Background: Cancer-related fatigue (CRF) during chemotherapy and after it and decreased body energy are common problems in patients that do not resolve with sleep and rest. Objectives: This study aimed to assess the effect of combination therapy of low-intensity exercise and slow stroke back massage (SSBM) on physical activity and fatigue intensity of patients undergoing chemotherapy. Methods: This clinical trial study was performed on 92 patients with cancer undergoing chemotherapy who were referred to the oncology wards of Baqhaiee-2 hospital Ahvaz-Iran (2018 - 2020). Patients were randomly divided into two groups. Intervention group patients who received three days a week for four weeks that each session 10 minutes for slow stroke back massage and 15 minutes’ low-intensity exercise. Control group patients who received usual care. Data were collected using a demographic questionnaire, Piper Fatigue Scale (PFS), and International Physical Activity Questionnaire (IPAQ) and then analyzed using SPSS software. Results: The results showed the intensity of fatigue decreased in the experimental group, and there was a statistically significant difference between the two groups (P = 0.05). The trend of physical activity increased in the experimental group; however, there was no statistically significant difference between the two groups (P = 0.68). Conclusions: The combination of low-intensity exercise intervention and slow stroke back massage had a positive effect on fatigue severity but no statistically positive effect on physical activity.

Abstract Purpose The Trials within Cohorts (TwiCs) design aims to overcome problems faced in conventional RCTs. We evaluated the TwiCs design when estimating the effect of exercise on quality of life (QoL) and fatigue in inactive breast cancer survivors. Methods UMBRELLA Fit was conducted within the prospective UMBRELLA breast cancer cohort. Patients provided consent for future randomization at cohort entry. We randomized inactive patients 12–18 months after cohort enrollment. The intervention group (n = 130) was offered a 12-week supervised exercise intervention. The control group (n = 130) was not informed and received usual care. Six-month exercise effects on QoL and fatigue as measured in the cohort were analyzed with intention-to-treat (ITT), instrumental variable (IV), and propensity scores (PS) analyses. Results Fifty-two percent (n = 68) of inactive patients accepted the intervention. Physical activity increased in patients in the intervention group, but not in the control group. We found no benefit of exercise for dimensions of QoL (ITT difference global QoL: 0.8, 95% CI = − 2.2; 3.8) and fatigue, except for a small beneficial effect on physical fatigue (ITT difference: − 1.1, 95% CI = − 1.8; − 0.3; IV: − 1.9, 95% CI = − 3.3; − 0.5, PS: − 1.2, 95% CI = − 2.3; − 0.2). Conclusion TwiCs gave insight into exercise intervention acceptance: about half of inactive breast cancer survivors accepted the offer and increased physical activity levels. The offer resulted in no improvement on QoL, and a small beneficial effect on physical fatigue. BACKGROUND Physical activity ameliorates chronic stress. Latest research suggests a relation between resilience and physical fitness. Beneficial adaptations of the hypothalamic-pituitary-adrenal axis, the sympathetic nervous system, the endocannabinoid system, and the tryptophan pathway, induced by an active lifestyle, are considered to be resilience conducive. However, detailed knowledge on the molecular link between the effects of acute and chronic physical exercise and an improved resilience is missing. OBJECTIVE Therefore, we implement a human exercise intervention trial addressing the main hypotheses: (1) web-based exercise training improves aerobic capacity of physically inactive adults, which (2) is accompanied by improved resilience. In this setting, we will analyze the relation of resilience parameters with innate and acquired aerobic capacity as well as circulating signaling molecules. METHODS 70 healthy physically inactive (<150min/week physical activity) adults (18-45y) will be randomly assigned to an intervention or control group. Participants in the intervention group receive weekly training utilizing progressive endurance and interval running individually adapted to their training performance via web-based physician support. A standardized incremental treadmill exercise test is performed before and after the intervention period of eight weeks to determine innate and acquired aerobic capacity. Before and after the intervention psychological tests and questionnaires that characterize parameters implicated in resilience (including impulse control, working memory, stress coping, emotion regulation) will be applied. Blood, and saliva will be sampled for analysis of lactate, cortisol, endocannabinoids, catecholamines, kynurenic acid, cell-free DNA and further circulating signal transducers. Statistical analysis will provide comprehensive knowledge on the relation of aerobic capacity and resilience, and furthermore, the capacity of peripheral factors to mediate the promoting effects of exercise on resilience. RESULTS The study was registered in October 2019. Enrollment began in September 2019. Out of 161 subjects, who were initially screened via telefon survey, 43 fulfilled the inclusion criteria and were included. Among the 17 participants in the intervention group, and 14 participants in the control group, who completed the study (total 31), no serious adverse event has been reported. Four participants withdrew during the programm (individual reasons), and 8 participants have not yet completed or begun the program due to the Covid-19 pandemic. Enrollment and data analysis are ongoing, and results are expected to be submitted for publication in October 2021. CONCLUSIONS Our study aims to demonstrate that an increase in physical activity with a simultaneous improvement in aerobic capacity is associated with an increase in resilience. This study design allows to investigate the effect of an active lifestyle on circulating molecular marker levels and their relationship to resilience factors. This will offer novel approaches for the diagnosis of depression and the therapy by exercise prescription. CLINICALTRIAL DRKS00018078, registered October 02, 2019, German Clinical Trials Register. Abstract BackgroundPhysical activity during pregnancy period is one of the issues with priority during pregnancy period. Researches show that women reduce their physical activity during this period and are unaware of the benefits on the health of the mother and embryology. Although researchers regarding physical activity during pregnancy have prepared many guidelines; it is not however clear why pregnant women do not perform physical activity and the effectiveness factors that facilitate the desired behavior. MethodsThe research population included all pregnant women aged 18 to 40 years with gestational age of 12-38 weeks referring to the healthcare centers in District 5 of Tehran Municipality, from 22 districts of the city who had eliminated their physical activity during pregnancy. Questionnaires of physical activity assessment questionnaire after educational intervention in pregnant women (PPAQ) and a questionnaire designed based on the results of the needs assessment and the dimensions of the PEN-3 model were used for assessment. This study is a Research Clinical Trial (RCT). Due to the nature, a quasi-experimental research design (pre-test, post-test) with the control group was used in this research. ResultsBased on the obtained results, the intervention based on E-learning has a significant effect of enabling factors to increase physical activity in pregnant women in the experimental group. In addition, the intervention based on E-learning has a significant effect on the training factors in order to increase physical activity in pregnant women in the experimental group. ConclusionBased on the research results, it can be said that there is a statistically significant difference between the experimental and control groups in the scores obtained from training factors in the pretest and the experimental group. Furthermore, it can be said that there is a statistically significant difference between the experimental and control groups in the scores obtained from the enabling factors in the pre-test and the experimental group.

The purpose of this study was to examine the effects of participation in an aerobic exercise intervention on daily activity occurring outside of the structured exercise sessions. Participants were randomized into one of the following 4 conditions: 1) No-exercise, 2) Low volume, low intensity exercise (LVLI), 3) High volume, low intensity exercise (HVLI), 4) Low volume, high intensity (LVHI). Physical activity was measured over 7 days with an accelerometer at baseline and during week 8 of the intervention. Activity was defined as: sedentary behaviour (SED; < 100 counts/minute), light physical activity (LPA; 100 to 1951 counts/minute), moderate-to-vigorous physical activity (MVPA; ≥1952 counts/minute), and total physical activity (TPA; LPA + MVPA). Activity was quantified as average total minutes per day of each SED, LPA, MVPA, and TPA. A one-way ANOVA was used to determine if time spent in SED, LPA, MVPA, and TPA changed from baseline to week 8. Seventy-one participants (No-exercise; n=12, LVLI n=17, HVLI n=24, LVHI; n=18,) with a mean age of 54 y and waist circumference of 110 cm completed 8 weeks of the intervention. There were no significant differences in SED, LPA, MVPA, or TPA between groups at baseline. There was no significant change in SED, LPA, MVPA, or TPA at week 8 compared to baseline (p>0.05). Similarly, there were no significant differences in activity variables between exercise conditions. Our observations suggest that daily activity patterns do not change with the implementation of an exercise intervention in men and women.

Abstract Background: The aim of the present study was to assess readiness to become or stay physically active according to the Stages of Change Model.Methods: The present quasi-experimental study was conducted on 100 women working in the healthcare centres of Bandar Abbas, Iran. The sampling method is clustering in type. The subjects were assigned into two groups of intervention and control. The collected data were analysed by SPSS-16 software using descriptive and inferential statistics, including independent-sample t-test, paired-sample t-test and Chi-square test.Results: Before the educational intervention, 19 subjects (0.38%) from the intervention group showed to have regular physical activity (4-5 stages). This number changed to 29 (0.58%) and 25 (0.50%) after three months and six months of intervention. A statistically significant difference was found before the intervention and 3 and 6 months afterwards (P˂.001). In the control group, no statistically significant difference was found between the pre-intervention and post-intervention (three months (P=.351) and six months (P=.687).Conclusion: The educational intervention based on the stages of behaviour change model showed to be effective in promoting the physical activity of employed women. These findings may benefit health education researchers and practitioners who tend to develop innovative theory-based interventions and strategies to increase the level of physical activity in women.

The school has been identified as a key setting to promote physical activity. The purpose of this study was to evaluate the effect of a classroom-based activity break on in-school step counts of primary school children. Data for 90 children (49 boys, 41 girls, 9.3 ± 1.4 years) from three Irish primary schools is presented. In each school one class was randomly assigned as the intervention group and another as controls. Children’s step counts were measured for five consecutive days during school hours at baseline and follow-up. Teachers of the intervention classes led a 10 min activity break in the classroom each day (Bizzy Break!). Mean daily in-school steps for the intervention at baseline and follow-up were 5351 and 5054. Corresponding values for the control group were 5469 and 4246. There was a significant difference in the change in daily steps from baseline to follow-up between groups (p < .05). There was no evidence that girls and boys responded differently to the intervention (p > .05). Children participating in a daily 10 min classroom-based activity break undertake more physical activity during school hours than controls.

Background: The patients under cataract sur-gery often experience anxiety not only during the surgery, but also prior to the surgery.Purpose: We sought to determine the effects of slow-stroke back massage on anxiety in patients undergoing cataract surgery. Setting: The study was conducted in the Amiral-momenin Hospital of Zabol city, south-east of Iran.Participants: A total of 60 candidates of cataract surgery participated in the study.Research Design: The participants were ran-domly allocated to either control or intervention groups. The intervention group received slow-stroke back massages, while patients in control group received routine interventions.Intervention: The slow-stroke back massage was performed on the patients assigned to the interven-tion group. The intervention was performed in the morning of the surgery day at 30 minutes before the surgery. The researcher performed each mas-sage session in a sitting position. The duration of each massage session was 15 minutes. Main Outcome Measures: Anxiety was assessed in the both groups in the morning of the surgery, before and immediately after the intervention. In-dependent samples Student’s t test, paired samples Student’s t test, and chi-squared test were used to analyze the data.Results: Anxiety was not significantly different between the two groups before and after the mas-sage (p = .816). On the other hand, paired samples Student’s t test showed a significant difference comparing the anxiety scores before (49.7±5.43) and after (45.16±3.89) the massage in the interven-tion group (p < .001). Conclusions: Based on our results, slow-stroke back massage, which is a low-cost and safe method, reduced anxiety in patients who were candidates for cataract surgery.

Abstract This study examined whether exergames could improve physical, functional, and cognitive functions in people with Down syndrome. Twelve adults with DS, aged over 35 (M = 50.35, SD = 7.45), were randomly assigned to a Wii-based program (n = 6) or a control group (n = 6), and completed physical (Chair Stand Test, 6-Minute Walk Test), functional (TUG, TUDS), and cognitive tests (Corsi, Barrage tests). The experimental group completed a 12-week Wii-based program. There was high intervention adherence and, compared with the control group, greater improvements were observed in the Wii-based exercise intervention group in physical fitness and functional outcomes (p &lt; .05), with no changes in cognitive outcomes.

Background & Aim: Human Immunodeficiency Virus (HIV) prevalence is increasing, and this disease has become a crisis for the modern world. Today, the survival of patients has been increased, such that HIV is considered a chronic disease. So, Paying attention to health-promoting intervention is necessary. Thus, the current study aims to determine the effect of educating lifestyle based on the Pender model on health-promoting behaviors in HIV patients. Methods & Materials: In this randomized clinical trial study, 70 HIV patients who had inclusion criteria were selected and then divided into intervention and control groups randomly. The intervention group received 6 one-hour education sessions weekly based on Pender lifestyle (nutrition, physical activity, stress management, spiritual growth, interpersonal relationships, and health responsibility). A demographic questionnaire and HPLP2 were used, which were completed by both groups before the intervention and 8 weeks after the intervention. Chi-Square, Fisher, Independent t, and ANCOVA statistical tests and SPSS 16 software were used to analyze data. Results: results showed that there was no significant difference in various dimensions of health-promoting lifestyle between two groups before intervention. However, intervention group scores for nutrition (28.08±6.23 vs. 23.58±6.04), physical activity (22.26±6.46 vs. 16.39±6.09), stress management (25.03±5.14 vs. 19.96±6.41), spiritual growth (29.49±6.11 vs. 25.45±8.54), interpersonal relationships (29.17±6.14 vs. 23.11±7.45) and health responsibility (28.36±6.06 vs. 23.89±5.74) were significantly higher than control group 8 weeks after intervention. Moreover, the total score of health-promoting behaviors had a significant difference in the intervention group compared to the control group (166.7±28.43 vs. 134.5±35.68, p<0.001). Conclusion: Based on the findings, it can be said that educating lifestyle based on the Pender model causes HIV patients to use health-promoting behaviors, which are recommended as a useful theory-based program for managers and providers of health services.

Abstract Background: The purpose of this study was to evaluate bone response to an 8-month aerobic gymnastics training program in young opioid addicted women. Method: One hundred and two young women with previous opioid addiction were divided into two groups: (a) low bone quality intervention experimental group (n=55); (b) low bone quality observed control group (observation group) (n=47). Intervention group took aerobic gymnastics regularly for 120 min·d-1, 5d·wk-1 for 8 months and completed follow-up testing. Substance use history and other life habit affecting bone quality were assessed by questionnaire-based interviews. Bone quality (stiffness-index, T-score, Z-score) was examined with quantitative ultrasound. Anthropometric characteristics (body weight, fat free mass, fat mass) were obtained by bioelectrical impedance analysis. Results: After 8-month intervention, the stiffness-index of bone quality increased significantly (before: 82±6, after: 108±14, p<0.05) in the experimental groups. However, the bone quality did not change significantly in the controls (observation group: before: 79±10, after: 77±13, p>0.05). Fat mass decreased in experimental group (Experimental group: before: 19.6±3.7kg, after: 18.8±4.0kg, p<0.05). Meanwhile, change of fat-free mass was the determination of change of bone quality in the experimental group. Conclusions: Our results suggested that aerobic gymnastics intervention can be an effective strategy for the prevention and treatment of drug-induced osteoporosis in detoxification addicts.
