Background: One of the health problems that can be caused by Perfluorooctanoic acid (PFOA) is Diabetes mellitus (DM). However, the causal relationship between Perfluorooctanoic acid and DM is still unclear, so it is necessary to look for some evidence regarding this relationship. The search for evidence is also equipped with a seven-step assessment of occupational diseases therefore it can assist occupational health doctors who find patients with DM and have a history of exposure to glutaraldehyde. Methods: Literature search using Pubmed, Cochrane and JSTOR databases. The keywords used are “PFOA” OR “perfluoroalkyl substances” OR “perfluorooctanoic acid” AND “diabetes mellitus”. Article selection was performed using the defined inclusion and exclusion criteria. Selected articles will be critically reviewed based on etiological studies from the Oxford Center of Evidence-Based Medicine. Result: It was in 5 selected articles that met the inclusion and exclusion criteria. The main finding after a critical review was that PFOA may increase the risk of DM. Conclusion: There is a relationship between PFOA and the incidence of DM in health workers. To establish the diagnosis of occupational diseases in health workers who are exposed to PFOA, seven steps of diagnosis of occupational diseases are used.

However, the causal relationship between glutaraldehyde and DKA is still unclear, so it is necessary to look for some evidence regarding this relationship. The search for evidence is also equipped with a seven-step assessment of occupational diseases therefore it can assist occupational medicine doctors who find patients with ACD and have a history of exposure to glutaraldehyde. Methods: Literature search using Pubmed, Scopus and JSTOR databases. The keywords used are ‘healthcare worker OR healthcare personnel’, ‘glutaraldehyde’ and ‘allergic contact dermatitis. Article selection was performed using the defined inclusion and exclusion criteria. Selected articles will be critically reviewed based on etiological studies from the Oxford Center of Evidence-Based Medicine. Result: It was in six selected articles that met the inclusion and exclusion criteria. The main finding after a critical review was that glutaraldehyde may increase the risk of ACD. Conclusion: There is a relationship between 1% glutaraldehyde exposure and the incidence of ACD in health workers. To establish the diagnosis of occupational diseases in health workers who are exposed to glutaraldehyde, seven steps of diagnosis of occupational diseases are used.

Respiratory system diseases are one of the main problems in contemporary occupational medicine and exceed one fifth of all occupational diseases. Objective of the review was search and analysis of literature on diagnosis of silicosis as an important occupational disease of respiratory organs, by computed tomography and high-resolution computed tomography, from evidence-based medicine viewpoint.

When the history of the COVID-19 pandemic is written, it is likely to show that the mental models held by scientists sometimes facilitated their thinking, thereby leading to lives saved, and at other times constrained their thinking, thereby leading to lives lost. This paper explores some competing mental models of how infectious diseases spread and shows how these models influenced the scientific process and the kinds of facts that were generated, legitimized and used to support policy. A central theme in the paper is the relative weight given by dominant scientific voices to probabilistic arguments based on experimental measurements versus mechanistic arguments based on theory. Two examples are explored: the cholera epidemic in nineteenth century London—in which the story of John Snow and the Broad Street pump is retold—and the unfolding of the COVID-19 pandemic in 2020 and early 2021—in which the evidence-based medicine movement and its hierarchy of evidence features prominently. In each case, it is shown that prevailing mental models—which were assumed by some to transcend theory but were actually heavily theory-laden—powerfully shaped both science and policy, with fatal consequences for some.

Descriptions of “evidence-based” approaches to medical care are now ubiquitous in both the popular press and medical journals. The term evidence-based medicine (EBM) was first coined in 1992, and over the last two decades, the field has experienced rapid growth, and its principles now permeate both graduate medical education and clinical practice. The field of EBM has been in constant evolution since its introduction and continues to undergo refinements as its principles are tested and applied in a wide variety of clinical circumstances. This review presents a brief history of EBM, EBM: fundamental tenets, a critical appraisal of a single study, reporting guidelines for single studies, a critical appraisal of a body of evidence, evidence-based surgery, and limitations in EBM. Tables list strength of evidence for treatment decisions (EBM working group), Oxford Centre for Evidence-Based Medicine revised levels of evidence for treatment benefits , “4S” approach to finding resources for EBM, critical appraisal of individual studies examining therapeutic decisions, reporting guidelines by study design, and key resources for evidence-based surgery. This review contains 6 tables and 85 references

A clinical decision on the use of complementary and alternative medicine (CAM) should be made based on evidence-based medicine (EBM) together with practitioner's knowledge and experiences. This chapter describes the process of EBM, including how to address a clinical question, do a systematic search for appropriate evidence with key search terms, appraise the evidence and make a clinical decision on CAM applications. An effective literature search should be performed by using a structured search strategy in searching biomedical and CAM databases, such as the National Center for Complementary and Alternative Medicine (CAM Citation Index). Few standard tools are recommended to evaluate the quality of CAM studies, i.e. the CONSORT extension for herbal interventions and STRICTA for RCTs of acupuncture. Additionally, some guidelines for designing RCTs in Chinese herbal medicine (CHM) can also be adopted to critique CAM literature. A clinical decision on choosing optimal CAM for patient care should be based on the current best evidence emerged from the EBM process.

This chapter provides a summary of a landmark historical study in cardiac surgery related to internal mammary artery ligation versus sham sternotomy for angina pectoris. It describes the history of the procedure and a summary of the study including study design and results, and relates the study to a modern-day principle of evidence-based medicine: blinding and sham surgery. Whether or not sham surgery is ethical remains under debate. Proponents for sham surgery agree that it should be used only when a question cannot be answered adequately by other methods. Cobb and his colleagues were among the first to demonstrate the value of sham studies in addressing important clinical questions.
