Background: Amavata is the most common form of inflammatory arthopathy seen in India. Among adult population below the age of 50 years this is the most common form of arthritis. For the present study, on Amavata as shaman therapy, Khanda Shunthi and Prasarni Avleha the Ushnaveeryadravya medicaments were chosen. Aims & Objective: The present research work aimed at to evaluate efficacy and establish safe use of Khanda Shunthi and Prasarni Aavaleha in Amavata. Materials & Methods: 40 subjects of Amavata fulfilling the inclusion criteria were selected from OPD and IPD of Desh Bhagat Ayurvedic Hospital, Mandi Gobindgarh, Punjab and randomly divided into two groups, group A and B, comprising each of 20 patients. Group-A subjects received Khanda Shunthi for 60 days, Group-B subjects received Prasarni Avaleha for 60 days. Assessments were done on 0th and 60th day of treatment. Results and Conclusions: In both the groups, highly significant results were observed in all the cardinal parameters with P value for fever and Hb are greater than 0.05 hence there is no significant difference in effect of Group A and Group B on fever and Hb. P values for all other symptoms are less than 0.05 hence we conclude that there is significant difference in effect of group A and group B on pain, swelling, stiffness, fever, ESR, walking time and grip strength. On comparison group A treatment is more effective than group B for all assessment criteria.

Objective: To compare the efficacy of dexamethasone 8mg versus ondansetron 4mg in preventing postoperative nausea and vomiting (PONV) in patients undergoing laparoscopic cholecystectomy Methodology: This quasi-experimental study was conducted at the Department of Anaesthesia and Intensive Care, Holy Family Hospital from 29th July 2018 to 28th January 2019.  Anesthesia was induced with propofol (2mg/kg IV) and Atracurium (0.5 mg/kg IV) was given to facilitate tracheal intubation. Nalbuphine (0.2mg/kg) was used as analgesic.Patients were randomly divided into two groups. Patients in Group A received 4mg ondansetron at end of surgery and Group B received 8mg dexamethasone at induction. Results: A total of 90 patients were included according to the inclusion criteria of the study. The mean age (years) in the study was 37.11+10.60 years. Frequency and percentage of nausea (up to 24 hours) among both the groups was 28 (62.2) and 28 (62.2) respectively (p-value = 1.0) while the frequency and percentage of vomiting (within 24 hours after extubation) was 28 (62.2) and 27 (60.0) respectively (p-value = 0.829) Conclusion: The study concluded that there was no significant difference dexamethasone and ondansetron in preventing postoperative nausea and vomiting.

Krodha is a Manasika Vikara which can be assessed by Droha done by another person. Here is an attempt made to evaluate the efficacy of Geetam and Chandra Anuloma Viloma Pranayama in the management of Krodha. Forty subjects fulfilling the diagnostic and inclusion criteria were selected and randomly assigned into two groups. Group A was treated with Geetam, and Group B were treated with Chandra Anuloma Viloma Pranayama. The effect of treatment was assessed based on symptoms of Krodha, Intermittent explosive disorder and Buss and Perry Scale. Both Geetam and Chandra Anuloma Viloma Pranayama showed a statistically significant effect on the assessment criteria. On comparison between two groups, there is no statistically significant difference between Group A and Group B. Thus, the null hypothesis is rejected, and the alternative hypothesis (H1) is accepted i.e., There is an equivalent effect of both Geetam and Chandra Anuloma Viloma pranayama in the management of the Krodha. Vata aggravation and Parihani (depletion) of all the Dhatus are inevitable in Vrudhavastha (old age). Hence there is subsequent vitiation of Vata which affects Asthidhatu (due to Ashraya and Ashrayi Sambandha) is common. Sandhigata Vata is one amongst them. The involvement of Marma with Vata Dosha and Dhatukshaya makes disease Yapya. Hence early management prevents disability in the old age. Snehana, Swedana and Basti Chikitsa are the prime line of management in Vatadosha. When the disease is Shula and Grahapradhana, affecting one body part, the Snigdha, Ekangasweda is the ideal treatment. So Snigdhapatra Pindasweda is selected. The need of Snehana, Brumhana, Vatashamana is best achieved by Snehabasti with Vatashamaka Taila (Dashamoola Taila). To assess the synergistic action, combined treatment is planned in the present study. With this in backdrop, a comparative clinical study was planned in 30 patients allotted into 3 groups of 10 each. Group A treated with Snigdhapatra Pindasweda, Group B treated with Matrabasti and Group C treated with both the treatment for 7 days. Within the group result showed statistically significant improvement in all the parameters. In between the groups comparison showed statistically significant difference in stiffness, tenderness, movement of knee joint, Walking - time and WOMAC- Index.

Dushta Vrana is a common and frequently encountered problem faced in surgical practice. The presence of Dushta Vrana worsens the condition of the patient with different complications and may become fatal. Local factors on wound like slough, infection and foreign body, affect the normal process of healing. A healthy wound in a normal body heals earlier with a minimum scar as compared to a contaminated wound. Therefore in this study all the efforts are made to make a Dushta Vrana into a Shuddha Vrana. Once the Vrana becomes Shuddha, Ropana of the Vrana will start. The objective of the study was to evaluate the clinical efficacy of Jatyadi Taila and Jatyadi Ghrita in Dushta Vrana. Clinically diagnosed 60 Patients of Dushta Vrana were randomly divided into two groups, each consisting of 30 Patients. Group A were treated with the Jatyadi Taila and Group B was treated by Jatyadi Ghrita. The results observed was based on the relief obtained on the subjective and objective parameters taken for consideration for this study viz, size of ulcer, discharge, smell, pain, burning sensation, itching and granulation were found significant (P Lass Than 0.05). On the basis of assessment criteria and overall result of treatment, the patients of Jatyadi Taila group showed better results when compared to Jatyadi Ghrita group. Even though statistically there is no much significant difference between the two groups, but by seeing the effect on individual parameters (subjective and objective) and over all response, Jatyadi Taila seems to be effective when compared to Jatyadi Ghrita. It is having more Ropana qualities when compared to Shodhana.

Background: Fear of poor wound healing have curtailed the use of diathermy for making skin incision. Scalpel produces little damage to surrounding tissue but causing more blood loss. Our aim of study was to compare electrocautery incision with scalpel incision in terms of incision time, blood loss, postoperative pain and wound infection.Methods: Total of 104 patients were included in the study undergoing midline abdominal surgery. Patients were randomized into electrocautery (group A) and scalpel (group B). The incision dimension, incision time and blood loss were noted intraoperatively. Postoperative pain was noted on postoperative day 2 using visual analog scale. Wound complications were recorded on every postoperative day till the patient was discharged.Results: 52 patients in each of the two groups were analyzed. There was significant difference found between group A and group B in terms of mean incision time per unit wound area, 8.16±1.59 s\cm2 and 11.02±1.72 s\cm2 respectively (p value=0.0001). The mean blood loss per unit wound area was found to be significantly lower in group A (0.31±0.04 ml\cm2) as compared to group B (1.21±0.21), p value=0.0001. There was no significant difference noted in terms of postoperative pain and wound infection between both groups.Conclusions: Electrocautery can be considered safe in making skin incision in midline laparotomy compared to scalpel incision with comparable postoperative pain and wound infection with less intraoperative blood loss and less time consuming.

Rajonivrutti condition manifests in the end phase of Jarawastha; but its Samprapti begins from the Sandhikala of Madhyamawastha and Jarawastha due to Vatavruddi in womens reaching Rajonivrutti. Postmenopausal Asthikshaya is a disabling disease, which renders women a bedridden life. Here 40 subjects diagnosed with postmenopausal Asthikshaya fulfilling the inclusion criteria were selected for the study and randomly categorized intp two groups as group A and group B each consisting of 20 subjects. Group A received Amapachana with Hinguvachadivati, Yastimadhu Siddha Ksheerabasti administered in Yoga Basti schedule followed by Tritiyatriphala Rasayana. Group B received Amapachana with Hinguvachadivati, Sadhyosnehapana with Amrita Ghrita, Sarvanga Abyanga with Murchita Tilataila followed by Sarvanga Swedana and Sneha Virechana was admistered with Eranda Taila followed by Tritiyatriphala Rasayana was given. Tritiya Triphala Rasayana selected for the present study by adapting all the general principles of prevention and management of Asthikshaya and as it is a well known Rasayana.

Background: Procedural sedation is required for multiple short duration procedures outside of the operating rooms especially in radiology and endoscopy suites. Intravenous anesthetic agent with rapid recovery profile is desirable in such circumstances. This study aims to compare two regimens of intravenous anesthetic agents. Aim: To compare the mean recovery time of propofol and midazolam with propofol alone for sedation in endoscopic retrograde cholangiopancreatography. Study Design: Randomized controlled trial. Settings: Department of Anesthesia, Shalamar Hospital, Lahore. Study Duration: June 2017 to December 2017. Methods: A total of 70 adult patients aged 20-60 years undergoing ERCP under sedation were included. Patients were given a combination of propofol and midazolam in group A while propofol alone was given in group B. After procedure, pts were transferred to recovery room and were followed for assessment of recovery time. Data were analyzed in SPSS vr 21, Independent t-test was applied & p-value ≤0.05 was considered statistically significant. Results: Significant difference was found in mean recovery time amongst both the groups. Mean recovery time in Group A (propofol and midazolam) was 19.29±4.50 minutes while in Group B (propofol alone) was 26.66±3.70 minutes showing statistically significant result with p-value = 0.0001. Conclusion: We conclude that mean recovery time with propofol plus midazolam is shorter as compared to propofol alone for sedation in ERCP. Abstractwas to assess efficacy of Tadalafil alone versus Tadalafil plus Pentoxifylline in the treatment of erectile dysfunction by using self administered IIEF-5 questionnaire.Two hundred and thirty seven patients presenting with ED at andrology OPD were evaluated for ED by a self administered IIEF (International Index of Erectile Function) questionnaire. Patients were systematically randomized by computer generated random table into two groups groups namely, group A: Tadalafil only group, group B: combination of Tadalafil + Pentoxifyl-line. All the patients were re-assessed by IIEF-5 questionnaire after 8 weeks of medical therapy. Statistical analysis was performed using student’s unpaired t-test, paired t-test, chi square test. p-value < 0.05 was considered statistically significant.Two hundred and thirty seven patients were included in the present study, in group A: 92 patients (78.6%) showed improvement in their IIEF score after 8 weeks of tadalafil treatment. While in group B, overall 104 patients(86.6%) showed improvement after combination of Tadalafil and Pentoxifylline. There was a statistically significant difference of percentage change in IIEF score was seen in group B (group A 90.7±15.2%, group B 95.6±13.4%; p value – 0.014). We found this difference even more statistically significant in patients with severe ED (group A 72.7±47.2%, group B 132.3±54.3%; p value – 0.000). There was no significant difference in between the two groups with regards to occur-rence of side effects.Both tadalafil and combination of Tadalafil + Pentoxifylline improve erectile function in patients of ED. Patients with severe ED showed much significant improvement in erectile function with combination therapy.

Abstract Background Atrial fibrillation (AF) is the most common sustained arrhythmia affecting humans. It is an electrical disturbance that leads to rapid, disorganized, and asynchronous contraction of the atrial muscle. In clinical practice, it accounts for approximately one-third of hospitalizations for cardiac rhythm disturbances. The incidence of AF increases from less than 0.1% per year in those under 40 years old to exceed 1.5% per year in women and 2% per year in men older than 80 years. Aim of the Work: to discuss the effect of variation in amiodarone use (including dosage and duration) on dysrhythmia recurrence in patients with new-onset AF in ICU. Patients and Methods This was a prospective observational study conducted over 6 months, 60 patients who fulfilled inclusion criteria were included in the study divided in two groups according to amiodarone dosage, each group is 30 patients: Group (A): received a loading dose of amiodarone followed by an infusion (1200mg amiodarone). Group (B): received a loading dose of amiodarone not followed by an infusion (300mg amiodarone). Results a significant positive correlation was observed between level of C-reactive protein (CRP) and the rate of AF recurrence. As regard effect of CRP on AF recurrence; in AF recurrent patients, mean is (191±77.3) with range from 15 to 352 which significantly differed from Non-recurrent AF patients, mean is (89±63) with range from 20 to 223 (p value &lt;0.001). AF recurrence was higher in group (B) than group (A),. In group (A) 8 patients had recurrent AF representing 26.7 % and in group (B) 19 patients had AF recurrence representing 63.3% (p value =0.004). Conclusion Patients with new-onset AF in (ICU) who are treated with amiodarone should receive a loading dose, immediately followed by an infusion.

Objectives: Role of niacin in decreasing cardiovascular accidents by lowering the levels of Apo-B in hyperlipidemic patients. Background: In hyperlipidemia, there are high levels of atherogenic lipoproteins leading to higher risk of atherosclerotic cardiovascular events. Patients with dyslipidemia use statins as a mainstay of therapy over last many decades. Recent studies show that apolipoproteins play a major role in formation of atheromatous plaque, thus there is an urgent need to study the effects of lipid lowering medication on apolipoprotiens levels. Study Design: Cross sectional analytical study. Setting: Sheikh Zayed Hospital Lahore (Department of Biochemistry and Chemical Pathology). Period: 12 weeks from July to Sep 2014. Materials and Methods: Recently diagnosed hyperlipidemic patients (n=44) were selected for the study purpose and divided into two equal groups; A and B. Each group was given different medication. Group A took only statin while group B took a combination of statin and niacin. Blood samples were taken at the start of medication and then after completion of 12 week time period. Results: At the start of the treatment there was no significant difference in the Apo B cholesterol level between the two groups (p value 0.972). However, after the end of 12 week duration, there was a significant reduction in the Apo level of group B taking statin and niacin as compared to group A taking statin alone (p value 0.003). Conclusions: Niacin has cardio-protective role when used in combination with niacin.
