Objectives: To highlight the knowledge, frequency, types, and the clinical aspects of overusing TCs on the face as well as the motivation for such practice, with the aim of rising the public awareness in Mosul City. Methods: This prospective observational study included 180 patients aged 13-53 years, with steroid dermatitis resembling rosacea "Iatrosacea" with history of using TCs on face > 1 month, were evaluated at Al-Qudis Health Center for Family Medicine. Patients contradicted using TCs, or with classical rosacea were excluded. Results: The majority were females (90%). Duration was 2 months to 7 years. The motives were fairness (55%) and melasma (19%). The commonly abused drug was Betamethasone Valerate (35%). The reported side effects were facial erythema (84%), telangiectasia (79%), rebound phenomenon (66%), rosacea (56%), and acne (42%). The exacerbation on stopping TCs (63%) and skin lightening (37%) were the main reasons behind continued use; 59% were unaware about side effects of TCs; 42% obtained the products from cosmetic shop/Beautician and 27% from nurse clinic. In spite of observable side effects, 74% of abusers didn't feel guilty for applying them for protracted periods. Conclusion: Inappropriate use of TCs for cosmetic purposes is still commonly encountered in our environment, with believe TCs will correct any facial imperfection. As fair skin is correlated with beauty and social privilege, both genders of different ages, social, and educational status are indulged in this practice that exposed them to medical and social problems. It is a multiphase issue necessitates collaboration of different sectors to raise public awareness and control their easy availability to overcome this dilemma.

Background/ Context: Corticosteroids, despite being a very effective line of treatment for various disorders is known to have a wide range of adverse effects. The use of systemic and topical steroids has been associated with cataract formation, however the ophthalmic side effects of application of steroid on skin away from the face has not been studied in depth. With increasing use of corticosteroids as rst line of therapy in treatment of various dermatological conditions, the ocular side effects, if any, of these preparations, cannot be overlooked. Aim: To study the incidence of cataract in patients using skin preparations of corticosteroids away from the face and also to correlate the dose and duration of steroid therapy to incidence of cataract. Materials & Methods: 50 patients with dermatological disorders who were being treated with dermatological steroids, on sites other than the face and periorbital region, for a period of more than six months consisted of the study group. The study group consisted of patients in the age group of 20 – 60 years. The study was conducted over a period of two years at a tertiary care hospital. Visual acuity, Slit lamp biomicroscopy, applanation tonometryand detailed fundus examination using 90D lens were carried out. Results: The incidence of cataract was signicantly higher in patients using dermatological steroids away from the face. Prolonged duration of use of these steroids also indicated a signicantly higher incidence of cataract. However, a larger population based study needs to be carried out to study the relationship between potency of steroid and incidence of cataract, although preliminary studies indicate a denite correlation between the two. Conclusions: Despite, steroids being used away from the face, there was a denite increase in the incidence of cataract. Before coming to any rm conclusions it would be advisable to reproduce the study in a larger population. However from the present pilot study it is prudent to advocate judicious use of innocuous looking steroid creams as they can cause signicant adverse effects

Tobacco consumption is a huge public health issue in India and its impact is especially devastating among the poor. Effective tobacco control should be a top priority, both as a health issue and as a method to reduce poverty. Tobacco use is deeply ingrained as a cultural practice and there are a myriad of tobacco types. We reviewed multiple determinants of tobacco consumption including socio-economic status, marriage, population growth, marketing strategies, and price. We also considered the tobacco burden including economic and social costs and adverse health impacts especially those resulting from oral cancer. We then addressed the history of tobacco control legislation in India and challenges in implementation. Tobacco consumption in India is continuing to increase despite tobacco control policy. Needed are more visible and aggressive anti-tobacco campaigns including increased public awareness of tobacco harms and active engagement of worksites and health professionals in promoting tobacco cessation.

Large scale censorship and control over the free flow of information on the internet that was already implemented on a large scale in many authoritarian countries in China in the past few decades has started to work its way through the more liberal and western countries including India, US etc. especially in the last decade raising concerns over privacy issues and the possibility of a dystopian future of tyrannical governments empowered by the use of digital surveillance technology to increase their power and make them essentially undefeatable on a level unforeseen in the history of humanity among many great thinkers in our era. In this paper, we wish to outline a method to not only combat but to completely eliminate both the possibility and current usage of all censorship and control over flow of information on the internet, hence heralding an era of free flow of information throughout the world and destroying practically all mind control that tyrannical governments can hold over their people, in essence ending the era of propaganda and tyranny from the face of this earth forever, using blockchain technology.

A 57-year-old shepherd was referred with a 2-week history of decreased visual acuity in both eyes. Optical coherence tomography (OCT) (Heidelberg Engineering GmbH, Heidelberg, Germany) revealed diffuse outer nuclear layer hyperreflectivity and indistinguishable external limiting membrane and ellipsoid zone. The patient announced to us that he took two 500 mg of closantel tablets (15.15 mg/kg) three days before the initiation of visual problems for sore throat as an antibiotic. Electroretinography displayed severely attenuated responses in both eyes. We decided to admit the patient with the presumed diagnosis of closantel retinal toxicity and treated him with intravenous methylprednisolone 1 g per day and intravenous erythropoietin 10000 IU twice a day, and reevaluation of the patient proved no change in his visual acuity on the third day of admission. Closantel is a veterinary drug with serious side effects in the human retina and central nervous system even in previously reported doses. Public awareness and appropriate drug labeling about its side effects could prevent accidental toxicity. OCT is a noninvasive and rapid diagnostic modality that should be done in suspected toxic retinopathy.

Abstract Background: Hypertension, known as the silent killer, is a major risk factor for cardiovascular disease. Awareness and treatment of hypertension is not appropriate in the world, and this has led to an increase in mortality and morbidity caused by uncontrolled hypertension.This study aims to estimate awareness, treated, and controlled hypertensive and relevant predictors in an adult Iranian population.Methods: This cross-sectional study was conducted on 10000 adults aged 20-69 years in Yazd, Iran. They were selected through multi-stage random cluster sampling in 2015-2016. Blood pressure was measured three-time with standard protocol by trained health workers. Those with a positive history of hypertension and using anti-hypertensive drugs, prescribed by a physician, were considered hypertensive. Hypertension was defined as systolic blood pressure ≥140 mmHg and/or diastolic BP of ≥90 mmHg. Uncontrolled hypertension was defined in accordance with recommended treatment targets by the Joint National Committee (JNC7). Logistic regression was used to assess the predictors of hypertension awareness, treatment and control.Results: The prevalence of hypertension was 37.3%, and the prevalence of pre-hypertension was 46.4%. 49.7% of People with hypertension were aware of their disease, and 71.5% of them were using antihypertensive drugs prescribed by physicians. Blood pressure was controlled in 38.9% of the treated patients. In the adjusted model, older age, female sex, and history of diabetes mellitus were positively associated with higher awareness. High physical activity, tobacco smoking, and diabetes are the only predictors of treated high blood pressure. Younger age, female sex, and higher education were determinants of controlled hypertension. Having health insurance was significantly correlated with awareness and control of hypertension.Conclusion: Hypertension is a public health problem in this population, which is not well controlled. Half of the patients were unaware. Intervention for increased screening coverage is needed. It should plan to raise public awareness about hypertension and improve hypertension control under the supervision of physicians. Implement a family physician program is recommended in the health system.

<p class="abstract">Granuloma faciale (GF) is a chronic condition characterized by asymptomatic erythematous plaque with prominent telangiectasia presenting usually over the face. Although the condition is benign, its treatment is often unsatisfactory. Therapeutic modalities that have been tried include topical steroids and topical tacrolimus sometimes enhanced with topical dapsone. Others include intralesional corticosteroids, antimalarials, isoniazid and pulsed-dye laser. We report a case of a 58 years old female with a 1 year history of a solitary slowly progressive plaque over the nose. Diagnosis of GF was made based on the histopathological findings. The patient was started on the combination of topical tacrolimus, intralesional corticosteroids injection and oral doxycycline for 3 months. The patient showed gradual improvement in 3 months without any side effects. This case supports previous papers of successful treatment of GF with topical tacrolimus. There was no recurrence at follow-up 18 months later. It also supports the use of combination therapy especially in resistant cases.</p>

This article examines the changing function of the Nyasaland police force between the 1890s and 1962. Initially, the police consisted of small groups of armed ex-soldiers, totally untrained in conventional police duties and employed by district officers in pressing labour and enforcing the payment of hut tax. In 1920, however, the authorities responded to the threat seemingly posed by the emergence of ‘dangerous classes’ – particularly labour migrants returned from the south – by forming a trained, centralized force, commanded in the Shire Highlands, though not elsewhere, by European police-officers. In the reorganized districts the police succeeded in protecting urban property. But so limited was the size of the force that the prevention and detection of crime was hardly attempted over the greater part of the country, while campaigns such as that against the Mchape witchcraft eradication movement foundered in the face of popular opposition.Substantial changes began in the mid-1940s in response to urbanization and the increasing complexity of police duties, coming to a climax in the 1950s as the colonial government struggled to maintain authority. At first the emphasis was on raising educational standards and improving conditions of service. But following the crisis of 1953, it switched to expanding police numbers and increasing the coercive power of the force; this process was accelerated in the aftermath of the 1959 emergency.Recruitment policies were influenced by the technical requirements of the authorities and by the ethnic stereotypes they evolved – a combination which resulted in the recruitment of a disproportionate number of Yao policemen in the first few decades and of more Lomwe and Chewa later. Policemen were attracted less by the rates of pay than by the privileges on offer. An inner corps of policemen spent their lives upholding colonial authority, but most could not be placed in a distinctive category of ‘collaborator’.

. Objective: To determine the frequency of diabetes mellitus and to assess the level of awarenessabout diabetes amongst persons attending a free health camp in cultural festival of Rahim Yar Khan. Study Design:Cross sectional observational study. Place and duration of study: Three days free health camp arranged in a culturalfestival in Rahim Yar Khan. Results: Over all frequency of type II diabetes was 19.38% and the frequency of diabetesincreased both in men and women with increasing age (p<0.001). Newly diagnosed subjects were 6.77% and thesedo not know that they were suffering from diabetes. 18.92% subjects were having glucose intolerance and 61.70%subjects were having random blood glucose level within normal limits. Diabetes was correctly defined by 57% subjectsand this was significantly associated with educational level, suffering from the disease and presence of diseaseamongst family members. 14.62% gave history of presence of disease in their families. As compared to females, maleswere more suffering from disease and these were consuming more foods and snacks than females (p<.0.01).Conclusion: High frequency of diabetes mellitus was observed amongst the study population. This may be due tohealth consciousness and higher level of educational status in the subjects who participated. Emphasis on healtheducation and preventive strategies are needed to increase public awareness and early detection of high risk groupsand risk factors of this common disease in our country. The lifestyle behaviors and environmental factors are strongmodifiers of diabetes The life style changes and interventions in early age may prove to be more effective thaninterventions done at a later stage.

Abstract Background Multisystem inflammatory syndrome in children (MIS-C) is a severe immune-mediated syndrome that occurs after COVID-19 infection. It mainly affects children and presents several clinical aspects. The cutaneous and mucous signs are very much part of the diagnostic criteria. The aimisto describe the clinical and evolutionary aspects of the muco-cutaneous signs during MIS-C Patients and methods It was a descriptive retrospective study conducted over a period of 9 months (March 1, 2020 - December 31, 2020) including children admitted to COVID isolation unit with suspicion of MIS-C syndrome. Results We included 17 patients. The average age was 7 years (1–11 years). The sex ratio was 2.2 (11 boys and 6 girls). There was o history of dermatological disease has been reported. Fever was present in all patients. Mucosal signs were present in 13 patients. Conjunctival hyperemia without purulent secretions was noted in 12 patients. Conjunctivitis was bilateral in 11 cases. A rash was found in 10 cases. It was macular (6 cases), maculopapular (2 cases) and vesicular cluster in one 1 case. It was itchy in 2 cases. It was located in the limbs (7 cases), thorax and/or abdomen (5 cases), pelvis (4 cases), palms and/or soles (3 cases) and the face in one case. Cheilitis was found in 6 cases and stomatitis or glossitis in 5 cases. oEdema of the extremities was present in three patients and oedema of the face was noted in one patient. All children received intravenous immunoglobulin therapy combined with high-dose corticosteroids and acetylsalicylic acid at anti-aggregating doses. The course was marked by the disappearance of the muco-cutaneous signs without recurrence in all cases. Conclusion Mucocutaneous involvement is a characteristic manifestation of multisystem inflammatory syndrome. It is one of the various diagnostic criteria for this syndrome.
