Одними из наиболее важных для уровня социального функционирования и адаптации составляющих клинической картины хронической ишемии мозга (дисциркуляторная энцефалопатия, болезнь малых сосудов) являются психопатологические симптомокомплексы: тревожные, смешанные тревожно-депрессивные, органоневротические (соматоформные), нозогенные расстройства, расстройства приспособительных реакций, неврастения, соматогенная астения. Раннее начало терапии хронической ишемии мозга (ХИМ) является важной задачей по предупреждению прогрессирования не только церебрально-сосудистой патологии, но и широкого круга коморбидных психопатологических расстройств. К пациентам с ХИМ необходим индивидуальный подход с учетом патогенетических факторов и выраженности как когнитивных, так и психопатологических расстройств с привлечением широкого круга средств, включая нейропротекторы. С этой целью в клинической практике используются для самостоятельного или комбинированного применения этилметилоксипиридина сукцинат и цитиколин. На фоне приема этих препаратов пациенты с различными формами ХИМ отмечают субъективное улучшение своего состояния в виде уменьшения утомляемости, общей слабости, эмоциональной лабильности, симптомов психической и соматической тревоги, а также повышения активности и мотивации деятельности, улучшения памяти, внимания и способности концентрироваться, что положительно отражается на их профессиональной деятельности. Anxiety, depression, somatoform disorders, nosogenic reactions, adjustment disorders, neurasthenia, somatogenic asthenia in patient with chronic cerebral ischemia (dyscirculatory encephalopathy, cerebrovascular disease) (CCI) are very important for the level of social functioning and adaptation. Early initiation of CCI therapy is an important task in preventing the progression not only of cerebrovascular pathology, but also a wide range of comorbid psychopathological disorders. Treatment of patients with CCI requires an individual approach taking into account pathogenetic factors and severity of both cognitive and psychopathological disorders. It should involve a wide range of drugs, including neuroprotectors. For this purpose the neuroprotective agents ethylmethyloxypyridine succinate and citicoline can be used for independent or combined use. Patients with various forms of CCI noticed subjective improvement during treatment which includes decreasing fatigability and general weakness, reduction of symptoms of psychic and somatic anxiety, fatigability, emotional lability, increasing activity and motivation, improvement of memory, attention, ability to concentrate that positively influences their professional activity.

The paper reviews the literature on vascular cognitive impairment (VCI), the diagnosis widely used in foreign neurological practice, as well as chronic cerebral ischemia (CCI) and dyscirculatory encephalopathy (DEP), the common diagnoses in Russian neurological practice. According to the etiology, risk factors, and manifestations, Stages I and II DEP largely corresponds to moderate VCI; Stage III DEP does to severe VCI. The results of the author’s studies show that a considerable proportion of patients followed up with a diagnosis of CCI, DEP, have no signs of chronic cerebrovascular disease (CVD), but suffer from primary or secondary headache, vertigo of various origins, emotional disorders, and other diseases. The diagnosis of CCI, DEP should be based on the presence of CCI, the reliable neuroimaging signs of chronic CVD, and the ruling out of other diseases. When treating and preventing VCI, CCI, and DEP, a premium is placed upon both non-drug (regular physical activity, smoking cessation, rational nutrition) and drug therapy aimed at normalizing blood pressure and blood lipid spectrum, preventing blood clots, and improving cognitive functions.

The paper presents the proceedings of the Round Table with the participation of leading neurologists, which is devoted to chronic cerebrovascular diseases. It is noted that chronic cerebral ischemia (CCI), or dyscirculatory encephalopathy (DEP), is one of the most common neurological diagnoses in our country. The pathogenesis, clinical presentations, diagnosis and treatment of CCI (DEP) and its matching with vascular cognitive impairment (CI), which is regarded in foreign literature as the main manifestation of chronic cerebrovascular disease (CVD) were considered. The authors analyze clinical trials evaluating the efficacy of vinpocetine (Cavinton) in chronic CVD, dizziness, CI, as well as the use of new vinpocetine formulations, such as Cavinton Comforte, in various neurological diseases, dysphagia in particular, in poststroke patients.

During the COVID-19 pandemic, healthcare workers (HCWs) have been subject to increased workload while also exposed to many psychosocial stressors. In a systematic review we analyze the impact that the pandemic has had on HCWs mental state and associated risk factors. Most studies reported high levels of depression and anxiety among HCWs worldwide, however, due to a wide range of assessment tools, cut-off scores, and number of frontline participants in the studies, results were difficult to compare. Our study is based on two online surveys of 2195 HCWs from different regions of Russia during spring and autumn epidemic outbreaks revealed the rates of anxiety, stress, depression, emotional exhaustion and depersonalization and perceived stress as 32.3%, 31.1%, 45.5%, 74.2%, 37.7% ,67.8%, respectively. Moreover, 2.4% of HCWs reported suicidal thoughts. The most common risk factors include: female gender, nurse as an occupation, younger age, working for over 6 months, chronic diseases, smoking, high working demands, lack of personal protective equipment, low salary, lack of social support, isolation from families, the fear of relatives getting infected. These results demonstrate the need for urgent supportive programs for HCWs fighting COVID-19 that fall into higher risk factors groups.

ABSTRACTBiofilms are a major source of medical device-associated infections, due to their persistent growth and antibiotic resistance. Recent studies have shown that engineering surface nanoroughness has great potential to create antibacterial surfaces. In addition, stimulation of bacterial metabolism increases the efficacy of antibacterial agents to eradicate biofilms. In this study, we combined the antibacterial effects of nanorough topographies with metabolic stimulation (i.e., fructose metabolites) to further decrease bacterial growth on polyvinyl chloride (PVC) surfaces, without using antibiotics. We showed for the first time that the presence of fructose on nanorough PVC surfaces decreased planktonic bacteria growth and biofilm formation after 24 hours. Most importantly, a 60% decrease was observed on nanorough PVC surfaces soaked in a 10 mM fructose solution compared to conventional PVC surfaces. In this manner, this study demonstrated that bacteria growth can be significantly decreased through the combined use of fructose and nanorough surfaces and thus should be further studied for a wide range of antibacterial applications.

Object. The success of treatment for delayed cerebral ischemia is time dependent, and neuronal monitoring methods that can detect early subclinical levels of cerebral ischemia may improve overall treatment results. Cerebral microdialysis may represent such a method. The authors' goal was to characterize patterns of markers of energy metabolism (glucose, pyruvate, and lactate) and neuronal injury (glutamate and glycerol) in patients with subarachnoid hemorrhage (SAH), in whom ischemia was or was not suspected.Methods. By using low-flow intracerebral microdialysis monitoring, central nervous system extracellular fluid concentrations of glucose, pyruvate, lactate, glutamate, and glycerol were determined in 46 patients suffering from poor-grade SAH. The results in two subgroups were analyzed: those patients with no clinical or radiological signs of cerebral ischemia (14 patients) and those who succumbed to brain death (five patients).Significantly lower levels of energy substrates and significantly higher levels of lactate and neuronal injury markers were observed in patients with severe and complete ischemia when compared with patients without symptoms of ischemia (glucose 0 compared with 2.12 ± 0.15 mmol/L; pyruvate 0 compared with 151 ± 11.5 µmol; lactate 6.57 ± 1.07 compared with 3.06 ± 0.32 mmol/L; glycerol 639 ± 91 compared with 81.6 ± 12.4 µmol; and glutamate 339 ± 53.4 compared with 14 ± 3.33 µmol). Immediately after catheter placement, glutamate concentrations declined over the first 4 to 6 hours to reach stable values. The remaining parameters exhibited stable values after 1 to 2 hours.Conclusions. The results confirm that intracerebral microdialysis monitoring of patients with SAH can be used to detect patterns of cerebral ischemia. The wide range from normal to severe ischemic values calls for additional studies to characterize further incomplete and possible subclinical levels of ischemia.

The purpose of the study. To study clinical and neuroimaging features of chronic cerebral ischemia (CCI) in polycythemia vera (PV).Materials and methods. 66 patients with PV were examined – the main group (43 men, 23 women; mean age 62.0 ± 3.4 years), of which 64 (97.0%) patients were diagnosed with CCI. The comparison group consisted of 85 patients with CCI (34 men, 51 women; mean age 67.7 ± 4.6 years), who developed against the background of cerebral vascular atherosclerosis and arterial hypertension. To identify cognitive disorders, we used Mini Mental State Examination (MMSE). Insomnia was studied in accordance with the criteria of the International Classification of Sleep ICDS‑22005. The quality of sleep was determined using a questionnaire from the Federal Somnological Center. Neuroimaging (MRI of the brain) was performed on Siemens Symphony 1.5 T and GE Signa 1.5 T tomographs.Results. Subjective symptoms CCI are characterized by a greater representation of asthenic and insomniac disorders. Transient ischemic attacks in patients with PV are significantly more common than in the comparison group, their frequency depends on the duration of PV. The revealed changes in MRI of the brain in the majority of PV patients with CCI are characteristic of multiinfarction vascular encephalopathy; in the comparison group, changes that characteristic for subcortical arteriosclerotic encephalopathy were more often recorded.

: Chronic cerebral ischemia is one of the common ischemic cerebrovascular diseases. Chronic cerebral ischemia can lead to brain dysfunction, and its pathophysiological mechanism involves inflammation, blood-brain barrier destruction, oxidative stress, and other factors. Due to it being difficult to detect, it is easily overlooked, and it is often only observed following onset of cognitive dysfunction. At present, there are few drugs for this treatment. DL-3-N-BUTYLPHTHALIDE (NBP), a compound extracted from celery seed, may play an important role in protecting against brain damage caused by chronic cerebral ischemia. Therefore, we pay more attention to the prevention and treatment of NBP on chronic cerebral ischemia.
