AimsTo determine the prevalence of anxiety and depression amongst participants with severe or complex obesity randomised and selected for bariatric surgery in a large multi-centre trial. To describe the change in prevalence of anxiety and depression amongst participants who had undergone bariatric surgery, within 6 months of randomisation and at 12 months post-randomisation.MethodThe By-Band-Sleeve (BBS) study is a multi-site randomised controlled trial evaluating the surgical management of severe or complex obesity and is the largest trial of its kind. Participants completed the Hospital Anxiety and Depression Scale (HADS) on study enrolment (pre-randomisation) and at 12 months post-randomisation. In this sub-study, we describe provisional data concerning the baseline prevalence of anxiety and depression along with change in median HADS symptom score amongst those who actually underwent bariatric surgery.Result758 participants met the criteria for study inclusion with 716 (94.46%) and 712 (93.93%) individuals fully completing questionnaires for HADS-A and HADS-D. At pre-randomisation, the prevalence of possible (HADS A/D = 8-10) and probable (HADS A/D >11) anxiety or depression was 46.19% (n 330/716) and 48.17% (n 48.17%) respectively. Paired and complete HADS-A and HADS-D questionnaires were available for 70.25% (n 503/716) and 69.94% (n 498/712) participants. There was a highly statistically significant decrease in median HADS-A and HADS-D scores at 12 months post-randomisation (Wilcoxon signed-rank test p < 0.001). This was coupled with a statistically significant reduction in the proportion of cases with possible and probable anxiety (–9.54%, p < 0.001) and also depression (–22.21%, p < 0.001) at 12 months post-randomisation.ConclusionOur results characterise the high rate of psychological comorbidity amongst patients with severe or complex obesity selected for bariatric surgery. Whilst bariatric surgery remains the most clinically effective treatment for severe obesity, its effects on long-term post-operative mental health outcomes are less clear. These findings contribute to the growing body of evidence calling for increased pre/post-operative mental health surveillance and integrated care for this cohort of patients.

Many investigations have reported the impact of resident involvement in urological surgeries. We aimed to conduct this systematic review to create enough evidence regarding this impact based on the outcomes obtained from the relevant studies. We have performed both manual and electronic search through many databases like Google Scholar, Web of science, PubMed, Scopus, Cochrane library, the international standard randomised controlled trial number registry, and the world health organization virtual health library to obtain all the relevant citations. Our intended outcomes included the risk of developing complications, operative time, and rate of reoperations. We have included 17 studies that reported the impact of residents’ involvement in urological surgical operations. while some studies reported some complications, the risk of developing complications was not significantly associated with residents' involvement according to most studies. Only one study reported that complications were significantly associated with residents’ involvement. The rate of reoperation and readmission after the primary surgery was also non-significant. On the other hand, most of the included studies showed that residents’ involvement was significantly associated with increased hospital stay and prolonged operative time. Our results indicate the residents’ involvement does not impact the overall safety of the urological procedures and that this practice should be encouraged with adequate supervision.

The death rate due to the Covid-19 virus based on the World Health Organization (WHO) dated April 19, 2020 was 152,551 people (WHO, 2020). According to data from the Ministry of Health, the percentage of confirmed positive Covid-19 on 27 May 2020 was 23,851 cases, and 1,473 cases died. Elderly people often experience psychological disorders such as stress, depression and anxiety, especially in the face of the Covid-19 outbreak because a lot of information about the Indonesian population has been confirmed positive and causes death to increase the level of anxiety. Efforts in overcoming anxiety that occurs in the elderly can be done with SEFT spiritual relaxation. Objective: To analyze the effect of Spiritual Emotional Freedom Technique (SEFT) therapy on reducing anxiety levels in the elderly in Grantung Village, Bayan District, Purworejo Regency. Methods: This type of research used a pre-experimental research design with one group pretest-posttest design. The sampling technique used total sampling with a sample size of 79 elderly in Grantung Village, Bayan District, Purworejo Regency. The data analysis technique used the Wilcoxon Signed Rank Test. The research instrument used HARS questionnaire sheet and SEFT therapy SOP. Results: The results showed that most of the anxiety levels of the elderly before being given SEFT therapy were in the moderate category of anxiety and after being given SEFT therapy, most of the anxiety levels in the elderly decreased to the mild category. Conclusion: There is an effect of SEFT therapy on the level of anxiety in the elderly during the Covid-19 pandemic in Grantung Village with a value of p = 0.000 (p value <0.05). Recommendations:for the elderly should apply SEFT therapy independently to deal with anxiety problems during the pandemic Covid-19.

This report presents the findings from the Australian component of the Phase III World Health Organization (WHO) Alcohol, Smoking and Substance Involvement Screening Test (ASSIST) randomised controlled trial investigating the effectiveness of a 5–10-min brief intervention (BI) for illicit drug use delivered in primary healthcare (PHC) settings. Participants (n=171) recruited from a South Australian PHC setting (sexual health clinic) who scored in the ‘moderate risk’ range on the ASSIST were randomly allocated to an intervention group or wait-list control group at baseline and were followed up 3 months later. The ASSIST was administered to both groups at baseline and follow up as a measure of relative risk. Those in the intervention group received a prescribed 10-step BI at baseline. The majority (n=63) of participants received the BI for amphetamine-type stimulants (ATS) or cannabis (n=17). There was a significant reduction in total illicit substance (P<0.001) and ATS Involvement (P<0.01) for those receiving the ASSIST-linked BI, compared with control participants. There was no significant effect on cannabis involvement. The results of this study demonstrate that the ASSIST-linked BI may be a reasonably easy and effective way of reducing illicit substance use by Australian PHC clients.
