EVALUATION OF ORTHOPEDIC TREATMENT EFFICIENCY OF PATIENTS WITH POST-TRAUMATIC DEFECTS OF THE UPPER JAW BY THE INDICATORS OF QUALITY OF LIFE

acquired maxillofacial defects are most often accompanied by pronounced functional and aesthetic disorders, which lead to limitation of life, social maladaptation and deep sociopsychological problems of the patient's existence. therefore, the purpose of our study was to monitor the effectiveness of orthopedic treatment of patients with post- traumatic and postoperative defects of the maxillofacial system by quality of life. the period of adaptation, fixation of the structure in the oral cavity, presence of inflammatory processes under the prosthesis, breakage and correction, convenience of use, diction, hit of food residues under the prosthesis and oral-nasal test were determined and analyzed. the study showed that the improved technique had a positive effect on each of the parameters and made it easier to adapt to dentures and improve the quality of life of patients in general. PHYTOHORMONAL ACTIVITY OF IMMOBILIZED CELLS OF SALT-TOLERANT COTTON RHIZOBACTERIA AFTER LONG TERM STORAGE

the phytohormonal activity of free and immobilized cells of b. subtilis bs-80, b. licheniformis bl-83, p. polymyxa pp-113 and p. amylolyticus pa-118 on phosphate flour have been studied. it was found that immobilized cells more actively synthesized iaa and gibberellins in comparison with free cells. the data obtained indicates that the phytohormonal activity of the salt-resistant cells of cotton rhizobacteria can be kept much better in the immobilized form than in the free form (in a refrigerator on jambs). ROLE OF T-786C ENDOTHELIAL GENE POLYMORPHISM OF NO - SYNTHASE IN THE DEVELOPMENT OF ACUTE CONTRAST-INDUCED NEPHROPATHY IN PATIENTS WITH CORONARY HEART DISEASE

contrast-induced nephropathy (cin) is the third considerable cause of acute kidney injury (aki) and makes up almost 10% of all acute kidney failure (akf) cases.objective. to study the role of polymorphism of enos-gene in the development of acute contrast-induced nephropathy (cin) in case of coronary heart disease.materials and methods. the prospective study was conducted in the national cardiology and therapy center named after academician mirsaid mirahimov from 2015 to 2018. a total of 184 patients with coronary heart disease (chd) aged 33-70 years (average age 55.2 ± 8.5 years) were examined to determine possible associative relationships between enos gene polymorphism (t786c) and the development of acute cin. of these, the group without cin was 152, and with the cin there were 32 patients who underwent coronary angiographic examination (cag). radiopaque contrast agents ultravist (iopromide) and omnipack (iohexol) were used. cin was defined as an increase in serum creatinine (scr) concentration by more than 25% from the initial level or by more than 0.5 mg/dl (44.2 μmol/l) and a decrease in glomerular filtration rate (gfr) after administration of an iodinated contrast agent within 48-72 hours in the absence of other reasons. a molecular genetic study was conducted to determine the t-786c polymorphism of the enos gene.results. the genotype frequency of the tt gene in the group with cin was 87.5%, and in the group without cin 69.7%, the reliability was p <0.05. but the connection of acute cin with the genotypes of ts and ss was not observed in both of the groups.conclusions. tt genotype t-786c polymorphism of the enos gene is a risk factor for the development of acute contrast-induced in patients with coronary heart disease. cts is the most common compression neuropathy with an incidence of 125-515/100 000. it is a result of compression of the median nerve by the transverse carpal ligament. it is observed in 2 to 5% of the general population, more frequently in women.in the last few years the scientific society has acknowledged the boston carpal tunnel syndrome questionnaire (bctq) and more and more frequently it is being used as a standard in cts patients. the literature also contains numerous studies on the diagnostics and treatment of cts using bctq.aim of study. to investigate the effect of a complex of physical factors on the reverse development of symptoms of moderate carpal tunnel syndrome using bctq and to compare the results of the bctq subjective rating (sss) (symptom severity scale) and function − fss (functional status scale) with objective assessment measured by the eng parameters of n. medianus materials and methods. 57 patients with mild to moderate degree of carpal tunnel syndrome clinically proven by electroneurographic study were examined. 38.60% of the patients were with right hand affected, 9 patients (15.80%) with left hand affected and 26 patients (45.60%) with bilateral involvement (n=26), in the last group, we examined both hands, i.e. 57 patients and 82 hands were investigated.based on the analysis of our own studies, we applied the following complex physiotherapeutic program to the patients involved in the study: ultrasound, electrophoresis with nivalin (galantamine), and traditional kinesitherapy program.results and discussion. by analyzing the results of the symptom severity assessment of the first part of the questionnaire we find that subjective complaints of patients decreased statistically significantly at the end of physiotherapy course from an average of 2.67 before treatment to 2.21 at the end of ftp (p <0.01). this trend continues within 1 − 4 months after the end of physiotherapy − average 1.79 and maintains up to 4 − 8 months when the mean of the results is 1.69 (p <0.001). we found that with regard to the recovery of the function of the affected hand and the way it affects some of the activities of daily living, the results of the second part of the bctq - functional status scale - show a statistically significant improvement at the end of the physiotherapy course 1.91 to an average of 1.63 (p <0.001), with a tendency for long-term retention and a slight decrease in the average of the results obtained to 1.37 within the first control examination at 1-4 months (p <0.001) and resistance to this trend in the following months (4-8) − 1.31 (p <0.001).based on the anova analysis and the calculation of the η coefficient, we proved that there is no significant difference in the results obtained from the boston questionnaire and those from the eng examination. there is a high percentage of overlap - from 53.10% to 95.60%.although the results of the questionnaire are based on the subjective assessment of the patient, there is a high percentage of overlap with those of the objective study (eng), which is a prerequisite for assuming that the questionnaire is highly reliable and valid and can be used as a tool for evaluating and analyzing a number of cт c studies, which is in line with leite's findings that the boston questionnaire is highly reliable and can replace any other non-standard methods for assessing cts severityconclusions. in the presented facts of the discussion impression makes the patient's influence both on the subjective complaints and the clinical symptoms as well as on the degree of restoration of the affected hand functions at the end of the physiotherapy course, the continuation of this trend up to 4 months after the end of the treatment and maintaining it for 4 to 8 months.although the results of bctq are based on the subjective assessment of the patient, there is a high percentage of overlap with those of the objective study (eng), which is a prerequisite for assuming that the questionnaire is highly reliable and valid and can be used as a tool for evaluating and analyzing a number of carpal tunnel studies, which is in line with leite's findings that the boston questionnaire is highly reliable and can replace any other non-standard methods for assessing cts severity Show More ... ... ANATOMIC-FUNCTIONAL STATE OF PELVIC ORGANS IN WOMEN WITH INFERTILITY AND OVARICOVARICOCELE

introduction. high informativeness and accessibility of ultrasound examination in infertile patients provides for the exclusion or confirmation of morphological changes in genitals, the degree of damage to the organ and the involvement structures, in particular, varicose veins of the gonadal veins, which can be explained by the emergence of ovarian dysfunction. in addition, the polymorphism of semiotics of echographic signs of pathological changes in pelvic organs in the examined patients confirms the complexity of determining the main and concomitant etiological factors of the emergence of functional infertility in women, the development of disorders of reproductive function and causes the need to involve other methods. the aim of the work was to study the anatomical and functional characteristics of the pelvic organs by ultrasound, medical diagnostic laparoscopy and hysteroscopy to determine the degree of morphological changes of the genitals in women with infertility and ovaricovaricocele. materials and methods. to solve the goals and objectives, 117 pregnant women of reproductive age (21-44 years old) with functional infertility were prospectively examined and divided into 2 groups for the comparative analysis: the main group was 62 women with infertility and varicose veins in the ovaries; a comparison group was 55 women with infertility without varicose veins. the study of the anatomical and functional state of the uterus, ovaries and fallopian tubes in women of the studied groups was performed by standard ultrasound examination on the philips atl-hdi 4000, philips hd 11-xe with the analysis of folliculogenesis. laparoscopic intervention was performed by the patients of the main group on the apparatus "karl storz" (germany) in the first phase of the menstrual cycle. diagnostic hysteroscopy in order to clarify the state of the uterus and endometrium was performed simultaneously with laparoscopy in the 1st or 2nd phase of the menstrual cycle using the equipment "karl storz" (germany).results. in women of the main group with a combination of functional infertility and ovaricoaricocele, there is a statistically significant increase in the percentage of incorrect position of the uterus - 36 (58.1%) in normal form and size of the uterus, both in the main group of women and in the comparison group - 53 (85,5%) and 48 (87.3%) cases respectively. analyzing the indicators of ovarian size and the number of antral follicles, a statistically significant difference was found in the direction of its reduction in women with functional infertility and ovaricovaricocele women without varicose dilatation of ovarian veins - 4.1 ± 0.1 cm3 vs. 5.8 ± 1 , 4 cm3 and 3.9 ± 1.1 versus 5.6 ± 1.4, respectively, in two groups. in addition, there is a tendency to reduce the size of the ovary of the protradiental age in women of all the studied groups with statistically significant rates in the patients of the main group. also, there is a difference in the right ovary volume relative to the left in women with infertility and ovaricovaricocele in the direction of decreasing the size of the latter, which may be due to the predominant localization of the enlarged gonadal vein, and this difference is statistically significant. in determining the functional state of the ovaries in the overwhelming number of women in all of the studied groups ovulation was noted, however, when studying the characteristics of the functional state of the yellow body according to echographic signs, decrease in the thickness of the endometrium in the middle of the luteal phase of the menstrual cycle in patients of the main group, as well as the absence in the overwhelming majority cases of its adequate secretory changes, which is statistically significant against the women of the comparison group. in patients, both the main group and the comparison groups, there is a decrease in the ratio of the volume of the yellow body and ovarian volume and the decrease in the thickness of the wall of the yellow body, respectively, against the control group women, which is an ultrasound sign of inferiority of the yellow body and lack of luteal phase of the menstrual cycle. in addition, women in the main group have a statistically significant reduction in the rates against the women in the comparison group. in women of the main group, a large percentage of varicose veins of the small pelvis are found in the absence of organic changes in the uterus and adnexies (73.3%) in the presence of infertility of obscure genesis.conclusions. ultrasound study in combination with color doppler of patients with ovaricovaricocele is a highly informative method of research, because it allows to perform topical diagnosis, to determine the morphological character of the lesion, to differentiate the type of pathological process and to identify the concomitant pathology of the pelvic organs. the conducted study suggests that ovaricovaricocele should be considered not only as an accompanying symptomatic changes in diseases of the female genitals, but also as one of the causes of ovarian dysfunction with delay or termination of growth of the follicles, change in their size, degenerative-dystrophic changes in the ovaries, which leads to a violation hormonal homeostasis and manifested by reproductive disorders. in the perspective of further research it is necessary to consider a complex analysis of the morphological and functional characteristics of the internal genital organs, which will establish the presence of a certain functional and organic gynecological pathology and will enable to diagnose the initial and minimal course of the pathological process of the pelvic organs, finally verify the final diagnosis and appoint pathogenetic treatment. conduct systematic analysis of the state of the supply of ukraine with medications with chondroprotective activity. the unified clinical protocol of primary, secondary (specialized) medical care allows doctors to control the state of a patient at every stage of illness and guarantees the effective treatment and prophylaxis of the locomotor system (lms) pathologies.as a result of the chondroprotective medication range analysis it was determined that the pharmaceutical market of ukraine is represented by 40 trade names of medications. the top market share of chondroprotective medications in ukrainian market belongs to the foreign producers (52.5%), while domestic medications occupy 47.5%. in general the nomenclature of ukrainian medications is provided by 6 manufacturing companies. three pharmaceutical companies are the leaders producing 5 names of medications each (12.5% each): pharmaceutical company “zdorovye” (kharkiv), prjsc “fitofarm” (bakhmut, formerly artemivsk, donetsk region), jsc “farmak” (kyiv). out of 40 trade names the medications are mainly produced: as coated tablets – 12 medications (30%), as solutions for injections – 11 items (27.5%), as capsules – 7 (17.5%), as soft dosage forms – 6 medicines (15%), and powders for oral administration – 4 items (10%). the process of dosing demulsifier is considered by changing its consumption depending on the consumption of oil emulsin at the installation inlet. the oil emulsion flow ratio and the rate of change of the demulsifier level, which leads to a change in the consumption of the demulsifier, is determined. the proposed process allows to reduce the cost of oil refining by increasing the accuracy of dosing. the whole complex oil processing unit (copu) is a single block module. the delivery set includes a technological unit, a control unit, furnaces and heat exchangers, automatic furnishing and maintenance units. copu should be equipped with valves and flow sensors as well as, other means of monitoring, control and regulation, as well as, instruments for testing and emergency protection systems. operation of copu control is carried out using an automated system, both remotely and locally. in the process of oil preparation, monitoring and measurement must be carried out the regulation of technological parameters, if necessary. various comprehensive training facilities correspond to various indicators of economic efficiency, in particular: indicator of energy consumption, complexity of technology; costs of installation, operation, maintenance of installations (including the cost of servicing personnel); the complexity of the oil preparation process. УЛЬТРАСТУКТУРНА ОРГАНІЗАЦІЯ ГЕМОМІКРОЦИРКУЛЯТОРНОГО РУСЛА ШКІРИ БІЛОГО ЩУРА НА РАННІХ ЕТАПАХ ПЕРЕБІГУ ЕКСПЕРИМЕНТАЛЬНОГО СТРЕПТОЗОТОЦИН- ІНДУКОВАНОГО ЦУКРОВОГО ДІАБЕТУ

this article is presented information about electron microscopic characteristic the angioarchitectonics of hemomicrocirculatory white rat skin net in experimental streptozotocin-induced diabetes mellitus. was used in the experiment 30 adult white male rats weighing 120-130 grams, which were maintained on a standard diet, had free access to food and water under normal conditions. the insulin-dependent form of type i diabetes mellitus was modeled by a single intraperitoneal injection of streptozotocin from sigma at a rate of 7mg per 100g of body weight of the animal (prepared on 0.1m citrate buffer, ph = 4.5). the development of experimental diabetes mellitus during 4 weeks was monitored by observing an increase in blood glucose, which was measured by the glucose oxidase method. studies were performed on rats with glucose levels of 12.00 mmol/l and above. the experiment was carried out in accordance with the provisions of the european convention for the protection of vertebrate animals used for experimental and other scientific purposes (strasbourg, 1986), council of europe directive 86/609 / eec (1986). rats were removed from the experiment by an overdose of intraperitoneal anesthesia using sodium thiopental (calculated at 25 mg / kg body weight of the animal). applying the method of electron microscopy (the study and photographing of the material was carried out on an electron microscope em-100 ak at an accelerating voltage of 75 kv and magnification 6000-8000 times). as a material for this examination, the skin intact from the internal surface of the thigh and the back of the white rats was used. the study showed that after 2 weeks of diabetes occure gradually and are progressing alterations of the skin's hemomicrocirculatory bed of white rats, which characterised mainly by small spasm of some vessels. the wall of the most vessels still had normal structure but revealed a narrowing of the capillaries due to the protrusion of the nuclear zone of the endothelial cells. after 4 weeks of the experimental streptozotocin-induced diabetes mellitus were already observed reconstruction practicaly all skin's microvessels of white rats. the lumen of the capillaries became irregular. the contours of the nuclei of endotheliocytes were elongated with an uniform homogen chromatyn, which is concentrated in lumps, thickened bazal membrane, observed proliferation of endothelial cells, which leads to narrowing of the lumen of blood vessels. at the end of the experiment, we observe the inclusion of compensatory properties of the organism - the wall of blood vessels of the hemomicrocirculatory bed of the skin thickens, while the lumen of the microvessels become narrows. the results of investigation can be used in the practice medicine for diagnosis and treatment of skin's diseases in diabetes mellitus. the article is devoted to the review of educational literature published in the printing houses of the largest muslim regions of the russian empire in the xix - beginning of the xx century. the author makes conclusion about general trends in religious education of muslims, the main elements of which was a maktab (elementary school) and madrasa (high school), and also lists branches of science, that were an obligatory part of the student’s program. is was noted that madrasas of each region had some sort of program, distinguished by in-depth study of a particular science. it is indicated that the superiority in the publication of muslim educational literature in the russian empire in this period belonged to kazan, which became the center of muslim printing. in daghestan and turkestan, where arab graphic printing firmly took its place in the early twentieth century, textbooks for muslim schools were also produced repeatedly and in large editions, as well as sold for affordable prices. the author lists and characterizes the most popular textbooks that were widely used in these regions, and concludes that educational literature, prayer books and ritual manuals were equally in demand by the local population.
