter is that it is not a case study, nor is it based on field experiences. It is in fact a critical exploration of the concepts to facilitate in building a theory in an interdisciplinary perspective.

In the era of patient centered healthcare, patients are educated, more aware and demanding than ever. However, there is a significant misalignment between patients and doctors due to improper communication resulting in broken patient-doctor therapeutic relationships and degraded quality of healthcare. This suggests that patients have a greater and mature role to play in their healthcare. The paper aims to fill this gap by studying the contribution of patients in their healthcare through patientdoctor communication in selected Indian multispeciality hospitals. Qualitative multi-case study was steered and in-depth interviews of thirteen patients, twelve doctors were conducted along with the secondary data analysis of more than 600 pages of the documents from the official websites of the sample hospitals. Grounded theory three level coding revealed the themes of contribution of patients in through effective communication. The results indicate that patients contribute to their healthcare through effective communication by demonstrating association with doctors, reflecting reciprocally, resolving communication challenges and supporting their overall treatment process. The paper extends the literature on patient’s contribution in their healthcare. It presents clear and succinct implementable implications and distinctive ways in which patients cooperate with the doctors, work mutually, improves communication and strengthen their overall healthcare process.

Interdisciplinary palliative care teams can improve quality of life by addressing the needs and experience of the whole person with chronic kidney disease, including physical, psychological, social, spiritual, cultural, end-of-life, ethical, and practical concerns. Nephrology teams can develop the essential skills to provide primary palliative care for uncomplicated problems, but consultation with a specialty palliative care team is warranted for more severe, complex, or refractory problems. Although specialty palliative care can be delivered in any care setting, it may be a scarce resource outside of a hospital or hospice. Nephrology teams should identify all the specialty palliative care resources available in their community and consider engaging palliative care experts not only in patient care but also in advisory, educational, and quality improvement activities.

Usage and nature of software systems have changed significantly. Due to this complexity of software systems has also grown exponentially. In these ever-changing requirements and environment in which software system is being used, maintaining quality of software system is very challenging and difficult. If user requirements are not met as expected, it’s called defect. To improve quality, it’s critical to understand and analyze these defects. In this study root cause analysis technique is used to analyze defects and their attributes, root cause of defects and corrective actions of defects. A quality model is designed based on defects, root cause of defects and tests. A quality algorithm is designed in this study depending on existing quality model, defects, tests and their attributes. This quality algorithm is executed on a software system to validate quality model. The results obtained are analyzed to understand the quality of the software system and how it’s different than existing quality model.
