Abstract Cognitive impairment and dementia are conventionally categorised according to their causative disease, such as Alzheimer's disease and cerebrovascular disease. Cognitive impairments of different aetiologies often share pathological cerebral blood circulation patterns, characterised by perfusion and resistance. Here, we show that these two features are associated with distinct types of cognitive impairment. Using carotid ultrasonography, we evaluated the circulation status of individuals with little to no subjective cognitive symptoms. We assessed individuals’ cognitive status using the Frontal Assessment Battery for executive function and Mini-Mental State Examination for learning and memory. Regression analysis showed that the hyper-resistance and hypo-perfusion components predicted the Frontal Assessment Battery and Mini Mental State Examination scores, respectively. These results are consistent with previous findings of hyper-resistance being associated with atherosclerosis, which disproportionately affects the frontal lobe and contributes to executive function impairments; meanwhile, hypo-perfusion disproportionately affects temporal lobe functions associated with learning and memory. We propose a novel concept that cognitive impairments fall into two subtypes regardless of the brain disease: hyper-resistance and hypo-perfusion subtypes. As these subtypes can be measured noninvasively and are treatable, this distinction would help design preventive and therapeutic strategies for dementia.

Objetivos: a) Verificar la cantidad de estudios efectuados en Iberoamérica sobre función ejecutiva (FE) en adultos mayores; b) Identificar los instrumentos utilizados para medir las FE en adultos mayores durante los años 2009-2018. Material y métodos: Se efectuó un estudio bibliométrico sobre las FE en adultos mayores de Iberoamérica. La búsqueda de información se hizo en las base de datos Scopus y Scielo. Se consideraron estudios iberoamericanos publicados en idioma español y portugués, utilizándose  ocho palabras clave y cinco indicadores registrados. Resultados: Se identificaron un total de 12 investigaciones originales en seis países iberoamericanos (España, Argentina, Chile, Colombia, Cuba, y Perú). La mayor cantidad de investigaciones relacionadas con FE en asultos mayores tuvo lugar en los periodos 2009-2010 y 2013-2014.  En estos estudios, se han utilizado varios instrumentos con el Trail Making Test (TMT) y la Frontal Assessment Battery (FAB) como los de uso más frecuente. Conclusiones: Parece existir un interés limitado por investigar las FE en adultos mayores de Iberoamérica. La mayoría de los estudios idntificados en esta indagación utilizaron como tests cognitivos el TMT y la FAB.

  Objetivo: Validar o Jogo das Mãos TATI enquanto instrumento de avaliação da coordenação motora e das funções executivas, em idosos institucionalizados. Metodologia: Foi recrutada uma amostra de conveniência de 10 idosos institucionalizados (idades entre os 81-92 anos) e cognitivamente preservados (considerando o resultado no Six Item Cognitive Impairment Test -6CIT). O protocolo de implementação incluiu o procedimento do Jogo das Mãos TATI, nas modalidades unilateral e bilateral, a Frontal Asssessment Battery (FAB, para avaliar as funções executivas), o teste de toque em discos da bateria de testes de aptidão física do EUROFIT (para avaliar a coordenação dos membros superiores) e a avaliação da experiência com o jogo. Resultados: Foram obtidas correlações elevadas e estatisticamente significativas entre a pontuação total da FAB e os resultados obtidos no procedimento do Jogo das Mãos TATI (ρ = - 0.74; p < 0.05; e ρ = - 0.77; p < 0.01, para as modalidades unilateral e bilateral, respetivamente) e entre os resultados obtidos no teste de toque de discos da bateria de testes de aptidão física do EUROFIT e no procedimento do Jogo das Mãos TATI (ρ = 0.72; p < 0.05; e ρ = 0.89; p < 0.01, para as modalidades unilateral e bilateral, respetivamente). Os idosos classificaram a experiência como globalmente positiva. Conclusões: Os resultados obtidos evidenciam o potencial do Jogo das Mãos TATI enquanto instrumento de avaliação das funções executivas e da coordenação motora, em idosos institucionalizados. Estudos futuros de validação deverão incluir amostras mais amplas e heterogéneas.  Resumen. El protocolo de implementación incluyó el el procedimiento del Juego de Manos TATI, en las modalidade unilateral y bilateral, la Frontal Assessment Battery (FAB, para evaluar funciones ejecutivas), la prueba de toque en la batería de pruebas de fitness de EUROFIT (para evaluar la coordinación de miembros superiores) y la evaluación de la experiencia con el juego. Resultados: Se obtuvieron correlaciones altas y estadísticamente significativas entre la puntuación total FAB y los resultados obtenidos en el procedimiento TATI Hand Game (ρ = - 0,74; p <0,05; y ρ = - 0,77; p <0,01, para modalidades unilaterales y bilaterales, respectivamente) y entre los resultados obtenidos en el test de aptitud física EUROFIT test de contacto con el disco de la batería y en el procedimiento TATI Game of Hands (ρ = 0,72; p <0,05; y ρ = 0,89; p <0,01, para modalidades unilaterales y bilaterales, respectivamente ). Los ancianos calificaron la experiencia como globalmente positiva. Conclusiones: Los resultados obtenidos muestran el potencial del Juego de Manos TATI como instrumento para evaluar las funciones ejecutivas y la coordinación motora en ancianos institucionalizados. The applied protocol included the TATI Hands Game procedure (unilateral and bilateral mode), the Frontal Assessment Battery (FAB, forexecutive functions assessment), the plate-tapping test of the EUROFIT testing battery (upper-limb coordination assessment), and the assessment of the game-based experience. Results: Large statistically significative correlations were found between the FAB total result and the Jogo das Mãos TATI results (ρ = - 0.74; p < 0.05; ρ = - 0.77; p < 0.01, for the unilateral and the bilateral mode, respectively), and the plate-tapping test of the EUROFIT testing battery results and the Jogo das Mãos TATI results (ρ = 0.72; p < 0.05; and ρ = 0.89; p < 0.01, for the unilateral and the bilateral mode, respectively). The game-based experience was globally classified as positive. Conclusion: Our results suggest that the Jogo das Mãos TATI can be used to assess motor coordination and executive functions in institutionalized older adults. Further studies are needed to replicate the results in larger and more heterogeneous samples.

Abstract Background Deficits of executive functioning (EF) are frequently found in neurological disorders. The Frontal Assessment Battery (FAB) is one of the most widespread and psychometrically robust EF screeners in clinical settings. However, in Italy, FAB norms date back to 15 years ago; moreover, its validity against “EF-loaded” global cognitive screeners (e.g., the Montreal Cognitive Assessment, MoCA) has yet to be tested. This study thus aimed at (a) providing updated normative data for the Italian FAB and (b) assessing its convergent validity with the MoCA. Methods Four-hundred and seventy-five healthy Italian native speakers (306 females, 169 males; mean age: 61.08 ± 15.1; mean education: 11.67 ± 4.57) were administered by the MoCA and the FAB. FAB items were divided into three subscales: FAB-1 (linguistically mediated EF), FAB-2 (planning), and FAB-3 (inhibition). Regression-based norms were derived (equivalent scores) for all FAB measures. Results Age and education were predictive of all FAB measures, whereas no gender differences were detected. The FAB and its sub-scales were related to MoCA measures—the strongest associations being found with MoCA total and MoCA-EF scores. FAB sub-scales were both internally related and associated with FAB total scores. Discussion The FAB proved to have convergent validity with both global cognitive and EF measures in healthy individuals. The present study provides updated normative data for the FAB and its sub-scales in an Italian population sample, and thus supports an adaptive usage of this EF screener.
